2022 Rivian R1T

The Office of Defects Investigation (ODI) has received two Vehicle Owner Questionnaires (VOQs) reporting left rear toe link separation in certain Model Year (MY) 2023-2024 Rivian R1S vehicles. Both VOQs report component separation while driving, causing the vehicles to swerve across multiple lanes of traffic. One incident resulted in a collision with an adjacent vehicle and roadside barrier.  In both separations, the bolt that maintains the integrity of the toe link fractured. Evidence collected from the complaint vehicles included repair histories, onboard video, imagery of the damaged components, and a police accident report. The two vehicles with reported separation have different vehicle histories. One vehicle received prior service and the other was involved in a previous collision. In both instances, the vehicles operated for multiple months and thousands of miles of usage with no apparent problems between the previous service or the previous collision and the failures that prompted this investigation. Rivian recognized the subject toe link joint’s sensitivity to service procedures in March of 2025 in its updated service procedures. In January 2026 through recall 26V-003, Rivian extended the improved repair procedure to vehicles that received toe link service prior to the March 2025 improvement. ODI is opening a Preliminary Evaluation (PE) to (1) assess the sensitivity of the rear toe link joint to foreseeable road and service conditions, (2) compare the physical failure evidence from the two VOQs to  identify apparent similarities and potential differences, (3) evaluate Rivian’s current toe link repair procedure, and (4) assess the in-field subject population’s toe links conditions. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

After gently removing the fabric cover of our Cybex Eternis S All-In-One car seat to wash it according to the manufacturer's care instructions, and then putting the cover back on, the car ride immediately following, the red plastic pieces designed to hold the cover in place completely snapped off. This structural failure exposed two sharp, unshielded metal screws directly in the child's seating area. Because these screws were left fully exposed by the broken plastic, they created an immediate laceration hazard. As a direct result of this defect, my young daughter's leg was cut by the exposed screws while using the seat.

SYSTEMS AFFECTED: Electrical System (12V Power Supply / Low-Voltage), Climate Control (HVAC System / Thermal Management), Towing / Recovery DESCRIPTION OF SAFETY HAZARD & DEFECT: My 2022 Rivian R1T experienced a sudden, complete electrical failure due to a defective HVAC system, resulting in a dangerous hard-bricked state that rendered the vehicle totally inoperable. Approximately 30 days prior to the incident, the vehicle underwent a comprehensive inspection at a Rivian Service Center where both the HVAC system and 12-volt battery system were tested and cleared. Shortly after, a primary internal failure/glitch within the HVAC equipment created a high parasitic power draw on the vehicle's low-voltage circuit while parked. The HVAC module failed to enter sleep mode, continuously draining the dual 12-volt AGM battery array below critical operating thresholds. This rapid, deep-discharge condition completely disabled the vehicle's primary high-voltage contactors and low-voltage control modules, locking out all vehicle functions, electronic doors, and drive controls. The vehicle could not be restarted, jump-started normally, or safely shifted into neutral, necessitating an emergency flatbed tow to the service center. Technicians confirmed that the primary HVAC equipment failure directly caused the full depletion and destruction of the 12-volt electrical system. This defect presents an immediate safety risk. An uncommanded HVAC-induced power drain that completely depletes the 12-volt system can cause sudden electrical failure, vehicle bricking, loss of hazard lights, and lock-out conditions, potentially stranded occupants in dangerous environments or busy roadways. Rivian is treating this catastrophic multi-system failure as non-warranty maintenance, refusing to address a clear pre-existing latent defect in their low-voltage power management and thermal architecture. In July of 2025 while driving my entire air suspension collapsed on the highway slamming the vehicle down onto it's bumpstops. Rivian covered a "fix" of this under warrant. In December the rear shock was leaking. Now in July of this year while driving the front air suspension collapsed to it's bump stop again.

The R1T has four cameras to assist the driver and provide warnings for lane changes, etc. The cameras are routinely failing while I am driving the vehicle. This started after an OTA software update. A software reset will turn the cameras back on, though they have repeatedly failed. Rivian attempted to correct the issue by replacing the front camera, which they determined was faulty. The cameras have continued to fail since then.

Vehicle gave a warning to service the electrical system and then shut down preventing any operation. Luckily I was in a parking lot and not on a roadway impeding traffic or jeopardizing safety while driving. Could smell burnt electrical from within cabin. Had to have towed to service center and explained that condensation from air conditioner leaked onto high voltage heater coils and shorted out electrical system. Seems like a poor design that this was even possible. Dealer indicated this was not uncommon.

Subframe has cracked. It is a known issue for my vehicle as the welded subframe isn’t strong enough to hold the weight of my vehicle over time. Thus compromising the structural integrity over time, leading to severe safety hazards like unpredictable handling, poor stability, and potential failure of critical components. This can lead to accidents if not resolved. Rivian currently knows this is an issue and charges customers who are out of warranty over $11k to repair.

12 volt battery was replaced under warranty on date 07/08/2024 at miles 14059. Again, 12 volt battery needs to be replaced at 29343 miles as of today's date 04/12/2026. This is highly unusual. Please look into it. Prakashkumar Patel, MD

Rivian is launching a technical service bulletin for some model year 2022-2026 R1T and R1S vehicles that might have an audible rattle originating from the B-pillar area when driving on rough road surfaces. Upon customer complaint, Rivian will apply anti-rattle tape near both 1st row seat belt retractors to prevent rattling.

Rivian is launching a technical service bulletin for some model year 2022-2024 R1T and R1S vehicles that could have inadequate adhesion between the outer door panel and an internal reinforcement. On affected doors, the exterior door handle might not sit flush with the outer door panel or might move slightly when the handle is operated. Upon customer complaint, Rivian will apply new adhesive between the reinforcement and the outer panel.

Rivian is launching a technical service bulletin for some model year 2022-2024 R1T and R1S vehicles that might have bubbles in the painted surfaces below the exterior door handles. On these vehicles, adhesive that secures an inner door reinforcement could allow contaminants to corrode the inner surface of the door over time, affecting the visual appearance of the door surface. Upon customer complaint, Rivian will repair and refinish the affected door(s). Note: This document supersedes RSB-56-25-002-1. Revision 2 updates the instructions for using the air wedge and inspecting the door panel prior to use.

Rivian is launching a technical service bulletin for some model year 2022-2024 R1 vehicles that may have ultrasonic sensors detached from the front fascia mounting area. After detachment, the ultrasonic sensor may become dislodged and hang inside the front fascia, causing customer notifications for sensor issues and potential sensor damage. Upon ultrasonic sensor detachment, Rivian will perform an ultrasonic sensor repair to secure the sensor to the front fascia.

Rivian is launching a technical service bulletin for some model year 2022-2024 R1T and R1S vehicles that may be susceptible to water ingress in the front HVAC assembly. This may result in damp or soggy carpets in the front of the vehicle. Upon customer complaint of wet carpet in the front of the vehicle, Rivian will inspect the component and replace any damaged parts, and reseal the case to prevent further water ingress.

Rivian is launching a technical service bulletin for some model year 2022-2026 R1T and R1S vehicles that may be prone to noise and/or vibration from the door areas, particularly buzzing and rattling sounds. This results in a perceived quality issue. Upon customer complaint of noise from any door trim panel, Rivian will modify several door trim panel components to alleviate the condition.
