Recalls, owner-reported complaints, investigations and safety data

Data refreshed

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
1
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

The defect: Isuzu Technical Center of America, Inc. (Isuzu) is recalling certain 2015-2017 Isuzu NPR, Reach, 2016-2020 Isuzu NPR XD, NPR HD, NQR, 2016-2021 Isuzu NRR, 2017-2020 Chevrolet 4500XD, 4500HD, 5500HD, and 2020 Chevrolet 5500XD vehicles. The U-bolt nuts for the rear axle may not have been properly tightened and may loosen, causing the axle to misalign.
The risk: The misalignment may cause the propeller shaft to separate, causing a loss of power to the rear wheels and a stall, increasing the risk of a crash.
The remedy: Isuzu will notify owners, and dealers will inspect and, if needed, replace and tighten the rear axle U-bolt nuts, free of charge. The recall began on April 23, 2021. Owners may contact Isuzu customer service at 1-866-441-9638. Isuzu's number for this recall is V2103.

The defect: Isuzu Technical Center of America, Inc. (Isuzu) is recalling certain 2011-2017 Isuzu NPR, 2008 NPR HD and NPR-HD Gas, 2011-2018 NPR HD and NRR, 2015-2018 NPR XD, 2012-2018 NQR, 2012-2017 NPR Gas and NPR-HD Gas, 2016-2017 Chevrolet Low Cab Forward 3500 HD, 3500 and 4500 and 2017-2018 Chevrolet Low Cab Forward 4500 HD, 4500 XD, 5500 HD and 5500 XD vehicles, equipped with certain Walter Kidde Plastic-Handle Fire Extinguishers, models FC10 and FC110. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or excessive force may be needed to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable.
The risk: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.
The remedy: Isuzu will notify owners, instructing them to contact Kidde to get a replacement fire extinguisher, free of charge. The recall is expected to begin in December 2017. Owners may contact Isuzu customer service at 1-866-441-9638 or Walter Kidde customer service at 1-855-262-3540 or they may also visit www.kidde.com and click on "Product Safety Recall" to order replacement fire extinguishers free of charge. Isuzu's number for this recall is V1705. Note: This recall includes fire extinguishers provided as a remedy for previous Isuzu recall 15V778.

The defect: Isuzu Technical Center of America, Inc. (Isuzu) is recalling certain 2011-2016 NPR, 2011-2017 NPR-HD, NQR, NRR, 2015-2017 NPR-XD, 2012-2016 Reach, NPR Gas and NPR-HD Gas incomplete trucks. The left rear wheel bearing hub locking bolts may not have been properly tightened during assembly.
The risk: The loose bolts can allow the wheel assembly to move outward from the axle housing, affecting the control of the vehicle and reducing braking performance, thereby increasing a risk of a crash.
The remedy: Isuzu will notify owners, and dealers will inspect the left rear wheel bearing hub locking bolts, tightening them as necessary, free of charge. The recall began on April 19, 2017. Owners may contact Isuzu customer service at 1-866-441-9638.

Unintended Movement

Opened Apr 19, 2019
Closed Dec 9, 2021

The Office of Defects Investigation (ODI) opened this investigation on April 19, 2020. ODI was made aware of seven consumer complaints related to model year (MY) 2012-2015 Isuzu N-series gasoline-powered trucks for unintended movement. ODI worked with Isuzu and peer manufacturers to understand the severity, failure mechanism and frequency of the issue. Attached is a report outlining these details, in addition to the investigation's chronology. This investigation is being closed with Isuzu releasing a technical service bulletin (TSB), with owner notification, to resolve the unintended movement issue in MY 2012-2017 vehicles. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency will continue to monitor the issue and reserves the right to take further action if warranted by new circumstances. Please review the attached closing report for full details.

Crash reported

Vacuum pump on the motor that supplies vacuum to the brake booster. We have replaced this pump (Isuzu PART #: 97241035) on this vehicle 3 times in less than a week. These parts were Isuzu brand and the exact part number for our VIN and application. Went to hit the brakes, the brakes were not working and extremely hard to press, and not slowing the truck down. Driver ran into another vehicle. Yes, the problem has been reproduced multiple times by our internal tech and local Isuzu dealer. Also can be found on different forums with exact issue (see attached). Each time the part has failed, we have sent it back to dealer. No prior warning until the brakes were applied and not working.

EQUIPMENT:OTHER:OWNERS/SERVICE/OTHER MANUAL

This bulletin provides information with regards to the Isuzu Commercial Vehicle (CV) Service Policies and Procedures Manual (SPPM). The Isuzu CV SPPM (sections 2.15 and 2.16) provides detailed procedures related to vehicle storage up to and beyond 30 days. Performing these required procedures at the specified intervals will properly maintain vehicles until they are retailed. Failure to follow the procedures outlined may result in repairs and additional costs that could have been avoided. We are providing dealers with this brief overview of the recommended procedures as a reminder to maintain older inventory. Failures resulting from improper, or lack of, storage procedures are not covered under the vehicle factory warranty.

This bulletin outlines the procedures for using the Alpine Exchange Program (the "Exchange Program"). The Exchange Program must be used when any Alpine radio or other component covered by this bulletin is repaired under warranty and may also be used for radio and component repairs not covered under warranty. Contact Alpine directly for pricing and payment methods applicable to radio and component repairs not covered under warranty.

ELECTRICAL SYSTEM; EXTERIOR LIGHTING:TURN SIGNAL

We have received reports of the front turn signal lenses cracking on Isuzu vehicles. These lenses are cracking from the effects of being washed with alkaline detergents. Alkaline detergents are typically used in industrial settings and are reactive with the paint, metals, and other materials used to make up the exterior components of Isuzu vehicles. Isuzu recommends that only neutral detergents be used when washing Isuzu vehicles. However, if alkaline detergents are used to wash an Isuzu vehicle, it is crucial that all traces of the detergent are rinsed away with water or wiped off as necessary. Allowing alkaline detergent to dry on the turn signal lenses will cause them to crack. Each Isuzu vehicle is originally equipped with an Owner's Manual that includes guidelines for the proper exterior maintenance of the vehicle. The following guidelines are typical of those found in an Isuzu Owner's Manual concerning the use of detergents on a vehicle's exterior.

The following is a list of the Isuzu-recommended Threebond product part numbers identified in the various Isuzu Workshop Manuals and bulletins. If a Threebond product is unavailable, we recommend that you utilize the chart below to find equivalent of similar products (when available) from Loctite and Permatex.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.

Higher-selling and older vehicles accumulate more reports. Counts are not failure rates and are not directly comparable between vehicles that sold in very different numbers. Older model years have had longer for reports to accumulate.
