Recalls, owner-reported complaints, investigations and safety data

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.
0
Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
0
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

POWER TRAIN:CLUTCH ASSEMBLY

Feb 12, 2002
1,530 units potentially affected
The defect: CERTAIN MEDIUM DUTY TILT CAB TRUCKS EQUIPPED WITH MANUAL TRANSMISSIONS HAVE A CONDITION IN WHICH THE CLUTCH MASTER-CYLINDER PUSHROD END THAT ATTACHES TO THE CLUTCH PEDAL PIN CAN WEAR PREMATURELY. THE FIRST NOTICEABLE EFFECT OF THIS WEAR COULD BE THE CLUTCH PEDAL NOT RETURNING COMPLETELY TO THE FULL-UP POSITION.
The risk: IF THIS OCCURS, THE FAST-IDLE ENGINE SPEED CONTROL MAY NOT FUNCTION. IF THE CLUTCH MASTER-CYLINDER PUSHROD END WEARS SUFFICIENTLY TO ALLOW THE ATTACHING END TO BEND OPEN OR BREAK OFF, THE PEDAL PIN WOULD DETACH FROM THE CLUTCH ROD LINK AND THE CLUTCH WOULD ENGAGE. IF CLUTCH ENGAGEMENT OCCURRED WHILE THE VEHICLE WAS STOPPED, WITH THE ENGINE RUNNING AND THE TRANSMISSION IN GEAR, AND IF THE BRAKES WERE NOT APPLIED, THE VEHICLE COULD MOVE FORWARD IF THE TRANSMISSION WERE IN A FORWARD GEAR, OR REARWARD IF THE TRANSMISSION WERE IN A REVERSE GEAR.
The remedy: DEALERS WILL INSTALL A NEW CLUTCH MASTER-CYLINDER ASSEMBLY AND CLUTCH PEDAL ASSEMBLY. OWNER NOTIFICATION BEGAN APRIL 23, 2002. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT CHEVROLET AT 1-800-222-1020 OR GMC AT 1-800-462-8782.

Due to the existing cellular provider not having roaming agreements in areas outside of Canada to support transmitting of the packet data required for Turn-By-Turn feature to operate. The customer has 3 options while traveling through the United Sates. First if the vehicle is equipped with a navigation radio they can request a "Destination Download" to the vehicle. Second if the customer uses the OnStar RemoteLink app they can send the directions to the vehicle. The third option would be to have the vehicle loaded with a "500 phone number" The customer will no longer be able to use their Hands Free Calling minutes when the new 500 phone number is loaded. If a customer chooses to restore Hands Free Calling in the future a new phone number will be assigned to the vehicle; previous phone numbers are not available.

EQUIPMENT:ELECTRICAL

This Preliminary Information communication provides information to the technician the steps needed to diagnose and repair the vehicles that may have unwanted or phantom phone calls. Technician will need to determine what type of call the customer is having that is unwanted. The vehicle may receive an incoming call just like any other phone. Typically the customer will hear the phone ringing in the vehicle. This scenario also includes incoming Bluetooth calls. One type of unwanted call can be made if the key pad is faulty the OnStar system will try to make a blue button call for shorted voltage in the circuits. The OnStar module could have an internal fault and produce a "Phone Unavailable" audio message also it may have a Diagnostic Trouble Code U1500 or B1000. Refer to diagnostics for the Diagnostic Trouble Code making sure to follow any applicable Bulletins or Preliminary Information sheets. A "Ready" or "OnStar ready" audio message heard while driving. In some cases it has been found that the customer is inadvertently pressing the ?push to talk? button on the steering wheel controls. On rare occasions, other issues in the steering column or Steering Wheel Control system may induce this concern.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.
