Recalls, owner-reported complaints, investigations and safety data

Data refreshed

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.
0
InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
0
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

Crash reported

THE VIN IS FOR MY 1996 MAZDA MIATA. MY COMPLAINT IS THE M291 GOING WEST BOUND TO 350 HIGHWAY IN RAYTOWN THAT VEERS TO THE WEST. THE EXIT, SEEMS TO DANGEROUS WITH CONCRETE BLOCKS SEPERATING LANES. MY SECOND INQUIRY IS A POSSIBLE CAR WRECK THAT MY DAUGHTER, [XXX] MAY HAVE BEEN INVOLVED IN. SHE DROVE A GREEN AND BLACK MUSTANG FORD CONVERTIBLE. WE LIVE APART AND AS SHE IS A YOUNG ADULT LIKE MOST, AS A MOTHER, I DON'T GET ENOUGH INFO, SOMETIMES. 2006 FOUND ME WITNESSING HER CAR OR ONE THAT LOOKED LIKE IT OVERTURNED ON 350 HIGHWAY GOING WESTBOUND ON THE CLOVERLEAF TO M291/470 HIGHWAY. AN AMBULANCE WAS THERE AND THE PASSENGER, HER FRIEND OUTSIDE OF THE CAR. I HAVE SEEN HER SINCE THEN, SO OBVIOUSLY SHE SEEMS ALRIGHT AND SAFE, HOWEVER, I WOULD SURE BE APPRECIATIVE IF SOMEONE COULD FOLLOW UP ON THAT IF ANYTHING SEEMS ASKEW. HER ADDRESS AT THE TIME WAS [XXX] WHERE SHE LIVED WITH HER OWN FATHER, [XXX]. HER DATE OF BIRTH IS 1/29/89. I AM [XXX]- OF [XXX]. MY DRIVER'S LICENSE # IS [XXX]. THANK YOU. TAKATA RECALL. AIR BAG LIGHT COMES ON CONTINUOUSLY AFTER TURNING ENGINE ON, NEVER GOES OFF. I BOUGHT THE CAR NEW, AND TOOK IT BACK TO THE DEALER. DEALER SAID THAT IT WAS NOT COVERED ON ANY RECALLS. I WAS INFORMED THAT THERE WAS A RECALL ON THE LATE 1995 MODELS, BUT NOT THE EARLY 1996 ONES. I WAS ALSO INFORMED THAT IT WOULD COST ME $900 TO $1000 TO FIX. AT THAT TIME, IT HAD TO REMAIN UNFIXED, AND THE LIGHT HAS BEEN ON CONTINUOUSLY SINCE. I AM STILL APPREHENSIVE WHEN I TURN THE KEY AND DRIVE IT.

AIR BAG COMES ON AND STAYS ON AFTER STARTING VEHICLE. DIAGNOSTIC CODE INDICATES SRS MODULE IS AT FAULT OR COULD BE ELECTRICAL CONNECTION PER DEALER. DEALER INDICATED SRS MODULE IS NOT AVAILABLE EVEN IF IT IS DETERMINED AS THE FAILURE AFTER REMOVING DASH TO ACCESS MODULE. VEHICLE ONLY HAS 64, 000 MILES. VEHICLE WILL NOT PASS SAFETY INSPECTION IN SOME STATES WHICH MAKES A MINT CAR WORTHLESS AND UNSAFE TO DRIVE.

WHEN I START MY CAR, ALL THE INDICATOR LIGHTS COME ON BRIEFLY AND THEN GO OUT - BRAKES, ABS, SEAT BELT, OIL, ETC. THE AIR BAG LIGHT ALSO COMES ON, AS IT SHOULD. BUT THEN IT STAYS ON AND DOES NOT GO OUT. MY MECHANIC TOLD ME THAT THE PROBLEM WAS DUE TO A FAULTY AIR BAG CONTROL MODULE, WHICH NEEDED TO BE REPLACED. BUT THEY THEN TOLD ME THAT MAZDA NO LONGER MAKES THIS PART AND THAT IT IS NOT AVAILABLE. THEY ALSO SAID THAT THE AIR BAG WOULD NOT DEPLOY UNLESS THE MODULE IS REPLACED AND THAT IT WOULD NOT PASS INSPECTION IN VIRGINIA.

In accordance with Federal regulations, follow the procedure listed below when replacing a speedometer. NOTE: Tampering with a motor vehicle odometer is strictly prohibited by Federal, and State laws. It is the dealer’s responsibility to comply with Federal, State, and local laws concerning odometer and related documentation requirements when providing a repair or replacement.  Mazda vehicles are manufactured with tamper-proof speedometers.  When a speedometer is replaced, the new speedometer will read zero (0) miles.  This procedure is extremely important to accurately represent actual vehicle mileage.  Prior to speedometer removal, complete the information required on the “Speedometer Replacement Label”. Labels can only be ordered through Mazda eStore.  In an effort to achieve a higher level of Customer Experience and retain the value of Mazda’s vehicles, MNAO Warranty Operations is allowing all Mazda dealers to utilize a third party, SPECMO Enterprises, Inc., for Instrument Panel Cluster (IPC) odometer setting service. With the use of SPECMO’s services, it is possible that the odometer will be set between 10 to 31 miles over the odometer mileage at the time of replacement for both warranty and customer pay repairs. Please make sure that the customer is aware that there may be a slight variance to the odometer, in which case the customer may have to accept the slightly higher mileage. Since odometer setting service is not part of the warranty repair, it is not warrantable nor required, but optional. However, Mazda Warranty policy provides after- warranty assistance (AWA) or DSA that can be utilized to cover this service expense.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.

Higher-selling and older vehicles accumulate more reports. Counts are not failure rates and are not directly comparable between vehicles that sold in very different numbers. Older model years have had longer for reports to accumulate.
