The defect: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024-2025 Toyota Tundra, Tacoma Hybrid, Tacoma, RAV4 Hybrid, Land Cruiser Hybrid, Lexus GX550, 2024-2026 Tundra Hybrid, 2025 Sequoia Hybrid, Crown Signia, Grand Highlander, Lexus TX500 Hybrid, Lexus NX350 Hybrid, 2025-2026 Grand Highlander Hybrid, and Lexus TX350 vehicles. The load carrying capacity modification label may display inaccurate added weight values. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."
The risk: A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.

The defect: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

The defect: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The electronic throttle control, vehicle speed control, and powertrain/transmission control system failed. When I press the pedal to accelerate there was no power. However I did not lose power to the vehicle as I can see the vehicle was in Drive mode but was not moving forward. I was on a decline hill and the decline helped me go downwards before a truck could hit me. This total loss of acceleration has occurred exactly 2 times within a 4-week period. The vehicle is currently located at the Lexus dealership service center and is available for immediate inspection upon request. My wife and small children were in the vehicle when the accelerator pedal went completely dead while driving. The sudden drop in speed in active traffic almost caused us to be catastrophically rear-ended by an 18-wheeler semi-truck. After one minute of zero throttle, the vehicle violently jerked, locked us into a highly dangerous 5 MPH limp mode on the road, jerked a second time, and then suddenly resumed normal speed. My family's lives were put at extreme risk. No. The vehicle is currently at the authorized Lexus dealership for inspection, but the service department states they cannot find any diagnostic trouble codes (DTCs) and have not yet reproduced the intermittent failure on a test drive. The vehicle is currently under inspection by the authorized Lexus dealership service department. It has not yet been inspected by the manufacturer's regional corporate specialists, police, or insurance. No. There were absolutely no warning lamps, check engine lights, dashboard messages, or symptoms prior to either of the two failures. The loss of acceleration happened instantly without warning. The vehicle's computer system failed to store any error codes despite the severe mechanical jerking and the sudden 5 MPH speed restriction.

We were driving down the highway and the vehicle started shaking. I immediately slowed down as I felt we had a flat. There was notification on the screen. I moved to the right onto the shoulder. Only after we were completely stopped did I see a low tire warning come on display. I slowly got out as the traffic was heavy. I could see the outer section of the tire came apart from both sides. As we were in a dangerous location someone called the police. Two police cars pulled behind us and provided us some safety. We agreed to swap out bad tire for spare as we were in a very dangerous location and needed to get vehicle moved before someone ran into all of us. We did look at the highway to ensure no objects were in the road. I didn’t see any while driving, but we wanted to verify to protect other drivers. There also weren’t any pot-holes in highway. We inspected tire after taking it off and there were no screws, nails, objects, or damage to the center tread/sides. The tire seemed to separate fairly cleanly on both sides. Tires were Continental Cross-contact 255/45 R22 107V LX20 (Eco plus) To repeat, no Warning Lights came on vehicle before tire came apart. I have sent message to my insurance company. No one has inspected tire and it is in my possession.

I am reporting two separate but related fuel-system defects on my 2025 Lexus TX500h (VIN [XXX]) that raise safety concerns and appear to be widespread across Lexus TX and NX hybrid models. FUEL FILLER DOOR FAILS TO OPEN: The electronic fuel filler door intermittently fails to open when the release is activated. It sometimes requires multiple attempts, and at times will not open at all, leaving the vehicle unable to be refueled. This creates a safety risk of being stranded with no ability to add fuel, including the risk of running out of fuel on the road or being unable to refuel in an emergency. The failure appears to worsen in cold weather. This is a widely reported problem on 2022-2025 Lexus NX and TX hybrid vehicles and has been attributed to a faulty electronic fuel-door lock actuator (referenced by owners and parts suppliers as OEM part number 77030-F6020). Owners are frequently forced to use a manual emergency release located behind an access panel in the cargo area in order to refuel. FUEL TANK CANNOT BE FILLED TO CAPACITY: On a near-empty tank, the fuel pump nozzle shuts off after only approximately 8 gallons, despite the tank's rated capacity of approximately 17 gallons. Adding additional fuel requires repeated, prolonged attempts and often only a few more gallons can be added. This significantly reduces usable driving range relative to what the vehicle's fuel gauge and range estimate imply, creating a risk of unexpectedly low fuel and reduced range in situations where refueling options are limited. Other owners have reported the same behavior, and in at least one documented case a Lexus Field Technical Specialist diagnosed the cause as a deformed/defective fuel tank requiring a redesigned replacement. Both conditions are present on my vehicle. I am reporting them so that they are documented at the federal level. No crash, injury, or fire has occurred. y 2025 Lexus TX500h experienced sudden and complete AC failure at fewer than 25,000 miles, 18 months after purchase. The selling dealer diagnosed a refrigerant leak from the AC condenser and denied warranty coverage citing road debris impact damage. No photographic documentation of a discrete impact event or specific leak point was provided to support the denial. This failure is consistent with a documented, systemic design vulnerability in the Lexus TX platform. The vehicle's wide-open front grille design allows road debris direct access to the AC condenser, which sits as the frontmost component in the cooling stack with no protective barrier. Multiple NHTSA complaints document identical failures on TX platform vehicles at extremely low mileages including 580 miles and 900 miles, with dealers uniformly denying warranty coverage. Operating without AC in Texas summer heat constitutes a safety hazard, particularly for child passengers. I am requesting NHTSA investigate this as a systemic design defect pattern across the TX platform. Attached photos include: (1) photos provided by the dealer as their evidence of "impact damage" — none of which document a specific leak point or impact deformation zone; and (2) photos taken by the owner of the condenser on the Westside Lexus loaner vehicle provided during this service visit, which show comparable fin condition and debris accumulation to the denied vehicle. The loaner vehicle photos demonstrate that this condenser condition is consistent with normal operation in this environment, not discrete impact damage.

On our 2025 Lexus TX350, less than 1 year old, and under normal driving conditions, our AC stopped working suddenly. Upon inspection at the dealership, they determined a rock or road debris came through the front grill and impacted the condenser, which caused a major refrigerant leak and ac to stop working. I believe this was due to a poor design of the lower half of the front grill, it’s wide open and offers little to no protection to ac and engine components. They should have a better honeycomb or similar grill to protect components. The refrigerant leak is an environmental and road safety issue, and living in hot climates like southern Nevada, when it’s 110 degrees outside, the heat inside the vehicle with no AC is a safety issue for children, adults, etc. The dealership did not cover repairs under warranty, and quoted $3,500 to repair either with cash or submit insurance claim.

2025 Lexus TX350, approximately 27,000 miles. Vehicle exhibits rotational clicking/vibration from front drivetrain during acceleration while turning. Dealer is attributing issue to brake pad and rotor wear. Lexus TSB L-SB-0033-24 Rev1 documents a known front drive shaft assembly defect on this platform. TSB applicability check has not been performed. Requesting investigation into whether this defect extends to 2025 model year production.

The contact owns a 2025 Lexus TX350. The contact stated that while driving 50 MPH, the brake pedal was depressed and felt spongy, and the vehicle failed to respond as intended, causing the braking distance to be extended. The vehicle was taken to the dealer, where the contact was informed that the brakes were in good working condition; however, the failure persisted. The contact stated that while attempting to raise the folded passenger's side third row seat to the upright position, the back of the seat became stuck to an unknown part behind the seat, causing the seat to fail to respond as needed. The vehicle was taken to the dealer, where the contact was informed that the unknown part had been replaced, and the seat could be raised; however, the failure persisted. Additionally, the contact stated that fuel tank door had failed to open intermittently while pressing the release button on the steering wheel, forcing the contact to release the fuel tank door by engaging the emergency release manual button inside the trunk. The dealer was notified of the failure, and an appointment was scheduled for a diagnostic test. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 3,000.
