2019 Honda Odyssey Fuel system Problems

Failed Component: Direct Fuel Injector Set. The components are currently on the vehicle and are available for inspection upon request. Safety Risk: The failing fuel injectors create a severe engine air/fuel imbalance, which causes engine hesitation, engine stumbling during acceleration, and unpredictable power loss. This puts my family at high risk of a collision, particularly when merging onto highways, crossing busy intersections, or accelerating from a stop. Furthermore, dumping unburnt fuel into the exhaust risks overheating the catalytic converter, creating a potential fire hazard. Dealer Confirmation: Yes. The problem was confirmed by AutoNation Honda 365 in Memphis, TN, on [XXX] (Mileage: 83,452). Manufacturer Inspection: The vehicle was inspected by factory-trained technicians at an authorized Honda dealership. A case was opened with Honda Corporate, but they refused to cover the repair, stating my specific VIN batch was excluded from their warranty extension, despite suffering the exact same failure. Warning Lamps & Symptoms: An "Emissions System Problem" warning and the Check Engine Light illuminated on [XXX]. Incident Description & Cause: Honda is fully aware of this dangerous defect. They issued Service Bulletin 21-010, acknowledging that "debris from the high-pressure pump or the fuel injector machining process can cause internal wear or clogging" in 2018-2019 Odysseys. Following the TSB 21-010 protocol, the dealer pulled code P219A and performed a "Cylinder AF Test." My vehicle failed the test (Cylinder 3 running rich). The Honda technician’s official notes state: "Fail air fuel test need injector set internal failure." Honda extended the warranty to 10 years/150,000 miles for this known manufacturing defect, yet they are arbitrarily excluding my 2019 Odyssey, which has the exact identical internal failure documented by their own dealer. XXX, I was driving my 2019 Honda Odyssey EX-L on a family trip to Florida when the dashboard lit up like a Christmas tree and the car went into neutral. We were on [XXX], close to [XXX]. My wife and I had to push the car off the side of the road and call a tow truck company. This occurred on a Friday night. The only place I was able to take the vehicle for repair was the nearest dealership. I told them what happened. The service writer tells me the issue is the fuel pump and high pressure fuel pump. The cost was $2,800. When I told them I was on vacation and this issue should've been resolved back in 2021 with the recall they told me nothing could be done. I had the recall work completed in 2021 with 12,000 miles on the van. The fuel pump & high pressure fuel pump problem occurred in June 2026 with 51,000 miles on the van. ​While driving my 2019 Honda Odyssey at highway speeds on the 60 freeway, the vehicle experienced a sudden and total engine failure/stall without prior warning. I had all my kids in the car as well. CHP advised to call Honda The sudden stalling resulted in an immediate loss of engine power, creating an unsafe scenario on a high-speed freeway. I was forced to navigate the vehicle across traffic lanes to pull over onto the shoulder safely.

The vehicle went into a limp mode due to programming and safety issue known by Honda. One of the six fuel injectors failed according to the fault codes at the dealer. I have reached out to Honda and they won't even acknowledge the safety issue nor are they willing to reimburse the cost of the emergency safety repair caused by the vehicle going into limp mode. They won't put anything in writing nor will they consent to any recorded calls. I have reached out to Brad Ortloff as well and he won't respond. What are they hiding? Why did NHTSA allow Honda to get away with extending the warranty on the fuel injectors for certain Honda Odysseys rather than recalling them? What is the difference between the vehicles that were included? There are numerous vehicles that weren't included that are failing for the exact same reason. I can't get any answers from Honda. They just say they are not licensed techs. Brad Ortloff won't respond. Why is NHTSA allowing Honda to put citizen's safety at risk? Will someone please investigate this safety issue?

Fuel pump injector needs to be replaced at approximately 68,000 miles. Honda issued an extended warranty on fuel pump injectors for 2018-2019 models but refuses to cover replace it. Check engine light came on due to injector failure. This poses an extreme safety risk.

The fuel injectors failed for the exact same reason as other VINs that Honda issued an extended warranty for. The fuel injectors failed without warning and went into limp mode. The vehicle barely moved in rush hour in an unfamiliar city with very busy traffic. I reached out to Honda and they could not provide an explanation as to why the warranty wasn't extended for the fuel injectors like they were for other VINs. In my opinion, Honda knows there is an issue, but has decided not to do anything about it. Hopefully, NHTSA can help with this potential safety issue. The dealer replaced the fuel injectors due them failing and the injectors are available for inspection.

The contact owns a 2019 Honda Odyssey. The contact stated that while driving at an undisclosed speed, the vehicle suddenly lost automotive power. No warning lights illuminated. The vehicle was towed to a local dealer where it remains, where it was diagnosed with a fuel pump failure. The vehicle was not repaired due to the costs. In addition, the contact stated that the vehicle was previously repaired at another local dealer under NHTSA Campaign: 21V215000 (Fuel System, Gasoline); however, it did not prevent a failure from occurring. Upon further investigation, the contact related the failure to NHTSA Campaign:23V858000 (Fuel System, Gasoline); however, the VIN was not included. The manufacturer was made aware of the failure. The failure mileage was approximately 80,000.

Honda states that the VIN number is outside of scope and is requesting an out of pocket repair. According to many online complaints, the VIN numbers need to be expanded.

When picking my child up from school, upon departure, the front end of the vehicle began violently shaking and sounding as if it wanted to shut down, simultaneously, a number of system failure warnings displayed on the dash. I was able to get the vehicle into a parking spot and turn it off. It would not restart. I called a tow truck and had it towed to a local garage as it was not safe to drive even if it did start. Upon the local mechanic pulling codes, they pulled the code P0087 indicating a failure of the fuel system, gasoline: delivery: Fuel pump. This incident put myself, my 3 children in the vehicle as well as other pedestrian children and adult pedestrians at great risk, because if I had lost control and not been able to park safely, the outcome may have been extremely different. The problem has been confirmed by local mechanic and is scheduled with the dealer in 5 days. There were no prior warnings or indications of the failure. Honda issued a recall in 2021 then closed it. It was later expanded by NHTSA in 2024. Then again closed. We never received notification of the recall either time. recall numbers follow: 21V-21520V-314 expanded recall: 23V-858/Honda TSB 24-022 Both, Im told are closed, which is unfathomable to me given the significant and known dangers attributed to this issue.

On May 2nd, we drove our 2019 Honda Odyssey XL-E to church, and as soon as we pulled into the parking lot, we started to see smoke coming from our front hood. We exited the car. Within 30 seconds, the car then went into flames, and exploded and the car is now unsalvageable. When I went online, it seems like this is a common occurrence with these Honda Odysseys. There were no warming lamps, messages, or lights that would have alerted me that something this serious would occur. We have taken our care to Honda for years for regular maintenance. We don't take it to any independent shops. Strictly to Honda for years. The most recent checkup/service was performed on February 11, 2026 where they performed the following services which they had recommended. 1) Oil Change 2)Engine coolant flush with conditioner 3)Transmission fluid change: 4) wheel alignment 5)Fuel tank additive, throttle body service, and injector. This is serious safety event and our family and children could have been killed in this accident. We have pictures, videos, police report, and the car has been taken to a salvage yard. We have filed a claim with State Farm.

Engine failure. Not available for inspection. Engine replaced and damaged engine return for core charge. While driving on the highway, without any for warning, engine stalled, loss of power and dash read “emissions system problem “. Vehicle was towed to Honda of Denton for diagnostic testing. The engine stalling and loss of power at highway speeds caused a great risk for my family in the van. We impeded traffic and could have caused an accident. There were no warning lights and/or engine system issues leading up to the engine failure. Honda stated that it’s is a common issue with the internal engine bearing failure. They also confirmed that my VIN is not listed on the recall list for the 2 million engines effected. Honda quoted a LKQ engine replacement. Declined and had replacement done at Caliber Collison.

[Incident Description & Manufacturer's Bad Faith] On April 6, 2026, an authorized Honda dealership (McConnell Honda in Montgomery, AL) diagnosed my vehicle (2019 Honda Odyssey EX-L, 143,097 miles) with DTC P219A (Rec replacing fuel injector set and fuel feed pipe at this time), requiring a fuel injector replacement. This exact mechanical failure is documented in Honda’s official Service Bulletin 21-010. The bulletin acknowledges that defective fuel injectors can cause engine misfires, loss of motive power, and engine stalling while driving. These conditions present an immediate and severe safety hazard. Honda established a 150,000-mile warranty extension limit for this defect in TSB 21-010. My vehicle has 143,097 miles and is well within this limit. However, American Honda Corporation has arbitrarily refused to repair the vehicle. Furthermore, on April 21, 2026, the assigned Honda Case Manager left a voicemail officially denying the repair. He explicitly refused my repeated requests to provide this denial in writing. In the voicemail, he falsely stated that my vehicle at 143,000 miles is "well outside the range" for assistance, directly contradicting Honda's own written 150,000-mile bulletin. Please review the attached documents, including the official dealership diagnostic report (DTC P219A), the repair estimate, and a copy of TSB 21-010. I have also secured the audio recording of the case manager's voicemail to prove their bad faith. Honda is knowingly leaving a dangerous vehicle with a confirmed, documented fuel system defect on the road, while actively providing false information to the consumer to avoid repair costs. I strongly request the NHTSA to investigate Honda's deceptive and dangerous practice.

I was driving to work on April 7th, 2026 and parked the vehicle at my company parking lot. Prior to parking, I noticed unusual smell and then parked the vehicle. I parked the vehicle around 8.10 am and I got the call from my facilities manager around 8.30-8.40 am that my car caught on fire and by that time I noticed Houston Fire Department extinguishing the fire. There were no check engine sign and this vehicle is a brand new vehicle when I purchased. Though there is no recall associated with my VIN #, the fire issue is similar to recalls for 2019 Honda Odysey. The recall numbers are 23V-751( Engine Fire Recall) & 20V-066 ( Electrical Wiring fire Recall)

"Emissions system problem" engine warning light has been on for quite some time. I know there is a service bulletin related to this issue, but my VIN is not covered. There are also several other warning lights that occasionally pop up, and my Honda dealer believes these are all related to the initial warning light. They mentioned that sometimes these systems start shutting down so the vehicle runs properly. Recently I received "transmission system prolem" (in conjunction with the slew of warning lights I have frequently been getting). No apparent issues until recently when I was driving on the interstate. The engine started to become very loud and when I looked at the RPMs it was very high, over 5,000 RPMs. The car wasn't shifting out of first gear at 55MPH. I immediately slowed down and got off the interstate as soon as possible. After arriving home, we unhooked the battery, re-installed, and all of the warning lights were gone. Again, the Honda dealer believes all of these issues were realted to the "emissions system problem", which is part of a service bulletin that should be covered by warranty extension. I beleive my VIN should be included, and the recent transmission system problem which happened on the interstate was very dangerous and could have ended very badly, had I not been in tune with what was happening.

The fuel injectors emaeem to be malfunctioning and the OBD for emissions is lighting up. It has been inspected by the dealer and confirmed it needs to be replaced. The OBD code is P219A and this causes misfire and potential stalling of the vehicle. Honda has issued several TSBs but none have my VIN. Some of the TSBs are TSB A18-025, TSB A21-010 and TSB ONLN9403292021. It has been inspected by Honda dealer and confirmed to be a issue but no action unless I pay them a lot of money. I would like to see if NHTSA can help to add my vin number to the list or help me get Honda to repair my vehicle.

I went to my van, and many alerts were going off on the dash about multiple systems failing and not working. I got the van to the dealer who said it was the fuel injectors that needed replaced. I found a service bulletin about this saying American Honda is extending the warranty on the fuel injection repairs to 10 years from the original date of purchase or 150,000 miles, whichever comes first. The dealer said there was not a recall on my vin and there was nothing they could do and I was forced to pay for the repairs.

Vehicle intermittently loses propulsion during acceleration when merging. Engine RPM remains low despite throttle input. After a delay, transmission re-engages abruptly with a metallic scraping noise. This creates unsafe merging conditions. This happened today, 2/14/2026 in 4 instances in the same trip. It has also occurred approximately 50 other times over the past six months when the mileage was as low as 66,100 miles.

My 2019 Honda Odyssey had an emissions code come up with OBD II sensor code of P219A which led to research revealing that the code was a result of fuel injector failure. I took my van to the dealer who revealed that based on my VIN, I am not eligible for the extended warranty issued by Honda in 2021. Life-Threatening Acceleration Lag: The vehicle is experiencing severe power loss during critical driving moments, for example, while entering intersections or attempting to re-accelerate into moving traffic. When I attempt to speed up, the engine fails to respond initially, leaving the vehicle stranded in the path of cross-traffic. The engine finally engages only because I am forced to floor the accelerator pedal out of necessity, which then causes the tires to squeal when power eventually returns. Intermittent Starting Issues: The vehicle frequently fails to start smoothly on the first attempt; a known symptom associated with the fuel pump impeller deformation described in Recall 23V-858. VCM Shudder and Hesitation: I am experiencing the distinct vibration and rhythmic shudder associated with the Variable Cylinder Management system. This symptom is most prominent during deceleration or coasting; specifically, when I lift my foot off the accelerator without applying the brakes, the system triggers a noticeable shudder as it incorrectly manages cylinder engagement/deactivation. Suspected Catalytic Failure (Codes P0420/P0430): Both catalytic converters are exhibiting inefficiency codes simultaneously, which are highly atypical and occur well before the expected component lifespan. This clearly points to upstream defects. Specifically, the catalytic substrate has been poisoned by carbon and phosphorus deposits from the documented oil-burning defect and has suffered thermal damage due to the improper fuel mixtures caused by the defective high-pressure fuel pump (Recall 23V-858).

I have had it tested at the shop and it states code P219A (air-fuel ratio variation of cylinder [bank 1, rear bank) which I am told has a warranty extension from Honda for 10 years or 150k miles. A MIL warning was displayed for Emissions System Problem flaging our vehicle for emissions testing and registration. The vehicle skips and does not accelerate well from 20-30mph making turns into a busy street unsafe. The computer reads a DTC of P219A. All of this matches the description in Honda Service Bulletin 21-010 which says it covers 2019 Odyssey of all trim levels. Local Honda dealer (Auto Nation Honda of Chandler in Chandler, AZ) and Honda America both refuse to extend this warenty to our vehicle for repairs. We took it to a third party Honda mechanic for a second opinion and they recommended we follow up on the Service Bulletin 21-010.
