The defect: ON CERTAIN TRUCKS EQUIPPED WITH A SPEED CONTROL SYSTEM, THE SPEED CONTROL DEACTIVATION SWITCH MAY, UNDER CERTAIN CONDITIONS, LEAK INTERNALLY AND THEN OVERHEAT, SMOKE, OR BURN.
The risk: THIS COULD RESULT IN AN UNDERHOOD FIRE.
The remedy: DEALERS WILL INSTALL A FUSED WIRING HARNESS. AN INTERIM NOTICE WAS SENT TO OWNERS ON SEPTEMBER 7, 2007, DIRECTING THEM TO HAVE THE SPEED DEACTIVATION CONTROL SWITCH DISCONNECTED TEMPORARILY. A SECOND NOTIFICATION LETTER TO OWNERS WAS IN FEBRUARY 2008 ADVISING THEM TO RETURN TO DEALERS FOR INSTALLATION OF THE REPAIR PART. OWNERS OF AFFECTED VEHICLES WHO DID NOT RECEIVE A PHASE 2 NOTICE AND HAVE NOT HAD THE RECALL COMPLETED WILL BE NOTIFIED BEGINNING AUGUST 6, 2008. OWNERS MAY CONTACT MAZDA AT 1-800-222-5500.

The defect: CERTAIN AFTERMARKET FUEL FILTERS SOLD UNDER THE PUROLATOR BRAND NAME, P/N F65277, SHIPPED FROM JANUARY 16 THROUGH JUNE 2, 2006, FOR USE ON THE ABOVE LISTED PASSENGER VEHICLES. DUE TO INCORRECT DESIGN, THE QUICK CONNECTORS MAY NOT FULLY SEAT WITH THE TUBE INTERFACE, EVEN THOUGH THE PERSON MAKING THE CONNECTION MAY BELIEVE THAT THE QUICK CONNECTOR IS FULLY ATTACHED.
The risk: FAILURE OF THIS CONNECTION COULD LEAD TO FUEL SPILLAGE AND, IN THE PRESENCE OF AN IGNITION SOURCE, A FIRE COULD OCCUR.

The defect: TIRE DESCRIPTION: FIRESTONE WILDERNESS AT, SIZE P235/75R15, PRODUCED AT THE DECATUR, ILLINOIS TIRE PLANT AND ALL FIRESTONE RADIAL ATX AND RADIAL ATX II TIRES, SIZE P235/75R15. OPERATION OF THESE TIRES AT LOW INFLATION PRESSURES, HIGH SPEED, AND IN HOT WEATHER, CAN CONTRIBUTE TO SEPARATION OF THE TIRE TREAD.
The risk: IF THE TREAD SEPARATES FROM THE TIRE, THE DRIVER CAN LOSE CONTROL OF THE VEHICLE, POSSIBLY RESULTING IN A CRASH CAUSING INJURY OR DEATH.
The remedy: THE REPLACEMENT/REIMBURSEMENT PROGRAM FOR THIS CAMPAIGN EXPIRED ON AUGUST 29, 2001. HOWEVER, CUSTOMERS SHOULD CONTACT FIRESTONE CUSTOMER SERVICE AT 1-800-465-1904 FOR POSSIBLE ASSISTANCE.

THE CONTACT STATED THAT THE AIR BAGS DID NOT DEPLOY UNTIL AFTER THE CRASH, AND THE SEAT BELT PRETENSIONER DID NOT TIGHTEN. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A COLLISION SHOP AND DEEMED DESTROYED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THERE WAS ONE INJURY THAT REQUIRED MEDICAL ATTENTION. THE CONTACT SUFFERED FROM A BROKEN NECK AND SPINAL DAMAGE. THE VIN WAS UNKNOWN. THE FAILURE MILEAGE WAS 287,000.

TL* THE CONTACT OWNED A 1999 MAZDA B-SERIES TRUCK. THE CONTACT STATED THAT WHILE DRIVING AT APPROXIMATELY 40 MPH, ANOTHER VEHICLE CRASHED INTO THE FRONT DRIVER SIDE OF THE CONTACT'S VEHICLE. THE AIR BAGS DEPLOYED. A POLICE REPORT WAS FILED. THE CONTACT SUSTAINED A FRACTURED VERTEBRAE, FLATTENED SPINAL CORD, PINCHED NERVE, AND CARPAL TUNNEL TO THE RIGHT HAND. IN ADDITION, THE CONTACT WAS KNOCKED UNCONSCIOUS FROM THE AIR BAG AND REQUIRED MEDICAL ATTENTION. THE VEHICLE WAS DESTROYED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. Some customers may complain of signs of oil on the struts or shock absorbers. In most cases the oil that is present is normal, and a result of the oil that remains on the shaft during the normal sweeping process of the shaft seal. Follow the inspection guidelines below to determine if the condition is a result of the normal sweeping process or a failure of the shaft seal.

POWER TRAIN

Some customers may experience a squeaking noise may be heard from the exhaust manifold when the idle speed drops from fast idle to normal idle speed during cold start. This is caused by a variable valve being stuck in the closed position. When returning from fast idle to normal, the stuck valve suddenly opens due to thermal change around it which causes abnormal noise. To eliminate this concern, the internal design of the exhaust manifold has been modified.

This service bulletin provides testing and charging procedures in order to promote proper battery maintenance, to deliver new vehicles with fully charged batteries, and to reduce unnecessary battery replacement cost. For new vehicles in stock at the dealer, inspect and maintain the battery periodically according to the repair procedure.
