Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

BRAKE LIGHTS INOPERATIVE
THIS PE WAS OPENED BASED ON TECHNICAL SERVICE BULLETIN (TSB) #05-6-19 ISSUED BY FORD MOTOR COMPANY AND SERVICE BULLETIN #09-008/5 ISSUED BY MAZDA MOTOR CORPORATION TO ADDRESS CONCERNS OF THE CENTER HIGH MOUNTED STOP LIGHT (CHMSL) WIRING HARNESS CHAFFING. THE TSB'S AFFECTED THE FORD SUPER CAB AND MAZDA CAB PLUS VEHICLES ONLY.SERVICE TECHNICIANS CONTACTED THE FORD HOTLINE BECAUSE IT WAS DIFFICULT FOR THE TECHNICIANS TO DIAGNOSE AND LOCATE THE CAUSE OF THE INOPERATIVE BRAKE LIGHTS.FORD AND MAZDA ISSUED THE TSB'S TO AID TECHNICIANS IN THE IDENTIFICATION AND REPAIR OF THE WIRE CHAFE CONDITION.OF ALL WARRANTY CLAIMS SUBMITTED TO FORD FOR BRAKE LAMP FAILURE, FORD IDENTIFIED ONLY 120 THAT WERE RELATED TO WIRE CHAFE NEAR THE CHMSL, AND MOST (115 OF THE 120) WERE REPORTED FOR THE SUPER CAB CHMSL. ODI'S REVIEW OF FORD'S WARRANTY CLAIMS AND COMPLAINT DATA REVEALS A DECLINING TREND FOR THE SUPER CAB AND THAT TREND CLOSELY PARALLELS THE TREND FOR THE STANDARD CAB AS WELL.A SAFETY-RELATED DEFECT HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.

The contact stated that the vehicle came to a complete stop after rolling across the street and through the neighbor’s fence. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 223,000. The contact stated that while the vehicle was having the air bag replaced, the contact became aware that the passenger’s side air bag had not been seated correctly. The contact asked the Service Manager to return the vehicle to have the mechanic properly seat the air bag and secure the air bag correctly. The contact stated that the Service Manager attempted to adjust and tighten the air bag. The contact stated that the vehicle had been repaired but not in a safe manner. The manufacturer had been informed of the issue. The failure mileage was approximately 228,000.

The contact owned a 2005 Ford Ranger. The contact stated while driving approximately 10 MPH, the vehicle struck the rear of another vehicle causing a fender bender. The contact stated that the front air bags deployed, and a powdery substance and metal fragments flew out of the air bag and landed on the contact. No injuries were sustained. No medical attention was required. A police report was filed. The vehicle was towed to an independent mechanic and deemed a total loss. The contact stated that he then received notification of the NHTSA Campaign Number: 17V880000 (Air Bags). An unnamed dealer and the manufacturer were notified of the failure and the contact was advised to call the NHTSA Hotline. The failure mileage was approximately 205,000.

I was in an accident and the airbags did not inflate. I was going 30-35 MPH, there was a patch of ice, and I hit a tree. The truck was totaled. I smashed my face on the steering wheel, fracturing a bone. Some weeks later I received a letter in the mail from Ford Motor Company with regards to a passenger side airbag recall. I am afraid that this could perhaps be a bigger problem than just passenger side airbags.

The contact stated while driving 40 MPH during rain weather conditions, the contact lost control of the vehicle while going around a curve, causing the vehicle to crash into a ditch. No air bags deployed. The contact suffered a head injury and several other unknown injuries due to the seat belt. The paramedic had to cut the seat belt to release the contact from the vehicle. The contact was taken to the hospital to seek medical attention. A police report was filed. The vehicle was towed and was deemed a total loss by the insurance company. The dealer was not contacted. Certain 2005-2014 Mustang, 2005-2006 GT, 2004-2011 Ranger, 2006-2012 Fusion & MKZ/Zephyr, 2006-2011 Milan, and 2007-2010 Edge & MKX Vehicles Driver & Passenger Airbag Module Quality Control Inspection: Ford Motor Company has determined that some vehicles claimed by dealers as repaired under Takata Airbag Safety Recalls 15S21, 17S42, 18S02, 19S01, and 21S12 may not have received the claimed repairs. The propellant in certain Takata airbag inflators can experience an alteration over time with exposure to certain climate conditions. A combination of time, high temperature fluctuations, and humidity contribute to the degradation of the propellant in the inflators. This degradation can cause the propellant to burn too quickly, creating high pressure inside the inflator, and in extreme cases causing the inflator to rupture in the event of a crash necessitating airbag deployment. A ruptured inflator can send inflator parts toward vehicle occupants resulting in serious injury or death. The Powertrain Control/Emissions Diagnosis (PC/ED) manual has been revised for diagnosing electronic throttle bodies (ETBs). The ETC_ACT and ETC_DSD PIDs should not used to diagnose possible ETB concerns. The IDS has a limited refresh rate when reading these PIDs and cannot display quickly enough to validate a concern. The PCM automatically monitors these inputs more accurately and will set diagnostic trouble codes (DTCs) when appropriate. Using these PIDs for diagnostics will lead to inaccurate results and improper ETB replacements. If a concern is intermittent and no DTCs are present, refer to historical DTCs and the PC/ED, Section 3 No DTCs Present Index chart for further information.
