While driving through normal rain with ordinary standing water on the roadway (NOT flood conditions), the rear bumper fascia detached from my Tesla Model Y. There was no impact, no collision, and no debris strike - there are no impact marks or scratches on the bumper. The fascia was pulled off by water drag, tearing a welded body mount bracket from the vehicle structure and damaging the underbody shield. This is a road safety hazard: a detached bumper fascia can drag on the roadway, separate completely, and become a large road obstacle for vehicles behind me. Tesla documented this exact failure mode in Service Bulletin SB-20-12-003 (rear fascia detachment when driving through standing water, with damage to harnesses and body fasteners/mounts), yet the defect persists in Model Y vehicles. Notably, another Tesla directly next to mine suffered the identical bumper detachment in the same rainfall at the same time - two identical failures in identical ordinary weather conditions, indicating a systemic design defect rather than driver error. Tesla Service denied warranty coverage, characterizing normal rain as 'flood water.'

On July 10, 2026, my 2024 Tesla Model Y (approx. 22,440 miles) lost all power (main/12V battery) while parked next to a Supercharger stall on Level 3 of the Harrah's Ak-Chin Casino parking garage, Maricopa, AZ. Ambient temperature was approximately 110F. My dog and cat were inside the vehicle. With the vehicle powered off, the key card, Tesla app, and interior door releases did not function, leaving no way to open the doors from outside or inside. Unable to find or access a working manual door release in time, we broke the rear quarter glass and damaged a door to physically extract the pets before they suffered heat-related harm. Maricopa Police Department and Ak-Chin Fire Department responded to the scene. This represents a serious safety defect: a loss of vehicle power should not eliminate all means of entry/egress, particularly when children or pets could be trapped in extreme heat. The vehicle, damaged door, and glass are available for inspection. Photos of the incident are available.

The rear hatch has shifted out of alignment, and has started to fall 5-10 inch's when opened intermittently. TESLA service can't fix the issue and pushing me to a body shop. There have been no accident or issues that would cause a body panel to shift over a quarter of an inch.

After about 2 minutes from leaving a patients house, I was driving at approximately 25mph when my frunk(front trunk) of my Tesla flew open and hit my windshield. There was no alert through the Tesla app or in the car itself. I stopped the car stepped outside and the frunk would not close after when I tried to close by hand, it would stop at the latch. When I closed it from the Tesla app it was able to close properly

On the evening of June 3, 2026, while driving on the I-575 Express Lanes in Cobb County, Georgia, the vehicle’s autopilot/driver assistance system malfunctioned and steered the vehicle into the express lane boom barrier. The boom bar struck the windshield, causing catastrophic windshield failure with glass imploding into the cabin. Glass shards covered the driver’s seat, floor, and dashboard. The driver sustained glass puncture wounds to both forearms and forehead, as well as neck, back, and right arm soft tissue injuries. Component failed: Autopilot/ADAS system. Vehicle is available for inspection. Tesla was notified directly at time of incident. Insurance company has been notified. SR-13 filed with Cobb County. No warning lamps or messages appeared prior to the autopilot malfunction. The root cause of the windshield damage was the autopilot system failure, not driver error or external road debris. Photographic documentation of vehicle damage and driver injuries exists.

I am writing to formally report a serious malfunction involving my Tesla vehicle. On my 2024 Tesla model y.I experienced an unexpected accelerator malfunction that caused the vehicle to surge forward uncontrollably. The incident occurred at approximately As I attempted to dive in slowly, the accelerator suddenly malfunctioned and my car creeped in slowly and rapidly increased power without my intention. Despite immediate attempts to brake and regain control, the vehicle continued to accelerate and subsequently collided with a gate, resulting in significant front-end damage. I firmly believe this was due to a mechanical or electronic failure in the vehicle’s accelerator or related control systems. The malfunction posed a serious safety hazard and could have resulted in injuries, and this has affected me mentally. I am requesting a full investigation into this malfunction, including an inspection of the vehicle’s electronic logs, sensors, and accelerator system to determine the cause of this unintended acceleration. I would appreciate your prompt response and guidance on the next steps to resolve this matter, including vehicle inspection, repair, and coverage options.

Finding random screws coming off the vehicle in various compartments. Two screws found laying in glove compartment that came off the assembly holding the inside of the glove box. A hex screw found in rear passenger arm rests storage compartment, after further look up, this screw became lose and fell off the arm rests storage compartment assembly.

Since I purchased the car 4 months ago my car has been going into the service center non stop for every issue you can imagine. Tesla spins and blames all issues and ignores me. Corporate says they can’t do anything and then ignore. Service center providing inaccurate or not providing service invoices.Deny my buyback requests. Repairs are failed and new damage occurs at every service appointment. New issues arise with the car every time I drive it. I rarely drive it as it’s not safe. Tesla continually cancels service appointments or does not have parts in stock. issues: -Safety restaurant system fault -electric motor issues: loud high pitched whistle/whine when driving. -car loosing power while driving -seat moving while driving -window clicking -water dripping out of dash vents. -Misalignments: frunk, trunk, trim -paint drips, bubbles, cracks, peeling -battery range issues -battery coolant fan issues -defects interior plastic surfaces -seat defects -hole left in front of car between dash and windshield with damages windshield. New dash uneven over airbag. -plastic trim falling off

I am writing to formally express my deep concern and dissatisfaction regarding a significant safety issue with my Tesla Model Y 2024, which I purchased directly through Tesla’s official website. To date, my vehicle has never been involved in any accident and has been consistently stored in a secure garage, ensuring that it remains free from external damage. Issue Description: Recently, I discovered that the glassroof of my Tesla Model Y has developed noticeable cracks.This issue is perplexing and unacceptable, especially considering the vehicle has not been subjected to any conditions that could cause such damage. The sudden appearance of these cracks suggests a potential manufacturing defect or poor assembly quality, both of which are serious concerns for vehicle safety and integrity. Previous Communication: Upon discovering the issue, I promptly contacted Tesla Service to report the problem and seek assistance. Unfortunately, my claim was denied without a satisfactory explanation or resolution. What component or system failed or malfunctioned, and is it available for inspection upon request? The "Actually Smart Summon" failed. It is available for inspection. How was your safety or the safety of others put at risk? The "Actually Smart Summon" function did not detect the post next to my vehicle as it was exiting the parking space. It immediately turn right into the post instead of pulling straight out first. I had very little reaction time. If there had been a person in the position of the post, this could have caused physical harm. My vehicle sustained damage to the front passenger door, its side mirror, and to the PPF material over these components. Has the problem been reproduced or confirmed by a dealer or independent service center? I don't know Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? When opening the feature on the app, it states: "Warning: Monitor vehicle's surroundings for people, animals, or unpredictable movement."

I purchased a 2024 TESLA model Y and received delivery of my Tesla vehicle on [XXX], at 4 PM EST. Upon receiving the vehicle, I noticed manufacturing defects on the tail lights, which I reported to Tesla Support immediately and scheduled an appointment on September 29. Unfortunately, no appointments were available at that time, and my service date was postponed to October 28, 2024. On [XXX], while traveling at low speeds, the rear passenger tire of my vehicle unexpectedly exploded. They are the factory-installed Continental tires 255/45/19. The car had to be towed to Tesla in Coral Gables, where they installed a temporary tire and quoted me $517.35 for replacement—despite the vehicle being less than a month old. Since delivery, I’ve also observed that my tail lights have accumulated water, indicating a potential safety hazard and manufacturing defect. I have contacted Tesla Support on multiple occasions, but they have not provided clear guidance on whether a loaner vehicle will be provided while these issues are addressed. The lack of timely and effective support for these escalating defects has been extremely frustrating and concerning. Primary Concerns: Safety hazards related to the tire explosion and water accumulation in the tail lights. Delays in service appointment scheduling. Lack of clarity and support from Tesla regarding service procedures, such as whether a loaner vehicle will be provided. Given these issues, I am requesting assistance from the NHTSA in addressing the quality and safety of this vehicle, as these defects and delays are affecting my ability to operate it safely and reliably. My tesla frunk has opened twice without warning or command on, [XXX] and [XXX]. This is after the 2024.20.3 update that was supposed to fix it. Jason from roadside assistance had me do a soft reboot and hopes it won't open in the future. Thankfully both incidents occurred in the parking lot. My Tesla (dark gray) was t-boned by a 2015 Hyundai (white) while it was passing the intersection on green light. The Hyundai hit my car when it tried to turn right on red light. My car was running at around 25 mph at that moment. About one second before the accident, my car's front collision warning went off, but everything was too late; the Hyundai's left front bumper hit my car. The crash did not cause any serious damage on the white Hyundai; as you can see in the photo, it barely got any dents, except its left front headlight cap was broken. The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.

While driving on highway at around 60,65 miles speed, I saw a warning that fronk is open. I dont think I pressed open fronk while driving and not sure even its possible while driving. Fronk wasnt open when I started the drive for sure else would have noticed the warning. Warning actually started after driving for 5,6 miles. I was on highway and took me some time to find a safe place to get on the shoulder and when I checked fronk was actually open. Im not sure how much an issue it was or if Tesla fronk can get loose and fly or hit the windscreen and cause visiblity issue or accident involving other vehilces but this was something which was a big concern for a moment while driving at high speed.
