The Full Self-Driving (FSD) system on my 2024 Tesla Model Y repeatedly causes the vehicle to weave or oscillate from side to side within its lane, particularly at highway speeds. The vehicle is available for inspection upon request. The most serious incidents have occurred at highway speeds while traveling in the left lane next to a concrete median. The vehicle has weaved so severely that it has nearly caused me to hit the concrete median. I attempted to steady the steering wheel with my hands, but this was not sufficient to stop the oscillation. I had to immediately disengage FSD to stop the behavior and regain stable control. This has happened more than once. The problem is independent of rain, wet pavement, poor weather, or unusual road conditions. I have experienced it in dry, clear weather and normal road conditions. There are no warning lamps or messages before the weaving begins. Tesla inspected the vehicle for this problem on July 22, 2026. Tesla Service found no physical defect and told me the behavior was a known software/firmware problem that could not be repaired at the service-center level. Tesla personnel indicated that similar behavior was known to be occurring in Europe. I was told that a future software update would correct the problem. Since then, Tesla has released multiple software/FSD updates and my vehicle has received subsequent updates. The weaving may have improved slightly on city streets and at lower speeds, but I have not observed meaningful improvement at highway speeds, where the greatest safety risk occurs. The weaving also creates a risk to surrounding traffic because the vehicle may appear to other motorists to be driven by an impaired or otherwise unsafe driver, potentially causing them to react to its unpredictable movement. Tesla has inspected and acknowledged the problem, but the high-speed safety issue remains unresolved and they do not expect any additional remedy from the dealer for my vehicle.

My 2024 Tesla Model Y (VIN [XXX]) has lost all forward camera-based safety and driver assistance functions. The vehicle displays alerts that Automatic Emergency Braking is unavailable, Park Assist is unavailable, adaptive headlight features are unavailable, and that self-driving is degrading and the vehicle may stop. Autopilot and Full Self-Driving will not engage. The side repeater cameras and the rearview camera still display images normally on the touchscreen, so the failure is not a loss of video display. The loss of Automatic Emergency Braking and forward collision warning means the vehicle has no automated protection against a frontal collision, and the loss of adaptive headlights and auto high beam means the high beams no longer dim for oncoming traffic. I performed a touchscreen reboot and a full power-off reset, and the condition returned. The vehicle is equipped with HW4 hardware and falls within the model years covered by NHTSA campaign 25V002000 (Tesla SB-25-00-001) concerning a computer circuit board short, though my VIN was not included in that recall population. I am reporting this because a vehicle sold with these active safety systems is now operating without them, and I believe other owners of vehicles in this build range may be experiencing the same failure. It has been towed to Tesla 5 times in the last 6 months. The car will all of a sudden do ALL of the following. Hood pops open Tailgate starts to go up and down The car lights start to flash - all of them The heater goes on 100 percent The seat and steering wheel warmers are activated The horn blows constantly The windows go up and down The stereo goes on full blast I cannot stop any of these things from happening. The contact stated that while driving at an undisclosed speed on the I-95 N to 175 W exit ramp, while approaching a traffic light in slow-moving traffic with the self-driving feature engaged, the vehicle failed to slow down as expected. To avoid a crash with the vehicle ahead, the contact veered to the right, leaving the roadway and traveling onto a slope and into the grass, where the vehicle struck a tree branch. There were no warning lights illuminated. A Police report was filed. There were no reported injuries or air bag deployment. The insurance company was notified of the issue. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified of the issue, and a case was opened. The failure mileage was approximately 19,891.

SEVERE DEFECT: Unprompted autonomous steering overrides manual driving. When Autopilot is OFF, Emergency Lane Departure Avoidance (ELDA) violently yanks the steering wheel, steering TOWARD hazards like oncoming traffic on the left or a mountain/rock wall past the curb on the right. This creates a severe collision risk, overriding safe manual input.DOCUMENTED PROOF: Provided Tesla Service (Palo Alto) over 12 timestamps and several dashcam footage. Technicians physically verified NO internal/external camera obstructions across TWO separate visits. Moving a transponder failed to resolve it.TESLA REFUSAL: Service Center has canceled TWO other service requests without fixing the defect, claiming it the 'system is working as intended.' Crucially, the Manager Jay explicitly admitted this dangerous steering override is a 'known issue for vehicles without FSD' operating on an older, separate software stack prone to these errors. To resolve this critical safety hazard, he explicitly suggested I purchase Tesla's premium Full Self-Driving (FSD) upgrade, claiming the FSD stack fixes it. Tesla is fully aware their base active safety software stack is defective and steers into hazards, yet demands a paid consumer upgrade to resolve it rather than issuing a safety patch or recall.

Car making excessive noise, service department refuses to address at Tesla even though various cases of the same issue are easily available online. Issue is with half shaft lubrication being dried but Tesla is trying to charge and claiming that the part hasn’t failed. But to me a part failing due to lack of lubrication can lead to hazard road conditions. I don’t want to put my kids safety in harm but Tesla tells me that no lubrication will not result in part failure

On XXX at approximately 7:23 a.m., a 2024 Tesla Model Y was traveling westbound on [XXX] in [XXX] in the middle lane at approximately 71 mph with Full Self-Driving (FSD) engaged. While traveling in traffic, the vehicle suddenly initiated a lane change toward the right lane without providing a warning or sufficient opportunity for the driver to respond. While the Tesla was executing the lane change, a vehicle traveling behind it collided with the left side of the Tesla. Following the impact, the driver manually took control and brought the Tesla to a complete stop on the right side of the highway. The system suspected of malfunctioning is Tesla’s FSD driver-assistance system, particularly its automated lane-selection and lane-change functions. The exact cause has not been determined. The vehicle and its electronic data were preserved. Currently, non-inspection was done. The unexpected maneuver placed the Tesla and surrounding motorists at risk because it occurred without warning at highway speed in active traffic and resulted in a collision. The driver had to intervene manually after the impact to control and stop the vehicle safely. At the time of this report, the suspected malfunction has not been reproduced or independently confirmed by a Tesla service center or independent repair facility. No technical determination of the cause has been received. A police officer arrived immediatly after the accident. An inspection was performed by Insurance company over a video. Before the unexpected lane change, there were no warning lamps, messages, audible alerts, or other symptoms indicating that he vehicle was about to initiate the maneuver or malfunction. While driving through normal rain with ordinary standing water on the roadway (NOT flood conditions), the rear bumper fascia detached from my Tesla Model Y. There was no impact, no collision, and no debris strike - there are no impact marks or scratches on the bumper. The fascia was pulled off by water drag, tearing a welded body mount bracket from the vehicle structure and damaging the underbody shield. This is a road safety hazard: a detached bumper fascia can drag on the roadway, separate completely, and become a large road obstacle for vehicles behind me. Tesla documented this exact failure mode in Service Bulletin SB-20-12-003 (rear fascia detachment when driving through standing water, with damage to harnesses and body fasteners/mounts), yet the defect persists in Model Y vehicles. Notably, another Tesla directly next to mine suffered the identical bumper detachment in the same rainfall at the same time - two identical failures in identical ordinary weather conditions, indicating a systemic design defect rather than driver error. Tesla Service denied warranty coverage, characterizing normal rain as 'flood water.'

I am reporting a safety‑critical issue with the automatic windshield wiper system on my 2024 Tesla Model Y. The vehicle uses a camera‑based “Auto Wiper” system, and it consistently fails to activate fast enough during heavy rain, spray from passing trucks, or sudden visibility loss. During moderate to heavy rain, the Auto wipers often stay on a slow or intermittent setting even when the windshield becomes fully covered with water. The system does not increase wiping speed quickly enough to maintain visibility. This results in several seconds where I cannot clearly see the road ahead. This problem is made worse because the Model Y makes it difficult to quickly switch to manual wiper control while driving on the freeway. The driver must look down at the touchscreen and navigate menus to adjust wiper speed, which is unsafe at highway speeds. The delay between losing visibility and being able to manually increase the wiper speed creates a dangerous situation. This behavior has occurred multiple times at highway speeds. When passing large trucks or encountering sudden water spray, the wipers do not react, and the windshield becomes momentarily opaque. This creates a serious risk of losing lane position or failing to see vehicles in front of me. This is a safety hazard because: 1.The Auto wipers do not clear the windshield when visibility is urgently needed. 2.Manual override requires taking eyes off the road to use the touchscreen. 3.The delay in response can lead to loss of visibility for several seconds. I am requesting that NHTSA investigate this issue, as it affects visibility and could lead to collisions, especially at highway speeds or during sudden heavy rain.

The automatic rain-sensing wipers on my 2024 Tesla Model Y fail to activate during rain from delivery date, leaving the windshield uncleared and forcing manual activation. To be clear, this feature does not work at. This happens consistently and creates a significant safety hazard due to impaired visibility while driving. Tesla uses a camera-based system rather than a dedicated rain sensor, and despite service visits and software updates, the issue persists. I believe this presents an undue safety risk that should be investigated.

• What the issue is: Intermittent suspension noise/clunking that suggests a possible failing component. • Why it’s a safety concern: Potential suspension failure could cause loss of control. • What Tesla did: Tesla refused warranty service related to a safety issue involving the suspension because they said they could not hear the noise, despite repeated reports. While driving, my vehicle released a loud pop and metal noise. After pulling over large bolts fell from the front of my car caused by a suspension issue. Tesla service later confirmed that the problem was due to improper torque in the suspension components, specifically the front lower links. This resulted in hardware failure, including bolts loosening or coming out of the suspension assembly. This is a serious safety concern, as it could have led to loss of control while driving. The vehicle required replacement of suspension components and hardware under warranty. This defect appears to be related to improper assembly or servicing rather than normal wear and tear. Tesla is now refusing to give me a detailed report of the manufactured defect cause.

At I-66 East and 495 Exit in Fairfax/Vienna, another car hit my Tesla car from the side at around 40-50 MPH. My car driver side had a heavy impact - Door scratched badly, rear window broke, rear tail lamp broke, rear tire got damaged, components on top of the front tire came off. Even the car was hit with this heavy force and had heavy damage, Airbags never deployed. Luckily THe driver didn't have any injuries as she was wearing a seat belt. How come the airbags never popped with this heavy impact accident? THe car that hit our car, didn't have as much damage as ours but their airbags deployed. Is this a defect with Tesla airbags?

On [XXX], while operating my 2024 Tesla Model Y in Full Self-Driving (Supervised) mode, the vehicle experienced an Uncommanded Steering Input leading to a collision. The vehicle was traveling at a very low speed of approximately 5 mph in a clear, simple environment. Despite this walking pace, the FSD software failed to identify a visible concrete curb and initiated a sudden, sharp turn directly into the obstacle without any prior warning or driver input. The fact that the system executed such a violent and erroneous maneuver at 5 mph indicates a fundamental failure in the Neural Network's object detection and path planning logic. If this logic failure had occurred at higher speeds or near pedestrians, the results could have been catastrophic. The local Tesla Service Center (Watertown, MA) refused to conduct a physical inspection or a log review of the steering torque. They issued a Repair Estimate of $1,217.26 while claiming 'no hardware fault codes' were present, thus ignoring the documented software malfunction. As a leaseholder of this vehicle with only 15,489 miles, I am reporting this as a significant safety defect that the manufacturer refuses to investigate. The contact owns a 2024 Tesla Model Y. The contact stated that while driving at various speeds, the TPMS displayed that the PSI of the tires was low. Additionally, a notification advising that the vehicle was experiencing low voltage and drainage and would shut down was displayed. The contact called towing services. The contact stated that the towing agent who assisted in towing the vehicle informed the contact that the tires were losing air, and the front passenger's side tire or wheel was defective. The vehicle was towed to a Tesla Service Center, where the power converter system(PCS) was corrected. Additionally, the technician informed the contact that the passenger's side tire was leaking air due to a rim defect, but the vehicle was not repaired, and the contact was advised to pick up the vehicle. The contact stated that the vehicle could not be driven at 20 MPH due to the failure. The contact had not retrieved the vehicle from the Service Center. The contact related the failure to NHTSA Campaign Number: 24V935000 (Tires), but the Tesla Service Center informed the contact that the recall repair was completed. The contact stated that NHTSA Campaign Numbers: 24V554000 (Latches/Locks/Linkages, Electrical System) and 24V935000 (Tires) were marked as complete on the Manufacturer's website; however, the NHTSA VIN tool displayed that the recalls were incomplete. Additionally, the contact stated that the driver's seat was not adjustable. The contact stated that while attempting to move the seat higher and forward, the seat turned inward towards the right. The vehicle was not repaired. The failure mileage was approximately 100,000.

SYSTEMIC POWERTRAIN SAFETY DEFECT: Catastrophic Loss of Propulsion & Failure of Dual-Motor Redundancy. 1. Incident & Safety Risk: While traveling in the far-left HOV lane of a 6-lane interstate highway, the vehicle experienced an uncommanded, unwarranted Automatic Emergency Braking (AEB) intervention during a controlled lane change. This sudden deceleration instantly triggered a complete powertrain collapse and total loss of propulsion. The vehicle came to a dead stop in a live highway lane. The driver (me), passenger, and our [XXX] infant were stranded in lane 3 of a 6 lane highway for ~35 minutes until Highway Patrol units arrived to manually push the vehicle across live traffic to the shoulder. This near-miss was an immediate, severe life-safety crisis not just for the occupants but also to others on the road. 2. Symptoms & Alert Codes: There were zero warning symptoms prior to the incident. Immediately upon the power collapse, the touchscreen displayed critical faults. Active diagnostic telemetry confirmed alert codes: DI_a137 (No Capable Drive Units) DI_a063 (System Graceful Power Off) 3. Manufacturer Confirmation & Inspection Status: The failure was confirmed in writing by the manufacturer’s service center. The diagnostic invoice explicitly states: "The rear drive inverter experienced an unintended reset due to an error. Sudden power reduction and shutdown while driving." The vehicle is currently held at the manufacturer's facility and is fully available for inspection. 4. Engineering Assessment: As a mechanical engineer, my assessment is the surge through the AEB event to the inverter’s DC-link was unable to damp this load, forcing a hardware logic reset. Crucially, the software logic failed to partition this fault. In a dual-motor architecture marketed as having built-in powertrain redundancy, a localized rear inverter reset should be isolated, leaving the front motor operational to safely navigate out of traffic, this did not happen. SAFETY DEFECT REPORT: Sudden Power Loss & Falsified Records 1. INCIDENT & 5-HR TOWING (XXX): On XXX at ~16:30, while executing a low-speed (<10 mph) left turn in Cambridge, MA, the vehicle had a sudden, total high-voltage failure mid-turn, losing all motive power and power steering without warning or error codes. Photos/videos were taken on-scene confirming ZERO physical impact or undercarriage collision. The dead vehicle stopped directly in the active lane, completely blocking traffic. With zero power, the driver and several helpful bystanders had to use extreme physical effort to manually push the dead car out of the road. AAA arrived at 18:00 but could NOT tow because total power loss completely locked all 4 wheels/brakes (no Tow Mode). AAA dispatched a 2nd specialized flatbed with dollies (arrived 21:00). Driver was stranded on-site for over 5 hours. Car reached Tesla Dedham Service at 22:00. On XXX at 10:00 AM, driver reported the driving power loss in person at Dedham, handed over key, and got a loaner. 2. WRITTEN NOTICE (XXX): On XXX, explicit written notice was sent to Tesla Service Manager Taso, re-confirming the exact mid-driving power loss hazard. 3. TESLA DIAGNOSIS & REPAIRS: Invoice #XXX confirms a severe high-voltage failure: "A hardware overcurrent event caused small high voltage battery brick imbalance and rear inverter failure." SYSTEMIC POWERTRAIN SAFETY DEFECT: Catastrophic Loss of Propulsion & Failure of Dual-Motor Redundancy. 1. Incident & Safety Risk: While traveling in the far-left HOV lane of a 6-lane interstate highway, the vehicle experienced an uncommanded, unwarranted Automatic Emergency Braking (AEB) intervention during a controlled lane change. This sudden deceleration instantly triggered a complete powertrain collapse and total loss of propulsion. The vehicle came to a dead stop in a live highway lane. The driver (me), passenger, and our [XXX] infant were stranded in lane 3 of a 6 lane highway for ~35 minutes until Highway Patrol units arrived to manually push the vehicle across live traffic to the shoulder. This near-miss was an immediate, severe life-safety crisis not just for the occupants but also to others on the road. 2. Symptoms & Alert Codes: There were zero warning symptoms prior to the incident. Immediately upon the power collapse, the touchscreen displayed critical faults. Active diagnostic telemetry confirmed alert codes: DI_a137 (No Capable Drive Units) DI_a063 (System Graceful Power Off) 3. Manufacturer Confirmation & Inspection Status: The failure was confirmed in writing by the manufacturer’s service center. The diagnostic invoice explicitly states: "The rear drive inverter experienced an unintended reset due to an error. Sudden power reduction and shutdown while driving." The vehicle is currently held at the manufacturer's facility and is fully available for inspection. 4. Engineering Assessment: As a mechanical engineer, my assessment is the surge through the AEB event to the inverter’s DC-link was unable to damp this load, forcing a hardware logic reset. Crucially, the software logic failed to partition this fault. In a dual-motor architecture marketed as having built-in powertrain redundancy, a localized rear inverter reset should be isolated, leaving the front motor operational to safely navigate out of traffic, this did not happen. I was driving the Tesla and coming up my circular drive(note I was driving uphill). The car suddenly accelerated and went out of control. I pumped the brake and took my foot off the accelerator but the car kept going and crashed into our house (sunroom) at end of the driveway at high speed > we went through the casement windows and brick foundation and would have gone through the other side window and down a hill and probably been killed if it were not for a heavy cast iron table that brought us to a stop. The airbags never opened.The car was totaled(the insurance company declared it a total lost and refused to repair and paid us a small amount , less than we paid for the car. This was the second repeat incident, the first one very similar was reported to NHTSA by my erhusband ,reported to NHTSA 0n 1/20/2025, 6.19 pm; complaint number 11637163 .after the first crash we tried to get Tesla to pick up the car . they refused. Instead they said they would check it remotely. We gave them the opportunity to inspect and repair the car between the first and second incident . They ignored us , do to their negligence we were almost killed. Please open an investigation. We have names and emails to show who we complained to.

SUDDEN ACCELERATION, I WAS DRIVING IN MY OWN DRIVEWAY AND A MODEST SPEED T15 MPH I HAVE A CIRCULAR DRIVEWAY AND I WAS ASCENDING UP THE CIRCLE WHEN THE CAR SUDDENLY ACCELERATED WITHOUT MY ACTION. i TOOK MYI FOOT OFF THE ACCELERATOR AND HIT THE BREAK BUT THE CAR DID NOT STOP OR SLOW DOWN. IT CONTINUED UP MY DRIVEWAY ON MY LAWN AND ACCROSS MY LAWN THROUGH A FENCE AND I WOUND UP IN MY NEIGHBORS LOT WITH CONSIDERABLE DAMAGE. I WAS SHAKEN BUT DID NOT SEEK MEDICAL ATTENTION AT THE TIME. MY NEIGHBOR AND MY WIFE ASSISTED. THE CAR WAS TOWED TO TESLA CONTRACTOR BODY SHOP -RECON. WHERE IT IS NOW. I HAVE REQUESTED THAT TESLA EVALUATE AND REPAIR THE CAR BEFORE I DRIVE IT AGAIN AND PICK IT UP FROM THE BODY SHOP. The contact owns a 2024 Tesla Model Y equipped with Continental Tires, Tire Line: Pro Contact, Tire Size: 255/45/R20, DOT Number: (N/A). The contact stated that several weeks after purchasing the vehicle, a neighbor informed the contact that a tire was flat. The contact attempted to pump air into the tire; however, the air immediately seeped out. Due to the failure, the vehicle was towed to an independent mechanic where the tire was replaced with the same brand tire. The contact stated that almost a month later, the tire pressure indicator light appeared on the infotainment screen while the contact's wife was driving the vehicle. The contact was called to the scene and attempted to inflate the rear passenger tire; however, the air seeped out of the tire. Upon inspection, the contact discovered multiple cracks along the inner sidewall which caused the tire to peel. The vehicle was towed back to the independent mechanic where the vehicle remained. The vehicle manufacturer was notified of the failure and referred the contact to the tire manufacturer. The tire manufacturer was notified and informed the contact that a tire representative would need to inspect the tire. The vehicle was not repaired. The tire and vehicle failure mileage was approximately 550.
