I am filing this complaint regarding a manual transmission failure on my 2017 Mazda MX-5 Miata (VIN: [XXX] ) at 55,000 miles. The failure occurred without warning while driving, causing immediate and significant transmission malfunction. Mazda issued Technical Service Bulletin 05 [XXX] c addressing this exact defect — defective synchros in 2nd and 3rd gear causing grinding, inability to engage gears, and transmission failure. My VIN falls within the TSB coverage range. The cutoff is sequential number XXX and my sequential number is XXX, placing me within the affected range. Despite this documented defect and my vehicle being under Mazda's Powertrain Limited Warranty at time of failure (under 60,000 miles), Mazda refused full coverage. The dealer offered only 50% goodwill. I escalated to the Mazda regional representative, who also declined full coverage and redirected me back to the dealer — evasive given the documented nature of this defect. This is not isolated. Multiple 2016–2017 MX-5 owners have reported identical failures, and a Mazda engineer publicly acknowledged on video that weak transmission tolerances cause gear teeth to shear under load. Active petitions are calling for a formal recall. While I was able to drive to the dealership, this failure occurred without warning. A similar failure at highway speeds or in a less favorable situation could result in complete loss of vehicle control, posing significant danger to the driver and surrounding traffic. I request that NHTSA: (1) investigate this defect across all 2016–2017 MX-5 Miata manual transmission vehicles, (2) determine whether a formal recall is warranted, and (3) require Mazda to honor full warranty coverage for VINs within the TSB range. Happened at US 101 Southbound 0.1 MILES SOUTH of RENGSTORFF AVENUE. “Power Steering Malfunction” light came on, then suddenly after 1 second, the car’s steering went stiff and suddenly braked while at freeway speed flow of traffic. Car then sharply veered left into center divider due to automatic sudden braking with no control of the steering wheel

I have 205/45/R 17 on a car that I drive on short trips maybe once a week or two. Driving on the highway, the tpms light came on and the handling was instantly strange. I pulled over and checked the tire, the sidewall was blown out on the passenger rear. I had it replaced. Now about 2 months later, the right rear went flat after a trip. I took it to be repaired and was told that the sidewall was splitting and it could not be repaired. I am concerned that this may not be coincidence.

Vehicles rear windshield spontaneously shattered. Vehicle was idle and upon starting the vehicle a cracking sound was heard in the cabin and the rear view mirror was unusable as the rear windshield was entirely shattered into <1 inch pieces. Small pieces of loose glass had fallen from both upper corners of the windshield. The windshield is still attached to the vehicle and rear visibility is severely limited. No third-party inspections have been executed. There was no previous damage to the windshield and the car was idling when the failure occurred. There were no indications of potential failure before the incident.

THE WINDSHIELD CRACKED WHILE THE VEHICLE WAS PARKED IN A LOT. THE CRACK APPEARS TO HAVE STARTED ON THE PASSENGER SIDE EDGE OF THE WINDSHIELD AND RAPIDLY PROPOGATED HORIZONTALLY ALONG THE CENTER OF THE WINDSHIELD TO ROUGHLY THE CENTER OF THE GLASS. THE CAR HAD 12000 MILES ON IT WHEN THE WINDSHIELD CRACKED.

AT 8300 MILES THIS CAR HAS DOZENS OF ROCK CHIPS IN THE WINDSHIELD, MOST OF WHICH WERE PICKED UP FROM DRIVING ON RELATIVELY CLEAN INTERSTATES AND SOME OF WHICH HAVE BEGUN TO SPIDERWEB. I BELIEVE THAT THE GLASS ON THIS VEHICLE IS ABNORMALLY SUSCEPTIBLE TO PICKING UP CHIPS FROM ROCKS WHICH WILL IN TIME CREATE STRESS RISERS AND CAUSE THE WINDSHIELD TO FAIL PREMATURELY. I TALKED TO A MAZDA REP AND THEY SAID THAT THERE WERE NO RECALLS ON THIS VEHICLE NOR WAS THE WINDSHIELD COVERED BY THE FACTORY WARRANTY. THEY DIRECTED ME TO TAKE MY COMPLAINT TO MY INSURANCE COMPANY. WHEN USING ANDROID AUTO WHILE THE VEHICLE IS IN MOTION THE TOUCHSCREEN IS DISABLED WHICH MEANS I NEED MY EYES ON THE SCREEN TO WATCH THE SELECTION MOVE BY TURNING THE SELECTION WHEEL RATHER THAN JUST TOUCHING THE SCREEN. ELECTRICAL SYSTEM; ENGINE
This SA supersedes the previously issued SA(s) listed below. The changes are noted in Red. SA-061/25 12/17/25 DESCRIPTION If you encounter any type of electrical concern (e.g, dead battery, engine no start, DTC(s) stored...), interview the customer before proceeding with normal MGSS diagnosis by asking if any aftermarket devices have been installed. Aftermarket devices may be the cause of the electrical concern. Below are a few examples of unusual electrical concerns caused by aftermarket devices. Examples: 1. Devices connected to the DLC-2 (battery discharge/DTC stored) 2. Devices plugged into the 12V outlet 3. GPS tracking device (engine no start/battery discharge) 4. Dash cam (DTC stored) 5. Non-certified USB cable (Apple Carplay™/ Android Auto™ will not connect) 6. Over-sized wheels/tires (incorrect GPS location) 7. DTC C0023:14 8. Decorative trim and/or cover on the START/STOP ENGINE button Example 1: Some electrical devices connected to the DLC-2 connector by insurance or mobile phone companies may cause: Battery discharge. The device may disrupt the CAN system battery saver mode. • IDS/MMDS will not communicate with the vehicle due to poor DLC-2 connection. The aftermarket device may cause DLC-2 female terminals to spread open.
SEATS; STRUCTURE:BODY
DESCRIPTION Some vehicles may exhibit scratches (1) on the rear of the seat back bar garnish (2) from contact with the convertible top (3). Due to assembly variations, the clearance between the seat back bar garnish and the convertible top is too small on some vehicles. REPAIR PROCEDURE 6. Install the seat back crossmember assembly from the vehicle according to the instructions on MGSS online or the Workshop Manual (section 09-10 SEAT BACK CROSSMEMBER ASSEMBLY REMOVAL/INSTALLATION). 7. Reinstall the rear seat back bar garnish while twisting the garnish so that the garnish can be kept closer to the seat back bar and away from the convertible top (for both left and right sides).
POWER TRAIN
This bulletin supersedes the previously issued bulletin(s) listed below. The changes are noted in Red. 01-008/23 06/05/23 Some vehicles may exhibit rattling noises from the engine compartment and oil leakage from the drive belt auto tensioner. This may be caused by insufficient sealing at the drive belt auto tensioner damper when the auto tensioner fully operates under both high ambient temperature and high load conditions. To eliminate this concern, the shape of the drive belt auto tensioner damper has been changed. REPAIR PROCEDURE If the tensioner is found to be "No Good" after completing the Drive Belt Auto Tensioner Inspection, replace it with a modified part according to the instructions on MGSS online:

To obtain a replacement VIN plate (bulkhead-mounted and visible through the lower-left area of the windshield), contact DAG (Dealer Assistance Group) using VIN Plate Request Form available on One Mazda. To obtain a replacement vehicle certification label (located on the Bpillar or front door), contact your assigned DSM (District Service Manager) to place the order. If DSM contact information is not available, refer to the Dealer & Field Contact Directory.

Mazda recommends dealers follow the scheduled maintenance tables as cited in the vehicle-specific workshop manual or owner's manual. Services performed outside of Mazda's factory scheduled maintenance are not recommended. These services are unnecessary and create unneeded expenses for your customers. SCHEDULED MAINTENANCE Refer to the applicable Workshop Manual or Owner's Manual for vehicle -specific manufacturer scheduled maintenance. Workshop Manual - Refer to section 00-00 SCHEDULED MAINTENANCE. Owner's Manual - Refer to section 8 Maintenance and Care. b. Note: Please refer to the information below outlining the services not included in Mazda's Factory Scheduled Maintenance: IMPORTANT: Recommending fluid flushes when not necessary can lead to customer confusion and dissatisfaction. ENGINE
This Service Alert supersedes the previously issued M-Tip(s) listed below. The changes are noted in Red text. MT-002/19 09/30/19 and 12/19/19 DESCRIPTION If an engine assembly is replaced after a catastrophic internal mechanical failure, clean the intake manifold internally before installing a replacement engine assembly. If the intake manifold is not cleaned properly, the replacement engine may fail prematurely. Metallic debris from the old engine (with catastrophic internal mechanical failure) may rest inside the intake manifold. When the new engine assembly is installed, debris in the intake manifold will be ingested into the new engine and cause internal damage. Clean the intake manifold internally to remove any debris from old engine before installing the new replacement engine assembly. If the intake manifold cannot be properly cleaned internally, replace it.
