Unintended and Uncontrolled Acceleration
On February 16, 2010, NHTSA's Office of Defects Investigation (ODI) opened RQ10-003 to determine whether the scope of prior Toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, Toyota recalls 07E-082, 09V-388, 10V-017, and 10V-023.NHTSA also requested information regarding potential electronic causes of unintended acceleration in Toyota and Lexus vehicles.I. Pedal Interference from Floor mats, Carpet Covers and Plastic Pads in Carpets.During the RQ10-003 investigation, ODI reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agency's analysis, NHTSA requested that Toyota recall additional vehicles.Toyota complied with the agency's request.The details of these recalls are set forth more fully in Toyota's reports to NHTSA pursuant to 49 CFR Part 573.These recalls are as follows:Recall 11V-112: (1) model year (MY) 2004-2006 Toyota Highlander and Highlander Hybrid; and, (2) MY 2004-2007 Lexus RX330,RX350, and RX400h (hybrid model). The total estimated population under this recall is 769,379 vehicles.This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles.This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles.This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration.After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles.That study has concluded.Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.

TL* THE CONTACT OWNS A 2009 LEXUS LX570. THE CONTACT WAS DRIVING 30 MPH AND ATTEMPTING TO STOP WHEN THE BRAKE PEDAL WAS ABNORMALLY DEPRESSED INTO THE FLOORBOARD. THE CONTACT STATED THAT ALL OF THE LIGHTS ON THE INSTRUMENT PANEL ILLUMINATED ACCOMPANIED BY A WARNING SIREN, INDICATING A BRAKE MALFUNCTION. THE CONTACT THEN ENGAGED THE SAFETY BRAKE AS THE VEHICLE COASTED IT TO A STOP. THE VEHICLE WAS TOWED TO A DEALER AND DIAGNOSED AS THE BRAKE PADS AND ROTORS BEING WORN AND A LACK OF BRAKE FLUID.THE CONTACT STATED THAT THE HE HAD BEEN KEEPING UP WITH THE RECOMMENDED MAINTENANCE OF THE VEHICLE. THE VEHICLE WAS LEFT WITH THE DEALER WHERE THE CONTACT WAS AWAITING REPAIRS. THE FAILURE AND THE CURRENT MILEAGE WAS 35,000. THE VIN WAS NOT AVAILABLE. UPDATED 02/15/12*BF UPDATED 03/12/12

I AM AN EXPERIENCED CPST (>800 SEATS PER YEAR) WHO YESTERDAY ASSISTED A MOTHER WITH INSTALLATION OF HER NEW GRACO SR-35 INFANT CAR SEAT W/ BASE MODEL # 1759703 IN HER 2009 LEXUS LX570 SUV. THIS COMPLAINT IS SPECIFIC TO A VEHICLE INTERIOR FEATURE THAT MADE IT VERY DIFFICULT TO CORRECTLY INSTALL THE CHILD RESTRAINT. THE VEHICLE BENCH SEAT IN THE SECOND ROW HAD LEATHER INTERIOR. THE DESIGN OF THE VEHICLE SEAT CUSHION INCLUDES A SUBSTANTIAL ROUNDED CUSHION THAT PROTRUDES FORWARD AT THE VEHICLE SEAT BIGHT. THIS SUBSTANTIAL THICKNESS OF MFG DESIGNED PADDING INTERFERES SIGNIFICANTLY WITH THE INSTALLATION OF AN INFANT SEAT BASE BECAUSE IT DISPLACES THE BASE OF THE INFANT SEAT SEVERAL INCHES FORWARD OF THE SEAT BIGHT. THE DISPLACEMENT OF THE BASE SEVERAL INCHES FORWARD OF THE SEAT BIGHT PROHIBITED SECURE INSTALLATION OF THE BASE WITH BOTH THE LATCH SYSTEM AND THE SEATBELT. TOYOTA SUVS OF SIMILAR BODY TYPE ALSO HAVE A SIMILAR SEAT CUSHION FEATURE HOWEVER IT IS NOT AS PRONOUNCED. ATTEMPTING TO INSTALL THE BASE ON TOP OF THIS CUSHION THAT PROTRUDES FROM THE SEAT BIGHT WAS A PROBLEM AS THE BASE EASILY SLIPS OFF THE PROTRUDING CUSHION ....CAUSING EXCESSIVE SLACK AND SIDE TO SIDE MOVEMENT OF THE BASE. THIS SLIPPING AND THE EXCESSIVE SLACK IT CREATES COULD PROVE PARTICULARLY PROBLEMATIC IN LIGHT OF THE FACT THAT THE OUTBOARD SEATING LOCATIONS OF THE 2009 LEXUS LX570 HAVE SEAT MOUNTED TORSO AIRBAGS. MY 9 YEAR OLD SON NEARLY STRANGLED IN FRONT OF HIS SISTER AND TWIN SIBLINGS (5 YEARS OLD) WHEN THE MIDDLE SEAT BELT OF THE THIRD ROW GOT TANGLED AROUND HIS NECK. THIS SEATBELT HANGS FROM THE CEILING. I COULD NOT DISLODGE THE BELT FROM AROUND HIS NECK, AS IT WAS NOT OBVIOUS HOW TO DISENGAGE THE CEILING LATCH. WE WERE VERY FORTUNATE TO STOP A PASSING CAR, RETRIEVE A PAIR OF SCISSORS AND CUT THE BELT TO FREE MY SON. HE WAS ON THE VERGE OF PASSING OUT FROM LACK OF OXYGEN. THE PARAMEDICS TOOK HIM THE TO THE ER, WHERE THEY DETECTED A RASH AROUND HIS FACE AND NECK THAT IS COMMON WITH STRANGULATION. HE WAS BADLY TRAUMATIZED, ALONG WITH THE REST OF US. I REPORTED THE INCIDENT TO LEXUS AND FOLLOWED UP NUMEROUS TIMES. EACH TIME THEY ADVISED ME THAT THEY WERE INVESTIGATING THE INCIDENT AND WOULD GET BACK TO ME. THEY NEVER DID DESPITE MORE THAN 6 EFFORTS TO SPEAK WITH SOMEONE. *TR

TL* THE CONTACT OWNS A 2009 LEXUS LX570. WHILE DRIVING APPROXIMATELY 35 MPH, THE VEHICLE ABNORMALLY ACCELERATED. THE GEAR SHIFT WAS PLACED INTO NEUTRAL AND THE VEHICLE STOPPED ACCELERATING WHILE THE ENGINE CONTINUED TO EXHIBIT UNUSUALLY HIGH RPMS. PRESSURE WAS APPLIED TO THE BRAKE PEDAL BUT THE VEHICLE WOULD NOT STOP UNTIL THE IGNITION WAS TURNED OFF. UPON EXAMINATION, THE CONTACT WANTED TO MAKE NOTE THAT THE FAILURE DID NOT INVOLVE THE FLOOR MATS. THE POLICE APPEARED ON THE SCENE TO ASSIST WITH PUSHING THE VEHICLE OFF THE ROADWAY. THE VEHICLE WAS NEITHER EXAMINED FOR THE CAUSE OF FAILURE NOR REPAIRED. THE FAILURE AND CURRENT MILEAGE WAS 20,000.
