Unintended and Uncontrolled Acceleration
On February 16, 2010, NHTSA's Office of Defects Investigation (ODI) opened RQ10-003 to determine whether the scope of prior Toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, Toyota recalls 07E-082, 09V-388, 10V-017, and 10V-023.NHTSA also requested information regarding potential electronic causes of unintended acceleration in Toyota and Lexus vehicles.I. Pedal Interference from Floor mats, Carpet Covers and Plastic Pads in Carpets.During the RQ10-003 investigation, ODI reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agency's analysis, NHTSA requested that Toyota recall additional vehicles.Toyota complied with the agency's request.The details of these recalls are set forth more fully in Toyota's reports to NHTSA pursuant to 49 CFR Part 573.These recalls are as follows:Recall 11V-112: (1) model year (MY) 2004-2006 Toyota Highlander and Highlander Hybrid; and, (2) MY 2004-2007 Lexus RX330,RX350, and RX400h (hybrid model). The total estimated population under this recall is 769,379 vehicles.This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles.This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles.This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration.After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles.That study has concluded.Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.

My vehicle caught fire a few days after being repaired by a licensed repair shop in California. The repair was performed by: Brodie's Tire & Automotive Address: 1276 Petaluma Blvd N, Petaluma, CA 94952 Phone: [XXX] After the fire, the repair shop admitted in writing that: • The intake manifold melted after the fire • The engine became “not repairable” • The fire occurred shortly after their repair • They offered to “fix it for free,” which indicates awareness of responsibility The fire caused severe heat damage to the engine compartment, electrical wiring, and surrounding components. The vehicle became unsafe to drive, and the engine can no longer operate. This incident presents a serious safety hazard because the fire happened shortly after a repair that involved components near the engine and fuel system. Improper or unsafe repair work may cause similar fires for other consumers. This incident is already under active investigation by: • California Department of Insurance (CDI) – Complaint #XXX• California Bureau of Automotive Repair (DCA/BAR) – Case No: XXX I am reporting this to NHTSA because the fire and melted engine components indicate a dangerous defect tied to improper repair work, which may pose a risk to other drivers. Unexpectedly found steel cable has snapped off & the spare tire was gone Like to report this could be a safety issue leading to an accident for fellow driver.

The contact owns a 2005 Lexus RX 330. The contact stated that there were horizontal and vertical cracks on the dashboard near the passenger’s side air bag housing. There was no warning light illuminated. The contact was concerned about the safety of the driver’s and passenger’s side air bags. The local dealer was contacted; however, the vehicle was not diagnosed or repaired. The contact was informed that the VIN was not under recall for the failure. The manufacturer was notified of the failure. The approximate failure mileage was 80,000.

Over past month Dashboard where airbag is under has become discolored and is CRACKING as if AIRBAG wants out. Other 2005 Toyota vehicles have been recalled bc of Dangerous AIRBAG issues. Should the AIRBAGS in 2005 Lexus’ be too? Afraid to drive now.

An AutoZone, Durlast brand brake master cylinder failed in less than one year. The piston in the master cylinder dislodged, preventing proper braking in at least two of the four disc brake calipers. The Duralast master cylinder has been retained for inspection. The safety of the driver, passengers, other motorists and pedestrians were placed at risk due to the malfunction of the shoddily manufactured brake component. Braking distance and stopping efficiency were adversely affected. The problem was confirmed by the automobile repair center. The manufacturer has, so far, failed to contact me about the defective brake component. No warning lamps or messages were associated with the master cylinder failure.

The contact owns a 2005 Lexus RX330. The contact stated while driving at approximately 45 MPH, the brake pedal was depressed but the vehicle was slow to respond. There were no warning lights illuminated. The vehicle was taken to an independent mechanic, who informed the contact that the Duralast brake master cylinder was inoperable. The contact was informed that the piston in the brake master cylinder had become dislodged, causing the braking system to become less efficient. The mechanic replaced the part. The local dealer was not contacted. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 210,000.

2005 LEXUS RX 330 Vin# [XXX] Problem: Dashboard crack and peel all over the surface happened on January 2021.I might be eligible for Lexus to cover this approximate $ 800.00 repair at no charge to me. This is WARRANTY ENHANCEMENT NOTIFICATION. Letter I never get from LEXUS. Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: •Minor damage: requires only buffing to repair. •Moderate damage: usually requires neutralizing, color sanding, and buffing. •Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. While the quality and durability of paint on Lexus vehicles is continuously being improved, it remains the dealer’s responsibility to protect and maintain the quality of the vehicle’s paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.
