My 2022 Honda Pilot Black Edition (VIN [XXX], 62,900 miles) has multiple intermittent safety‑critical failures involving the transmission, acceleration, adaptive cruise control, and HVAC blower. These issues occur without warning and without diagnostic trouble codes. Transmission/Acceleration Failure: Most recent incident was XXX on Highway 71 during heavy traffic. The vehicle twice experienced rapid deceleration with no ability to reach a safety stop. RPM dropped below 1000, then spiked to 6,000–8,000 while the vehicle continued to lose speed. Acceleration returned only after several seconds. This also occurs during highway merging and uphill acceleration. Additionally, when disengaging cruise control and immediately pressing the accelerator, the vehicle loses speed, RPM drops abnormally low, then spikes before power returns. Adaptive Cruise Control/Honda Sensing Failure: On XXX the vehicle rapidly decelerated when a semi‑truck was beside me, despite no vehicle ahead. The week prior, while driving several states away, setting cruise at 70 mph caused sudden deceleration and cruise resetting itself to ~54 mph. These events resemble phantom braking and radar mis‑targeting. HVAC Blower Failure: During a long multi‑state trip the week prior, the AC blower (speed 3–4) intermittently stopped blowing for several seconds, then resumed at the same speed with no change to the display. These failures affect powertrain, forward collision avoidance, vehicle speed control, electrical systems, and cabin equipment. The dealership (Roush Honda, Westerville OH) cannot replicate the issues and reports no stored codes. These defects pose significant safety risks. The contact owned a 2022 Honda Pilot. The contact stated that while the vehicle was parked in a parking spot with the brake pedal depressed, the engine revved unexpectedly and independently accelerated and crash into a Ford Bronco, a Jeep Cherokee, another vehicle, and two parking signs. There were no warning lights illuminated during the failure. The contact had made three other stops prior to the crash. There was no injury sustained. The air bags deployed as needed. A Police report was filed. The vehicle was towed to a tow lot. The local dealer was contacted, and the contact was informed that the failure was possibly caused by a stuck floor mat. The dealer declined to inspect the vehicle. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The insurance company was contacted and deemed the vehicle a total loss. The owner had not yet released the vehicle to the insurance company. The failure mileage was approximately 56,000.

Brake light wont cut off and drains battery. Car wouldn't start and once battery was charged all steering, cruise control, hill assist, lane assist and other lighs came on. 7 total alerts came on and car struggled to go up hills and speed up after braking at lights. Brake light stayed on entire drive and didnt go off even when the car was off.

I was driving home at 4am on I-95 with my children in the car. My car has had a history of "juttering" and making a vibrational noise, but on the way home there was an issue with acceleration when using the gas pedal, and slow to gain speed. There were times when the gas pedal didn't respond to my foot depression at all, and did not accelerate. When I was almost home, many lights came on my dash including: Blind Spot warning, emissions systems problem, transmission problem apply parking brake when parked, stop driving when safe transmission problem see manual. The car is not driveable at this time and sitting in my driveway. I have no idea where to tow it to or what to do or how much it will cost

Windshield cracked majorly after being hit by a small rock on the highway. Even after having it replaced and the camera recalibrated, 7 safety warnings of the ADAS come on. Reportedly, the frontcamera has failed, which is concerning in a car that has less than 50k and is just about 4yrs old. I believe this is a larger issue, the windshield truly should not have broken that easily and the camera failing so soon is problematic. It is also a very expensive fix.

The contact stated that while reversing into the garage, the accelerator pedal was depressed and the vehicle continued to accelerate before crashing into the garage. On another occasion, while driving into the parking lot, the brake pedal was depressed; however, the vehicle independently accelerated and crashed into a store. There were no warning lights illuminated. The driver was not injured but sought medical attention at the hospital and was treated for three days. A police report was filed. The air bags did not deploy. The vehicle was towed to a tow lot. The contact was informed by a friend that the failure was related to the throttle body. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The vehicle was towed to an unknown location and deemed a total loss by the Insurance provider. The failure mileage was unknown. The VIN was not available.

“The vehicle experiences phantom braking while driving, where the Collision Mitigation Braking System (CMBS) activates without warning even when there are no vehicles or objects in front of me. The vehicle suddenly brakes on its own, creating a major safety hazard and risk of rear-end collisions. The issue has happened multiple times during normal driving at different speeds. Warning lights related to the collision mitigation system and passenger airbag system have appeared intermittently. The blind spot monitoring system has also failed to properly detect nearby vehicles at times. The vehicle was previously taken to the dealership/service department and remained there for an extended period for inspection and attempted repairs, but the problem was not fully corrected. After getting the vehicle back, the same symptoms and braking issues continued. This issue puts myself, my passengers, and other drivers at risk because the vehicle may suddenly brake unexpectedly in traffic.

My 2022 Honda Pilot failed to engage Drive after reversing, despite the Drive button being pressed. This resulted in an unintended backward motion & a collision with another vehicle. 1.What component or system failed or malfunctioned…? The Electronic Transmission Shift-by-Wire system (push-button shifter) failed to respond to a gear change request. Additionally, related electrical systems including the dashboard infotainment system & the integrated USB charging ports have malfunctioned. The vehicle & its components are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The vehicle experienced unintended motion. While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. There were no specific transmission warning lights prior to the gear selection failure; however, the persistent electrical issues in the dash area suggested a systemic electronic malfunction.

I turned my car on one day and warning lights for all of my safety sensing features came up. Intermittently they work for a little bit. I brought it to the dealer, and they said that there was damage to the sensor, but there has been no damage to the front end of the car. Their reply was perhaps a pebble came up from the road while I was driving. It seems to me that that is a fault in the manufacturing if it is that easy for all of the sensing features to stop working. I attempted to sort this out with the dealer and with Honda corporate offices with no luck.

I was operating a 2022 Honda Pilot and had just made a left turn into a parking lot. I was slowly pulling into a parking space directly in front of a dance studio with a large floor-to-ceiling window. There were four children and a dance instructor standing inside near the window. As I was coming to a stop to park, my foot was on the brake. At that moment, the engine suddenly revved loudly and the vehicle accelerated forward unexpectedly without any additional throttle input. There were no warning lights, messages, or prior symptoms before this occurred. Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle was inspected by insurance and they did not total it. It is going to be repaired. The vehicle has been inspected by insurance representatives. The manufacturer has not inspected the vehicle at this time. I did not call police because I was not hurt, the kids were not hurt and the building only had a couple of scratches on the brick.

The front windshield camera is causing the brake system error light to come on and triggering the safety system to malfunction. This failed camera system disables safety features and causes lane assist, cruise control , lighting features, back-up camera, driving assist, and the collision system to be disabled, causing a major safety concern. This front camera system has failed and has been diagnosed by the Certified Honda dealer that my vehicle was purchased from. My vehicle is 4 years old and has 35k miles, and the system has failed. Each component was inspected and issues diagnosed by the dealer. The warning light first displayed on the dashboard and showed as a "brake system" error light. Under the break system error 6 errors displayed they read : 1) adaptive cruise control problem 2) Lane keeping assist problem 3) collision mitigation system problem 4) Road departure mitigation system problem 5) auto-high beam problem 6) transmission system problem

Lots of warning lights going off in car. Took it to the dealership and was told it’s the front camera and would cost over $2k to fix. Car is new, less than 5 years old. Should not be having this issue on a new car. After researching found there are lots of people with the same issue. Seems to me it’s a manufacturing issue and Honda should be responsible to fix.

It is constantly breaking when cars are coming the other way. It almost caused a vehicle to almost hit me. Also the auto shut off has almost caused accidents because it causes my car to stall and I wish I could keep it off and not have to push the button off every time.

As I was traveling from Houma, La on a major highway to an out of state meeting. I started to hear a slight tapping sound and pulled over to inspect the sound. Upon inspection it seemed as though it was coming from the lower engine, or exhaust near the engine area. I located the nearest Honda dealership which was Community Honda of Lafayette with intentions of dealership inspection. It was approximately a 20 minute drive from the current location. As I was approaching the exit to the dealership the warning system lit up an check engine light. The Manager at the dealership took the vehicle to the back and ran a scan and responded that it sounds like an exhaust leak. I got back on the road for my destination. Approximately an hour later while I was on [XXX] all of the warning lights lit up at once, the engine jerked and shut off. I pulled over to inspect the vehicle again. I restarted the vehicle to work myself to a safe exit. The vehicle would not drive over 20mph. Once at a safe location I made concessions to have the vehicle carried back to my local Honda dealership, Barker Honda in Houma, La Upon inspection by the Honda technicians it was found to have irreparable damage to the engine internals. Technician states this could occur to low oil and/ or lack of maintenance; however, the technician was perplexed by the failure because all maintenance and records in their system showed that the vehicle was properly maintained by their staff. The ADAS camera has failed. It is extremely common. When it fails, all the lights for EVERY system of ADAS comes on. None of them work. And it is not possible to turn off. Furthermore it sets codes for the brakes and turns on the brake warning lights. So none of the brake assist systems work either. Honda refuses to warranty any of these systems when the camera fails. It is across their entire line of vehicles with ADAS. If they can’t build a functional system, it should not have the system. Also, the system functions extremely poorly, when it was operating. It will drive the vehicle, unassisted, down to 20 mph, then just shuts off and lets the vehicle coast, unguided. Also, if the cruise is set and lane keeping on, and it can’t find lane lines, particularly where lanes merge, it will often veer left, into oncoming traffic. I have tested both of these, when the system was functioning “correctly”. The cruise and lane keeping just shuts off, at any speed, and lets the car go where ever it would without input. It is extremely dangerous.

The contact owns a 2022 Honda Pilot. The contact stated that the entertainment system was blank and had shut down. The message “My Map Off” was displayed. The vehicle was restarted, and the rearview camera was inoperable. The contact linked the failure to NHTSA Campaign Number: 23V431000 (Back Over Prevention). The local dealer was contacted, but the vehicle was not diagnosed or repaired. The manufacturer was contacted and assisted with resetting the system. The failure mileage was approximately 24,000.

My vehicle multipurpose camera unit failed. The entire advanced driver assistance system shut down and stopped working. This error randomly appeared. My vehicle has 48,000 miles on it. I received error codes for each of the components that they were not working. This meant no adaptive cruise control, no automatic emergency braking, no blind spot warning, no forward collision warning, no lane departure warning, no lane keeping assistance, no parking collision warning, no rear cross traffic warning. My safety was put at risk as the components that provide safety warnings were not operative. I took the car to my Honda service shop and they provided the following diagnostic: [Other]: Verify customer concern that theirs multiple lights on on the vehicle, then check for codes notice theirs code for U3000-49 multipurpose camera unit/ millimeter wave radar internal electric failure. Then disconnect one of the connectors for sensor to single one out to see if when clear the code to see if stays on or not, after clearing the code lights still came on and code pop on. Recommend start first replacing with a new multi purpose camera, and retest. I paid for the repair as it needs fixed for safety purposes. The cost was $1,950.00. Honda should be responsible for covering this as it is some type of camera malfunction and the vehicle is only 3 years old.

All the sudden the lights in dash went off reading lane keep assist failure, adaptive cruise control failure, and automatic brake system failure. Nothing was working, lights stayed on, took to auto body shop, they said I need a whole new front windshield camera and for it be recalibrated, that will cost more than $2000. Honda is saying this isn’t under warranty or recall but it seems like it keeps happening to many Hondas when you look up the same problem so it should be a recall.

When I started my 2022 Honda Pilot Special Edition (VIN: [XXX] ) in a parking lot, the vehicle immediately behaved abnormally. As I attempted to turn into a space, the car unexpectedly accelerated and moved in an unintended direction. Despite pressing the brake pedal firmly, neither the standard brakes nor the automatic braking system responded. Shifting into Park did not stop the vehicle, and the transmission did not engage to hold the car. The vehicle continued moving through the parking lot, striking property and other vehicles. The entire event felt like a loss of braking and control, with both the brake system and gear selector failing to function as intended. This was a sudden and dangerous incident that could have caused serious injury. The Honda dealer has confirmed there are no open recalls or campaigns for my VIN, but the event aligns with reports of unintended acceleration and brake failure in other 2022 Honda Pilot vehicles. The camera on the windshield fails when then the windshield gets too hot when you are driving and causes the safety systems to stop functioning ( lane assist, blind spot, etc). See attached pictures. This is a major design flaw and Honda should recall and replace the cameras on all 2022 Honda pilots. I live in the desert in El Paso, Texas and this happens when you are driving . See my attached pictures that occurred at 83 degrees and 93 degrees, respectively. Please help me before there is a fatal injury.

The foreward collision warning is too sensitive and the alert message and steering wheel shaking are distracting. Especially driving on a two lane road, the forward collision messages occurs under normal driving situations. Simply a vehicle approaching in their lane in the opposite direction triggers the warning. You get a large orange box popping up on the dash screen which diverts your attention from the road to the screen. It's an obnoxious panic message. Also, the steering wheel shakes to alert you, which startles you. I fear the mechanism could malfunction and take over steering and cause an accident. I know how to drive and I don't need the absurd "assistance". I submitted another report for the auto idle problem. These driver assist things are horrid. They don't enhance the driving experience and seem to be items that unnecessarily tack on to the cost of purchasing a vehicle. We have complained to the Honda dealer and car mechanics about these matters. Regarding Forward Collision Warning, no resolution was provided. This is an ongoing problem and so an approximate date below is not applicable. Thank you Thank you.
