Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

Engine Stall
On August 26, 2010, Toyota submitted a Defect Information Report to the National Highway Traffic Safety Administration (NHTSA) regarding problems with Engine Control Modules that could result in engine stall while driving in certain model year (MY) 2005 through 2008 Toyota Corolla, Corolla Matrix and Pontiac Vibe vehicles equipped with the 1ZZ-FE engine and two-wheel drive and manufactured between April 19, 2004 and January 2, 2008.Toyota's report indicated that cracks may develop at certain solder point or on varistors on the ECMs.According to Toyota, such cracks generally resulted in engine warning lamp illumination, harsh shifting or engine no-start.In some cases, if the cracking occurs on particular solder points or varistors, the engine could stop while the vehicle is being driven.Toyota will notify owners of subject vehicles that they should return their vehicles to a Toyota or Pontiac dealer.Dealers will be instructed to inspect the production number of the ECM for each vehicle and replace the ECM if necessary.On November 30, 2009, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE09-054) to investigate alleged engine stall while driving in model year (MY) 2006 Toyota Corolla and Corolla Matrix vehicles.In its response to ODI's information request, submitted on March 2, 2010, Toyota indicated that it had identified two possible conditions with ECMs used in MY 2005 through 2007 Toyota Corolla and Matrix vehicles:1) cracks forming between the IC and the board caused by an improperly cured conformal coating applied to the circuit boards after the soldering process is completed; and 2) cracks occurring in a variable resistor (varistor) due to overheating.During the manufacturing process a glass coating is created on the surface of the varistor.Between March and August 2010, Toyota began collecting ECMs from the field to further investigate failure modes that may result in engine stall.Toyota also conducted durability testing on ECMs collected from in-use vehicles to assess whether the problems may be a continuing trend. In addition, they conducted durability testing in order to assess whether this problem was a continuing trend.Toyota found that 4 of 32 collected ECMs malfunctioned after thermal shock testing.Toyota also confirmed that cracking could develop at solder points on the ECM circuit board that could lead to engine stall while driving and that this trend was likely to continue.The manufacturer failure data in this resume is limited to MY 2005 through 2007 Toyota Corolla and Corolla Matrix vehicles.Based on Toyota's recall, this Engineering analysis has been closed.

O2 (oxygen) sensor is faulty even after replacing it several times.

These 3 things may be related to each other because they all are connected to the steering wheel. My car can be scheduled for an inspection regarding these safety problems. 1. The Airbag warning light is sometimes on when I drive the car. I'm worried it won't work if I'm in a wreck. 2. The horn doesn't work when I press on the part of the steering wheel that should make it work. It does make a loud sound when I press the panic button on my key fob, but only when the car is turned off. I had to stick my head out of the window and yell at a driver who was getting close to hitting my car in a parking lot. I was parked, and they were backing out of their parking stall. They stopped inches away from my car. 3. The cruise control does not work in the car. I think by using this feature, and maintaining a steady speed, it reduces the chances of accidents. How was your safety or the safety of others put at risk? See answers #1, 2 and 3 above. Has the problem been reproduced or confirmed? #1. No. A dealer told me it would cost me $100 to have them check the airbag light. #2. The horn was checked by an independent service center. #3. No. Has it been inspected? #1. This car has been serviced two different times for recalls for the airbag not working correctly. I wonder if the horn and cruise control wires were damaged, or not put back together correctly, when the recall work was being done, while owned by my sister-in-law. #2. No. #3. No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? #1. The warning lamp for the airbag started coming on around August 15, 2024. #2. No. It didn't work on February 11, 2023 - when I bought the car. #3. No. It didn't work on February 11, 2023.

The contact's client owned a 2005 Toyota Matrix. The contact stated that while her client was driving at approximately 35 MPH, he crashed into another vehicle that had merged into his lane of traffic. The air bags deployed upon impact. Her client claimed that shrapnel was released from one of the air bags upon deployment; however, the claim had yet to be verified by the contact. Upon investigation, the contact discovered that the vehicle was currently under NHTSA Campaign Number: 18V025000(Air Bags) which she linked to the failure. The contact sought medical treatment at the hospital for a severe burn to an arm (arm unknown) as a result of the air bag deployment. He was also treated for pain to his neck, back and both knees. The whereabouts of the other driver were unknown. A police report was filed. The vehicle was towed from the scene to an unknown facility. The manufacturer had yet to be notified of the failure. The vehicle had been destroyed as a result of the crash. The failure mileage was unknown.

I took my vehicle for the airbag recall. When the technician gave me my car back I noticed the airbag warning light was still on. The technician said the warning airbag light is something else but he could not say what. Please add additional recalls to fix passenger airbag. But he said “the light was on before the recall” and I would have to pay to find out what the warning is for. I waited 2 hours for the recall work to be finished and when I entered my car the warning light was still on. This vehicle has not been in an accident. This situation I read in the reviews online is happening to this car and many complaints. I feel unsafe and I think all possible recalls should be put in place for this to be taken care of by Toyota. It is dangerous and it is a liability. Please help me get this resolved with AutoNation Toyota in Winter Park. RECEIVED RECALL NOTICE FROM TOYOTA. CALLED LOCAL DEALERSHIP WHERE PURCHASED - THEY TOOK INFO AND STATED THEY WOULD GET PARTS AND CALL TO SCHEDULE. THEY NEVER CALLED BACK. WE CALLED TOYOTA CUSTOMER EXPERIENCE CENTER TO INFORM WE WERE NOT GETTING OUR RECALL SERVICE. THEY STATED THEY WOULD CONTACT THE DEALERSHIP TO HAVE THEM CALL WITHIN ONE BUSINESS DAY TO INITIATE SERVICE. NO ONE CALLED. I CALLED TOYOTA AGAIN, THEY PUT ME ON HOLD AND AFTER 12 MINUTES ON HOLD I GAVE UP. TOYOTA IS NOT FOLLOWING THROUGH ON THIS RECALL SERVICE WHICH IS A DEADLY CONDITION.

I HAVE TRIED SO MANY TIMES REQUESTING TOYOTA RECALL TEAM FOR REMEDY. THEY CONNECT ME TO THE LOCAL DEALER ON TELEPHONE. THREE TIMES THE LOCAL DEALER GAVE ME APPOINTMENTS. BUT, WHEN I HAVE GONE AS PER THE APPOINTMENTS, THE DEALER TOLD ME THE PART WAS NOT AVAILABLE. IN GENERAL, THE COMPANY UNABLE TO DO THE RECALL REMEDY.

To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
