Tesla released a new version of FSD: 2026.20.6.11 V14 Lite for my 2022 Model Y Long Range. I installed it overnight and the next morning, several systems become intermittent. The backup camera stopped working immediately, and FSD would drop out, with the message "Take control immediately". I would then lose traffic visualization and other advanced features. Eventually the system would recover and start the cycle all over again. Several times per drive. I took the car to the dealer and they diagnosed it as a failed HW3 computer. The cost to repair it was $2,939 out of pocket since my car was 2 months out of warranty. The car was 100% functional before the V14 Lite update. The dealership denied the problem was related to the update, but the internet now has numerous instances of this problem, and attribute it to the computer running much hotter under this new update, causing components to fail. Our update was installed while the car was in our garage at a temperature in the 70’sF and immediately caused the computer failure before the car even left the garage. I requested and was given the failed computer in case any independent investigation is needed

The Autopilot/FSD computer (driver-assistance ECU) on my 2021 Model Y (HW3) began failing intermittently on August 2, 2026, days after installing a software update -FSD v14 "Lite" branch. The component is in the vehicle and available for inspection. While driving at highway speed in high ambient temperatures (112–125°F), the system repeatedly disengaged with "Cruise control unavailable" (alert DI_a175) and "System error — take over immediately" warnings, and later alert APP_w46u on FSD engagement. Failures included: FSD and Autosteer aborting within seconds of engagement or disappearing from the menu entirely; complete loss of driving visualizations (no lane lines or vehicle overlays); rear camera blacking out; loss of speed-limit recognition and automatic turn-signal cancel. Both FSD and Autosteer Beta failed identically, indicating the fault is in the shared Autopilot computer/camera pipeline, not one feature. Because the same camera/compute stack supports automatic emergency braking, forward-collision warning, and blind-spot warning, the status of those safety systems during the failures is UNKNOWN — I had to complete long desert highway stretches fully manually, with repeated unrequested hand-backs of control creating a sudden-takeover risk. The fault is thermal-load related and intermittent: it recurred at high ambient temperatures, sometimes recovered after a rest or power-off reset, but on August 4 one full power-off reset at 88°F ambient failed to restore the system (it later self-recovered), suggesting progressive hardware degradation rather than ambient heat alone. Not yet reproduced by a service center: a Tesla service appointment is scheduled for August 13, 2026, and a service request with fault codes was filed in-app while the faults were active so remote diagnostics could capture them. While operating my 2022 tesla model y, the vehicle suffered a catastrophic hardware failure of the onboard vehicle control computer. This total computer failure resulted in an immediate, sudden loss of electronic power steering assist while the vehicle was in motion. The steering wheel became incredibly stiff and heavy, creating a severe and hazarders driving situation. the vehicle was towed to a tesla service center, where technicians confirmed the onboard computer hardware requires a complete replacement. A vital computer failing prematurely on a 2022 vehicle presents an extreme safety risk of crash or injury due to sudden loss of vehicle control.

I was driving at approximately 30-35 mph on a 30 mph residential road with my son in the front passenger seat. My vehicle unexpectedly collided with a parked, unoccupied car on the right side of the road, impact occurring between my front passenger side and the rear driver side of the parked vehicle. Following the initial collision, the vehicle continued past a small tree, mounted the sidewalk, and struck a second parked car before I was able to manually bring it to a stop 2–3 houses further down near an intersection. Throughout this sequence—with the vehicle traveling at ~35 mph—neither the Automatic Emergency Braking (AEB) activated nor did any airbags deploy. Fortunately, neither my son nor I sustained catastrophic injuries, though my chest struck the steering wheel during the impact, causing ongoing pain my and mental trauma for my son. I am currently receiving medical advise. Given the speed and sequence of impacts, it is terrifying that the vehicle’s primary safety systems failed to engage before, during, or after the collision. I am seeking a thorough technical investigation into why Automatic Emergency Braking and airbag deployment failed in this scenario with Tesla Model Y

FSD failed when on FSD to go to local Walmart, in Walmart parking lot while on FSD, vehicle was parking it self by backing up and it hit shopping cart rack. It failed to see physical shopping cart rack and stop and park. It could have hurt someone if someone was there, software and 3 cameras didn’t avoid the accident.

As a 100% T&P Disabled Veteran relying on advanced driver-assistance for mobility, I purchased a 2022 Tesla Model Y Performance with Hardware 3 (HW3), which was actively advertised and sold as possessing all necessary hardware for safe, autonomous operation. In practice, the vehicle suffers from safety-critical driver-assist defects—including unexpected disengagements and erratic system choices—due to fundamental HW3 processing and memory bandwidth limitations. Operating a vehicle marketed as autonomy-ready, but constrained by hardware incapable of handling complex traffic scenarios safely, forces constant manual intervention and creates an unacceptable collision hazard for a disabled driver.

Tesla Y on the freeway, FSD on(Full Self Driving), driving at 70 mph. I saw the cars ahead are slowing but my car was not slowing, so I braked and held the steering wheel firmly. However, my car still hit the car in front at 36 mph. The airbag deployed and luckily I only sprained my thumb in both hands in holding the steering wheel very hard. If I didn't take over my car would have hit the car in front at 70 mph. I downloaded Tesla "Vehicle Data Report" and it shows that the "FSD was active, Cruise Control State was active, Driver Monitoring Detection was detected and I applied the brake. I used FSD for over 3 years and no problem at all, until the accident. The Tesla Data shows that the FSD malfunctioned that caused the accident but my insurance told me that it was my fault and they increased my insurance for $227 a month.

FSD / vision system , no warnings , the vehicle is turning left and right on a straight road sunny conditions. I have a video that it wouldn’t let me attach to the case. I had Tesla service review this and they told me it’s standard behavior, this never occurred before and is dangerous and unusable. Autopilot has an issue that Tesla themself can’t detect and when I get into an accident it’ll Be too late. I’ve been complaining about this for 3 months and Tesla keeps telling me it’s standard behavior, this never happened before the car used to drive straight, brand new tires front and back , blanket and aligned the car drives straight while not in autopilot, issue is directly related to FSD and Tesla autopilot.

This week, I was driving my Tesla at approximately 65 mph in the middle lane of a six-lane freeway. Autopilot was engaged. Traffic was moving normally, and I was not aware of any obstacle or hazard that would have required emergency braking. Without any warning, the vehicle suddenly applied the brakes and rapidly decelerated until it came to a complete stop in the middle travel lane of the freeway. I was not pressing the brake pedal. The event occurred unexpectedly and created a potentially life threatening safety hazard. my vehicle came to a stop in a high-speed traffic center lane! After I disengaged Autopilot and pressed the accelerator, the vehicle responded normally. Immediately following the incident, the display showed the message “Autopilot unavailable.” That message later disappeared, and no additional warning messages remained on the screen. I contacted Tesla to report the incident. Tesla reviewed the vehicle’s logs remotely and later discussed the results with me by telephone. During that conversation, I was told that the event was considered an “anomaly.” I was also told that the vehicle logs reflected the event approximately one hour from when it actually occurred and that the logs indicated the vehicle was not operating in Autopilot at the time of the incident. This information was inconsistent with the event, because Autopilot was engaged when the sudden braking occurred. I could not even move to berm. Tesla did not request that I bring the vehicle in for a physical inspection. Instead, I was told there was no reason to have the vehicle examined. During the same conversation, I was encouraged to consider subscribing to Tesla’s Full Self-Driving (Supervised) system, which requires an additional monthly subscription, rather than continuing to use autopilot as a result of a question I asked about two plans. The contact’s wife owned a 2022 Tesla Y. The contact stated that while the vehicle was parked, his wife shifted the gear shifter into drive(D), and the vehicle independently accelerated. The Autonomous feature was not engaged. The brake pedal was depressed; however, the vehicle failed to respond as needed, and the front center of the vehicle crashed into a pile of cinder blocks, drove through a gate, and crashed into a dumpster, where it came to a stop. As a result of the impact, the front driver's side tire detached, and the dumpster was damaged. The contact stated that another person helped remove the driver from the vehicle due to the front driver door being unable to open. The air bags deployed as intended. No warning lights were illuminated. The vehicle was towed to a tow yard and was deemed a total loss. A police report was filed. The driver had pain in the back and sought medical attention. The contact's wife was informed of a slight fracture of an unstated bone in her back and ribs, and had to return to the doctor several times for medical assistance. The contact was certain there was no user error to have caused the crash The manufacturer was notified of the failure. The failure mileage was unknown.

While in Full Self Driving mode in my Tesla, it was making a right turn into a parking lot when it suddenly violently accelerated. Before I could gain control it crossed the parking lot entrance, climbed the curb, and came to rest on a lawn in front of a business. Luckily no one was hurt. It appears the car will be totaled by the insurance company at the completion of their investigation.

On Feb 25, 2026 My 2022 Tesla model Y with only 25k miles on it suddenly stopped knowing where it was and almost drove me into a ditch. All cameras stopped working and the gps was stuck in the same wrong location. (Service Mode showed Primary DAS ECU shows "AP IN COREBOOT" and "Status: INIT." Secondary B shows nominal. Features unavailable. GNSS shows 0 satellites in open sky.) Tesla service said I was outside of warranty and “There is an internal processor issue with your autopilot system that will prevent updates from installing. The only solution is replacing the computer on April 9, 2026, 12:22,” I had had other computer related issues I brought up during regular service visits but weren’t recorded and I was told software updates would fix such as: delayed rearview camera upon startup, navigation volume resetting, headlights staying on for extended periods after exit, and lockout issues. I asked Tesla to document the specific failure codes which they never sent. They said “Tesla diagnostics are showing the primary Autopilot computer is not booting properly, which is preventing cameras, software updates, and related systems from functioning. Because the computer is not initializing, a firmware reinstall will not complete successfully. At this point, replacement of the computer is the required repair to restore normal functionality.” My car had a safety recall during its service appointment that they could not install software for unless I paid $3200 to replace computer. I had total loss of rearview camera (FMVSS 111 violation) and loss of Active Safety Systems (AEB/FCW), but Tesla refused to repair a federally mandated safety defect if I didn’t pay the full repair cost.

INCIDENT SUMMARY On April 16, 2026, at approximately 2:06 PM local time, my 2022 Tesla Model Y was traveling on a multi-lane divided roadway in the Austin/Lakeway, Texas area with Autopilot engaged. Traffic in my direction of travel was heavily congested and traveling with 32 mph. Without warning, alert, or any input from me, Autopilot commanded an uncommanded left steering maneuver. The vehicle veered left out of its lane and into a painted buffer separator area. The direction of travel pointed directly toward a building, an electrical transformer/utility box, and a landscaped area containing mature trees. I immediately recognized the unexpected maneuver and manually gripped the steering wheel to override Autopilot. I steered the vehicle to avoid collision with the building and the electrical box, both of which posed serious collision and electrocution risks. In the approximately 1.5 to 2 seconds available to me, I successfully redirected the vehicle away from the building and the transformer. The vehicle came to rest after striking a tree in the landscaped area adjacent to the road. The vehicle was totaled. Driver and curtain airbags deployed. I was wearing my seatbelt, was alert, and sustained no major injuries as a result of my intervention and the successful redirection of the vehicle.

I am reporting a serious safety concern involving my Tesla Model Y. While driving, the vehicle’s main center screen (MCU) suddenly failed and entered a continuous reboot loop. This resulted in the loss of critical driving information and controls, including navigation, camera display, and vehicle system status. The contact stated that they rented a 2022 Tesla Model Y. The contact stated that the vehicle was supervised in autonomous mode when it suddenly turned left while driving at approximately 45 MPH, causing it to hit an electrical box nearby. The vehicle then steered itself back onto the road and shortly after, turned off the road towards the right and crashed into a tree, where it came to a stop. There were no warnings or notifications that were displayed during the incident. The air bags did deploy. The vehicle did not allow the driver to correct the direction of the vehicle or disable the autonomous mode setting. The vehicle had caught on fire. The contact and the other passenger, sitting in the front passenger seat of the vehicle at the time, were able to exit the vehicle. The fire department was called and extinguished the fire. The police department was contacted, and a police report was filed. The vehicle was towed and totaled. The contact received a broken left wrist, a sprained right foot, and seat belt bruising to their lower stomach and chest area. The passenger in the vehicle received seat belt bruising. Medical attention was needed. The contact rented the vehicle through Turo and informed the person they had rented from. The contact was not sure if the incident was reported to the dealer or manufacturer by the owner. The contact stated their attorney contacted the manufacturer and informed them of the incident in an attempt to retrieve data information from the vehicle at the time of the incident. Following a 25-day repair at a certified collision center due to an earlier accident, the vehicle's Full Self-Driving (FSD) and Active Safety Systems began failing. The vehicle repeatedly failed to keep speed and failed to stay centered in the lane. On March 25, 2026, while traveling at low speed on a public roadway with FSD actively engaged, the system failed to maintain a stable path and incorrectly steered the vehicle into a curb, causing property damage to the wheel. This failure put my safety and the safety of surrounding traffic at risk by executing an unpredictable steering maneuver that the active safety systems failed to prevent. The vehicle was taken to the manufacturer's service center twice with this specific complaint prior to the curb strike. The service center investigated and officially concluded that this was not a software issue, stating that they could do nothing else. They deferred the resolution back to the collision shop, citing likely camera/sensor physical misalignment from the 25-day body repair. The collision shop is now refusing to review the vehicle or inspect the hardware, claiming lack of visible exterior damage despite the internal service team stating this is a latent hardware alignment defect. The vehicle is currently in the same failing condition and available for inspection upon request. No warning lamps or error messages appeared on the screen prior to the failure or the curb strike. The symptom of unpredictable lane tracking first appeared immediately upon receiving the vehicle back from the collision repair earlier this year. Note: I have a formal data privacy request pending with Tesla to pull the exact chat logs showing their refusal to service this hardware defect. I will provide these to the investigator upon follow-up.

The vehicle was delivered with a critical safety defect in the braking system and inoperable safety software (GPS/FSD/Navigation). Upon further inspection by the dealer's own technician, it was confirmed that the vehicle had 'EXTREMELY POOR BRAKE FEEL' and 'EXTREME RUST BUILD UP' on all pads and rotors, which violated Tesla’s 6mm minimum safety standard for used vehicles. Additionally, a physically damaged windshield harness caused the failure of GPS and FSD features. These defects posed a significant safety risk to myself and my family during operation. The dealer has documented and confirmed these safety issues but is now refusing a buyback and is retaliating with storage fees for my rejection of the unsafe product. No warning lights appeared prior to the technician's discovery of the brake defects.

Around 02/10/2026, all of ADAS functions of my 2022 Tesla Model Y has suddenly stopped working, and the rearview was just black. On 02/27/2026, Tesla service diagnosed the car and tole me that the car computer should be replaced and its total repair cost would be about $3,200. My car's mileage was 51,000 at the onset of the problem, so just passed the standard 50K warranty. My car has total loss of rearview camera (FMVSS 111 violation) and loss of Active Safety Systems (AEB/FCW), but Tesla refused to repair a federally mandated safety defect if I don't pay the full repair cost.

The contact owns a 2022 Tesla Model Y. The contact stated that while charging the vehicle or using the onboard navigation system to search for a charging station, the entire infotainment central processing unit shut down unintendedly. The contact stated that there was an abnormally low humming sound, and approximately 30-seconds later the infotainment screen shut down unintendedly. The contact had taken the vehicle to a local dealer where the battery and the coolant system were checked. The contact was advised by the dealer that they were not able to isolate the failure and needed to run additional diagnostic testing. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 22V296000 (Back Over Prevention, Equipment); however, when the dealer was contacted about the recall, the contact was advised there was no recall associated with the VIN. The manufacturer was not informed of the failure. The failure mileage was approximately 70,000.

On January 9th my wife got into an accident in our 2022 Tesla Model Y vehicle. She reported that as she was backing out of a prking spot in a strip mall, the vrhicle suddenly accelerated backward involuntarely which resulted in colliding with a vehicle which was approaching from the rear side. My wife reported the speed was so violent that she felt dizziness. When she took the car bto the Tesla shop for repair by the insurance, the Tesla repair people discounted her reporting and did not do any follow-up to investigate the issue. Note the onboard camera recorded the swift movement of the vehicle and I will try to upload it. We have read that such problem was looked into by the NTHSA in the past and that it is being revisited again. Note that that Tesla has determined the vehicle as being totalled and has kept it at their shop. Please let us know your thoughts on this matter so if this is a serious problem that it can be addressed so it won't happen to others. My wif was lucky she didn't get hurt and also she didn''t heart others. Please note I am currently traveling out of the country and will be be back in the US on March 12, 2026. On December 30th, 2025, my wife experienced a serious and unexpected safety event while using Autosteer in one of our Tesla Model Y cars (we own two of them). The version of the vehicle software in use was v12 (2025.26.8) and the Full Self Driving (Supervised) Software was v12.6.4. The vehicle came to a complete stop at a red traffic light on northbound San Gabriel Blvd in Pasadena at the intersection that controls entry to the I-210 West. After waiting for approximately 55 seconds, Autosteer initiated forward movement into the intersection while the traffic light was still red. The vehicle’s movement was not caused by driver input. My wife’s feet were away from the pedals at the time. There was no visible lead vehicle movement or other obvious external trigger. Fortunately, my wife was attentive and immediately disengaged Autosteer, brought the vehicle to a stop, and carefully reversed back into a safe position. Moments later, multiple fast-moving vehicles crossed the intersection in front of her path. Had she not intervened promptly, there was a realistic risk of a serious side-impact collision. This serious incident was reported to Tesla on 12/30/2025 including a video of the incident. In my correspondence, I requested that all relevant vehicle telemetry, Autosteer/FSD logs, and any uploaded video associated with this time and location be preserved and reviewed. As of today (7 weeks later) they have not followed up with me. The 18-second video was recorded by an independently installed Viofo dashcam, which clearly shows: . the vehicle stopped at a red light, . the light remaining red, . and the vehicle beginning to move forward unexpectedly.

In 9/2025 I noticed that the vehicle software was defaulting to a speed limit of 55 mph. I noticed that when in any form of autopilot and FSD it would accelerate to that speed. This happens in any situation before the car encounters a sign or an area that it's internal map corrects it, which is not common outside of major routes in Idaho. This includes paved marked roads, graded roads, single track dirt roads, my driveway and garage. Sometimes the car controls it's speed consistent with conditions, sometimes not. It can happen unexpectedly, and that's the issue. Where I have learned this is a problem, I of course turn off the AP or FSD and drive myself, however, if I am in driver assist mode and things are going well and the speed limit in the vehicle changes suddenly to the default 55 mph, it can take a moment for me to react. In October 2025 I reported this to my Tesla Service center as well as Tesla's over the air and on line email reporting options (from which I got no response). The service center entered a ticket directly with Tesla, and told me that that Tesla was aware. The explanation that came back was that it was due to a mapping update for the internal map of the car. They said it was the result of Google Maps setting the default speed limit in Idaho, which is where I live, to 55 and that this was due to a September 2025 update. They said it might be corrected with the next update, which has not occurred at this point (January 2026). This speeding behavior can happen suddenly and unexpectedly and seemingly randomly, and has the potential to injure pedestrians, the driver, and property, not to mention that it is also exceeding by far the legal speed limit. Drivers from other areas that may not be aware of the issue are likely to be caught unaware. There is absolutely no reason for the default speed to be 55, ever. It's unsafe, and unnecessary. It is rare to see a speed limit of 55 at all in Idaho. Most of the roads are above or below that speed limit.
