The brake pedal was depressed; however, the vehicle failed to stop as intended. No warning lights were illuminated. The contact stated that the failure occurred while the vehicle was being parked in the residence driveway. The vehicle crashed into the right side of the residence before coming to a stop. The air bags did not deploy. The contact stated that the right side of the garage was damaged. No injuries were sustained. The contact stated that the failure was a known failure with the vehicle. The contact stated that an unknown recall with a similar failure was completed in 2022. The vehicle was towed to a local tow yard. The vehicle was deemed a total loss by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 65,000.

Vehicle: 2021 Ford Mustang Mach-E VIN: [XXX] Mileage: 56,005 I am filing this complaint to bring to NHTSA's attention a safety concern regarding Ford Motor Company's decision to make the Blue Cruise driver-assistance system available only via a paid subscription beginning January 2027, at a cost of $49.99 per month. Blue Cruise is a hands-free highway driving assistance system — a Level 2 driver-assistance safety technology. The hardware enabling this system was installed in my vehicle at the time of purchase and was represented as a feature of the vehicle. Ford is now moving to disable this safety feature on existing, in-service vehicles unless owners pay an ongoing subscription fee. I respectfully request that NHTSA examine whether a manufacturer's ability to remotely disable or withhold active safety technology on vehicles already in service — technology that was sold as part of the vehicle — raises concerns under federal motor vehicle safety standards. Specifically: 1. Consumers purchased these vehicles with the reasonable expectation that installed safety hardware would remain functional. 2. Ford's decision to monetize this feature post-sale, on vehicles already in service, effectively degrades the safety capability of those vehicles for owners who cannot or will not pay. 3. The precedent this sets — manufacturers using over-the-air software controls to disable safety features for revenue — warrants regulatory attention. I am not alone in this concern. There are thousands of Blue Cruise-equipped Mustang Mach-E, F-150, and Expedition owners facing this same situation. I respectfully ask NHTSA to investigate whether Ford's subscription model for Blue Cruise is consistent with federal vehicle safety obligations, and whether any regulatory guidance is warranted to protect consumers from the post-sale removal of safety technology. The front camera system does not function which causes all forward collision safety features to become unavailable, as well as any cruise control or automatic high beam lights. Seems to be a common issues and is related to faulty fiber optic cables.

This is a safety issue. The front collision assist is not working. I have returned the car to the Ford Dealer (Biener Ford) several times but the dangerous issue persists. I'm not the only one. There are many complaints listed on the web and the Ford Mach E forum. Ford needs to recall their EV Ford Mach-E vehicles and fix this dangerous safety issue once and for all.

The car gives two messages, "Front Camera Fault Service Required" and "Pre-Collision assist not available." The cruise control does not work, however, the front camera does work. I had the vehicle scan done at the Ford dealer and was told it needs a 7 hour software update, after which it may need cables replaced. I do not understand how software goes bad. I do understand something may have happened to cables but that would not be checked until they had charged me $ 1,400 for a software update. This is a safety issue and, from what I read on the Mach E forum, is not where near unique to my vehicle.

Vehicle: 2021 Ford Mustang Mach-E Component: Image Processing Module A (IPMA) / Front Camera / Forward Collision Avoidance System The IPMA and front camera system have failed, disabling Pre-Collision Assist, AEB, forward collision warning, and adaptive cruise control while driving. The vehicle is available for inspection upon request. Warning messages "Front Camera Fault Service Required" and "Pre-Collision Assist Not Available" first appeared August 24, 2022 and have recurred consistently over nearly four years. No accident, physical damage, or obstruction was present at any occurrence. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The problem was confirmed by a Ford dealership in February 2026. Their repair order documents: "RAN IPMA SELF TEST AND FOUND DTC SET U3000-49... HARDWARE TEST FAILED... FOUND TSB RELATED REC REPLACE MODULE." The dealer contacted Ford, who authorized 50% off the module replacement only, characterizing it as a "loyal customer discount" rather than a warranty or goodwill repair. I declined to pay for the fix. NHTSA Campaign 11028164 (superseding TSB 25-2156) covers this exact failure. Ford's own TSB acknowledges the defect and the dealer confirmed it applies to my vehicle, yet owners are being quoted up to $8,000 for a known safety system failure. I'm requesting NHTSA investigate whether this warrants a recall. The front camera has failed and it is available for inspection upon request. The safety of myself and everyone in my car is at risk when the pre-collision assist is not available due to this components failure. Yes, it is a daily occurrence and the camera never stays on for long. A fix was attempted once by a dealer. The component has not been inspected by either of those parties. Only the dealer whom the car was purchased from. Yes, warning messages pop up every time in the car and in the phone app about the issue. They first appeared before car purchase when the dealer fixed it in early February. Then it resumed again in late February/March.

An error message intermittently pops up on my vehicle that says, "Front Camera Fault", causing collision assistance and adaptive cruise control to suddenly stop working. This is a major safety issue, as a failure of adaptive cruise control while under operation may lead to a risk of an accident. Ford is aware of this issue and has issued TSB 23-2155, but they are treating it like a standard repair and not covering vehicles out of warranty. This should be a safety recall. The car would intermittently give an error saying pre-collision assist not available and now this error is permanent. There is a technical service bulletin for it 23-2155 Lots of people have been complaining about this in the mach e forum https://www.macheforum.com/site/threads/front-camera-fault-pre-collision-assist-not-available-errors.18710/page-69#post-1026803 This causes people to drop out from the automatic driving bluecruise without notice and adaptive cruise control becomes unusable.

There are many reports of the Mustang Mach E "accelerating unexpectedly." This usually happens in parking lots or other situations involving low speed and a rapid turn of the wheel. I know exactly what the problem is, because it happened to me this morning. While parking in my parking garage, my car rapidly accelerated uphill and I only narrowly avoided hitting a wall. Here is why: There are two interconnected problems. The first is that the "cruise control on/off" button is located on the far left of the interior of the steering wheel, which makes it easy to accidentally press when doing a sharp, quick one-handed turn of the wheel (like when parking). When this happens, CC engages while in the act of parking. This is exactly what happened to me. I accidentally hit the button while doing a sharp twist of the wheel, I saw the CC engage, the car rapidly accelerated uphill without my foot on the gas pedal, and I would have hit the wall in front of me if I had been a split-second slower in hitting the brake. The second problem is that there is supposed to be a failsafe for accidentally activating CC at low speed, BUT IT DOESN'T ALWAYS WORK. Page 224 of my manual says "Press the button to activate system. When the system activates, the set speed is equal to whichever is greater, the current vehicle speed, or 15 mph...." This part is true, and this is how the unexpected acceleration to 15mph occurs when parking. But the manual continues: "If the speed is too low, or other conditions are not correct for adaptive cruise control activation, the system enters standby mode. When you are below 15 mph ... adaptive cruise control does not activate unless you are following another vehicle." This is FALSE. This did not work. Clearly the CC mistook the wall in front of me for "following another vehicle." And so, when I accidentally activated CC while parking, it lurched forward UPHILL to 15mph toward a wall a few feet away. Ford needs to move the button and/or fix the failsafe.

Camera system that allows hands free driving, lane departure warning, collision warning and adaptive cruise control fails and all system offline. Extremely widespread on the 2021 models. Looks to be a cable route to the camera that was installed incorrectly. Went to dealership, they said they see this in almost all of the Mach E vehicles. Usually begins failing when hitting bumps -- system goes in/out. After a while, system drops out completely and all safety systems relying on them fail.

When in cruise control, any speed, car suddenly accelerates This happens while driving, or sometimes after stopping and then resuming previous set speed. For example I was driving in cruise control at 38 mph, when the car suddenly sped to 50mph. Or when on a highway, driving at 55mph, the car suddenly accelerated to 70mph. Started about 3 weeks ago.

The contact owns a 2021 Ford Mustang March E. The contact stated that while reversing, the back over prevention camera image was frozen, and the driver was unable to view the images behind the vehicle. The vehicle was taken to the local dealer who determined that the back over prevention camera was faulty; however, the VIN was not included in NHTSA Campaign Number: 25V315000 (BACK OVER PREVENTION). The vehicle was not repaired. The contact stated that the vehicle had experienced the failure listed in the recall. The failure mileage was 7,500.

I was driving on the highway when the car gave me an error saying that the front camera is faulty and the collision warning system was giving an error. This stopped my cruise control and lane departure settings. I had just got my car back from Ford for this issue where they replaced a $7000 computer part and said it was fixed. This issue has been happening since I purchased the car in Dec 2024.

Front Camera Fault Service not available. Pre-Collision Assist Not Available. These faults have been showing up for over a year. I have taken my car to the dealer about 10 times, to no avail. Every time you hit a bump it happens. As I write this my car is in the shop again. The internet is rife with people complaining about this problem.

A KNOWN faulty camera problem for 100s of Ford EVs. Starts off as very intermediate warnings until it stops working all together. This problem doesn't allow you to use cruise control and a host of other standard safety features. This should be a recall, I can't imagine someone getting hurt or worse because ford isn't getting ahead of this thing.

Car gave me a warning to bring car to dealer, who told me I need to replace the High Voltage Battery Junction Box. This part was recalled on other models, but not my specific model, despite the cars using the same part. Dealer said it will cost $3,593 to replace and Ford customer service says there is no recall for it. Also, I intermittenly have gotten a warning that the front camera will not work for forward collision warning or cruise control. It often happens after a bump in the road, but also happens at other random times.

Front camera fault. Pre collision systems don’t work without it. Large online community with the same issues but ford refuses to acknowledge it’s an issue. They won’t help even if errors start happening before warranty is over. And once warranty is over they charge large amounts to fix. There is a TSB out for the issue and it seems like a pinched coaxial cable. Our safety is at stake and the number of occurrences can’t not be random. The front camera system stopped working, so I lost access to Pre-Collision Assist—(e.g., Collision Warning, Automatic Emergency Braking) as well as all other driver assistance feature. This would occur suddenly while I was driving (sometimes at highway speeds) where I'd have to swerve during a turn while using hands-free driving. Every time I start the car now, I have a permanent error message telling me the safety systems aren’t available. I’ve tried restarting the vehicle and checking for any obvious problems, but nothing has worked. The dealer told me it’s a hardware problem with the camera or its wiring, but fixing it would cost more than I can afford ($4K+). Since Pre-Collision Assist is a key safety feature, not having it working puts me—and possibly other drivers—at greater risk of an accident. This seems to be a common issue with Mustang Mach-Es, and I’m worried about the safety impact. I hope NHTSA can look into this and see if a recall or some other action is needed.

I am the owner of a 2021 Ford Mustang Mach-E and have experienced repeated failures of the front-facing camera system, which directly impacts critical safety features such as automatic emergency braking, lane-keeping assist, and adaptive cruise control. When the camera fails, these safety systems become unavailable or behave unpredictably, putting my safety and that of others at risk. The issue began around 40,000 miles. It is typically preceded by dashboard warnings such as “Front Camera Fault” or “Driver Assistance Features Unavailable.” I paid out of pocket for a repair at an authorized Ford dealership, but only four months later, the exact same issue happened again. Now, Ford is requiring me to pay for the repair again, despite this being a repeated safety-related failure of the same component. The dealership confirmed the problem both times, and the front camera module is still available for inspection if needed. Ford has refused to cover the cost under warranty or acknowledge any broader defect. I am also aware that other Mustang Mach-E owners have reported similar failures online. Given the recurring nature of the issue and the impact on critical driver assistance systems, I believe this is a serious safety concern that warrants a formal recall and investigation by NHTSA.
