The defect: VCI Emergency Vehicle Specialists (VCI) is recalling certain 2017-2018 Ford F-550, 2014 E-350 and E-450, 2015 F-250 and F-450, 2016 E-450, 2017 E-350, F-450 and E-450, 2018 F-350, 2010 E-450, 2005-2006 E-450, 2009 E-450 and Chevrolet C4500-based Type I and Type III ambulances equipped with ASA Electronics Voyager monitors used to display the back-up camera image. The monitors may unexpectedly revert back to the factory default settings which may cause the camera image to be reversed.
The risk: The driver may inadvertently turn the wrong direction to avoid an object behind the vehicle, increasing the risk of a crash.
The remedy: VCI Emergency Vehicle Specialists will work with ASA to notify the owners, and remedy the displays, free of charge. The recall began May 9, 2019. Owners may contact ASA Electronics at 1-800-384-4400.

The defect: Farber Specialty Vehicles (Farber) is recalling various specialty vehicles modified by Farber and equipped with model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation. The platform side plate of the affected wheelchair lifts may crack.
The risk: If the platform side plates crack, the lift platform can separate from the lift and come to rest against the vehicle's lift door. When the doors are opened, the platform may fall out, increasing the risk of injury to the lift operator.
The remedy: Farber will notify owners to take their vehicles to a Ricon dealer or service center who will install supplemental platform support bumpers. Any platform that has already started cracking will be replaced, free of charge. The recall began on March 24, 2015. Owners may contact Farber customer service at 1-800-331-3188, or Ricon customer service at 1-800-322-2884. Farber's number for this recall is 14E-041.

The defect: Elkhart Coach (Elkhart) is recalling certain model year 2006-2014 Ford E-350 and Ford E-450 vehicles manufactured January 1, 2006, to May 1, 2014 and equipped with certain model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation. The platform side plate of the affected wheelchair lifts may crack.

ODI has received 1 complaint and reports from 4 transportation fleets alleging 182 switch failures.Each fleet operates mini buses built on a Ford E350 or E450 cutaway chassis.The OEM HVAC blower switches have melted, smoked, burned or experienced open flame and become inoperable on vehicles in each fleet.Some drivers have reported a burning smell or smoke coming from the dash.One fleet reported a vehicle fire.No injuries were sustained.However, the bus was carrying special needs children when the driver noticed smoke coming from the dash.The driver was able to evacuate the children prior to the vehicle becoming engulfed in flames.This PE has been upgraded to an Engineering Analysis (EA) to determine if the blower motor switch failures create a situation where a vehicle fire is possible and presents a safety risk to the driver and other passengers.

The contact owns a 2006 Ford E-450. The contact stated while driving 15 MPH there was a high-pressure oil leak. There were no warning lights illuminated. The vehicle was taken to an independent mechanic who diagnosed the failure to be high-pressure oil leak and EGR failure. The vehicle was repaired; however, failure reoccurred. The manufacturer was notified of the failure. The approximate failure mileage was 64,000.

PULLED UP TO STOP SIGN. WHEN MAKING RIGHT TURN, ONTO INTERSTATE FRONTAGE ROAD, THROTTLE QUIT WORKING, WRENCH LIGH AND CHECK ENGINE LIGHT CAME ON. COASTED TO SIDE OF ROAD. VEHICLE HAD TO BE TOWED IN TO FORD DEALER. WAS TOLD ELECTRONIC THROTTLE CONTROL SHUT DOWN VEHICLE, AND THROTTLE BODY WOULD HAVE TO BE REPLACED. HAD IT HAPPENED ANOTHER MINUTE LATTER IT WOULD HAVE FAILED IN THE MIDDLE OF AN INTERSTATE HIGHWAY.

THIS SAME PROBLEM HAS BEEN REPORTED BY PEOPLE COMPLAINING ON BLOGS. WHILE IN MOTION THE VEHICLE WILL CUT OUT. ALL POWER JUST STOPS AND THE VEHICLE ROLLS TO A STOP. IT CAN BE RESTARTED. THERE IS NO INDICATION WHEN THIS WILL HAPPEN. IT HAS HAPPENED ON THE EXPRESSWAY, AT A REPAIR FACILITY, STOPPING AT A TRAFFIC LIGHT, GOING DOWN THE ROAD. SPEED IS NOT A COMMON FACTOR. THE KEY HAS ONE OR TWO OTHER KEYS ON THE "KEY CHAIN". THERE IS NO SPECIAL WEATHER SITUATION. THE PRELIMINARY REPORT REGARDING THE FIRE ONBOARD A COMMONWEALTH OF MASSACHUSETTS SOLDIERS HOME OWNED HANDICAPPED TRANSPORTATION BUS INDICATES THE FIRE CAUSE MAY HAVE BEEN ELECTRICAL IN NATURE. THE VEHICLE IN QUESTION IS A 2006 FORD E- 450. AN ODOR OF SMOKE WAS REPORTED BY THE DRIVER, ENIO LOPEZ, JUST AFTER ASSISTING A VETERAN ONTO THE BUS AT THE CHELSEA SOLDIERS HOME. UPON REPORTING THE ODOR OF SMOKE, THE DRIVER WAS INSTRUCTED TO REMOVE THE VETERAN FROM THE VEHICLE AND SHUT THE VEHICLE DOWN. THE FLEET SERVICE ADMINISTRATOR FOR THE CHELSEA SOLDIERS HOME, UPON INVESTIGATING THE ODOR OF SMOKE LOCATED A FIRE IN THE RIGHT FRONT OF THE BUS IN THE ELECTRICAL PANEL. THEY CONTACTED THE CHELSEA FIRE DEPARTMENT AND GRABBED THE EXTINGUISHER ONBOARD TO ATTEMPT TO EXTINGUISH THE FIRE AND PREVENT FURTHER DAMAGE. UPON DISCHARGING THE EXTINGUISHER, THE FIRE DID NOT GO OUT AND SEEMED TO BE INTENSIFYING ALSO SMOKE AND FLAMES WERE VISIBLE FROM THE ENGINE COMPARTMENT. A CAMPUS SECURITY OFFICER RESPONDING TO CALLS FOR MORE EXTINGUISHERS ASSISTED THEMIN FIGHTING THE FIRE. TWO MORE EXTINGUISHERS WERE PROVIDED TO THEM AND BY AN ENVIRONMENTAL SERVICES SUPERVISOR AND EMPLOYEE. CHELSEA FIRE DEPARTMENT ARRIVED ON SEEN AND FINISHED EXTINGUISHING THE FIRE, DISCONNECTING THE BATTERY AND OVERHAULING THE INTERIOR OF THE BUS TO EXTINGUISH THE REMAINING POCKETS OF FIRE. ONCE THE FIRE WAS COMPLETELY EXTINGUISHED THE BUS WAS TURNED BACK OVER TO THE COMMONWEALTH OF MASSACHUSETTS SOLDIERS HOME AND WAS TOWED TO ITS CURRENT LOCATION BEHIND VINNIE'S PLACE. *TR

NEW TIRES PURCHASED FOR CHURCH BUS WITHIN THE LAST 2 YEARS. OF THE 6 TIRES ON THE BUS, 4 HAVE EXPERIENCED TREAD SEPARATION. THE LATEST INCIDENT WAS ON 8/10/12. THE BUS WAS TRAVELING ON I-75 NEAR OCALA, FL. TRAVELING AT THE POSTED SPEED LIMIT OF 70, THE BUS EXPERIENCED A COMPLETE TREAD SEPARATION OF THE RIGHT FRONT TIRE. THE DRIVER WAS ABLE TO SAFELY GUIDE THE BUS INTO THE EMERGENCY LANE. NO ONE WAS KILLED OR INJURED. A NEW TIRE WAS INSTALLED AND WE HAVE THE OLD TIRE IN OUR POSSESSION. THE OTHER 3 TIRES EXPERIENCED THE SAME FAILURE BUT WE NO LONGER HAVE THOSE TIRES. *TR

AS I APPLIED THE BRAKES AND ATTEMPTED TO TURN THE BRAKES AUTOMATICALLY LOCKED UP AND WOULD NOT RELEASE. I WAS IN TRAFFIC THE TRUCK WOULD NOT MOVE IN THE MIDDLE OF THE HIGHWAY. IT ALSO HAPPENED AGAIN AS I ATTEMPTED TO BACK INTO THE DRIVEWAY. I APPLIED THE BRAKES AND BEGAN TURNING THE STEERING WHEEL THE BRAKES AGAIN WENT TO THE FLOOR AND WOULD NOT RELEASE. ASK THE DEALERSHIP AND THEY SAID OH YEAH THAT HAPPENS SOMETIMES ON THOSE VEHICLES. *TR

Mixing MERCON LV and MERCON SP in the same transmission is acceptable. After 30-Jun-2023, MERCON SP ATF will no longer be manufactured and will only be available until inventory depletes.

These SKUs are Trailer Brake Control Modules. The customer communication requested return of unsold inventory due to an internal software issue. The internal software issue leads to a false check trailer brake message and Trailer Brake Controller fault whenever the brake pedal is pressed. This will cause the part to be replaced, due to repetitive dashboard warning light, even though it sends signals to the trailer brakes properly.
