Recalls, owner-reported complaints, investigations and safety data

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.
0
Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
0
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

The defect: Chrysler (FCA US, LLC) is recalling certain 2022-2026 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention from improperly sealed seams. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."
The risk: Air bags with insufficient pressure retention increase the risk of ejection and injury during a crash.
The remedy: Dealers will replace the side curtain air bags, as necessary, free of charge. Owner notification letters were mailed May 14, 2026. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06D, 10D, and 11D. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning April 2, 2026. This recall expands and replaces previous NHTSA recall numbers 25V302 and 25V573. Vehicles previously repaired under a prior recall will need to have the new remedy completed.

Radio "ETMR1H for 26MY and 27MY RU Pacifica, Voyager and Grand Caravan a recurring failure mode is to display a black screen or a blank center screen when using the submenus. This black screen may occur when preparing the RU for pre-delivery. Please do not replace the radio. The solution is to reset the radio and retry the submenus to assure that the issue is resolved. If the issue resolution is not in place, it is recommended that a hard reset be done for the ETMR1H. The hard reset can be done by disconnection of the radio completely and reconnecting. Another way is to completely disconnect the positive battery terminal such that the radio is completely without power and reconnecting. Once again, please do not remove and replace the radio with the failure mode of a black or blank screen. Current SW U42.13 is the production level SW for the ETMR1H and will be updated in the next few months."

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.

Higher-selling and older vehicles accumulate more reports. Counts are not failure rates and are not directly comparable between vehicles that sold in very different numbers. Older model years have had longer for reports to accumulate.
