The contact stated that while driving at an undisclosed speed on the I-95 N to 175 W exit ramp, while approaching a traffic light in slow-moving traffic with the self-driving feature engaged, the vehicle failed to slow down as expected. To avoid a crash with the vehicle ahead, the contact veered to the right, leaving the roadway and traveling onto a slope and into the grass, where the vehicle struck a tree branch. There were no warning lights illuminated. A Police report was filed. There were no reported injuries or air bag deployment. The insurance company was notified of the issue. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified of the issue, and a case was opened. The failure mileage was approximately 19,891.

SAFETY DEFECT REPORT: Sudden Power Loss & Falsified Records 1. INCIDENT & 5-HR TOWING (XXX): On XXX at ~16:30, while executing a low-speed (<10 mph) left turn in Cambridge, MA, the vehicle had a sudden, total high-voltage failure mid-turn, losing all motive power and power steering without warning or error codes. Photos/videos were taken on-scene confirming ZERO physical impact or undercarriage collision. The dead vehicle stopped directly in the active lane, completely blocking traffic. With zero power, the driver and several helpful bystanders had to use extreme physical effort to manually push the dead car out of the road. AAA arrived at 18:00 but could NOT tow because total power loss completely locked all 4 wheels/brakes (no Tow Mode). AAA dispatched a 2nd specialized flatbed with dollies (arrived 21:00). Driver was stranded on-site for over 5 hours. Car reached Tesla Dedham Service at 22:00. On XXX at 10:00 AM, driver reported the driving power loss in person at Dedham, handed over key, and got a loaner. 2. WRITTEN NOTICE (XXX): On XXX, explicit written notice was sent to Tesla Service Manager Taso, re-confirming the exact mid-driving power loss hazard. 3. TESLA DIAGNOSIS & REPAIRS: Invoice #XXX confirms a severe high-voltage failure: "A hardware overcurrent event caused small high voltage battery brick imbalance and rear inverter failure." Tesla replaced Rear Drive Unit Inverter (3DU) and Pyrotechnic Battery Disconnect (Pyrofuse). 4. FALSIFICATION OF SAFETY ISSUE: Despite 5-hr towing from wheel lock-up, XXX in-person report, and XXX written notice detailing mid-turn power loss, Tesla falsely recorded on invoice: "customer states: vehicle will not charge." On July 10, 2026, my 2024 Tesla Model Y (approx. 22,440 miles) lost all power (main/12V battery) while parked next to a Supercharger stall on Level 3 of the Harrah's Ak-Chin Casino parking garage, Maricopa, AZ. Ambient temperature was approximately 110F. My dog and cat were inside the vehicle. With the vehicle powered off, the key card, Tesla app, and interior door releases did not function, leaving no way to open the doors from outside or inside. Unable to find or access a working manual door release in time, we broke the rear quarter glass and damaged a door to physically extract the pets before they suffered heat-related harm. Maricopa Police Department and Ak-Chin Fire Department responded to the scene. This represents a serious safety defect: a loss of vehicle power should not eliminate all means of entry/egress, particularly when children or pets could be trapped in extreme heat. Component or system that failed: The GPS navigation system and cellular connectivity module in my 2024 Tesla Model Y (VIN: [XXX] ) have intermittently lost signal and become completely nonfunctional multiple times over the past month. Tesla's service documentation indicates this affects the vehicle's connectivity hardware or managing software. The vehicle is available for inspection upon request. Safety risk: Loss of GPS navigation while driving creates a safety hazard when traveling unfamiliar routes, in low-visibility conditions, or during emergencies. Loss of cellular connectivity simultaneously disables Tesla's roadside assistance and the vehicle's calling capability. The vehicle acknowledges this risk by displaying a yellow caution alert reading "Poor GPS location accuracy — Additional attention may be required" on the primary touchscreen while in motion, with no advance warning before the failure occurs. Reproduced or confirmed by a dealer or service center: Yes. On May 22, 2026, Tesla Service Columbia-Two Notch Rd, 6301 Two Notch Road, Columbia, SC 29223, confirmed this as a known issue. The service advisor stated Tesla is aware of the defect but has no repair available and instructed me to submit in-car bug reports each time it occurs. Inspected by manufacturer or others: Yes, by Tesla's service center on May 22, 2026. No independent, police, or insurance inspection has been performed. Warning lamps or prior symptoms: A yellow triangle caution icon with the message "Poor GPS location accuracy — Additional attention may be required" appears on the primary touchscreen during each failure, but only after GPS has already degraded — no advance warning is given. On December 2, 2025, my Tesla was charging in the garage of my residence in [XXX] , when a fire appears to have originated in or around the vehicle area. Before the fire, I smelled smoke/burning and also heard popping sounds. The fire spread rapidly and resulted in the destruction of my home, creating an immediate and severe risk to the safety of all occupants and anyone nearby. This incident exposed those present to danger from flames, smoke inhalation, toxic fumes, and structural collapse, and it caused emergency evacuation and displacement. The fire department identified the garage or vehicle area as the origin. The exact component or system that failed is unknown at this time, but the circumstances indicate a possible failure involving the vehicle, charging system, battery system, or related electrical components. I am not aware of any prior battery, charging, or electrical warning messages before the incident. Emergency responders and insurance representatives were involved after the fire. I previously attempted to notify Tesla of this incident, but I did not receive a response. Because this event involved a catastrophic fire and total residential loss, I believe it should be reviewed as a serious vehicle safety matter and that all relevant vehicle data, charging data, components, and related evidence should be preserved for inspection. Driver side front window auto rolled back up after pressing the button to once to roll it down. This happened while driving while the right hand was on the steering wheel. The left hand got caught inbetween the window glass and the window frame as it auto rolled back up on its own. It caused bleeding and pain to the left hand.

Center console USB component part#1522264-00-B, available for inspection. USB-C charging port that had been plugged in for more than a year, started smoking and wire melted while vehicle was parked. If this would have happened while my wife was on the highway on the way to work and smoke suddenly appeared, it would have cause her to be startled and degrade her ability on a busy highway. This would have most likely have created a traffic incident in Memphis on highway 240. I asked the dealer if there were any reports of defective part and I was told no. I brought the components in to show but the parts manager or service staff were not interested in the details or my concerns that smoke had been detected and that it melded the metal piece of USB-C charger to that TESLA part. I bought another replacement part from TESLA, but I am not utilizing the charging port for fear of smoke/fire. There were no warning lights or indication prior to the day of event where my wife observed burnt plastics and visible smoke. On November 26th called me in a panic that the car was smoking. She went out to our vehicle during her break and she smelt burnt plastic and observed smoke from the middle console. Shortly after she called me at my work, and after making sense of what she was telling me, I told her to unplug the USB-C but the parts manager melted of and the metal piece was stuck in the charging port. I left my work and after 30 minutes got to the vehicle, the burnt plastic was still evident but smoke no longer visible. I detached the panel with the part unplugged from power source, thank you YouTube for the DIY video. Pictures will be included with this report, but format doesn’t allow for video upload. The contact owns a 2024 Tesla Model Y. The contact stated that while driving at various speeds, the TPMS displayed that the PSI of the tires was low. Additionally, a notification advising that the vehicle was experiencing low voltage and drainage and would shut down was displayed. The contact called towing services. The contact stated that the towing agent who assisted in towing the vehicle informed the contact that the tires were losing air, and the front passenger's side tire or wheel was defective. The vehicle was towed to a Tesla Service Center, where the power converter system(PCS) was corrected. Additionally, the technician informed the contact that the passenger's side tire was leaking air due to a rim defect, but the vehicle was not repaired, and the contact was advised to pick up the vehicle. The contact stated that the vehicle could not be driven at 20 MPH due to the failure. The contact had not retrieved the vehicle from the Service Center. The contact related the failure to NHTSA Campaign Number: 24V935000 (Tires), but the Tesla Service Center informed the contact that the recall repair was completed. The contact stated that NHTSA Campaign Numbers: 24V554000 (Latches/Locks/Linkages, Electrical System) and 24V935000 (Tires) were marked as complete on the Manufacturer's website; however, the NHTSA VIN tool displayed that the recalls were incomplete. Additionally, the contact stated that the driver's seat was not adjustable. The contact stated that while attempting to move the seat higher and forward, the seat turned inward towards the right. Incident summary (Sanford, FL – Appaloosa Court – Dec 2, 2025): In the early morning on Tuesday, December 2, 2025 (around 5:00–5:15 a.m.), a fire occurred at a residence located on [XXX] . A Tesla located in the garage was reported as being involved, and the fire spread to the garage that totaled the house The vehicle was reportedly charging in the garage when smoke/fire was noticed. Emergency responders (Sanford Fire Department and Sanford Police) responded to the scene. The incident resulted in property damage and at least one injury reported as non-life-threatening (per local reporting). Fire forensics stated that the fire started and caused by the TESLA around the charging plug area by the rear left passenger door. I am writing to formally report a serious malfunction involving my Tesla vehicle. On my 2024 Tesla model y.I experienced an unexpected accelerator malfunction that caused the vehicle to surge forward uncontrollably. The incident occurred at approximately As I attempted to dive in slowly, the accelerator suddenly malfunctioned and my car creeped in slowly and rapidly increased power without my intention. Despite immediate attempts to brake and regain control, the vehicle continued to accelerate and subsequently collided with a gate, resulting in significant front-end damage. I firmly believe this was due to a mechanical or electronic failure in the vehicle’s accelerator or related control systems. I am writing to formally raise a complaint regarding a serious malfunction that occurred in my Tesla Model Y (2024) following a recent software update pushed by Tesla. A couple of days ago, my vehicle received a new over-the-air software update. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for May 21, 2025, at 2:15 PM. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: The problem was introduced by Tesla's software, The malfunction affects critical safety systems, and There is imminent risk to safety, I believe Tesla should bear responsibility for the repair and ensure the vehicle is safe to operate. This situation warrants urgent attention, both from a consumer protection and vehicle safety standpoint. I request that Tesla take full accountability and cover the repair costs immediately, or offer a rollback/fix to the software that caused the issue. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic.
