No Restart After Auto Start/Stop Engages
On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. Inadvertent Automatic Emergency Braking
On March 7, 2024, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24008) to assess reports of inadvertent activation of the Automatic Emergency Braking (AEB) system in model year 2019-2022 Honda Insight and 2019-2022 Honda Passport vehicles. The complainants allege activation of the AEB system with no apparent obstruction in the vehicle's path, resulting in rapid vehicle deceleration. Honda indicated that it is aware of a total of 412 reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and stated that some customers possibly had an inadequate understanding of the AEB system and its limitations. However, many consumer complaints received by ODI allege that Honda dealerships were unable to reproduce the condition or state that Honda dealerships informed the consumer that this is considered normal AEB operation. To date, ODI has received a total of 106 consumer complaints of inadvertent activation of AEB in the subject vehicles.In total, ODI reviewed 475 reports involving vehicles with unique VINs that may relate to the alleged defect. Of the reports reviewed by ODI, three (3) allege a crash caused by the inadvertent activation of AEB and two (2) allege injury. This Engineering Analysis is being opened to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. Further, the scope of the investigation is being expanded to include assessment of model year 2023 Honda Passport vehicles. Automobile warnings include: "Audio is Turned OFF" "Network Communication Lost" "MOST Net Off" Loss of all audio, screen, and backup camera. Most common when starting the car or hitting imperfections along any roadway. I need all audio, screens, and backup cameras functioning completely and predictably to ensure safe driving. Vehicle was recalled twice already for MOST cable and audio defects. Dealership refused to investigate related issues on XXX. The contact owns a 2019 Honda Passport. The contact stated that due to prolonged sunlight exposure, the interior foam fabric of the driver’s and passenger’s side sun visors were warped and swollen. The swelling caused the interior parts of the sun visors to poke through the exterior fabric coverings, creating a bulge that obstructed the view while driving. Additionally, the interior plastic housing had separated. The sun visors failed to stay in place and flapped around loosely, striking the driver’s head while driving. The manufacturer was notified of the failure. The vehicle was taken to the dealer and the failure was confirmed. The failure mileage was 54,700.

The contact owns a 2019 Honda Passport. The contact stated that the infotainment system screen intermittently turned off while driving approximately 65-80 MPH. The messages "Network Communication Lost" and "Most Net Off" were displayed. The failure caused the rear-view camera to become inoperable and the speakers to make crackling sounds, which distracted the driver. The contact stated that the vehicle was taken to the dealer and a second dealer, Honda of Princeton (987 State Rd, Princeton, NJ 08540), where the FAKRA connector was replaced several times, but the failure persisted. The dealers attempted to repair the vehicle nine times over four years. The contact related the failure to Warranty Extension: 21-012 (2019-20 Passport/Pilot MOST Bus Network Connectors) and NHTSA Campaign Number: 23V431000 (Back Over Prevention); however, the VIN was not included in the recall. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 62,000.

The contact owns a 2019 HONDA Passport. The contact stated while driving the vehicle at an undisclosed speed, he noticed that the vehicle had reduced power and could not accelerate very well, accompanied by an odd noise coming from the engine bay. The contact stated that he had an independent mechanic confirm that the noise was a possible engine issue leading to the destruction of the camshaft and other components. The vehicle was taken to a dealer where they confirmed a main bearing connecting rod failure and a code stating a lack of oil in the engine, although the vehicle has shown a good history of maintenance. The contact stated he had gotten an emissions warning light illuminated on the instrument panel. The contact stated the issue he noticed was the odd sound, which first sounded like a ticking noise and became louder over time. The contact also noted the dealer had discovered metal shavings in the oil pan. The manufacturer was notified of the failure. The dealer confirmed the failure. The vehicle was not repaired.

Car began making loud rattling noise which proceeded to a larger grinding noise in the engine. Car sudden wouldn’t accelerate and had to be shut off. It wouldn’t restart and once it did get jumped produced an intensely loud grinding rattling noise. Car had to be towed to Honda which upon diagnosis was found to have metal shards in the oil pan and diagnosis to replace entire engine. Honda is denying recall on this engine though it is very similar engine to what they currently have recalled for their V6 engines in Pilots, Odysseys ect

I am writing to formally request goodwill assistance regarding a significant hardware failure in my 2019 Honda Passport. My vehicle is currently experiencing diagnostic trouble code U3000-49 (Internal Electronic Failure of the Multipurpose Camera Unit), which has completely disabled my Honda Sensing suite, including Adaptive Cruise Control, Lane Keeping Assist, and Collision Mitigation. While my vehicle has approximately 76,000 miles and is out of the original bumper-to-bumper warranty, I am deeply concerned that this is a premature internal electronic component failure rather than normal wear and tear. Why I Am Reaching Out: Widespread Hardware Defect: The U3000-49 internal circuit board failure is a known, heavily documented issue impacting Honda Sensing-equipped vehicles (such as the closely related Honda Pilot and Odyssey models built on the same architecture). Safety Impact: This failure shuts down primary active safety features that were a core part of why I purchased a Honda. A structural component failure disabling safety features after roughly 7 years of ownership is unreasonable for a brand with Honda's reputation for longevity. Exclusion from Current Actions: I am aware of ongoing class-action litigation regarding these exact front sensor camera failures in Honda vehicles. While my specific vehicle model year/trim configuration is not currently named in the active scope of those specific lawsuits, the mechanical and electronic reality of the part is identical. Replacing this unit and performing the mandatory ADAS static calibration quotes out to over $1,500–$2,000 at local service centers—an extreme financial burden for a manufacturing-related internal circuit board failure. Given my loyalty to the brand and the safety-critical nature of these systems, I am asking Honda to step in and offer goodwill assistance to cover or heavily subsidize the replacement and calibration of this failed multipurpose camera unit. The vehicle engine is dying at intersections and going into “neutral” on its own when stopped. This has been ongoing for over a year, Honda denies any issues. It is becoming more of a safety problem more recently as the vehicle “downshifted” on its own while driving on the highway. (On that instance, the vehicle went from approx 45 mph to a slow crawl - just enough to pull off the road to safety).

What component or system failed or malfunctioned, and is it available for inspection upon request? The rear subframe, specifically the passenger-side rear suspension mounting bracket, is failing due to severe, premature corrosion. How was your safety or the safety of others put at risk? My safety was put at extreme risk because a local authorized Honda dealership fraudulently closed out an active, critical safety recall (NHTSA Campaign Number 26V-365) without actually performing the mandated repair. They returned the vehicle to me to drive on public roads with a heavily rusted, structurally compromised rear subframe where the flange seams are actively separating. Driving this vehicle poses a critical, ongoing risk because the rear control arm mounting points could fracture and fail. Such a failure would cause the rear wheels to become misaligned or inadequately retained, resulting in a sudden and catastrophic loss of vehicle handling, stability, and braking performance. Has the problem been reproduced or confirmed by a dealer or independent service center? The local authorized Honda dealership explicitly acknowledged the open recall during my initial service visit and stated they needed to order the remedy parts. They subsequently contacted me to bring the vehicle back under the pretense that the parts had arrived. They released the vehicle back to me and marked the recall as officially completed in their system. However, physical inspection of the undercarriage confirms the passenger-side subframe bracket remains completely bare, unreinforced, and heavily corroded. Because official corporate recall notices confirm that the required replacement parts are globally backordered until the fall, it is physically impossible for the dealership to have performed the installation, confirming they falsified the repair record and returned an unsafe vehicle. Prior to the dealer visits, I experienced significant shuddering and abnormal vibrations from the rear suspension.
