Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492
Pedestrian alert sounds
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

AFTER THE RECALL APPOINTMENT ON 5/3/2019,THE SIGN "ABS" STILL COMES ON ALL THE TIME IN THE PANEL.BESIDES,THE AUTO AUDIO SYSTEM FAILS WHEN THE IGNITION IS ON. WHEN THE CAR IS NOT ON THE AUDIO SYSTEM COMES ON AGAIN. THIS MAY BE CHECKED BESIDES THE ABS SIGN.

I WAS DRIVING MY CAR ON ANTIOCH RD. CRESTVIEW, FL AND HAD STOPPED AT THE STOP SIGN TO JUMP ONTO P.J. ADAMS RD. WHEN TRAFFIC PERMITTED I TIRED TO PULL OUT THE CAR LUNGED FORWARD A FEW FEET PUTTING THE HOOD AREA COMPLETELY IN THE ROAD WHEN THE D LIGHT STARTED FLASHING AND THE CAR STOPPED MOVING . THE MOTOR REVES UP BUT THE CAR WONT MOVE. I WAS ALMOST HIT BYE A SEMI TRUCK . AFTER REPLACING ATT SENSORS THE D LIGHT IS STILL FLASHING AND THE CAR WONT MOVE . I HAVE EXQHAUSTED ALL REPAIRS THAT I CAN DO AND WAS FORCED TO SCRAPE THE CAR BECAUSE IT WAS MY ONLY WAY OF GOING AND AFTER SPENDING ALMOST 1000BUCKS WAS STILL UNABLE TO FIX THIS CAR. ITS VERY DANGEROUS IN THE FACT THAT I COULD HAVE BEEN HIT AND KILLED DUE TO THIS UNEXPECTED AND SUDDEN SHUT DOWN WITH NO WARNING

WHEN STARTING FROM AT STOP THE CAR HESITATES AND SHUDDERS WHEN YOU PRESS THE GAS PEDAL UNLESS YOU LET THE CAR IDLE AND ROLL FORWARD FIRST. IT ALSO HAS PROBLEMS WHEN YOU HAVE THE CRUISE CONTROL ON FOR MORE THAN 10 MIN. AFTER APPROX 10MIN IT ACTS LIKE I HAVE TURNED OFF AND BACK ON THE CRUISE CONTROL SEVERAL TIMES BEFORE IT DOES TURN OFF AND THEN YOU CAN TURN THE MAIN CRUISE CONTROL SWITCH OFF AND BACK ON AND THEN RESET THE CRUISE BUT IT ONLY STAYS WORKING FOR 1-2 MIN BEFORE SHUTTING ITSELF BACK OFF.

THERE IS A SHUTTERING IN THE TRANSMISSION THAT OCCURS WHEN THE CAR IS ATTEMPTING TO GET OUT OF FIRST GEAR. THE SHUTTERING IS BECOMING MORE VIOLENT. THE CAR HAS ALSO SLIPPED OUT OF DRIVE (WHILE ON THE ROAD) AND WOULDN'T ACCELERATE. I'VE READ THAT THE SHUTTERING ISN'T FAIRLY COMMON IN THIS MODEL AND/OR TYPE OF TRANSMISSION.

I DROVE THIS 2003 HONDA CIVIC HYBRID TOOK MY BABY HONESTLY FROM THE SHOPPING MARKET. IT WAS JUST AT THE RIGHT TURNING CORNER, THE VEHICLE SUDDENLY LOST POWER TO MOVE, I IMMEDIATELY PRESS THE EMERGENCY BUTTON, GOT OUT OFF THE VEHICLE LET OTHER AUTOS PASSED ME AHEAD BY THE SIDE WAY. LATER SOME KINDLY GUYS HELP ME PUSHED THE CAR TO A PARKING LOT NEAR BY. THE TRANSMISSION WAS OUT OF WORK. AFTER MY FRIEND HELP ME TOOK BABY HOME, THE VEHICLEL WAS TOWED BACK. GLAD IS NOT ON THE HIGHWAY. IT ONLY JUST PASSED 100K MILES. THE TRANSMISSION FLUID WAS GOOD, OUTPUT SPEED SENSOR IS OK. I THINK NOT ONLY ME HAVE THIS QUALITY PROBLEM. IT IS TOO DANGEROUS FOR US WITHOUT A SIGN.

WHEN DRIVING AT 50MPH ON A NATIONAL HIGHWAY 95S HEADING TO PHILADELPHIA AROUND 11AM,THE SPEED OF THE CAR SUDDENLY REDUCE TO 20MPH AND THEN STALLED AND STOPPED MOVING. I HAD TO PULL ASIDE TO CHECK THE ENGINE. AFTER SOME THIRTY MINUTES OF REST,I HAD TO RETURN HOME WITH AN IDEA OF GETTING THE CAR CHECKED BY THE DEALER.THE DEALER REORTED OF HAVING NO PROBLEM IN THE ENGINE AND I HAD TO TAKE THE CAR BACK AND RUNNING AS CAREFULLY AS POSSIBLE WITHOUT USING FOR A LONG TRIP. I HAD SOME DENTS DUE TO OTHER DRIVER WITH RASH DRIVING AND HITTING THE CAR.I NEED TO GET THE DENTS FIXED AS SOON AS THE PRESENT WINTER SEASON TURNS FEASIBLE FOR EASY RUNNING.

JERKING AND NOISES GOT WORSE OVER A ONE WEEK PERIOD. DEALERSHIP, WHERE ALL SERVICE HAS OCCURRED FOR THIS CAR, SAYS IT NEEDS A NEW TRANSMISSION ($4800) AND IS NOT WORTH REPAIRING. THE MILEAGE IS QUITE LOW FOR THIS PROBLEM, 133,000. WE'VE HEARD THIS IS A COMMON PROBLEM FOR THESE HYBRIDS 2002-2005.

"TAKATA RECALL" 2003 HONDA CIVIC HYBRID WITH IMA AND ENGINE LIGHTS WENT OFF. NO CHARGE TO BATTERY CAUSE THE CAR TO MALFUNCTIONS. SUCH AS ELECTRICAL STEERING WHEEL STOP WORKING. ENGINE LOST POWER AND STOP WHILE DRIVING ON CITY STREET AT STOP SIDE. I CHANGED NEW DIEHARD 12V BATTERY AND ALSO CHECKED AND VERIFIED THE IMA BATTERY IS GOOD (EACH CELL ABOVE 7 VOLTS). I THINK THE CAR NEED TO UPDATE SOFTWARE FROM HONDA. PLEASE HELP US FOR THE RECALL TO UPDATE SOFTWARE. MANY PEOPLE HAVE THE SAME PROBLEM AND THEY POST THEIR STORY ON YOUTUBE

ENGINE AND ENGINE COOLING
Service bulletin - Emission certification labels that were replaced on a small number of vehicles may have been replaced with a label listing incorrect information. The affected emission labels are replacement service parts, and were not installed at the factory; they were installed during postmanufacturing repairs that required replacement of the underlying part. This will cause the vehicle to be non-compliant with emissions regulations.

Dealer message - Today, American Honda is notifying dealers of a voluntary noncompliance recall for a small number of Honda vehicles due to an error on the replacement (service part) underhood emissions labels. American Honda previously notified CARB and the EPA for this action.
