On 8.1.26, while operating my 2019 Honda Odyssey, the vehicle suffered a catastrophic, un-commanded automatic emergency braking event followed by a total failure of the brake-release fail-safe mechanisms. While driving in active traffic, the vehicle's forward-facing sensor system suffered a false-alarm trigger. Due to a severe design and component vulnerability, the Electronic Parking Brake (EPB) switch suffered a simultaneous internal mechanical and electronic hardware failure. Because the EPB switch failed internally at the exact moment of the automatic braking event, the system failed to command a brake release. The vehicle’s wheels locked completely, leaving the vehicle completely immobilized in active traffic. This multi-system failure resulted in an immediate, life-threatening crash hazard. By random circumstance and the grace of God, a catastrophic rear-end collision was avoided, but this was a severe near-fatal safety event. A professional automotive diagnostic scan confirmed that the vehicle's computer retained 2 distinct fault incident codes documenting the internal hardware failure of the EPB switch. The dealership confirmed that the system provides absolutely no advance warning to the driver, and fails entirely "in the event," meaning the vehicle remains safe until it suddenly and unpredictably locks the brakes on the highway. The vehicle was repaired out-of-pocket for $297 to replace the defective switch. I am filing this complaint because an internal electronic switch failure must never result in an un-commanded, un-releasable wheel lockup while a vehicle is in motion. This indicates a fatal flaw in Honda's braking fail-safe architecture. This issue must be investigated immediately before it results in a future serious or fatal acciden.

The incident is that the rear brakes keep wearing much faster than the front. New brake pads do not last after only less then 10,000 miles of driving. Dealership and mechanic expect that there is a caliber issue with the 2019 model that is causing the rear brakes to wear too fast. Manufacturer should remedy this issue.

The vehicle was in reverse and started to accelerate uncontrollably in reverse. The car came to a stop after hitting a wall and a car. The uncontrolled acceleration happened when we went about 1 foot onto grass while in reverse. The emissions light was going on and off from time to time. We believe it was inspected by a honda dealership at some point. The vehicle is involved with insurance at this point.

On April 13, 2026, I brought my 2019 Honda Odyssey to a Honda dealership in Chicago, IL for a dealer-initiated recall service. The dealer contacted me and asked me to come in. Before the recall service, the vehicle had zero warning lights, zero error codes, and was operating completely normally. Immediately after the recall service was completed, the following warning lights appeared on the dashboard: Brake System warning, Hill Start Assist Problem, VSA (Vehicle Stability Assist) warning, and additional stability/steering system warnings. None of these warnings existed before the service. I returned to the dealership the same day. The service team cleared all error codes. However, within minutes of driving off the lot, all warning lights reappeared. This confirms the issue is persistent and not a one-time post-update self-check. I returned a second time. The service manager stated the warnings were not caused by the recall service and refused to investigate further. The dealer quoted $800 for diagnostic work. I also contacted American Honda by phone and was told they could not intervene. With the Brake System and VSA warnings active, the anti-lock braking system and vehicle stability control may not be functioning properly. This puts me and other drivers at risk, especially in emergency braking situations and on slippery roads. This pattern of multiple safety warnings appearing immediately after recall service on 2019 Honda Odysseys has been reported by other owners. The vehicle and its warning lights are available for inspection upon request.

Multiple safety systems on my 2019 Honda Odyssey Elite have simultaneously failed, including LKAS, ACC, Collision Mitigation Braking System, Auto High-Beam Headlights, and other driver-assist features. The dashboard displays warnings for nearly every safety-related system, and the vehicle disables these features without warning. **Detailed Description of the Problem:** Whenever I start my vehicle, multiple error messages are displayed related to the Lane Keeping Assist System (LKAS), Adaptive Cruise Control (ACC), Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Auto Headlights, and other safety features. These warnings appear together and persist even after restarting the vehicle. Apparently, this issue is widespread among 2018–2020 Honda Odyssey owners, with many reporting the same cluster of system failures. The problem appears to be related to a malfunction in the vehicle’s electrical system, sensors/ cameras, or control modules. In my case, the failures occur without any external damage, weather event, or driver action that would explain the sudden loss of safety functionality. When these systems fail, the vehicle disables critical safety features such as automatic emergency braking, lane keeping, adaptive cruise control, and automatic headlights. This creates a significant safety risk, especially at highway speeds or in low‑visibility conditions. **Frequency:** The issue occurs whenever the vehicle is started, affecting my driving everyday.

The impact on the car is we no longer have cruse control (basic feature) and every time we turn on the car we see many warning lights turn on. Some other feature that does not work is lane assist does not work. According to what shows up to on the dashboard there are problems for "road departure mitigation system Problem", "Auto high-beam problem", "Adaptive cruse control problem", "Lane keeping assist problem", "collision mitigation system problem", "break system", "left sliding door problem" A quick search on the web indicate this a common issue with Honda car (including Acura). [XXX] Another problem we're seeing is driver Side sliding door does not close correctly or open correctly. We have since stop using this door since it's very dangerous for our kids since while opening, it would not open all the way and close on the kids. similar issues [XXX] These two things are broken under 65000 miles and less than 6 years owning the car. Collision Mitigation Braking System (CMBS) My 2019 Honda Odyssey slammed on the brakes while traveling 70+ MPH on a clear freeway. There was no warning and nothing in the road. I have also had issues with the braking warning light flashing for no reason while traveling down the roadway. I called the Honda dealership and inquired about the braking issues and was told unfortunately it can happen, especially if I drive on windy roads.

The contact owns a 2019 Honda Odyssey. The contact stated that while driving at an undisclosed speed, the ABS warning light illuminated. In addition, the contact stated that several unknown warning lights were also illuminated. The vehicle was taken to the dealer, where it was diagnosed with a failed camera system. The contact was informed that the camera system needed to be replaced, and the software needed to be updated. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 87,395.

At restaurant on 6/1/25, backed out of parking spot. Drove about 30 ft to parking lot exit. Turned onto side street to go to light and turn on main street. About 3 or 4 car lengths from exit to light. Side street sloped down about 20-30 degrees. Brakes would not stop van. Not sure if brakes slowed down van as going slow. However, brake pedal pushed hard down to floor but did not stop van. If hit gas pedal van would have accelerated and engine raced causing acceleration and more damage. No damage to back of vehicle I hit. Very minor damage to my van. Tried yesterday and today to replicate (in isolated locations and with no other vehicle near) the van’s conditions at time of incident. I will be contacting dealer. There is no recall. Do not know if anyone else had this problem or if dealer can find any issue. Notifying NHTSA to advise of this potential systemic issue. I’ll follow up with results of interaction with dealer.

All the symbols related ADAS suddenly popped in my car with no rain or snow or any accidents. When I checked with Honda dealership, they said it caused due to software issue and nothing related hardware and it needed 2500$ for replacing the entire system. If it is software causing this issue why would customer had to pay for it, and it could be a recall. I noticed similar issue for all 2019 and above models.

A number of warnings popped up: adaptive cruise control problem, lane keeping assist problem, collision mitigation system problem, road departure mitigation problem, and auto high beam problem. Only light that is on is my brake system light. I read a bit on some forums and it is likely to be the the front camera. Apparently the Multipurpose Camera Unit (MCU) fails often on these vehicles, causing these errors and the cruise control to stop working.

The whole ADAS system went down and a brake system light came on. Van isn’t in warranty but isn’t old either. Just out of warranty. Lots of safety items disabled because of a faulty camera that’s too expensive to replace on our own, so now we operate a mini-van without all the extra safety items that ADAS gives you. Guess ADAS was never intended to improve safety if it only works for a couple of years.

The front facing monocular camera of the ADAS system has failed on this 2019 Honda Odyssey with approximately 50,000 miles. The failure of this camera has rendered the ADAS systems inoperable including adaptive cruise control, automatic emergency braking, forward collision warning, lane departure warning, and lane keeping assistance. This appears to be a common complaint/failure mode amongst many similar Honda models 2018 - 2022. Additionally, within a span of five years, two doors have malfunctioned, and the electrical system required replacement. These defects have jeopardized the safety of my family. I'm experiencing several error messages on my 2019 Honda Odyssey, which include issues with the Brake System, Lane Keeping Assist System (LKAS), Auto High-Beam, Road Departure Mitigation System, Collision Mitigation System, and Adaptive Cruise Control (ACC). I have a 2019 Odyssey Elite and the following lights come on: Adaptive cruise control problem (ACC); Auto high-beam problem (A); Road departure mitigation system problem; collision mitigation system problem; lane keeping assist problem (LKAS); brake system lights. My problem is intermittent. I can make it go away most of the time by pouring water over the camera or spraying it from the windshield wipers fluid, then turning the car off and back on. That works sometimes. This makes me think it may be due to heat as the windshield (especially the black web lines around the camera) gets very hot. I also have noticed that at night or in cooler parts of the day the lights do not come on most of the time (have not kept a log of problem yet). One problem could be heat but cooling off with water does not always work. In that case, I lightly tap the camera case with a small rubber hammer (it is locate in front of my rear view mirror), turn the car off and back on, and the lights go out. Water plus tapping works and sometimes tapping alone works. This makes me theorize that the weather (heat in my case but I have seen other say it is cold due to snow) causes the cable/wiring within the camera to loosen (meaning weather [heat or cold] sensitive) cable connectors. Thus cooling with water and or light tapping fixes the problem until the next episode occurs. Who knows it could even be the windshield glass. As I said earlier, there is a black webbing type glass etching around the camera. Some say it is the battery, but I have yet to see a post where changing the battery fixed the problem. When the lights are off, all safety features (mitigation, adaptive cruise control, cruise control) work. I assume auto high beam and brakes system will work also. Forgot to add that when I turn the car off and back on, I have to press enter on the steering wheel a couple of times before the lights go off.

Over 20 times our van has breaked when it was not needed. This usually happens when going up a hill and turning, or when a car slows but not enough to need to break. In several instances we have almost been rear-ended due to the sudden breaking. We try to turn off the service, but it has to be turned off each time. With the amount of traffic we have in the area, this is very dangerous.
