Owner-reported complaints naming the driver-assistance system, filed with NHTSA

Data refreshed

Driver assistance reportsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

My name is [XXX], we bought a brand new 2019 Honda Odyssey Elite, we have had problems with this vehicle since day one. They have lied and covered up the issues with this vehicle starting with the infotainment center, transmission slippage, fuel pump and the front camera has been overheating giving us the message that it is overheating and all the lights come on. They have stalled us on these issues and now the extended warranty is up and we want something done about this. We have babied this vehicle the entire time since new and it seems like one problem after another, at this point we are very disappointed from the way we are being treated from Honda. The latest is the front sensor on the front mirror that pops up all the lights on the dash, no one seems to be able to duplicate the problem, now they are on all the time and conveniently the extra extended warranty we bought have since expired, there was a lot of recalls on this vehicle and no one ever seems to be able to send us the recall letters. We really thin the mirror sensor and the transmission are definitely safety issues especially when the mirror shuts all the safety off. The transmission shifts hard at times and we pray that Honda will step up to the plate and do the right thing and fix the other recall issues on this same vehicle as well. We have other Honda vehicles that we have not had any issues with, we just can't understand why we are being treated in this manner, we just want the vehicle fixed. In January 2026, my 2019 Honda Odyssey (approximately 110,000 miles) suddenly began displaying multiple simultaneous warning messages on the dashboard, including Adaptive Cruise Control Problem, Lane Keeping Assist Problem, Collision Mitigation System Problem, Road Departure Mitigation System Problem, Auto High-Beam Problem, and a Brake System warning light. These warnings do not clear and are present every time the vehicle is started. As a result, the following safety systems are completely disabled: Forward collision avoidance / automatic emergency braking — the vehicle no longer detects or warns of potential frontal collisions Auto high-beam headlights — no longer switch automatically Adaptive cruise control and lane keeping assist — inoperable Road departure mitigation — inoperable In addition to disabling these safety systems, the persistent warning messages significantly obstruct the main instrument cluster display, making it difficult to see basic driving information including speed, fuel level, and odometer/mileage. This is a serious distraction and visibility concern in itself, separate from the loss of the safety systems. I have not yet had the vehicle professionally diagnosed, but based on extensive research into this issue — including reviewing the NHTSA complaint database — this appears to be a widespread and well-documented defect affecting 2018-2020 Honda Odyssey vehicles. The pattern matches hundreds of other complaints describing failure of the front-mounted Multipurpose Camera Unit (MCU), frequently associated with diagnostic trouble code U3000-49 (Multipurpose Camera Unit/Millimeter Wave Radar Internal Electronic Failure). Other owners report repair costs in the range of $1,500 to $2,600 for camera replacement and recalibration. This represents a serious safety concern: a single component failure disables the vehicle's primary collision-avoidance and driver-assistance systems simultaneously, while also obstructing basic instrument cluster vis

The vehicle’s front camera/Honda Sensing system has failed, causing multiple safety features to become unavailable simultaneously, including forward collision warning, collision mitigation braking, lane departure warning, lane keeping assist, adaptive cruise control and related systems. The failure produces multiple warning messages and disables safety systems that the vehicle was equipped and marketed with. This is a recurring issue reported by other Honda owners and is currently the subject of a proposed class action lawsuit, Jones et al. v. American Honda Motor Co., Inc., Case No. [XXX]. The lawsuit alleges a defective front camera system affecting 2018–2025 Honda vehicles, including the Odyssey. I believe this constitutes a safety defect because the failure can occur unexpectedly and simultaneously disables multiple driver-assistance and collision-avoidance safety systems. I am requesting that NHTSA investigate this issue and require Honda to issue a safety recall and provide a free repair. The multipurpose windshield camera module in my 2019 Honda Odyssey experiences an internal electronic/power failure (Diagnostic Trouble Code U3000-49) while driving. This failure triggers a cascade of warning lights across the dashboard, instantly and without warning disabling the entire Honda Sensing driver-assist suite—including Collision Mitigation Braking (CMBS), Lane Keeping Assist (LKAS), Road Departure Mitigation (RDM), Adaptive Cruise Control (ACC), and Auto High Beams.

Safety Complaint – 2019 Honda Odyssey Elite Front Camera Failure I own a 2019 Honda Odyssey Elite with approximately 60,000 miles. The front windshield camera used by the Honda Sensing system failed. When the camera failed, the vehicle displayed approximately 11 warning messages/errors involving multiple safety-related systems, including the Brake System, VSA, EPS, Electric Parking Brake, Brake Hold, Hill Start Assist, and Honda Sensing features such as ACC, LKAS, and CMBS. This was concerning because one camera failure caused multiple safety systems to become unavailable or display warning messages. The vehicle’s driver-assistance and safety functions were significantly affected. I took the vehicle to a Honda dealer, and the dealer diagnosed the front Honda Sensing camera as defective and replaced it. The total cost was approximately $2,600, including the camera, parts, labor, and calibration. I believe this is an unusually expensive failure for a critical safety component, especially on a vehicle with only approximately 60,000 miles. The front camera is not a normal wear item and is essential to several Honda safety and driver-assistance systems. I am also concerned that similar Honda owners have reported failures involving Honda Sensing cameras. I would like NHTSA to investigate whether there is a recurring or widespread defect involving the front Honda Sensing camera on Honda Odyssey vehicles, particularly model years 2018–2020. I respectfully request that NHTSA review complaints and repair records related to this issue and determine whether further investigation or a safety recall is warranted. I am submitting this complaint because I believe owners should not have to pay approximately $2,600 to repair a potentially recurring defect in a critical safety component.

We were backing up the vehicle and the children started playing around acting like they were pushing the vehicle. The rear camera showed they were on the back corners of the vehicle and I think also the rear camera. As I pulled away all the safety notifications started to appear - adaptive cruise control, lane keep assist, high beam automation and a brake system error. The vehicle was taken to the dealership in Richmond, Kentucky who advised that the issue was the front camera that is in the rearview mirror mount area. I asked how this could be the front camera when everything happened in the rear of the vehicle. They advised they didn't know how but that is what their code readers are recognizing as the issue. They also stated that they have several individuals reporting this issue to them each month but there are no recalls due to this. This seems to be a safety defect as I cannot see how children playing around at the back of the vehicle affects the 'front' camera in turn affecting so many safety items.

In August 202, the multipurpose monocular camera failed. My car is a 2019 Honda Odyssey EXL. I was not involved in any accident or incident that would have caused this to fail. When this camera fails 5 warning codes popup: collision mitigation braking system failure , lane keeping assistance system failure, adaptive cruise control failure, road departure mitigation and auto-high beams stop working. The dealer priced the repair at ~$2,100. Many reports are coming up with this failure and it seems driven by overheating of the camera module, a filed camera component, or other unknown reasons.

I'm filing a formal safety complaint regarding my 2019 Honda Odyssey, VIN [XXX], & requesting an engineering-level review by American Honda. I've brought this vehicle to an authorized Honda dealer numerous times for recurring & increasingly serious electrical, engine/power-loss, & transmission problems. Despite thousands of dollars in repairs, the problems continue. The vehicle has unexpectedly shut off/lost power; the power sliding doors have failed & required manual opening; low-battery warnings have appeared; & the radio/infotainment system repeatedly loses power and resets. I have photographs showing the message: “Anti Theft System–This system has lost power.” More seriously, the vehicle has malfunctioned in Park, Reverse, while shifting into Drive, & while turning. Most recently, the transmission would not engage properly and the vehicle had to be shut off and restarted multiple times before it would go into Drive. Delray Honda documented this transmission failure/no-drive incident on its August 2026 repair order. In May 2026, I brought the vehicle to Delray Honda for a Check Engine Management System warning. Repairs were performed, but the warning returned within days. Delray Honda then replaced the power distribution module, costing me approximately $2,000. I've spent thousands more dollars addressing these recurring problems without resolution. I understand Honda/NHTSA Recall 19V-299 involved certain 2018–2019 Odysseys and a loss of power to the Transmission Control Unit. My Odyssey appears to have been manufactured around the time of those Hondas that were covered by that recall. I'm not claiming without diagnosis that the defect is identical, but the similarity of the symptoms warrants an urgent investigation. I request that Honda open a formal safety case, review my complete service history, DTCs and TCM/PCM data, determine whether my vehicle has the same or a related defect, and review my prior repair costs for reimbursement. Thank you. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT

In December 2025, my multi purpose camera failed on 2018 Honda Odyssey EXL seemingly out of nowhere. There was no accident or incident that would have caused this to fail. With the failure of this system, my collision mitigation braking system, lane keeping assistance, adaptive cruise control, road departure mitigation and auto-high beams no longer work until this camera is fixed. The dealer has confirmed the failure and the repair is over $2,000 to fix by Honda. The component has not been inspected by police or insurance, just Appletree Honda in Asheville, NC. The warning lamps for all of the systems affected show up everytime I start the car.

We have Multipurpose Camera Failure. No damage or accidents. It just started failing. There are many reports of this and the camera is on back order with Honda. They are charging $2100 to fix it. The official Honda Odyssey Forum online has countless complaints on the same issue without resolution by Honda. Honda has acted in bad-faith towards their customers since this defect impacts the vehicle's advanced safety features for which buyers incur additional expenses, endangering the lives of passengers. Honda has acted in bad-faith towards their customers by designing a system that requires continuous and substantial expenditures without resolution because it is an engineering problem, i.e., the placement of the radar behind the large, thick plastic H emblem obstructs the radar. I hereby ask that the NHTSA investigate this issue and order Honda to offer a solution in the form of a RECALL that permanent addresses the problem. Our experience after contacting Honda Corporate to get some help is this is a money maker for them.

I have received warning lights associated with safety features controlled by front facing windshield camera indicating that the ACC, auto braking, lane departure and front collision avoidance features were not working. This has happened once before just over a year ago and I was told by the dealer that the camera needed to be replaced. I did so, at a cost of over $2K. All features worked again after the costly replacement, until last month, just outside of the one year warranty on the new camera. This is clearly an issue with either the camera or a connected electrical issue. This van also has a breach in the body wields at the rear lift gate opening in which water is able to seep into the interior of the body behind the interior rear side panels. During heavy rains the carpet on the right side trunk area is wet and can be tracked to coming from behind the right rear interior molding. I have not yet removed the molding so I am unsure if any electrical components connected to the front camera and associated safety features are located there and might be causing these safety feature issues. At a minimum, both of these issues, the camera constantly quitting, as well as the breach in the body allowing moisture intrusion, should warrant a forced recall for Honda to address and correct each item.
