Recalls, owner-reported complaints, investigations and safety data

Data refreshed

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

Latest Nov 9, 2006
InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
4
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

Fleet Vehicle Recall Completion Audit

Opened Nov 18, 2010
Closed Dec 28, 2015

On November 18, 2010, NHTSA's Recall Management Division (RMD) opened an audit query (AQ10-001) to investigate recall remedy completion by rental car companies on safety recall campaigns. It was expected to provide the agency an indication of how completely and how quickly rental car fleets perform necessary recall-related remedies on the vehicles owned and then rented/leased for use on the roadways.RMD queried three (3) major motor vehicle manufacturers (Ford, Chrysler, and General Motors) and requested data regarding completion figures for fifteen (15) discreet safety recalls.Analysis of this data demonstrated that rental car recalls completion was neither complete nor consistent. The rental car companies representing the largest market share in the industry were then queried as to their respective policies for the rental, lease, and sale of vehicles with open recalls. The data and responses from the manufacturers and the rental car companies can be found in the document repository by accessing the ?Associated Documents link above. In December 2015, Congress enacted the Fixing America's Surface Transportation (FAST) Act.One section of the Act prohibits rental car companies from renting, leasing, or selling motor vehicles subject to open safety recalls. With the passage of the FAST Act and its prohibition, there is no continuing need for the agency to investigate rental car recall completions.Accordingly, this investigation is closed. The agency reserves the right to open any future investigations, including audits of individual recalls, or groups of recalls, or rental car companies, to examine more closely safety recalls performance and completion issues in this area.

ENGINE STALLING

Opened Jun 22, 2006
Closed Nov 29, 2006

IN A NOVEMBER 8, 2006 LETTER, DAIMLERCHRYSLER CORPORATION (DCC) SUBMITTED A DEFECT INFORMATION REPORT TO NHTSA CONCERNING TWO SEPARATE DEFECT CONDITIONS THAT COULD RESULT IN ENGINE STALL WHILE DRIVING IN MODEL YEAR (MY) 2005-2006 CHRYSLER PACIFICA VEHICLES (NHTSA RECALL NO. 06V-432).MY 2005 VEHICLES BUILT IN AUGUST AND SEPTEMBER OF 2004 ARE BEING RECALLED TO CORRECT A DEFECT IN THE SECONDARY FUEL PUMP MODULES, WHICH MAY HAVE AN IMPROPER PRESS FITTING ON THE INLET THAT CAN BECOME DISLODGED AND SHUT-OFF FUEL FLOW.ALL MY 2005-2006 VEHICLES BUILT BETWEEN JULY 8, 2004 AND NOVEMBER 11, 2005 ARE BEING RECALLED TO CORRECT A POWERTRAIN CONTROL MODULE (PCM) SOFTWARE ISSUE WITH THE EVAPORATIVE EMISSIONS SYSTEM WHICH CAN ALSO CAUSE STALLING.FOR BOTH CONDITIONS, ENGINE STALL INCIDENTS TEND TO OCCUR DURING LEFT TURNS. THERE WERE 17,375 VEHICLES BUILT DURING AUGUST AND SEPTEMBER 2004 THAT WERE AFFECTED BY BOTH THE FUEL PUMP MODULE AND PCM SOFTWARE CONDITIONS.THERE WERE 30 VOQS SUBMITTED TO ODI, 58 CONSUMER COMPLAINTS TO DCC AND 829 FIELD REPORTS RELATING TO VEHICLES PRODUCED DURING THIS PERIOD.WHILE THESE ARE MOSTLY RELATED TO THE FUEL PUMP CONDITION, SOME MAY HAVE BEEN CAUSED BY THE SOFTWARE FAULT.THE RESULTING STALL WHILE DRIVING FAILURE RATE FOR THIS PERIOD IS 5,278 INCIDENTS PER 100,000 VEHICLES.THERE WERE TWO CRASHES ALLEGED TO HAVE RESULTED FROM THESE INCIDENTS.THERE WERE 1,021 WARRANTY CLAIMS FOR FUEL PUMP MODULE REPLACEMENT THAT APPEAR RELATED TO ENGINE STALL (5.9% CLAIM RATE). THE POPULATION FOR THE REMAINING VEHICLES INCLUDED IN 06V-432 (I.E., THOSE ONLY AFFECTED BY THE SOFTWARE CONDITION) IS 114,523.THERE WERE 97 VOQS, 124 CONSUMER COMPLAINTS TO DCC AND 454 DCC FIELD REPORTS RELATING TO THESE VEHICLES.THE RESULTING STALL WHILE DRIVING FAILURE RATE IS 589 PER 100,000 VEHICLES.THERE WERE SEVEN CRASHES ALLEGED TO HAVE RESULTED FROM THESE INCIDENTS.THERE WERE 3,211 WARRANTY CLAIMS FOR PCM REFLASH THAT APPEAR RELATED TO ENGINE STALL IN THE TOTAL POPULATION OF 131,898 VEHICLES (2.4% CLAIM RATE). ALL CALCULATIONS ARE BASED ON UNIQUE VINS (MULTIPLE/REPEAT FAILURES NOT INCLUDED IN THESE CALCULATIONS) WITH NO DUPLICATION ACROSS CATEGORIES.DCC'S RECALL WILL REPROGRAM THE PCM SOFTWARE AND REPLACE THE FUEL PUMP MODULES IN ALL AFFECTED VEHICLES.THIS ENGINEERING ANALYSIS HAS BEEN CLOSED.

FUEL TANK PUNCTURES

Opened Jun 15, 2006
Closed Oct 11, 2007

EA06-010 WAS OPENED BASED ON INFORMATION GATHERED IN PRELIMINARY EVALUATION PE06-009.PE06-009 WAS OPENED BASED ON INFORMATION OBTAINED IN A PEER INFORMATION REQUEST AS PART OF A PRELIMINARY EVALUATION (PE05-050) OF FUEL TANK PUNCTURE IN MODEL YEAR (MY) 2003 THROUGH 2006 NISSAN MURANO VEHICLES.IN JANUARY 2006, NISSAN SUBMITTED A DEFECT INFORMATION REPORT TO ODI THAT IDENTIFIED A DEFECT IN THE FUEL TANK SHIELDING IN APPROXIMATELY 209,000 MY 2003 THROUGH 2006 NISSAN MURANO VEHICLES.TO REMEDY THE PROBLEM, NISSAN INSTALLED SHIELDS ON THE EXPOSED FRONT SURFACES OF THE RECALLED VEHICLES.PEER DATA COLLECTED FOR MY 2004 THROUGH 2005 PACIFICA VEHICLES DURING PE05-050 INCLUDED 120 FIELD REPORTS DOCUMENTING TANK REPLACEMENTS DUE TO PUNCTURE OR DAMAGE BY ROAD DEBRIS AS PART OF A PARTS RESTRICTION PROGRAM CONDUCTED AT THE REQUEST OF THE TANK SUPPLIER (INERGY). ANALYSIS OF THE INFORMATION PROVIDED BY CHRYSLER DURING PE05-050, PE06-009 AND EA06-010 SHOW THAT THE FRONT SURFACE OF THE FUEL TANKS IN THE SUBJECT PACIFICA VEHICLES IS NOT COMPLETELY PROTECTED FROMROAD DEBRIS BY THE FLOOR PAN OR OTHER BODY STRUCTURE.THE TANKS WERE COMPARED WITH PEER VEHICLES, INCLUDING SEVERAL IDENTIFIED BY CHRYSLER AS SHARING SIMILAR DESIGN CHARACTERISTICS (UNSHIELDED HIGH-DENSITY POLYETHYLENE PLASTIC FUEL TANKS IN A SADDLEBAG CONFIGURATION).SOME OF THE PEER DESIGNS REVIEWED BY ODI PACKAGED THE TANKS IN RECESSED CAVITIES IN THE FLOOR PAN OR USED SHIELDS TO PROTECT EXPOSED SURFACES ON THE FRONT OF THE TANK.IN GENERAL, THESE TANKS HAD THE LOWEST COMPLAINT RATES FOR TANK PUNCTURE BY ROAD DEBRIS. HOWEVER, OTHER PEER DESIGNS REVIEWED BY ODI HAD PACKAGING SIMILAR TO THE SUBJECT VEHICLES AND SOME OF THESE ALSO HAD SOME COMPLAINTS OF TANK PUNCTURE.NONE OF THE PEER VEHICLES ANALYZED BY ODI HAD TANKS PUNCTURE DATA COMPARABLE TO CHRYSLER'S FIELD REPORTS FROM ITS PART RESTRICTION PROGRAMS, SO THAT DATA COULD NOT BE USED IN PEER COMPARISONS. ANALYSIS OF THE COMPLAINT DATA SHOWS THAT THE RATE FOR THE SUBJECT VEHICLES IS ABOUT HALF THAT OBSERVED IN THE RECALLED MURANO VEHICLES AND NOT SIGNIFICANTLY HIGHER THAN SOME OF THE PEER VEHICLES ANALYZED WITH SIMILAR DESIGN CHARACTERISTICS.TESTING BY THE NHTSA VEHICLE RESEARCH AND TESTING CENTER (VRTC) SHOWED DIFFERENCES BETWEEN THE MURANO AND PACIFICA FUEL TANK PACKAGING AND EXPOSURE TO ROAD DEBRIS THROWN BY THE FRONT TIRES.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL CONTINUE TO MONITOR COMPLAINTS AND OTHER INFORMATION RELATING TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES AND TAKE FURTHER ACTION IN THE FUTURE IF WARRANTED.SEE DOCUMENT FILE FOR ADDITIONAL INFORMATION REGARDING THIS RESUME.

ENGINE STALLING

Opened Feb 27, 2006
Closed Jun 22, 2006

ON FEBRUARY 27, 2006 RQ06-002 WAS OPENED IN RESPONSE TO 60 COMPLAINTS OF ENGINE STALL, MOSTLY DURING LEFT-HAND TURNS, IN MODEL YEAR (MY) 2004 TO 2006 CHRYSLER PACIFICA VEHICLES THAT WERE OUTSIDE THE SCOPE OF RECALL 04V-113.IN MARCH 2004, DAIMLERCHRYSLER INITIATED RECALL 04V-113 ON 34,561 MY 2004 PACIFICA VEHICLES TO CORRECT A CONDITION THAT COULD RESULT IN ENGINE STALL WHILE MAKING A LEFT-HAND TURN AFTER A COLD-START.DURING RQ06-002 IT WAS DETERMINED THAT RECALL 04V-113 DID CORRECT THE SPECIFIC CAUSE OF THE ENGINE STALLING CONDITION FOR THAT SPECIFIC POPULATION.HOWEVER, TWO OTHER CONDITIONS WERE IDENTIFIED THAT COULD ALSO CAUSE ENGINE STALLING DURING TURNING MANEUVERS:(1) AN ISSUE WITH MISBUILT FUEL PUMPS THAT PRIMARILY AFFECTED ABOUT 17,000 MY 2005 PACIFICA VEHICLES BUILT DURING AUGUST AND SEPTEMBER 2004; AND (2) A SOFTWARE CONDITION AFFECTING VAPOR PURGE IN ABOUT 131,000 MY 2005-06 PACIFICA VEHICLES BUILT FROM JULY 2004 THROUGH NOVEMBER 2005. AN ENGINEERING ANALYSIS, EA06-013, HAS BEEN OPENED TO FURTHER EXAMINE THE SCOPE, FREQUENCY AND SAFETY CONSEQUENCES ASSOCIATED WITH EACH OF THESE CONDITIONS IN THE SUBJECT VEHICLES.

Bought car from a private seller and wasn't informed of MASSIVE and UNSAFE DAMAGE TO ENGINE CRADLE. The engine cradle is nearly completely rotted out. Don't know if rust has eroded other parts until a mechanic can look extensively at the engine cradle. I have been trying salvage yards and 9 out of the 10 Pacifica's I have inquired about the engine cradle is rotted beyond repair. It has to be replaced. The fee for replacing and labor is going to be anywhere from $400 to $2500. and the vehicle has only 153÷k on it. This is a huge and reoccurring issue with the Chrysler Pacifica after I did research.

I got in the car started it as usual, shifted it in (D), pulled out of my church parking lot, got about 5 miles away from my church stopped at a red light. The light turned green I accelerated as usual. At this light is a slight incline about 15% ways up the incline it just stopped pulling. I pressed the gas heat the engine Rev but no power to the wheels. So I shifted it in auto stick mode and it wouldn’t change out of second gear I left it on the highway for about 3 days.I went back trying to reset the trans by turning the switch on and holding the gas pedal for 30sec as seen in Chryslers forum and YouTube yet still no power had the car towed back to Alabama when I go to church in Tennessee which was about 50 miles away or so. Get it home after paying a huge towing bill. Because my insurance only covered like 15miles. I got it home try to shift it in R, D, and The Autostick. however, it finally started going back to 1st gear. Unfortunately it still wouldn’t pull. I’ve looked up the problem and there is a recall for this, However when I call Chrysler it doesn’t cover my vehicle

A few weeks ago we took our car to a mechanic to have the brakes worked on as a part of routine maintenance. When we picked it up, he noted that when he had it up on the lift, he saw that the engine cradle was very badly rusted. He said that we shouldn't drive it anymore because of the risks involved. I tried to crawl under the car when we got home (there isn't a ton of clearance) but I saw what he was talking about. At that point we looked online and found hundreds and hundreds of similar experiences and complaints. We were left dumbfounded at the risks this introduces...thinking about what would happen if this mechanic hadn't noticed the issue and we'd been driving down the interstate with our kids when the thing decided to give way. And the fact that the manufacturer knows of this issue and has done nothing is beyond disgusting. Fortunately we found out from the mechanic, not from the engine falling out. Even still, this means that we're left with a worthless car. While our car is abandoned in the driveway hoping for Chrysler to right their wrong, we still see lots of similar vehicles on the road. I'm guessing that their owners don't know that they're driving death traps either. We had a 2008 Tacoma with similar frame rust issues and Toyota recalled it almost a decade after it was manufactured, and replaced the entire frame. Chrysler should have to do the same to repair their shoddy and negligent design, or at least compensate us for the complete loss of value so that we can have some money to put towards its replacement. I also think that this issue needs to be publicly noted for all similar model years. We'd purchased this from a Chrysler dealership, and had checked the Carfax and Consumer Reports before purchasing this vehicle, and none of them noted these issues even though the vehicle was a few years old at the time. Vehicle has been repaired.*JB

If the holes are damaged or elongated the transmission should be replaced to prevent further damage to the transmission/flexplate or pump bushing due to the inability to properly align the engine to the transmission during the repair. 3. All 62TE equipped vehicles. If pump bushing is scored or damaged and the dowel pin alignment holes are ok, replace the pump assembly as opposed to complete transmission. 4. All 62TE equipped vehicles. If vehicle is jerking/slipping with MIL code(s) P0740 or P2764, check TCC Solenoid operation. If TCC Solenoid is ok, the codes are indicative of a torque converter failure. Replace the torque converter.
