Tesla released a new version of FSD: 2026.20.6.11 V14 Lite for my 2022 Model Y Long Range. I installed it overnight and the next morning, several systems become intermittent. The backup camera stopped working immediately, and FSD would drop out, with the message "Take control immediately". I would then lose traffic visualization and other advanced features. Eventually the system would recover and start the cycle all over again. Several times per drive. I took the car to the dealer and they diagnosed it as a failed HW3 computer. The cost to repair it was $2,939 out of pocket since my car was 2 months out of warranty. The car was 100% functional before the V14 Lite update. The dealership denied the problem was related to the update, but the internet now has numerous instances of this problem, and attribute it to the computer running much hotter under this new update, causing components to fail. Our update was installed while the car was in our garage at a temperature in the 70’sF and immediately caused the computer failure before the car even left the garage. I requested and was given the failed computer in case any independent investigation is needed
The Autopilot/FSD computer (driver-assistance ECU) on my 2021 Model Y (HW3) began failing intermittently on August 2, 2026, days after installing a software update -FSD v14 "Lite" branch. The component is in the vehicle and available for inspection. While driving at highway speed in high ambient temperatures (112–125°F), the system repeatedly disengaged with "Cruise control unavailable" (alert DI_a175) and "System error — take over immediately" warnings, and later alert APP_w46u on FSD engagement. Failures included: FSD and Autosteer aborting within seconds of engagement or disappearing from the menu entirely; complete loss of driving visualizations (no lane lines or vehicle overlays); rear camera blacking out; loss of speed-limit recognition and automatic turn-signal cancel. Both FSD and Autosteer Beta failed identically, indicating the fault is in the shared Autopilot computer/camera pipeline, not one feature. Because the same camera/compute stack supports automatic emergency braking, forward-collision warning, and blind-spot warning, the status of those safety systems during the failures is UNKNOWN — I had to complete long desert highway stretches fully manually, with repeated unrequested hand-backs of control creating a sudden-takeover risk. The fault is thermal-load related and intermittent: it recurred at high ambient temperatures, sometimes recovered after a rest or power-off reset, but on August 4 one full power-off reset at 88°F ambient failed to restore the system (it later self-recovered), suggesting progressive hardware degradation rather than ambient heat alone. Not yet reproduced by a service center: a Tesla service appointment is scheduled for August 13, 2026, and a service request with fault codes was filed in-app while the faults were active so remote diagnostics could capture them. I was driving at approximately 30-35 mph on a 30 mph residential road with my son in the front passenger seat. My vehicle unexpectedly collided with a parked, unoccupied car on the right side of the road, impact occurring between my front passenger side and the rear driver side of the parked vehicle. Following the initial collision, the vehicle continued past a small tree, mounted the sidewalk, and struck a second parked car before I was able to manually bring it to a stop 2–3 houses further down near an intersection. Throughout this sequence—with the vehicle traveling at ~35 mph—neither the Automatic Emergency Braking (AEB) activated nor did any airbags deploy. Fortunately, neither my son nor I sustained catastrophic injuries, though my chest struck the steering wheel during the impact, causing ongoing pain my and mental trauma for my son. I am currently receiving medical advise. Given the speed and sequence of impacts, it is terrifying that the vehicle’s primary safety systems failed to engage before, during, or after the collision. I am seeking a thorough technical investigation into why Automatic Emergency Braking and airbag deployment failed in this scenario with Tesla Model Y

On Aug. 9, 2026, my 2022 Tesla Model Y experienced a sudden low-voltage electrical failure that completely immobilized the vehicle. Earlier that morning, the Tesla app sent a push notification indicating low-voltage battery service was needed. I tapped it, but it provided no actionable warning. I immediately checked Maintenance. At 9:06 AM, the app displayed “0 Recommended” and “No maintenance is recommended for your vehicle at this time.” Less than an hour later, the vehicle completely failed. Nothing functioned. We could not open the doors or trunk, access the vehicle normally, or operate vehicle functions. The vehicle could not be put into a condition for normal towing; it would have had to be dragged/pulled onto the tow truck. Tesla later documented “Electrical system is unable to support all features” and is replacing the 12V battery. This may be relevant to NHTSA PE25010.

During normal driving on roadway the vehicle pulled to the left and driver was not able to steer right to avoid travelling into oncoming traffic and struck another vehicle from opposing traffic. The vehicle gave no notice prior and was not in automatic mode.
While operating my 2022 tesla model y, the vehicle suffered a catastrophic hardware failure of the onboard vehicle control computer. This total computer failure resulted in an immediate, sudden loss of electronic power steering assist while the vehicle was in motion. The steering wheel became incredibly stiff and heavy, creating a severe and hazarders driving situation. the vehicle was towed to a tesla service center, where technicians confirmed the onboard computer hardware requires a complete replacement. A vital computer failing prematurely on a 2022 vehicle presents an extreme safety risk of crash or injury due to sudden loss of vehicle control.

The heat pump failed after experiencing temperatures below 10 degrees fahrenheit. There was no warning until after the failure, when I received a notification that the climate system needed maintenance. All heating/cooling, including defrosting/defogging the windshield, is inoperable. The service center confirmed that this is a heat pump failure and requires a replacement of the entire system, and a Tesla employee indicated that this is a common issue in the first 6-9 months of this cooling system design. This is well documented across many forums, and there is a class-action lawsuit in Quebec, Canada related to this failure.
HVAC system failure in sub-freezing weather causing loss of windshield defrost and cabin heat. Tesla has acknowledged this as a known hardware design limitation of the heat pump/octovalve system but is refusing repair coverage outside of warranty. This condition affects driver visibility and winter driving safety.

--torn bushings on both the lateral and compliance links --my understand is that torn bushings, if they completely failed, could cause a wheel to lose its rigid control causing the control arm to shift wildly, causing severe steering wander, loud clunking noises over bumps, unstable braking, and rapid, uneven tire wear. In worst-case scenarios, it can lead to a sudden loss of vehicle control and at worst cause an accident --also, this can also cause alignment issues and excessive tire wear--in my case, the torn bushings caused the front tires to chunk --three dealerships, including Tesla, confirmed the issue, including that it likely impacted the alignment and tires --the earliest symptoms were uneven tire wear: 1) all four tires from my first set were chunking and had to be replaced; 2) the front two exhibited the same chunking and have yet to be replaced
I am reporting a serious safety issue involving severe, difficult-to-detect inner-edge tire wear on my Tesla that resulted in a tire failure on Highway 101. The tire failed while I was driving on the highway, creating a dangerous situation and significant traffic disruption. After the failure, I discovered severe wear concentrated on the INSIDE edge of the tire. The concerning part is that the normally visible tread appeared serviceable. An owner can therefore believe the tire is safe while the difficult-to-see inner portion may be dangerously worn. I am concerned that Tesla’s suspension/alignment geometry, including camber/toe settings, vehicle weight, or related components may contribute to excessive inner-edge tire wear. NHTSA should investigate whether this is occurring on other Tesla vehicles and whether owners are being adequately warned to inspect the entire tire, particularly the hidden inner shoulder. I also experienced a very concerning mileage discrepancy. My vehicle showed approximately 36,000 miles before traveling to Tesla for service. During the trip to the dealership, the mileage associated with my vehicle appeared to increase to approximately 56,000 miles. Tesla subsequently referenced the higher mileage in connection with service/maintenance coverage. I request that NHTSA investigate whether Tesla vehicles have a pattern of excessive hidden inner-tire wear capable of causing sudden tire failures; whether suspension, alignment, camber, toe, vehicle weight, or related components contribute to this condition; whether Tesla has received similar complaints; and whether owners are adequately warned about this difficult-to-see wear. I also request review of the approximately 20,000-mile mileage discrepancy and whether the mileage Tesla relied upon accurately represented my vehicle’s actual accumulated mileage. I have video evidence and can provide photographs, Tesla service records, mileage information, and other documentation.

The rear lift gate will not close. Defective hinges and electrical strut causing miss alignment and inability to close.
On Saturday, [XXX] at around [XXX] going west on [XXX] toward [XXX] stopped at the intersection of [XXX]. While driving my 2022 Tesla Model Y, Performance, I was stopped at a red light. When the light turned green and I began accelerating normally through the intersection, both passenger-side doors unexpectedly unlatched at the same time. The windows on both passenger doors lowered slightly, and the vehicle displayed a warning indicating the doors were open and to drive carefully. This created an immediate safety hazard, as the doors opened while the vehicle was in motion without any intentional action from the occupants(there were 0 occupants there). This is the second time this exact issue has occurred under similar circumstances, with the previous incident also happening as I accelerated from a red light. The vehicle should not allow multiple doors to unlatch while driving. I have reported the issue to Tesla for inspection and request that this incident be documented and investigated as a potential safety defect. I was going less that 10 mph when this occurred. Thankfully, my toddler was not sitting in the back seat and there were no passengers in the car. No injuries. Hello. I brought my Tesla Y to Tesla service local at 7077 W. Sahara Av., Las Vegas, NV. To fix noise in the breaks on Aug 28, 2025, After that on the same day i went to drive Tesla and I started feeling severe ear pressure, heart pressures, severe body pain from high EMF exposure, also severe electric shocks from the floor, seats, back of the seat, wheel, it has been constantly ongoing for already 6 six month since Aug 28, 2025. It’s when ever I am getting inside my car, driving, or just when it’s parked, I feel all that severe pain in the body, severe ear pressure, severe heart pressure, pain in hands, legs from high EMF exposure inside Tesla, also electric shocks from the floor, back of the seat, seat, wheel. My Tesla Y caused severe damaged t my health. I have all those symptoms and pain only when I am inside the car, when I am not in the car, I don’t feel any pain, and I don’t feel all of those severe symptoms and pain in my body. I have this car from 2024, it was perfectly fine, except that during hot season in Vegas, when the temperature is above 80 F, the glass roof got extremely hot, and it burned my head severely despite I cover with sun protector glass roof. But besides that I didn’t have any issues with my Tesla Y. Only after I brought my car to Tesla service on Aug 28. 2025, same day Tesla started causing me severe pain from High EMF and high Voltage causing electric shocks. I contacted local Tesla service many times and tried to address this issues. They didn’t help. I got response from Tesla manager at W. Sahara “ hope it will resolve by itself”:(( no one addressed this issue seriously. I noticed, every time I broght my Tesla Y to local Tesla services here in Las Vegas, the EMF exposure and electric shocks became even more extreme, worst like a 100 times more:(( I honestly after these horrific experience with Local Tesla services I DONT TRUST THOSE TESLA TECHS anymore. PLEASE HELP AND CONDUCT HIGH EMF AND HIGH VOLTAGE INSPECTIONS FOR MY CAR!
One day I was exiting the vehicle, and as I swung my leg out to get out of the car the outboard panel broke. Nothing caught on it, it just broke as my weight shifted. As the technician came out to inspect it for repair, it was claimed that this was a common repair. The seat controls are on this panel, so when this happens, this means wires are loose, while the seat automatically moves for use. This changes how the seat positions itself, and the panel interferes with the door closing properly, the seat position.

Seat belt locked up on my son(8yr old) on the back of passager seat. he can't unbuckle. It got tight. I have to drove to fire department ask for help. We had 3 fire fighters helping him to get out of it. they can't. so the fire fighters have to cut the seat belt.

"I have a Tesla Model Y that experienced what looks like a steering malfunction, with documented data and repair records already in hand. "While driving our 2024 Tesla Model Y when the steering wheel suddenly turned itself, hard left, and driver couldn't regain control. The car went up a hillside, between a utility/ electricity pole and an electrical box, coming to a stop just inches from the pole. No injury, but real vehicle damage." "I pulled Tesla's own vehicle data log for that exact moment. It shows the steering angle went from +362° to -358° — basically two full wheel rotations — in about 1.5 seconds, while the torque sensor barely registered any force. Then the wheel stayed pinned at full-left lock for about 8 seconds while the car was actively accelerating in Drive." There was a fault code 14 mention in the data file right before the incident. "Tesla told us their review of the data showed 'no malfunction.' But afterward, Tesla's own repair records show they replaced the steering gear assembly and the drive unit — after we complained the steering felt stiff once we got the car back from the first repair."
Model Y (VIN: [XXX]). On XXX, I brought my vehicle to Tesla Service for replacement of the front wheel hub assembly and paid approximately $1,000 for the repair. Shortly after picking up the vehicle, I noticed abnormal knocking and clicking noises while turning the steering wheel. Because I was concerned about the safety of the vehicle, I sought a second opinion from an independent repair facility. On XXX, the independent shop inspected the vehicle and found that two wheel hub mounting bolts had not been properly tightened and that one mounting bolt was completely missing. The repair invoice states that the vehicle was being operated in an unsafe condition and that continued operation could have resulted in wheel hub failure and a serious accident. The shop corrected the condition by replacing the missing bolt and properly tightening the wheel hub assembly. I incurred an additional expense of $284.47, which would not have been necessary had the original repair been completed properly. The passenger-side (right) headlight assembly suffered an internal mechanical failure and is stuck aimed too high. It cannot be calibrated or lowered to factory specification. This poses a severe safety risk by blinding oncoming traffic at night. The vehicle was inspected by a manufacturer service center on July 13, 2026. The factory technician confirmed in the official service notes that the right-side headlight is 'internally faulty and will not lower the headlight height to specification,' requiring a full assembly replacement. This is the second time this exact OEM component has failed under normal use; the entire assembly was previously replaced for a failure just over two years prior. No external damage, collision, or impact caused this internal mechanism to fail.
My driver side headlight assembly stopped working. The headlight assembly includes the driving lights, high beams, and turn signals. I am open to having this product inspected upon request. This impacts the safety of me and others due to decreased visibility and the inability to appropriately indicate when I am turning. The dealer has not reviewed this yet, but I have a scheduled appointment on Friday, April 24. Vehicle has not been inspected by any third-party, improving manufacturer, police, insurance, or other representatives. There were no warning, lamps, messages, other symptoms of the problem prior to failure. The failure first appeared at around 55,000 miles, which was 5000 miles beyond my warranty. I believe that Tesla is pushing out updates that intentionally cause light assemblies and other high priced items to stop working after the warranty. there are multiple instances people replacing the same fixture after their warranty period has expired. Tesla charges $2000 to replace one light fixture. I believe this is fraudulent and corrupt business practices by Tesla.

The contact’s wife owned a 2022 Tesla Y. The contact stated that while the vehicle was parked, his wife shifted the gear shifter into drive(D), and the vehicle independently accelerated. The Autonomous feature was not engaged. The brake pedal was depressed; however, the vehicle failed to respond as needed, and the front center of the vehicle crashed into a pile of cinder blocks, drove through a gate, and crashed into a dumpster, where it came to a stop. As a result of the impact, the front driver's side tire detached, and the dumpster was damaged. The contact stated that another person helped remove the driver from the vehicle due to the front driver door being unable to open. The air bags deployed as intended. No warning lights were illuminated. The vehicle was towed to a tow yard and was deemed a total loss. A police report was filed. The driver had pain in the back and sought medical attention. The contact's wife was informed of a slight fracture of an unstated bone in her back and ribs, and had to return to the doctor several times for medical assistance. The contact was certain there was no user error to have caused the crash The manufacturer was notified of the failure. The failure mileage was unknown.
My 2022 Model Y, 213k miles, battery health 77%, no issues until Beginning of November 2025. After Tesla’s update, BMS_a079 popped up, capped charge at 22%. No dead cells, no imbalance—pure software bug remotely triggered by Tesla. Service refused written diagnosis, only said ‘nine thousand dollars for new battery. Risk of stranding.

The car stopped in a busy freeway saying electric issue and doesn’t give sufficient time to drive to shoulder. This is the 2nd time this happened within 2 weeks. First time, tesla said high voltage controller had an internal issue with voltage sensing. the hv controller was replaced along with pyrotechnic fuse. Tesla assured this shouldn’t repeat and it repeated again. This has kept us in a catastrophic situation. I request immediate attention to this error and matter.
I have repeatedly had electrical failure in the vehicle in the form of a "Reduced Power Alert" which renders the vehicle with significant limitation to drivability. This places the vehicle in "limp mode" and this frequently occurs on the highway. Once this Alert appears the vehicle has limited propulsion, limitations to steering and braking as well as other electrical systems. It has been to the dealership for repair on 4 occasions without remedy. It is leaving us on the highway in a dangerous situation repeatedly. It's not a one time event. There were 20 Alerts showing on the screen when I limped it into the dealership once again on [XXX] having nearly wrecked on [XXX] just before the [XXX] exit. The reason this is so dangerous is the failures occur at higher speeds such as highway driving. Leaving one with an immediate loss of forward motion as if you've hit the brakes but vehicles behind you have no warning. This is going to end up with someone getting rear ended or worse. Social media indicates it is a prevalent and not well understood recurring problem. Tesla has been quietly repurchasing vehicles from angry owners. Dreadful. HV failure failed on 2/12/26 @ 62k miles. Multiple errors (BMS_d265 Replacebatterypack, BMS_a079_Max_charge_limit_exceeded, and BMS_a029_dSOC_limiting. Frequent errors while driving, especially during acceleration on highway which were confusing/distracting. The errors seem to indicate danger with the vehicle and to not charge the vehicle/limit the charge to prevent further damage to the vehicle/surroundings. Took into service center on 2/13/26. Confirmed need a replacement HV Pack.
We live on a road with a steep embankment. My wife backed across our driveway and when she put the car from reverse to drive it accelerated uncontrollably. She hit our mailbox, then a power pole across the street, a driveway across the street then over the embankment where she hit a retainer wall another wooden railing and finally came to rest against our neighbors house. If she wouldn’t have caught the edge of the house she would have went down approximately 150 foot embankment. Not only the car accelerated on its own it is supposed to automatically brake, it didn’t and the airbags never deployed. This all this happened right after it went through an automatic update. The car was totaled, with my wife getting a concussion and a lot of muscle injuries. There was no warning lights prior to event and we reported it to Tesla and all they did was send camera images which we had. The car is now in possession of Wawanesa Insurance because it was a total loss

A tire experienced a sudden bead-area blowout while the vehicle was traveling at freeway speed. There was no pothole, curb, debris, or roadway impact. The failure occurred approximately ten days after installation. Inspection shows failure at the tire bead adjacent to a wheel defect. The wheel was represented as new at installation but shows evidence of prior material removal and cosmetic reconditioning, including rounded aluminum exposure and surface striations inconsistent with fresh impact. The compromised wheel-tire interface caused sudden bead unseating and rapid air loss.
GOODYEAR TIRE, 255/40R20 101W XL I have had to replace 5 tires on my Tesla in 13 months. 3 were from nails, one was from running over a pothole and one was flat from an unknown cause. I have been told by multiple people in the car maintenance field that the tires are not appropriate for the weight of the vehicle therefore are far more prone to imbedded nails and other spontaneous road hazards. I only do basic city driving and avoid industrial/construction zones when able. Also, 2 out of the 5 times I have had a tire with low pressure, the vehicle has indicated the wrong tire. For example, the most recent incident showed my tire pressure at 23 in the front driverside tire, when checked manually the pressure was reading 45 psi in that tire. Went to Tesla and they confirmed the sensor needed to be replaced and made an appointment to have that fixed in 2 weeks when they were available. I continued to drive the vehicle since I was told it “was just a sensor”. The next day, the passenger rear tire was flat but indicated appropriate pressure for that tire on car’s reading. I was horrified that I continued to drive on a low tire for an entire day and did not know because the car was indicating the wrong tire was low in pressure. This could’ve resulted in a blow out or other threat to my safety. Supposedly my sensors have been replaced when I have asked for them to be but the sensors reading the wrong tire pressures is a chronic issue. Tesla is aware the tires are prone to flats, which is why they will not sell you tire coverage. I feel unsafe in this vehicle.
