AS A RESULT OF THIS, AND THE PRECEDINGINVESTIGATION, RQ99-003, GM HAS EXPANDED THE POPULATION OF SAFETY RECALL 98V-150 (REPLACEMENT OF THE WINDSHIELD WIPER CONTROL MODULE) TO INCLUDE AN ADDITIONAL 1.77 MILLION VEHICLES.INCLUDED ARE VARIOUS 1994-1997 MY M/L VANS, S/T TRUCKS AND SUVS, & C/K TRUCKS AND SUVS.SEE ATTACHED TECHNICAL REPORT FOR DETAILS.

SEAT BELT ANCHORAGE SEPARATION
ODI HAS RECEIVED ONE (1) COMPLAINT ALLEGING THAT THE 2ND ROW PASSENGER-SIDE SEAT BELT RELEASE BUCKLE ANCHOR DETACHED FROM SEAT FRAME DURING A SIDE IMPACT CRASH.DURING THE CRASH, THE OCCUPANT WAS THROWN FROM HIS SEAT, PARTIALLY EJECTED THROUGH THE REAR QUARTER WINDOW OF THE VEHICLE.OCCUPANT SUSTAINED INJURY.GM HAS SUBSEQUENTLY CHANGED SEAT BELT ANCHORING DESIGN IN MY 2000. ODI HAS COMPLETED A PRELIMINARY INVESTIGATION AND HAS DETERMINED THAT FURTHER INFORMATION IS NEEDED TO DETERMINE WHETHER A SAFETY DEFECT EXISTS.

TL* THE CONTACT OWNS A 1995 CHEVROLET TAHOE. THE CONTACT STATED THAT THE CORNER OF THE FRONT DRIVER'S SIDE SEAT BRACKET HAD FRACTURED, WHICH CAUSED THE SEAT TO ROCK AND BECOME UNSTABLE. THE DEALER (GOOD CHEVROLET, 325 SW 12TH ST, RENTON, WA 98057, (425) 336-0640) STATED THAT THERE WAS NO RECALL ON THE VEHICLE. THE MANUFACTURER HAD NOT BEEN CONTACTED. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE FAILURE MILEAGE WAS UNAVAILABLE.

THE REAR DRIVER SIDE WINDOW BECAME INOPERABLE, FOLLOWED BY THE ENTIRE DOOR (IT WON'T OPEN AT ALL). THEN, THE REAR PASSENGER SIDE DOOR WOULDN'T OPEN FROM THE INSIDE, FOLLOWED BY THE DRIVER DOOR AND RECENTLY THE PASSENGER DOOR. THE DRIVER DOOR, FRONT AND REAR PASSENGER DOORS CAN ONLY BE OPENED BY ROLLING THE WINDOW DOWN AND OPENING THE DOOR FROM THE OUTSIDE HANDLE. WE NOTICED THAT AROUND THE SAME TIME THE DOORS WENT OUT, SO DID THE BACK BRAKE LIGHT PANELS (REPLACING THE BULBS AND HOUSING UNITS DON'T MAKE A DIFFERENCE). THE BACK BRAKE LIGHT PANELS ARE INTERMITTENT AND SEEM TO WORK ON OCCASION (IF WE HIT A BUMP OR SOMETHING). THIS IS A SEVERE SAFETY ISSUE AND I'M AFRAID WE WILL HAVE TO JUNK OUR TRUCK DUE TO IT. EXCESSIVE CORROSION ON POORLY COATED/TREATED BRAKE LINES LED TO A FAILURE TO STOP AT A STOP SIGN AND ALLOWED THE VEHICLE TO COAST INTO TRAFFIC AT AN INTERSECTION ON A BUSY MAJOR HIGHWAY. USE OF DOWNSHIFTING, THE EMERGENCY BRAKE, AND SHIFTING INTO 4WD WAS THE ONLY WAY A MAJOR ACCIDENT WAS AVOIDED. This Preliminary Information communication provides information to the technician on how to unlock radios that are Delco TheftLock. Technician will use the radio to get the scrambled code. Technician will need to contact the radio unlock center to retrieve the unlock code from the scrambled code. The PI update to version D is to advise the unlock procedure is temporarily unavailable. E version update is to add administrative details.
