2023 Toyota Tundra Driver assistance Problems

Owner-reported complaints naming the driver-assistance system, filed with NHTSA

Data refreshed

Driver assistance reportsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

Multiple vehicle systems have malfunctioned, including the transmission, infotainment/display screen, and backup camera. Most recently, the backup camera has stopped functioning, preventing a rear view while reversing. The vehicle is available for inspection upon request, along with any service records documenting prior repair attempts. The transmission issues created an unsafe driving condition due to unpredictable shifting and hesitation. In addition, the backup camera is no longer functioning, significantly reducing rear visibility while reversing and increasing the risk of striking a pedestrian, child, vehicle, or object. These defects pose a safety risk to both the driver and others around the vehicle. The vehicle has been presented to an authorized Toyota dealership on multiple occasions for repair of various issues. Most recently, I sought service for the inoperative backup camera but was refused service. Previous concerns have been documented through dealership service visits. The vehicle has been inspected by an authorized Toyota dealership during previous repair attempts. It has not been inspected by police or insurance representatives. The vehicle experienced ongoing drivability and infotainment issues before the backup camera failed. The backup camera eventually stopped functioning entirely.

Forward facing Camera's: After vehicle was recalled for a refacing camera issue and repaired for that recall, now the forward facing camera does not work properly. When coming to stop or while stopped the forward facing camera's no longer work. They only come on when I get close to something. They also no longer read speed limit signs. When stopped at a cross walk if a child runs in front of the vehicle I do not get a warning until the child is directly in front of the vehicle. This could cause me to hit a child based on the height of the child and being blocked by the hood of the vehicle. I need the forward camera's to activate at 5 miles per hour or slower and stay on when stopped until going over 5 miles per hour like they did before the backup camara recall was installed on my truck. This is an on going issue starting June 18,2026. I took it to the dealership July 14,2026 and they did not correct the issue. This issue could case a child to be run over!

was waiting for the traffic light to turn green. Vehicle in drive with foot on brake. the traffic light turned green, I took my foot off the brakes and pushed on the accelerator. Nothin happened. I saw another vehicle approaching at a high rate of speed behind me so I continued to mash on the accelerator. right before being hit by the car behind me my vehicle lurched forward a few feet then stopped again. I looked at the dash display and saw a "Hybrid System Malfunction" warning. Because of the traffic situation/location it was not safe to just sit in the road so I continued to pump on the accelerator. each pumped moved the vehicle forward a couple of feet. I did not turn of the engine because I was unsure what would happen so I continued to do that for about a mile or so until I could get to a safe place to pull over. during this time, I almost got rear-ended and side-swiped several times. Once stopped I attempted to start the vehicle several times and got the "Hybrid System Malfunction" display and the vehicle would not start. I waited for about an hour with the vehicle off and restarted the engine. I stayed off the freeway and took backroads to the closest dealer service center where it is currently located. When I dropped it off the service rep said he hasn't seen anything like this. Prior to this incident I had no issues with the truck and it was last serviced by the dealership on March 4, 2026.

While driving on a two-lane highway at approximately 80–85 mph and attempting to pass another vehicle, the engine suddenly shut off without warning. The vehicle lost all propulsion, and steering assist was significantly reduced, creating a dangerous situation and nearly resulting in a head-on collision with oncoming traffic. Warning lights including check engine and low power steering illuminated immediately after the stall. The vehicle would not restart for several minutes and was temporarily disabled in the roadway. The vehicle was later inspected by a dealership, which confirmed through diagnostic data that multiple engine stall events occurred at highway speeds. The inspection also identified system-wide electrical voltage drops and multiple communication faults between control modules. The dealership attributed the issue to a loose battery terminal; however, the diagnostic report indicates prior voltage irregularities and multiple affected systems, suggesting a broader failure condition. This incident is consistent with an active safety recall affecting this vehicle, which states that the engine may stall during driving. The failure has not been reproduced since the initial incident, but the vehicle has documented evidence of multiple stall events and remains a safety concern. The vehicle is available for inspection upon request. Due to the sudden loss of power at highway speed and the inability to safely control or accelerate the vehicle, this issue poses a serious risk to the driver and others on the road.

Throttle response is unpredictable. From a stop sign, acceleration is difficult to predict. Sometimes a one second delay occures before acceleration power is effected. With the time delay the throttle has been pressed further than normal. In heavy traffic, a collision could be possible. The problem got worse after a firmware update, during a safety recall to replace rearview touch screen. Pedal Lag problem is noted on Toyota Tundra forum.

My reverse stopped working, it just shows a black screen when I switch to it. The rest of my entertainment console works, it's only when I put it in reverse that it goes black. I saw that there was a recall issued for 2024 and 2025 Toyota Tundras, but I believe that the 2023 should be included as well. It looks to be a common issue, as I saw 2023 Tundras with a higher trim package also had a recall for the backup camera.

NHTSA SAFETY RECALL DELAY COMPLAINT Complainant: [XXX] State: Illinois Vehicle: •Year/Make/Model: 2023 Toyota Tundra SR5 Crew Max •VIN: [XXX] •Current Mileage: approx. 16,000 miles Recall Number: Toyota Safety Recall 25TB06 Dealer Holding Vehicle: Romeoville Toyota 684 S. Weber Road Romeoville, IL 60446 ________________________________________ Description of the Safety Issue and Delay I am filing this complaint to report an excessive and unreasonable delay in remedying a safety-related recall on my vehicle. My 2023 Toyota Tundra SR5 Crew Max has been out of service at an authorized Toyota dealership (Romeoville Toyota) since June 18, 2025 due to Toyota Safety Recall 25TB06. As of today, the vehicle has remained unrepaired for 224+ days with no confirmed completion date. Despite repeated inquiries, neither the dealership nor Toyota has provided a definitive timeline for completing the recall repair. While a rental vehicle has been provided, I remain without access to my own truck and have experienced an extended loss of use far exceeding what would be considered reasonable for a safety recall remedy. A manufacturer safety recall is intended to correct a defect within a reasonable period of time. A delay exceeding seven months leaves the consumer without their vehicle and raises concerns about Toyota’s ability to timely and adequately remedy safety defects once a vehicle is removed from service. This prolonged delay undermines the purpose of a safety recall and places an unfair burden on the consumer. I am requesting that NHTSA review this matter and intervene to ensure Toyota Motor North America provides a timely and appropriate remedy, including repair completion or alternative relief. Unknown. Intermittently when merging into highway traffic from the onramp, the truck will stall and I will lose acceleration. There will be a loss in power and the truck speed will not change. instead it will just coast with no ability to change speed. I have owned the truck for 1 year and 3months. I advised the dealer and they can not duplicate the issue.

There have been multiple times, since owning the vehicle, when there is dangerous throttle lag. I step on the gas to turn and nothing happens for a few seconds. It can be pretty scary when there is a car coming and the truck just rolls for a few minutes before accelerating. It’s become a major safety concern for my family.

While pulling out into traffic, my truck will suddenly hesitate. It also caused a vehicle to hit me as the truck pulled out into the lane, then suddenly completely lost power. Dealer was unable to replicate. Has happened to me several times when at a stop light. This is slightly safer as traffic behind me is stopped or slowed. It feels like a throttle issue as it completely looses power

I get random lags in thorttle engagement when starting from complete or rolling stops. When this happens, it behaves like there is a delay between the throttle input and the engine resoponse. Push the throttle to go, and no go, and after a delay of usually part of a second, then it jerks and goes. Most times there is no problem and the throttle responds correctly, but this issue occurs randomly almost every time I drive.

The contact owns a 2023 Toyota Tundra. The contact received notifications of NHTSA Campaign Numbers: 25V657000 (Back Over Prevention), 25V322000 (Exterior Lighting), and 25V767000 (Engine and Engine Cooling); however, parts to do the recall repairs were unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The contact stated that while driving 55 MPH in stop-and-go traffic, the engine failed to operate as needed, and the contact was able to coast into a nearby parking lot, where the vehicle became immobile. There were no warning lights illuminated. The vehicle was towed to the dealer, but was not diagnosed. The vehicle was not repaired. The manufacturer was informed of the failure and confirmed and not received a report from the dealer. A case was opened with the manufacturer. The failure mileage was 34,736.

The truck “detected” a warning collision when there was no one in front of me which made the engine lag. There for causing me to come to an abrupt stop from 40 mph. I stepped on the gas to get back up to speed with traffic beside and behind me and there was no power almost causing an accident. This is the 3rd time this truck has done this in 6 months.

Tundra has had and has throttle lag. Lag is so noticeable that other drivers notice. Following lag is a burst of power causing a jerk and or lunge forward. This is after a long pause. Happens on a roll, or start-stop situation. My truck has had the toyota tsb applied a long time ago.

I experience delayed throttle response and throttle lag daily. Acceleration is unpredictable. The vehicle barely moves in some cases when pressing the gas pedal. Other times, the tires will chirp. The vehicle will sometimes hesitate so long to accelerate, that I've almost been T-boned and I've held up traffic. It seems like the truck thinks I'm going to run into something, even when there's nothing in front of me. The delay is sometimes 3-5 seconds before it will allow any acceleration. This seems like a huge safety issue, not being able to get out of the way of oncoming traffic and having such an unpredictable, and often delayed throttle response.

Over the course of ownership I have had frequent instances of loss of power and/or lack of acceleration resulting in almost stalling out while trying to take off. This causes dangerous situations when trying to pull out into traffic because you never know when the vehicle might hesitate and crest unsafe distances between you and other moving vehicles.

My 2023 Toyota Tundra has an intermittent issue where the throttle lags or hesitates when accelerating from a complete stop. The problem occurs unpredictably and causes the truck to fail to respond when pressing the gas pedal. The issue appears to be related to the engine or throttle response system, and the vehicle is available for inspection upon request. This malfunction puts my safety and the safety of others at risk because the hesitation can occur while turning left across traffic or merging into traffic, leaving the truck momentarily stalled in the path of oncoming vehicles. The problem has not yet been officially reproduced or confirmed by a dealer or service center, but it has happened multiple times under normal driving conditions. The vehicle has not been inspected by the manufacturer, police, or insurance representatives at this time. There were no warning lights, messages, or other indicators before or during the hesitation; it happens without any prior notice.
