The defect: Mitsubishi Motors North America, Inc. (Mitsubishi) is recalling certain 2022-2024 Outlander and 2023-2025 Outlander PHEV vehicles. Due to a software error in the in-vehicle infotainment system (A-IVI), the rearview camera image may freeze, or not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk: A rearview image that does not display properly reduces the driver's view of what is behind the vehicle, increasing the risk of a crash.
The remedy: Dealers will update the A-IVI software, free of charge. Owner notification letters are expected to be mailed June 30, 2025. Mitsubishi's number for this recall is SR-25-001. This recall replaces and expands previous NHTSA recall number 23V345. Vehicles previously repaired under recall 23V345 will need to have the new remedy completed.

The contact owns a 2025 Mitsubishi Outlander PHEV. The contact stated that while driving approximately 25 MPH and pulling into the neighborhood through the slow-access gate, while the vehicle was approximately one foot from the gate, the brake pedal was depressed and the vehicle unintendedly accelerated. The vehicle crashed into the gate, causing the iron rails to detach. The contact was standing on the brake pedal with excessive force while attempting to steer the vehicle. The steering wheel was turned, and the vehicle suddenly came to a stop. There were no warning lights illuminated. There were no reported injuries. A police report was not filed. The contact was able to drive to the residence. The contact called a tow truck company, and the vehicle was towed to the local dealer. After several days, the dealer was unable to duplicate the failure after performing several system scans. The vehicle was not diagnosed or repaired. The insurance company was notified of the failure, and the vehicle was towed to a body shop. The manufacturer was not notified of the failure. The failure mileage was approximately 500.

The contact owns a 2025 Mitsubishi Outlander PHEV. The contact stated upon pulling into her neighbor's driveway while depressing the brake pedal, the vehicle accelerated unintendedly and crashed into a brick wall that was 4 feet away. The brick wall was damaged. The air bags did not deploy. The contact stated that upon shifting to reverse while depressing the brake pedal, the vehicle accelerated unintendedly onto the road. The contact continuously depressed the brake pedal and the vehicle stopped in the middle of the road. The contact turned off the vehicle for a moment. The contact then restarted the vehicle and was able to safely pull to the side of the road. The contact stated that the regenerative braking mode was engaged at the time the incident occurred. The contact stated that there was a passenger in the front passenger's side seat at the time the incident occurred. The contact stated that the mason who provided an estimate of the brick wall damage and repair stated that the brick wall was severely damaged as if the vehicle that crashed into it was going at a high rate of speed. There was no injury sustained. No police report was filed. The vehicle was towed to an auto collision repair shop to be repaired. The manufacturer was notified of the failure and a case was filed. The manufacturer referred the contact to the dealer for assistance. The dealer was notified of the failure, but the vehicle was not diagnosed or repaired. The failure mileage was approximately 985.
