Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

DELAYED TRANSMISSION ENGAGEMENT
MS. CLAIRE M. TIEDER OF RESTON, VA, SUBMITTED A DEFECT PETITION TO THE NATIONAL HIGHWAY TRAFFIC SAFETY ADMINISTRATION (NHTSA) BY LETTER DATED JANUARY 11, 2004, REQUESTING A DEFECT INVESTIGATION BE CONDUCTED INTO DELAYED TRANSMISSION ENGAGEMENT ON MODEL YEAR 2004 BMW XI VEHICLES.BMW'S RESPONSE TO NHTSA'S INFORMATION REQUEST INDICATED THAT CERTAIN BMW 3-SERIES VEHICLES EQUIPPED WITH THE GM5 TRANSMISSION MAY EXHIBITED A FORWARD 'PARK' TO 'DRIVE' ENGAGEMENT DELAY FROM2 TO 30 SECONDS.HOWEVER, THE CONDITION CAN OCCUR ONLY DURING A VEHICLE COLD START, AFTER THE VEHICLE HAS BEEN AT REST FOR MORE THAN EIGHT HOURS.DRIVERS EXPERIENCING THE DELAY HAVE LEARNED TO TAKE PRECAUTIONARY AND COMPENSATORY MEASURES.BWM WILL REPLACE AFFECTED TRANSMISSIONS EXHIBITING THE DELAY, AFTER THE CONDITION HAS BEEN VERIFIED, WITH AN IMPROVED UNIT.NO ALLEGED CRASHES, INJURIES, OR FATALITIES HAVE BEEN REPORTED TO EITHER BMW OR NHTSA RELATING TO THIS ISSUE.BASED ON ANALYSIS OF NHTSA'S CONSUMER COMPLAINTS AND BMW'S DATA, THE PETITION IS DENIED.FOR DETAILS, SEE FEDERAL REGISTER, VOL. 69, NO. 91 (MAY 11, 2004).

The contact owns a 2004 BMW 325 CI. The contact stated that while driving at an undisclosed speed, the driver's side rear turn signal light became inoperable while attempting to make a turn. There was no warning light illuminated. The contact stated that the rear turn signal light failed to function properly. The vehicle was taken to an independent mechanic, where it was diagnosed that the rear tail lamp functions had failed and needed to be replaced. The vehicle was not repaired. The dealer was contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 250,000.

AFTER RECALLED AIR BAGS REPLACED, ABOUT 6 MONTHS LATER NEW AIR BAG SENSOR LIGHT CAME ON AND DIAGNOSED AS PASSENGER SEAT AIR BAG SENSOR MAT NEEDS TO BE REPLACED. JUST RANDOMLY THE LIGHT CAME ON AND STAYED ON. I SEE THIS HAS BEEN A PROBLEM IN BMWS AND THERE WAS AN EXTENDED WARRANTY FOR OTHER MODELS. THIS SHOULD BE A RECALL AS IT IS A SAFETY ISSUE AS BAGS WILL NOT DEPLOY.

TL* THE CONTACT OWNS A 2004 BMW 325CI. THE CONTACT STATED THAT UPON MAKING A RIGHT TURN AND A STOP, HE NOTICED THAT HIS RIGHT TURN SIGNAL HAD FLASHED ERRATICALLY UPON ACTIVATION. THE CONTACT TOOK THE VEHICLE TO AN INDEPENDENT MECHANIC WHO INFORMED HIM THAT THERE WAS A RECALL ON HIS VEHICLE UNDER NHTSA CAMPAIGN NUMBER: 11V438000 (EXTERIOR LIGHTING). THE CONTACT CALLED CHAPMAN BMW CHANDLER (7455 W ORCHID LN, CHANDLER, AZ 85226) WHERE HE MADE AN APPOINTMENT TO HAVE THE VEHICLE REPAIRED UNDER THE RECALL. THE DEALER CALLED BACK AND INFORMED HIM THAT HIS VEHICLE WAS NOT INCLUDED IN THE RECALL. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND CONFIRMED THAT THE VEHICLE WAS NOT INCLUDED IN THE RECALL. THE MANUFACTURER THEN REFERRED HIM TO NHTSA FOR ASSISTANCE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 90,000.

I CALLED MY DEALER LAST YEAR TO SCHEDULE TO FIX MY RECALL ON MY CAR. UNFORTUNATELY I DID NOT WENT TO MY APPOINTMENT. NOW ONE OF THE BAGS EXPLODED, AND I CALLED THE DELEGAR AGAIN TO DO MY RECALL, BUT TO MY SURPRISE THE HAVE MY CAR IN RECORDS AS REPAIR!! AND I TOLD THE THAT NEVER HAPPENED. THEY PROMISED TO CHECK AND CALL ME BACK AND HAS NOT HAPPENED EITHER. THIS IS THE BMW SAN ANTONIO TEXAS DELEGAR. I BELIEVE THIS IS FRAUD! I WANT TO HAVE THE RECALL DONE AND I WANT TO MAKE SURE THEY DON'T GO AWAY WITH IT. I DON'T HAVE ANY DOCUMENTATION TO UPLOAD ABOUT IT BECAUSE IT DOESN'T EXIST. HEADLIGHT WIRING INSULATION ON STANDARD BMW HEADLIGHTS IS BRITTLE AND FALLING OFF EXPOSING BARE COPPER WIRES WITHIN THE HEADLIGHT ASSEMBLY. IN ORDER TO PROTECT THE HEADLIGHTS I HAVE HAD TO WRAP THE WIRES IN ELECTRICAL TAPE AND SHRINK WRAPPING. THIS ISSUE HAS EXISTED FOR SEVERAL YEARS.

Diagnosis of Compound Brake System Complaints in M Vehicles: 1. Visible cracks are caused by high thermal stresses during rapid heating and cooling of the brake discs. Braking from high speed with cold brake components (less than 212 degrees F) increases the likelihood of cracks to form.

SERVICE ACTION - HEAD AIR BAG HOLDER RETROFIT: A rattle noise may develop from the head air bag (ITS) gas generator contacting adjacent component. This noise can be heard while driving on an uneven roadway.

Discharged Battery Energy Diagnosis Must Be Performed: The electrical system of BMW vehicles has been subject to an ongoing development process over the last few years. This has led to increased demands being placed on the battery. This document covers important information for the dealer on how to handle "discharged battery" complaints. In order to properly repair the vehicle the first time in the workshop, it is very important that the diagnostic test plans are performed to completion, with all results taken into consideration. Without performing the energy diagnosis test plan, the root cause of the discharged battery cannot be identified. This increases the chances of a repeat repair.
