Loss of Power Brake/Steering Assist Post Recall 22V089 Inspection.
The Office of Defects Investigation (ODI) opened Recall Query (RQ) 24003 on February 27, 2024, to evaluate the scope and remedy effectiveness of NHTSA recall 24V097. On February 16, 2022, Ford Motor Company (Ford) issued a safety recall, NHTSA recall 22V089, for certain 2021-2022 Ford E-350 and E-450 vehicles equipped with dual rear wheels. According to the filing, specific hydraulic lines could leak resulting in a sudden loss of power steering and power brake assist, increasing the risk of a crash. The remedy involved a dealer inspection of the lines and their replacement as necessary. Only vehicles that failed the inspection received updated parts. On February 9, 2024, Ford issued a new recall, NHTSA recall 24V097, to replace the specific hydraulic lines on any vehicle that had not yet been inspected and on any vehicle which had passed inspection within the previous twelve months. However, vehicles which had passed the inspection more than twelve months ago would not be included within the scope of the new recall, as Ford deemed them to have a low likelihood of failure. ODI opened this RQ to better understand Ford's scoping of recall 24V097, specifically concerning the inspected-only vehicles not included in the new recall filing. ODI sent Ford an information request letter on April 18, 2024. Ford responded, in full, on May 31, 2024. ODI reviewed Ford's data which showed that the vehicles not included in the new recall had a predicted failure rate of less than 0.25% within 10 years from the date of inspection. By comparison, the vehicles included in the new filing have had a failure rate of approximately 2% prior to being recalled again. ODI analysis of complaint and warranty data further supports Ford's assessment that vehicles that passed inspection more than twelve months prior to recall 24V097 are unlikely to experience the defect condition. ODI is closing this RQ without a manufacturer action. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. The contact owns a 2022 Ford E-450. The contact received notification of Customer Satisfaction Program: 24N03 (Hydro boost or Hydro max Power Steering Pressure and Jumper Line Replacement). The contact stated that while driving 70 MPH, the brake pedal was depressed but the vehicle failed to stop immediately. The contact downshifted the transmission from fifth to first gear to stop the vehicle. Additionally, the vehicle experienced a loss of power steering assist while turning. There was no warning light illuminated. The vehicle was towed to the dealer, where it was determined that the hydro boost power steering pressure and jumper line needed to be replaced. The vehicle was not repaired. The contact related the failure to NHTSA Campaign Number: 24V097000 (Steering, Service Brakes, Hydraulic); however, the VIN was not included in the recall. The manufacturer was made aware of the failure and a case was opened. The failure mileage was 12,750.

We operate numerous 7.3L Ford engine equipped vehicles in our fleet of ambulances. Over the past year we have experienced 4 engine failures of this 7.3L engine in vehicles ranging form around 60,000 to 90,000 after exceeding the service recommendations stated from the manufacturer. I have contacted the Ford Pro Fleet experts and have asked repeatedly about what could be causing the failures, trying to figure out if we are doing something wrong in our maintenance or operation. They assure us we are not doing anything wrong, however we continue to lose engines. Each time the repair bill is around $12,000. We have been lucky that Ford has offered financial assistance, but now that has gone away. At first, we were told that we were the only ones experiencing these failures, but now I am hearing from other operators of similar vehicles that they are experiencing the same concern. We cannot afford to keep replacing engines in relatively new vehicles, especially when there is an apparent defect in parts or standard operation. Symptoms we experience that lead to engine replacement are loss of power while driving caused my multiple cylinder misfires. Loss of power while running an emergency call pose a significant safety risk to operators and patients.

The contact owns a 2022 Ford E-450. The contact had received notification of NHTSA Campaign Number: 22V089000 (Steering, Service Brakes, Hydraulic) and the vehicle was taken to the dealer to be repaired under the recall. The contact stated that the recall repair was not performed; however, the dealer inspected the vehicle and informed the contact that the recall repair was not needed, and that the vehicle was good to be driven. The contact stated that while driving at an undisclosed speed, the power steering and the service brakes were inoperable. The contact stated that the steering failed to function as intended. Additionally, the contact stated that while depressing the brake pedal, the brakes failed to respond as needed. The contact was able to stop the vehicle. The vehicle was towed to Castle Ford in Michigan City. The vehicle was diagnosed with failed power steering connection line and a failed brake hydro boost unit. The vehicle was then repaired under NHTSA Campaign Number: 22V089000 (Steering, Service Brakes, Hydraulic). The contact stated that the failure occurred thirty days after the vehicle was serviced at the dealer and the dealer declined to complete the recall repair. The contact stated that the failure occurred while out of town. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 4,000.

I am a mechanic at Huron Valley Ambulance. Ford has a recall that covers some of our Ambulances. Recall NO: 22S08 for the power steering lines coming apart. I have been having issues with the hoses coming apart even after the recall has been performed. The Ford dealer will not cover this repair after the recall has been done. This is a safety issue as both the steering and braking are effected. It is also time consuming and expensive to repair. I feel this recall is not correcting this problem and needs to be addressed.

The tire pressure monitoring system fails frequently while being driven. The first few times I pulled off the road and checked the pressures, and they were normal. I had the system checked at a Ford dealership and they couldn't find any problem. It failed again. I returned to the dealership and they said they couldn't find a problem and there is nothing they could do for me. So I drive away from the dealership and the same thing happens. Believing that they can't do anything I keep driving and manually checking pressures. Problem returns. I went to a different Ford dealership and they said they found a faulty sensor and replaced it. The same problem came back after less than a few miles. I needed gas so I filled up and realized that after turning the ignition off and restarting, the light on the dash and the warning went away. I returned to that dealership and they said after hearing my explanation. They couldn't do anything unless the fault was active at the time I brought it to them. So I was told to drive it until I got the safety warning and to not shut it off until I got to a Ford dealership. Really!!! Not all Ford dealerships will service the E450s with RVs built on the chassis. Reasonable? No way! There is a problem with the system that Ford should be able to correct. I don't feel safe having to pull over and check tire pressures on a busy highway. And it's not necessary if they address the problem and fix it

Ford and Lincoln vehicles equipped with wired keyless entry keypad systems and accessory wireless keyless entry keypad systems may or may not come with a wallet card containing the master code. Unlike the integrated wired keypad, the accessory wireless keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool or from the label printed on the body control module (BCM). The Factory Keyless Entry Code application within the diagnostic scan tool will not provide an applicable master code for the accessory wireless keypad. If the wallet card for an accessory keypad is not available, the "Wireless RF Keypad Diagnosis Guide" can be referenced and provides direction on how to retrieve the master code. This guide is located under the Workshop Manual tab > Accessories > Installation > Wireless RF Keypad Diagnosis Guide > PPT C > Step C2.<br /><br /> Note: The only available keypad for some vehicles from the assembly plant is the accessory wireless keyless entry keypad. Some of the vehicles listed in the Model statement above may exhibit an upper engine ticking or tapping noise at idle. Customers may also report experiencing rough engine performance, engine misfire, or a malfunction indicator lamp (MIL) with diagnostic trouble code (DTC) P0301-P0308 in the powertrain control module (PCM). This may be due to camshaft and/or valve tappet having excess wear within the engine assembly.
ENGINE; POWER TRAIN
Some 2021-2025 Expedition/Navigator/F-150/F-Super Duty/F-53 Motorhome/F59 Commercial Stripped Chassis/F-650/F-750/Transit vehicles equipped with a 2.7L/3.5L (non hybrid)/6.8L/7.3L engine and 2021-2026 E-Series may exhibit a rattle/scraping noise from the bellhousing area on start-up. Workshop Manual (WSM), Section 303-00 Engine System - General Information > Diagnosis and Testing and 303-06 > Starting System has been updated to address this condition. Additionally, WSM, Section 303-01 > Removal and Installation > Engine to Transmission Spacer Plate has been added to address this condition. Refer to the revised WSM sections for normal diagnosis.
POWER TRAIN
Some 2021-2025 F-150 vehicles equipped with a 5.0L/5.2L engine, 2021-2025 F-Super Duty/F-53 Motorhome/F59 Commercial Stripped Chassis vehicles equipped with a 6.8L/7.3L engine and 2021-2026 E-Series/F-650/F-750 vehicles equipped with a 7.3L engine may exhibit a rattle/scraping noise upon startup that may remain after the engine is running. This may be due to the starter motor drive gear not retracting after starting. To correct this condition, inspect the starter motor drive gear and shaft. If the starter motor drive gear slides back and forth by hand and/or shows signs of scoring or damage, replace the starter motor following Workshop Manual (WSM), Section 303-06 Starting System > Removal and Installation > Starter.
POWER TRAIN
Some of the vehicles listed in the Model statement above may exhibit an incorrectly illuminated malfunction indicator lamp (MIL) with diagnostic trouble codes (DTC) P0420 and/or P0430 stored in the powertrain control module (PCM). This may be due to the software in the PCM.
