Haldex Brake Chamber Failures
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE21018) based on eleven Vehicle Owner Questionnaires (VOQ). These VOQs included seven wheel-end fires on various Kenworth and Peterbilt trucks.Several of the VOQs were submitted by a maintenance facility stating that the fires were due to power spring fractures within brake chambers manufactured by Haldex Commercial Vehicle Systems (Haldex). According to the reports, the fractured spring punctured the brake chamber diaphragm, resulting in a partial loss of compressed air to the park brakes, causing the brakes to drag and overheat. ODI sent an information request letter to Haldex on August 3, 2021. On October 17, 2021, Haldex submitted a complete response. In this investigation, ODI analyzed thousands of warranty claims as well as fleet reports of brake chamber replacements. ODI gathered information from multiple vehicle manufacturers and reviewed legal testimony alleging defects and laboratory reports that raised concerns about spring failures initiated by corrosion. ODI found that spring failures were often detectable as soon as they occurred. In the subject incidents, after detecting the failures, some operators continued driving to a maintenance facility rather than stopping and waiting for a mobile technician to address the issue, increasing the risk of fire. While over twenty entities purchased the subject component from Haldex, the VOQs that ODI received involved only PACCAR vehicles. Haldex completed replacement campaigns for several fleets operating PACCAR vehicles, reducing the risk of further failures. Brake chambers have a limited service life. Industry guidance generally recommends replacement every two to four years depending on operating conditions, with some manufacturers and fleets recommending even shorter intervals. Vehicles operating in salt-belt states—where the majority of VOQs originated—require more frequent replacement than vehicles in less corrosive environments. Haldex’s standard warranty is four years and unlimited mileage. ODI determined that the average time in service for warranty claims was 4.9 years and 260,000 miles. Currently, all of the chambers subject to this investigation are between five and ten years old. Given the absence of recent failures and because the subject components are now older than their expected service life, ODI is closing this investigation. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. Quarter Fender Failures
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE21-015) based on one Vehicle Owner Questionnaire (VOQ) and supporting fleet reports. The reports indicated that the subject quarter fenders were prone to cracking and/or separation.After a comprehensive review of the data related to the quarter fender failures on the subject vehicles, ODI did not identify the existence of a safety-related defect trend. Quarter fender failures resulted in a warranty claim rate less than one percent (<1%) of the total vehicle population. All separations, including those due to tread separations, road hazards, etc., accounted for less than point one percent (<0.1%) of the total vehicle population.ODI discovered that the minimum recommended gap between the top of the quarter fender and the bottom of the trailer was not known or being followed. The optional quarter fenders were supplied by Fontaine Spray Systems (FSS), who recommended a two inch (2) gap. ODI analyzed the claim data using a calculated gap. A calculated gap using the top of the quarter fender flap and assuming the trailer is level with the top of the fifth wheel hitch was necessary, as PACCAR doesn't know specifically what trailer a purchaser will couple to the tractor. It is safe to assume no trailers on the market extend below the upper fifth wheel plate, but many do provide additional clearance over the wheels where the quarter fenders are located. ODI determined that 94 percent of warranty claims and 84 percent of separations violated FSS' recommendation.ODI confirmed that the VOQ vehicle and the fleet report vehicles also violated FSS' recommended gap. In one fleet the gap was negative point three inches (-0.3).PACCAR has updated its vehicle specification process and issued a customer advisory notice to existing owners. In light of the low rate of reported failures and separations, further use of agency resources does not appear to be warranted. The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if warranted by the circumstances.
Intermittent Loss of Power Steering Assist
On May 1, 2020, the Office of Defects Investigation (ODI) opened PE20006 to investigate complaints of intermittent loss of power steering assist in Daimler Truck North America (DTNA), Volvo Trucks of North America (Volvo) and PACCAR vehicles equipped with a ZF/TRW (TRW) THP60 steering gear. The investigation focused on 2010-2020 DTNA Freightliner Cascadia, M2 and WST; Volvo VN, VAH and VHD and PACCAR 337, 365, 389, 567, 579 model vehicles.Field reports and interviews with fleet drivers allege that high power steering fluid temperatures could result in internal seal and bearing failures causing pressure fluctuation, affecting left and right-hand turns. The inability of the driver to steer the vehicle as intended can increase the risk of a crash and injury. ODI analyzed data provided by TRW, DTNA, Volvo, and PACCAR in response to individual information request letters.ODI found that most failures occurred within the first two years and/or 50,000 miles of service. The claim rate decreased with time and miles in service and remained less than 0.5%. ODI normalized data from each of the manufacturers and determined that there was not a substantial difference in the failures.Additionally, TRW improved a seal in the steering gear in December 2015.Trucks built on or after 2016 (as 2017 and newer model years) show a decreasing trend of failures. In view of the overall low failure rate, the nature of early-life failure, and failure rate trending downward for model years 2017 and newer, this Preliminary Evaluation (PE) is closed. Unit 1 towing Unit 2 was traveling eastbound in the inside lane of IH-20. Unit 1 suffered a tire failure resulting in a blowout of the front left steering tire. Unit 1 towing Unit 2 steered towards the center median striking the guardrail and the concrete overpass barrier. Unit 1 and towed Unit 2 then rolled one-quarter roll right-over-left. Unit 1 and Unit 2 became engulfed in flames. Unit 1 came to rest in the center median, facing east, resting on the left/ driver side, with significant burn damage and not drivable. Unit 2 remained coupled to Unit 1 and came to rest in the center median, facing east, resting on the left side with significant burn damage and not towable. The driver of Unit 1 suffered severe, non-life-threatening injuries. Every 12-18 months I need to change something called a flexible pipe part# 1860417. A crack happens to form on the same end of the part. I asked Peterbilt worker about it and he said that they do see this part changed out a lot.

VEHICLE INVOLVED IN WHEEL END FIRE THAT DESTROYED THE ATTACHED TRAILER AND LOAD. DAMAGE TO TRACTOR WAS CONTAINED TO THE REAR CHASSIS AND BACKSIDE OF THE CAB. VEHICLE WAS IN MOTION ON INTERSTATE HIGHWAY WHEN THE DRIVER SAW SMOKE COMING FROM THE TRACTOR DRIVE WHEELS. TIRE BLEW SOON AFTER AND FIRE COULD NOT BE EXTINGUISHED BY ON BOARD FIRE EXTINGUISHER. THE FIRE PREVENTED ACCESS TO FIFTH WHEEL RELEASE. DRIVER WAS UNABLE TO SEPARATE TRAILER FROM TRACTOR TO PREVENT FURTHER DAMAGE AND LOAD LOSS. TOTAL DAMAGES IN EXCESS OF $250K FORMAL FIRE INVESTIGATION AT STORAGE LOT AND SUBSEQUENT LAB INSPECTIONS CONCLUDED THAT A HALDEX 3030 GOLDSEAL CHAMBER TO BE THE ROOT CAUSE. THE PARKING SPRING HAD FAILED AND PUNCTURED PARKING SIDE DIAPHRAGM. THE SUBSEQUENT LEAK CAUSED PARKING BRAKES TO PARTIALLY APPLY WHILE VEHICLE WAS MOVING. THE PRIVATE FIRE INVESTIGATION FIRM HIRED BY THE FLEET PROVIDED X-RAYS SHOWING THE SPRING FAILURE PRIOR TO DISASSEMBLY. THIS FLEET HAS DOCUMENTED NUMEROUS HALDEX GOLDSEAL 3030 CHAMBER FAILURES ON ITS OTHER SAME YEAR,MAKE MODEL TRACTORS. FAILURES ARE OCCURRING AT ABNORMALLY HIGH RATES AND WITHIN THE FIRST 2 YEARS FROM DATE OF IN-SERVICE.
