Multiple vehicle systems have malfunctioned, including the transmission, infotainment/display screen, and backup camera. Most recently, the backup camera has stopped functioning, preventing a rear view while reversing. The vehicle is available for inspection upon request, along with any service records documenting prior repair attempts.The transmission issues created an unsafe driving condition due to unpredictable shifting and hesitation. In addition, the backup camera is no longer functioning, significantly reducing rear visibility while reversing and increasing the risk of striking a pedestrian, child, vehicle, or object. These defects pose a safety risk to both the driver and others around the vehicle.The vehicle has been presented to an authorized Toyota dealership on multiple occasions for repair of various issues. Most recently, I sought service for the inoperative backup camera but was refused service. Previous concerns have been documented through dealership service visits.The vehicle has been inspected by an authorized Toyota dealership during previous repair attempts. It has not been inspected by police or insurance representatives.The vehicle experienced ongoing drivability and infotainment issues before the backup camera failed. The backup camera eventually stopped functioning entirely.

While driving the vehicle on a public roadway at approximately 70mph, the vehicle suffered a sudden and catastrophic powertrain failure without timely warning. The check engine light illuminated, the physical engine oil pressure gauge dropped to absolute zero, and the dashboard immediately flashed a critical "Hybrid System Malfunction" warning. > This mechanical and electrical failure triggered an instantaneous, total loss of motor power while traveling live lane with traffic behind me. The vehicle immediately stalled and became a completely inoperable, unresponsive rolling hazard. The vehicle failed to restart and left me stranded on a narrow shoulder which was very hazardous as traffic coming at high speeds could have easily rear ended me. The vehicle required a flatbed tow to an authorized Toyota dealership, and after submitting a case with corporate hoping for a trade assist(as my wife no longer feels safe in this chassis), I have been told it will take up to 45 days to process this request. I made a significant investment in this vehicle in January of 2023 and have since methodically kept up with maintenance, with hopes this vehicle would last me 10+ years. I find it highly inexcusable for Toyota to not consider speedily helping a loyal customer in this situation and making it right in a timely manner. I am now being offered a vehicle the same as mine of the current year and with me having to put over 22K in equity to close the gap. I should not be left on the hook for something that put my safety at risk and now has left me without the ability to travel as my family has lost their main method of transportation. Manufacturing defects should not come at the customers expense.

The 2023 Toyota Tundra Hybrid’s powertrain design may be insufficient to maintain basic safety maneuvers (specifically "limp mode" or emergency clearing) during an engine failure while operating under high-load conditions in mountainous terrain. This creates a life-threatening hazard for the occupants and other motorists. Statement of Fact: As an owner of a 2023 Toyota Tundra Hybrid, I primarily utilize this vehicle to tow a 9,000 lbs travel trailer through the high-altitude, steep-grade regions of Utah. These routes frequently consist of narrow, two-lane mountain passes with no shoulders and steep drop-offs. Failure Under Stress: Engineering logic suggests that engine stress is highest during heavy towing on steep grades. If a primary engine failure occurs in these conditions, the hybrid system’s battery and electric motor capacity are, in my experience, inadequate to move a combined Gross Combined Vehicle Weight (GCVW) of approximately 15,000+ lbs to a safe location. While the hybrid system may allow a light, unladen vehicle to limp to a shoulder under "ideal conditions," it lacks the torque and sustained power to move a heavy trailer on a grade once the internal combustion engine is compromised. In the event of a failure, the vehicle would likely become "dead in the water" in the middle of a live, narrow lane on a blind mountain curve. Without the ability to reach a turnout or even a marginal shoulder due to the weight of the load, the vehicle becomes a stationary target for other high-speed mountain traffic, posing a severe risk of rear-end collisions and multi-car accidents. I am requesting that the NHTSA investigate the performance of the Tundra Hybrid’s "fail-safe" modes specifically under maximum towing capacity on grades.

While operating my 2023 Toyota Tundra TRD Pro under normal conditions, I started the vehicle and activated standard factory equipment including the seat heaters and window/mirror defrosters. Shortly thereafter, I noticed a slight acrid odor inside the vehicle, which quickly became more pronounced. I drove approximately 200 feet, and immediately returned to my driveway. Upon shutting the vehicle down, I observed smoke coming from the engine compartment. I opened the hood and confirmed smoke and signs of an electrical fire originating in the engine bay. I used a fire extinguisher to extinguish the fire and then disconnected the vehicle’s battery. No injuries occurred. The vehicle was not restarted and was subsequently towed to an authorized Toyota dealership for inspection. Visible damage is present in the engine compartment, including the main fuse/relay box and associated electrical components. The incident occurred without collision and during normal vehicle operation.

2023 Toyota Tundra experienced repeated engine stalling and vehicle lock-up while driving on public roads, including in an intersection. During one incident, the driver was thrown forward into the steering wheel. A Toyota dealer attempted repair by replacing the neutral/park safety switch, but the repair failed and the dangerous condition persisted immediately after. The vehicle has been removed from use due to safety concerns. The issue is consistent with known engine stalling concerns affecting certain 2022–2024 Toyota Tundra vehicles. The vehicle is unsafe to operate.

While driving today the engine check light turned on, and the engine turned off (stalled) while in the middle of the road with a good amount of traffic around me. I pulled off to the side of the road and gave the engine a minute. I turned the truck back on again, the engine sounded very rough turning on and rough while driving. After a minute or two of driving, the engine light turned on again, and the engine stopped (stalled) again while in the middle of the road. I stopped this time for two minutes or so and turned the engine on again. I then proceeded to try to make it home, which I accomplished, but the truck's engine sounded rough and with a chattering sound (engine knocking) even when idling. I have received a recall notice letter concerning " Certain 2022-2024 Model Year Tundra Vehicles Engine May Stall During Driving." I possess the letter with the NHTSA Recall Number and the Toyota Recall Number. In the letter, it mentions the condition for which it is being recalled. The conditions are as stated in the letter. "Machining debris may not have been cleared from the engine when it was produced. In the involved vehicles, this can lead to engine knocking, engine rough running, engine no start, and/or a loss of drive power. A loss of drive power while driving can increase the risk of a crash." My truck has experienced all of the disfunctions that are stated in the recall letter. I would like for Toyota to look into this and remedy this problem before it ends in a major fatal crash or any other unfortunate circumstance. Thank you!

This is ridiculous if there’s debris in the engine of a risk for it and risk for the engine to go I want a new vehicle there’s 18k miles on this truck brand new shape and I will be putting my family in this. This is a risk I’m not willing to take. This needs to be bought back

The engine of my 2023 Toyota Tundra failed at approximately 34,500 miles. The failure involved sudden loss of oil pressure and severe internal damage, with metal shavings found in the oil and oil filter. The oil filter itself detached during the incident and was later recovered with metallic debris inside. The vehicle and the recovered filter are available for inspection upon request. While accelerating from a stop light on a busy morning public road, the engine suddenly lost power, illuminated multiple warning lamps, and entered limp mode with “low oil pressure” and “engine malfunction” messages. The truck rapidly leaked oil, creating a roadway hazard and putting me, my wife & son, and other drivers at risk of accident. I was able to limp the vehicle home .5 miles from the incident at reduced speed, but complete engine seizure was a possibility. Toyota of Fort Walton Beach inspected the vehicle, confirmed metal shavings in the oil pan, documented codes P05202 and P052049 (low oil pressure and oil pressure sensor stuck in range), and noted “oil was very metallic.” The vehicle was inspected by Toyota dealership technicians and corporate representatives. I also have possession of the oil filter and photos/videos from the inspection. At the moment of failure, the dash displayed low oil pressure, check engine light, and a message regarding engine/electronic malfunction. Prior to this event, there were no warning signs — the truck had been serviced two weeks earlier at 33,360 miles, and I drove approximately 1,200 miles without incident until the catastrophic failure. This failure matches the description in Toyota Recall 24V-381. My vehicle’s build date (February 27, 2023) is less than two weeks outside the published recall production window, yet the symptoms and inspection results are identical. I respectfully request NHTSA investigate whether additional 2023 Toyota Tundra vehicles outside the current recall window are affected, as my case demonstrates the same hazards.

Whenever it rains in DFW, vehicle emits a foul/musty odor from the HVAC system. Only happens when it rains. HVAC setting has always been the same, "Lo" temp, AC Mode ON, Recirculation mode ON, fan speed varies. Taken vehicle to two different dealerships and techs unable to duplicate. It doesnt rain much here in TX so it is hard to get it taken care of while still within warranty. My family and I use this vehicle primarily and I am concerned for my kids' lung health as they are [XXX], [XXX], and [XXX] years old as of writing. Hesitation when trying to accelerate. When I start my truck in the morning And hit the gas there’s a hesitation before it will move and When I come to a stop sign and hit the gas there’s always a hesitation. When the truck finally starts to move it jumps quickly forward from pressing down on the gas pedal and it’s almost caused me to rear end someone multiple times.

On February 25, 2025, I took my 2023 Toyota Tundra VIN# [XXX] to Stokes Toyota Beaufort, SC, for its 45,000-mile service. Just 300 meters after leaving the dealership, my truck caught fire, causing significant damage and putting my safety at risk. I am sharing pictures of the incident and the warning messages I received on the dashboard. I was on my way back to the dealership when this happened. At first, the truck was shaking and losing power. I managed to make a U-turn to return to the dealership, but then the engine stalled. At that moment, I started seeing warning messages on the dashboard. I immediately called the dealership to inform them that I was experiencing issues with my truck. While still on the phone with the dealership, I noticed smoke coming from the passenger side of the hood. Moments later, flames started emerging from the hood. I quickly hung up and called 911. 1- What component or system failed or malfunctioned? I believe the engine and possibly the brakes failed (based on the dashboard warning messages). 2- How was your safety or the safety of others put at risk? Within minutes, my truck burned completely. I am grateful I didn't have my children with me at the time. 3- Has the problem been reproduced or confirmed by a dealer or independent service center? I received formal letters from Toyota Manufacturer, Toyota Distributor, and the dealership, stating that they are sorry for what happened but cannot offer any assistance with my claim. 4- Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Yes, the insurance company, fire marshal, dealership, Toyota, and Engineering Analysis Associates (EAA) have all inspected the vehicle. 5- Were there any warning lamps, messages, or other symptoms of the problem prior to the failure? Yes, the truck was shaking, losing power, and eventually stopped completely, displaying warning messages on the dashboard. Fuel gage will randomly reset itself to full when the tank quantity is less than full or fuel gage will not reset to full after the gas tank is filled. A manual reset of the computer is required to force the gage to reset to an accurate reading. Driver is at risk of running out of fuel due to the gage overstating the quantity of fuel remaining. Numerous reports of this situation are found on youtube along with instructions on how to manually reset the software to force the gage to reset. Gage has malfunctioned 4 times in less than two months. Dealer could not find an issue with the system and requires the vehicle to be brought in during the malfunction. Issue started around 8000 miles and 22 months after initial delivery to owner.

Notified of safety recall for this in August of 2023, to date, 10/10/2024, the fix has not been installed or available for my vehicle. Initially notified of a recall for the same issue on April 28th, 2023. I was under the impression this had been remedied during one of the services performed between April and August of 2023, with the dealer telling me the August 2023 recall was a different recall for the same part as the remedied part was recalled as well.

The contact owns a 2023 Toyota Tundra. The contact stated while driving 45 MPH, there was electrical smoke emerging from the starter. The low power and battery warning lights illuminated. The vehicle was taken to the local dealer, where it was diagnosed that the starter needed to be replaced. While driving, there was a loud clucking sound detected, and the engine seized. The contact stated that the failure was related to NHTSA Campaign Number: 24V381000 (Engine and Engine Cooling). The vehicle was towed to the local dealer, where it was diagnosed and determined that the engine needed to be replaced. The vehicle was not repaired. The manufacturer was contacted, opened a case, and referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 34,000. The VIN was not available.
