Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

firstly, WHY, would a vehicles doors need to be opened while going down the road? WHY, should I have to apply the child lock to the door to ensure that my passengers do not open them? Does the passenger door have a child lock? shouldn't the doors stay locked while the vehicle is operating? please offer me some type of explanation as to why the doors have to have the ability to be unlocked and opened by any passenger

I recently bought a 2007 Infiniti FX35 and the dashboard is completely melted away. It's sticky and there are cracks from the driver side to the passenger side. This is dangerous as it can cause the airbag to deploy! The previous owner has a dashboard cover on it which I can't remove and take pictures of because it's STUCK by the melting material

Driver Seat detached from floor causing great risk to operator, passengers and other vehicles or pedestrians. Seat frame broke previously and was reported to Jackie Cooper Infiniti. They requested I report it to corporate Infiniti which I did but any relief was denied. The broken frame was a cause for concern but last evening the seat became loose to the point where the rear can be lifted now from the floor.

THE ENTIRE DASH OF MY VEHICLE IS COVERED WITH BUBBLES BECAUSE OF AN INFERIOR PRODUCT IN PRODUCTION. THESE BECOME WORSE EVERY SUMMER BECAUSE OF THE HEAT AND THE DASH BECOMES A GOOEY MESS. HOW CAN I BE SURE THAT THE AIRBAG WILL DEPLOY OR DEPLOY CORRECTLY WHEN THIS MESS IS ALL OVER IT? I WAS NEVER MADE AWARE OF ANY RECALL, BUT WHEN CONTACTING THE COMPANY WAS TOLD IT WAS PAST DEADLINE AND THEY WOULD DO NOTHING. I WANT TO BE SURE THAT IF THE AIRBAG IS NEEDED, THAT IT WILL WORK CORRECTLY.

I'VE CALLED INFINITI AND ANOTHER NUMBER ON A LETTER I KEEP GETTING 877-481-0083 MANY, MANY TIMES AND TOLD THEM I HAVE NOT OWNED THIS CAR FOR OVER 15-YEARS AND FOR THEM TO TAKE MY NAME OFF THE LIST. NO LUCK AND THEY ARE WASTING TIME AND MONEY AND NOT FINDING THE REAL OWNER!!!

I HAVE HAD MY CAR FOR 4 YEARS NOW AND IT WAS GARAGE KEPT WITH ONE PREVIOUS OWNER SO I'M TOLD. SINCE I'VE HAD THE CAR I HAVE NOT BEEN ABLE TO PROVIDE A COVERING FOR THE VEHICLE. FOR THE FOUR YEARS I HAVE HAD THE DASH FEEL LIKE IT'S MELTING. IT'S STUCK TO ANYTHING I'VE PUT OR LAID ON IT AND CAUSES CRACKS AND RUN LINES WHERE THE CRACKING HAS TAKEN PLACE. DURING COLD MONTHS IT SHRINKS AND DURING THE SUMMER MONTHS EXPANS AND STARTED FEELING STICKY AND MELTING. HAS MADE SOME BUTTONS STICK THAT ARE CLOSE THE THE DASH BEFORE SO HAVING AN INCONVENIENT OF BEING STUCK ON SHUFFLE FOR MUSIC IS A THING THRU THE SUMMER. OVER TIME IS GETTING WORSE AND I CAN'T USE ANYTHING TO CLEAN THE STICKY STUFF BECAUSE I'M AFRAID TO MAKE IT WORSE. WILL SEND IMAGES WHEN SOMEONE CONTACTS ME ABOUT THE PROBLEM THRU EMAIL. TL* THE CONTACT OWNED A 2007 INFINITI FX 3.5. THE CONTACT STATED THAT WHILE DRIVING AT 35 MPH, SHE ATTEMPTED TO DEPRESS THE BRAKE PEDAL BUT HER BRAKES FAILED AS THE CHECK ENGINE LIGHT ILLUMINATED. DUE TO THE FAILURE, THE CONTACT CRASHED INTO ANOTHER VEHICLE IN FRONT OF HER. THE AIR BAGS DID NOT DEPLOY UPON IMPACT. A WITNESS INFORMED THE CONTACT THAT THE BRAKE LIGHTS WERE ON BUT NOTICED THAT HER VEHICLE WAS NOT COMING TO A STOP. THE PARAMEDICS WERE CALLED TO THE SCENE AND THE CONTACT AND HER TWO CHILDREN WERE TRANSPORTED TO A LOCAL HOSPITAL VIA AMBULANCE. THE CONTACT WAS TREATED FOR INJURIES TO HER NOSE, RIGHT KNEE, AND RIGHT SHOULDER. THE CONTACT'S SON WAS TREATED FOR MULTIPLE SCARS TO THE LEFT SIDE OF HIS BODY, HE ALSO EXPERIENCED SORENESS AS A RESULT OF THE ACCIDENT. THE CONTACT'S OTHER CHILD DID NOT SUSTAIN ANY INJURIES; THE OTHER DRIVER WAS NOT INJURED IN THE ACCIDENT. A POLICE REPORT WAS FILED AT THE SCENE. THE VEHICLE WAS TOWED INITIALLY TO AN INDEPENDENT TOW YARD AND THEN TO THE CONTACT'S HOME. THE CONTACT'S INSURANCE THEN HAD THE VEHICLE TOWED TO ANOTHER INDEPENDENT MECHANIC FOR INSPECTION; THE VEHICLE WAS THEN DETERMINED TO BE TOTALED. THE DEALER NOR THE MANUFACTURER HAD BEEN NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 100,000.

I WAS DRIVING ON A HIGHWAY AT 55MPH AND STARTED TO BRAKE TO MAKE A RIGHT HAND TURN AND THE ENGINE DIED. THIS HAPPENED TWICE IN TWO DAYS. THE CHECK ENGINE LIGHT ALONG WITH THE SLIP AND VDC LIGHT CAME ON. THIS TURNS OUT TO BE A COMMON PROBLEM WITH NISSAN AND THEY RECALLED SOME CARS FOR IT. IT'S EITHER THE CAMSHAFT OR CRANKSHAFT POSITION SENSOR. THIS IS A VERY SERIOUS SITUATION. IM JUST GLAD I WASN'T DRIVING ON THE INTERSTATE WHEN THIS HAPPENED. SHOULD BE A FIX OR RECALL ON ALL CARS AFFECTED BY THIS.

This bulletin lists the criteria and provides an application form for such replacements. · The “VIN Plate/Label” (metal plate or vinyl label) is attached to the instrument panel or body on the driver side at the base of the windshield (see Figure 1). · The “Certification Label” (vinyl adhesive label) is located on the lower area of the driver side B-Pillar (see Figure 1).
