BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

The defect: Ford Motor Company (Ford) is recalling certain 2015 F-250, F-350, F-450, Expedition, Lincoln Navigator, and MKC vehicles. The rearview camera printed circuit board terminal header connector may be loose or damaged, which can result in a blank or distorted image.
The risk: A blank or distorted rearview camera image can reduce the driver's view of what is behind the vehicle, increasing the risk of a crash.
The remedy: Dealers will replace the rearview camera, free of charge. Owner notification letters were mailed October 16, 2025. Owners may contact Ford Customer Service at 1-866-436-7332. Ford's number for this recall is 25S42. This recall expands previous recall number 22V-082.

The defect: VCI Emergency Vehicle Specialists (VCI) is recalling certain 2017-2018 Ford F-550, 2014 E-350 and E-450, 2015 F-250 and F-450, 2016 E-450, 2017 E-350, F-450 and E-450, 2018 F-350, 2010 E-450, 2005-2006 E-450, 2009 E-450 and Chevrolet C4500-based Type I and Type III ambulances equipped with ASA Electronics Voyager monitors used to display the back-up camera image. The monitors may unexpectedly revert back to the factory default settings which may cause the camera image to be reversed.
The risk: The driver may inadvertently turn the wrong direction to avoid an object behind the vehicle, increasing the risk of a crash.
The remedy: VCI Emergency Vehicle Specialists will work with ASA to notify the owners, and remedy the displays, free of charge. The recall began May 9, 2019. Owners may contact ASA Electronics at 1-800-384-4400.

The Office of Defects Investigation (ODI) opened this RQ to investigate allegations that the scope and remedy of safety recall 13V-535 were inadequate. Recall 13V-535 was limited to a relatively small population of Ambulance Package vehicles and ODI received complaints of pickup trucks and Fire Engine Prep Package vehicles being affected by the same defect. Ford manufactured approximately 555,985 subject vehicles between February 22, 2010 and January 30, 2015 that may be affected by defective EGT sensors and Powertrain Control Module (PCM) software calibration. The subject vehicles are equipped with four (4) EGT sensors to monitor engine operating conditions. An EGT sensor fault may produce false exhaust over-temperature signals. If these signals are produced, the Powertrain Control Module (PCM) implements a Failure Management Effects Mode (FMEM) and the driver receives a Stop Safely Now (SSN) system warning on the instrument cluster message center. The FMEM triggers a 70% reduction in available engine power over a period of no more than 45 seconds, during which it may not be possible to accelerate or maintain vehicle speed. The PCM then commands the engine to shut down when vehicle speed reaches approximately 1 mph. There is no immediate restart capability. Depending on the PCM software calibration, ignition key position and ambient temperature conditions, the engine may not restart until the end of a 10-60 minute cool down period.Ford and its component supplier(s) made numerous changes to the PCM software calibration and the design, material and manufacture of the EGT sensors over the period of subject vehicle production. This continuous product improvement affected vehicle performance regarding the alleged defect. Ford acknowledges the defect condition in the subject vehicles but argues that an unreasonable risk to motor vehicle safety is limited to Ambulance Package and Fire Engine Prep Package vehicles where commanded shutdown could impact emergency response and patient care. Ford also argues that the defect trend is in decline. Ford also initiated a new safety recall (15V-175) to address the 13V-535 remedy concerns in the Ambulance Package vehicles and Fire Engine Prep Package vehicles. The 15V-175 remedy reprograms the PCM with a new software calibration that stops a single EGT sensor fault to cause the vehicle to enter the FMEM shutdown mode.Ford also initiated an EPA emissions recall (14E03) for all subject vehicles that reprograms the PCM with the software calibration employed in safety recall 15V-175. Finally, Ford initiated a Customer Satisfaction Program (15M01) providing extended warranty coverage of 8 years or 80,000 miles for the the EGT sensors on certain MY2011-2012 subject vehicles.Due to the EPA emissions recall, that had a 66.6% completion rate in Ford's December 2015 progress report to the EPA, the defect trend is in decline. ODI is not aware of any crashes, deaths or injuries related to the alleged defect.ODI believes the alleged defect poses a safety risk but notes that the EPA emissions recall is addressing the safety problem.Compelling issuance of a duplicate recall would not represent a wise use of agency resources.Accordingly, this Recall Query is closed.This action is not a finding that no defect exists and NHTSA will take further action if such action is warranted.

THIS VEHICLE IS SHUTTLE BUS, TRANSPORT PASSENGERS FROM CHICAGO O'HARE AIRPORT TO FOUR POINTS HOTELS AND BACK 24/7. THIS VEHICLE IS OWNED BY FOUR POINTS HOTEL AT SCHILLER PARK, IL AND I WORK AS DRIVER. ON OCTOBER 8 2017 JUST AFTER 7 PM I WAS DRIVING THE BUS WITH PASSENGERS ON BOARD FROM THE AIRPORT TO THE HOTEL WHEN I HEARD NOISE FROM THE REAR I PULLED OVER AND STOPPED THE BUS. AFTER CHECKING I SAW THE REAR LEFT WHEEL IS COMING OFF. ALMOST ALL THE NUTS AND BOLTS ARE GONE BOLTS SNAPPED OFF WHILE THE BUS WAS IN MOTION. THE BUS WAS TOWED TO M&M AUTOMOTIVE INC. (708 456 6629) M&M MAINTAIN THE HOTELS BUS. THE SAME ( IDENTICAL) PROBLEM TOOK PLACE ON 11/20/2016, BUT THE REAR LEFT WHEEL COME OFF. THE TRUCK SURGES/ENGINE REVS. FORD DOES NOT KNOW WHY ITS HAPPENING, MULTIPLE PARTS HAVE BEEN CHANGED. SEVERAL FORD ENGINEERS HAVE BEEN INVOLVED WITH NO RESOLUTION. THE TRUCK HAS BEEN AT THE DEALER FOR 7 MONTHS, PARKED FOR THE LAST 1 MONTH WITH NO ACTIVITY. I BELIEVE IT IS A SAFETY ISSUE, DUE TO THE FACT THE TRUCK WILL SURGE FORWARD ON ITS OWN. FORD NOW SAYS IT NOT A WARRANTY ISSUE, BUT WE STILL ARE UNDER THE 5 YR / 60,000 MILE POWER TRAIN WARRANTY. THIS IS A RECOGNIZED PROBLEM FOR MOUNTAIN SUPPLY AND SERVICE AND WE ARE TRYING TO RESOLVE IT BEFORE SOMEONE GETS HURT, AND WE FEEL THAT IT IS NOT OUR RESPONSIBILITY AT THIS POINT, SINCE WE HAVE BROUGHT IT TO FORD'S ATTENTION.

WHILE DRIVING AT VARIOUS SPEEDS RECEIVE LOW FUEL PRESSURE / REDUCED ENGINE POWER WARNING. TRUCK ENTERS "LIMP HOME HOME" ALMOST IMMEDIATELY. REDUCED POWER AT HIGHWAY SPEED PUTS DRIVER/PASSENGERS IN HARM AS ONE CAN'T MAINTAIN SUFFICIENT SPEED TO SAFELY EXIT THE HIGHWAY. FORD WAS UNABLE TO DETERMINE ROOT CAUSE ON FOUR OCCASIONS. ON FINAL OCCURRENCE DEALER STATES FORD CORPORATE WANTS TO DISMANTLE FUEL SYSTEM TO DETERMINE IF FUEL CONTAMINATION IS PRESENT. FURTHER STATES IF CONTAMINATION IS FOUND REPAIR AND DISMANTLE ARE OUTSIDE OF WARRANTY AND OWNERS RESPONSIBILITY. ISSUE BEGAN WITH 10K MILES ON THE VEHICLE FINAL INCIDENT OCCURRED AT 18K MILES. TRADED FOR ANOTHER BRAND OUT OF CONCERN FOR PERSONAL SAFETY. FORDS LACK OF CONCERN AND BLATANT ATTEMPT TO PASS THE BLAME TO CONSUMERS AND FUEL SUPPLIERS HAS LOST THEM A CUSTOMER FOR LIFE.

PICKUP - 6.7L DIESEL ENGINE - RUNS ROUGH DURING EXHAUST REGENERATION PROCESS - BUILT ON OR BEFORE 2/23/2016 ISSUE Some 2015-2016 F-Super Duty 250-450 Pickup vehicles equipped with a 6.7L diesel engine and built on or before 2/23/2016 may exhibit a runs rough/misfire condition only during the exhaust regeneration process. This condition typically occurs on light throttle tip in between 64-113 Km/h (40-70 MPH) and will not set diagnostic trouble codes (DTCs). The concern may be intermittent and a knocking noise may be present during the concern. SERVICE PROCEDURE 1. Reprogram the powertrain control module (PCM) to the latest calibration using Integrated Diagnostic System (IDS) release 99.03 or higher. Make sure you are connected to the internet when entering module programming to obtain the latest updates. Calibration files may also be obtained at www.motorcraftservice.com 2. Did the customer indicate the message center displayed a Cleaning Exhaust Filter message prior to the concern? a. Yes - proceed to Step 3. b. No - proceed to Step 6. 3. Remove the left cylinder head. Refer to Workshop Manual (WSM), Section 303-01. 4. Replace all eight (8) exhaust valves and valve seals. Refer to WSM, Section 303-01. (Figure 1)
