NHTSA Vehicle Safety Complain To whom it may concern, My Name is [XXX] I am submitting this complaint regarding my 2019 Ford F-250 Super Duty equipped with the 6.7L Power Stroke diesel engine (VIN: [XXX]). At approximately 137,900 miles, my vehicle suddenly lost engine power while driving and stalled without warning, creating a serious safety hazard. After stalling, the truck would not restart and had to be towed to a repair facility. The repair shop diagnosed a catastrophic failure of the diesel high-pressure fuel injection pump. According to the repair invoice, fine metallic flakes were found throughout the entire fuel system, contaminating the fuel lines, fuel tank, injectors, and related components. Because of this contamination, the shop determined that the entire fuel system required replacement, including replacement of all eight fuel injectors, cleaning of the fuel tank and fuel lines, replacement of the fuel filter, and installation of an upgraded diesel injection pump. The estimated repair cost exceeded $11,000, resulting in a significant financial burden. I understand that Ford Safety Recall 24S78 addresses a high-pressure fuel pump issue on certain 2020–2022 Ford Super Duty trucks equipped with the same 6.7L Power Stroke diesel engine. Although my 2019 model is not included in the recall, my vehicle experienced what appears to be a very similar failure that resulted in sudden loss of engine power while driving and complete contamination of the fuel system. I am not asserting that my vehicle is covered by the current recall. Rather, I respectfully request that NHTSA investigate whether the same or a substantially similar defect may also affect certain 2019 Ford F-250 Super Duty vehicles equipped with the 6.7L Power Stroke diesel engine. If the investigation determines that these vehicles are affected by the same safety defect, I ask that the recall be expanded accordingly. The contact owns a 2019 Ford F-250 SD. The contact stated that while driving 70 MPH, there was an abnormal diesel odor on the inside of the vehicle. The contact stated that the vehicle failed to exceed 65 MPH, and there was white to gray smoke coming from under the hood. The contact pulled over with the engine still running and after opening the hood, the contact became aware of diesel fuel squirting out from the secondary fuel filter. The contact stated that the fire consumed the vehicle. The fire was coming from underneath the driver's side hood and driver’s side wheel well. The Fire Department was contacted and extinguished the fire coming from the vehicle. The vehicle was towed to an impound lot, and it was deemed a total loss by the insurance company. The manufacturer was contacted by phone and was made aware of the failure. The failure mileage was approximately 144,000.

I experienced a severe, violent, and uncontrollable front-end suspension and steering wheel oscillation—commonly known as the "Ford Super Duty Death Wobble." While traveling at highway speeds approximately 70mph, I came across a bridge expansion joint (nothing unusual) and immediately upon impacting the joint, the front axle began shaking so violently that it caused a sudden, near-total loss of steering control. The violent shaking made it nearly impossible to maintain a straight lane of travel, risking a catastrophic multi-vehicle collision. The steering wheel jerked violently side-to-side, forcing me to rapidly and dangerously decelerate in active highway traffic to regain control of the vehicle. This defect created an extreme, unpredictable safety hazard. I immediately let-off the accelerator with some braking to stop the oscillation and regain control. A local specialty alignment shop previously attempted an adjustment, as I had experienced this a couple of months before (although not as severe as this most recent incident) on [XXX] in eastern [XXX]. The adjustment temporarily masked the underlying issue for approximately 2.5 months before the violent oscillation returned as I am reporting now. The local Ford dealership (Victory Ford) has confirmed that the vehicle qualifies for Customer Satisfaction Program 20B02 (Steering Linkage Damper Replacement), demonstrating the manufacturer's awareness of this dangerous defect. However, no formal safety recall has been issued. Aside from Fords damper replacement online research and an independent alignment shop recommends replacement of track bars, bushings, steering damper, etc. with heavy duty parts. I am filing a complaint against Capital Auto Mall Premier regarding the sale of a 2019 Ford F\-250 Lariat \(VIN [XXX]\) that I purchased on XXX. The vehicle was represented as being in good working condition at the time of sale. However, almost immediately after taking possession of the vehicle, I began discovering numerous mechanical, electrical, and safety\-related defects. Due to the number of concerns, I had the vehicle independently inspected shortly after purchase. The inspection identified multiple significant issues, including transmission hard shifting, transfer case clunking during shifting, battery failure requiring replacement, steering and suspension concerns, excessive play in front suspension components, and numerous diagnostic trouble codes stored in the vehicle's systems. In addition to these findings, I discovered issues involving vehicle lighting, winch operation, and other features and equipment that were not functioning properly despite being expected to be operational at the time of sale. These issues became apparent within days of purchasing the vehicle. The quantity and severity of the defects lead me to believe that the vehicle was not in the condition represented during the sale and that material defects may not have been properly disclosed before the transaction occurred. As a result of these issues, I have incurred costs associated with inspections and diagnostics and am obtaining additional repair estimates to determine the full cost of necessary repairs. The defects significantly impact the vehicle's safety, reliability, usability, and value. I have supporting documentation, including the purchase agreement, financing documents, independent inspection reports, diagnostic findings, photographs, repair estimates, and records of communications regarding the vehicle. The contact owns a 2019 Ford F-250. The contact stated that while driving approximately 65 MPH on a cross-country move, the rear windshield was loose and was shaking inside the frame. The contact stated the rear glass remained attached and hanging within the frame. The vehicle was taken to a dealer to be diagnosed. The dealer replaced the rear window frame and window bracket, and the vehicle was repaired. The manufacturer was notified of the failure but offered no assistance. The failure mileage was approximately 80,000.

At around 139,000 miles i started to noticed some intermittent noise coming from the drivetrain. I took it to two different repair shops and was told it was tire noise. over time the grinding noise increased to the point of shuddering and jerking while shifting with no warning lamps showing. At 144,500 miles I contacted my aftermarket warranty and they said to take it to a certified shop to get it checked out. They did not have any preferred shops in my area so I took it to US Automotive. At 145,000 miles the shop contacted the warranty company and proceeded with investigation of the issue. After inspection and tear down they confirmed the transmission had the typical failure for the 10R80 10-speed automatic transmissions. They turned over there findings and the warranty company which has agreed to the findings and has offered a replacement used transmission with 125,000 miles on it and will also not cover the full cost to install the used unit. I feel that this is fraud and outside of the warranty agreement. I am now paying the expense to put the original transmission back into the truck to try to take it the 2.5 hours back to the dealership which sold me the truck along with the warranty to see what they will do in the matter. I hope it doesn't fail on the highway on the way.

My truck transmission changes gears randomly while in motion. It is terrifying when it happens in traffic and the truck stalls and looses power then kicks back in with a loud noise. I stopped driving it since March of 2026.

Original Fuel pump stated intermittent long cracks about 35,000 miles. And continues to do so. It seems like FORD uses the all time favorite “ we cannot recreate the issue” in order to send the customer on their way and not take ANY responsibility whatsoever ,WOW! Its rediculous that they don’t want to acknowledge the issue after thousands of customers complain. Our whole family has been riding in this vehicle when the truck experienced the truck lag and turn off at highway speeds. Does someone have to be hurt or killed by this for this company to finally own up? I hope none does!!! Make it right FORD!!! Yet they changes and updated the newer trucks pumps after this, why? REDICULOUS

While driving at highway speeds, I drove over an expansion joint on a bridge and experienced violent shaking of the vehicle. This included violent shaking of the steering wheel and interior of the truck. I also slowed down to under 30 mph on the highway to stop the shaking. This occurred several other times and, upon researching, found it to be called a "death wobble". I brought my truck to a Ford dealership service center and mentioned the death wobble, which was immediately acknowledged. This cost $1758 out of pocket to fix. From research I have done, it seems all suspension components should not need any type of repair or replacement prior to reaching 50,000 miles, with some components lasting as much as 100,000 miles.

My husband was driving our 2019 F250 on the highway at over 75 mph and the truck totally shut down/stalled without warning, could have killed him or another person, lucky he was able to coast to the shoulder and stop, we took to the Ford dealership and was caused by the faulty CP4 fuel pump the truck was made with that has had class action lawsuits and recalls, we are out of pocket over $14,000 to rebuild entire fuel system due to the fact that fuel pump is NOT compatible with our diesel fuel leading to metal shavings build up in fuel system. Our truck still has no recall on this and it should and we should be reimbursed for the extremely expensive and lengthy repair, this is a very dangerous issue. The mechanic also stated he "duplicated" the stall when he go on the highway and hit speeds over 60 mph, thank goodness he is ok as well.

The contact owns a 2019 Ford F-250 equipped with Cooper Tires, Tire Line: Discoverer AT3, Tire Size: 295/60/R20, DOT Number: (N/A). The contact stated that while driving 60 MPH and hauling a trailer, the vehicle jolted into oncoming traffic and started wobbling. The contact regained control of the vehicle and eventually steered to the side of the road and parked the vehicle. The contact became aware of a flat spot on the tire, and the spare tire was placed on the driver’s side front. The contact continued driving, and the wobbling became controllable at speeds under 50 MPH. The vehicle was taken to the residence, where the contact observed that the driver's side front tire had tire separation in the middle of the tire. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The tire failure mileage was 37,000. The vehicle failure mileage was 52,000.

Without warning while I was driving at ~70 mph on the highway the engine in my truck died and left me with no power brakes or power steering in busy traffic. This is a very scary situation to be in. I pulled over and checked the truck best I could. Started it back up and carefully pulled back out onto the highway when it died again. I took it to the closest Ford service center and after diagnosis, they told me the CP4 pump is failing and will need to be replaced at a cost of ~$14,000. My research shows this is a common issue for Ford for a certain year range which my truck is in and they have recently paused the recall addressing the issue so the recall is not showing up when searching my VIN. I would appreciate Ford claiming that this is a safety issue and reimburse me for my out of pocket costs.

2019 Ford F-250 original fuel pump or related part connected cause hard and prolonged starting in vehicle. Prolonged starts can damage other parts if left unfixed. This is a Ford manufacturing issue and could cause a person possible harm from an unknown development from the pump and related affected parts. Dont wait until humans are injured to address this problem especially when dealing with potential combustible explosive materials. 2019 F-250 6.2 84k miles fuel pump check valve has gone back no a new issue it’s all over the web with problems most are seeing it with even 40k when you put a new one in you get less then that or equal amount and it goes out.

The contact owns a 2019 Ford F-250. The contact purchased four aftermarket steel rims for the vehicle. The vehicle was taken to have the rims balanced, and the contact was advised by the technician that the steel rims were not rated for a 3/4-ton vehicle. The contact stated that the seller's website had listed the rims as compatible for a 3/4-ton vehicle. The contact returned the rims to the seller; however, the contact felt that the seller should have correctly identified the wheels for the correct vehicle weight, because the vehicle was used to haul trailers.

The contact owns a 2019 Ford F-250. While driving at approximately 20 MPH, there was an abnormal sound coming from underneath the vehicle, and the check engine warning light illuminated. The vehicle was taken to a dealer to be diagnosed; however, the dealer stated that the cause of the abnormal sound and the illuminated check engine warning light was unknown. The contact was notified of an unknown recall; however, the VIN was not included. The vehicle was not repaired. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline to have the VIN added to the recall. The failure mileage was approximately 205,000.

Backup camera distorted, wavy lines, and sometimes blank screen. I have seen other trucks of same year and model that have recalls and dont understand why mine don't. It happens all the time.

We believe the fuel line came off of the fuel filter. The truck caught fire and went up in flames. There is nothing left of the truck other than the frame. CHP and the fire department came to the scene. They put the fire out and the truck has been taken to the tow yard. Nothing has been inspected yet.

The back window shattered after turning on the rear window defroster. This put back seat occupants in harm. It impaired my vision. There were no warnings to this failure. My insurance company has been notified. Independent auto repair & glass repair shops have inspected the damage. Window must be replaced.

After sitting for extended durations the engine experiences a no start or long crank condition. Research and discussion with local mechanic indicate this is an issue with a failed/failing fuel pump and/or fuel pump module. It is my understanding a recall exists for similar failures on other ford models.
