On June 14, 2016, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE16-006 to investigate 135 complaints alleging incidents of yaw rate sensor malfunctions resulting in unexpected Vehicle Stability Control (VSC) activations and steering pull while driving in model year (MY) 2001-2002 Toyota Sequoia vehicles.ODI identified a total of 958 incidents in complaints provided by Toyota or submitted to ODI by consumers.ODI conducted the following work to assess the alleged defect in the subject vehicles: 1) evaluated the design of the subject yaw rate sensor; 2) initiated vehicle testing and instrumentation; 3) performed failure analysis on subject components collected from the field; 4) analyzed field data for evidence of system faults; and 5) reviewed the performance of the subject system in fault simulations testing conducted by Toyota.ODI review indicated that an overwhelming majority of incidents are related to flashing VSC slip indicators on the instrument panel accompanied by warning buzzers and grinding noise or vibration caused by pulsed brake application to one of the front wheels.Yaw rate sensor malfunctions resulting in VSC brake applications is accompanied by a brief duration of longitudinal deceleration with minimal changes in vehicle speed and will not result in abrupt lane change or loss of vehicle control due to steering controllability.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety related defect does not exist.For additional information, see the closing resume appendix and the investigation file for PE16-006.

I own a 2001 Toyota Sequoia SR5, which I purchased new in Colorado and moved to Virginia in 2004. I received a Toyota Limited Service Campaign Program notice in 2013 about excessive corrosion on the frame. Alexandria Toyota inspected the vehicle and applied a Corrosion-Resistant Compound to the frame. They also replaced the spare tire carrier assembly due to rusting. Over the years, I've had mechanics mention the rust on the frame but Toyota never issued a recall on the 2001 Sequoia. In November 2024, the following dashboard lights remained on while driving: Vehicle Skid Control (VSC) OFF, VSC TRAC and Brake Lights. I took it to Steven’s Auto Clinic in Alexandria. They found low brake fluid levels were causing the brake system failure. The brake pipes are leaking due to heavy rust. In addition, rust was destroying the under carriage. The brake system and frame would require extensive maintenance. They warned me NOT to drive the car. They refilled the brake fluid to allow me to take the car home. Reading the Complaints under Structure on the NHTSA website, dozens of customers over the years mention serious rusting on the frame of their 2001 Sequoias. I believe Toyota had a recall to replace the frame on the Tundra but never included the 2001 Sequoia even though it was built on the Tundra frame. I now have a vehicle I can’t drive, due to safety concerns, and I can’t sell, except for parts. The Sequoia should have been included in the Tundra recall. Toyota needs to be held accountable. I can provide any paperwork for my 2001 Sequoia including the Limited Service Campaign Program notice and the repair order from Stephen’s Auto Clinic if necessary. I can also provide the vehicle for inspection.

The contact owns a 2001 Toyota Sequoia. The contact stated while driving at various speeds, he became aware of a cranking sound and the vehicle suddenly started jerking and shaking. An unknown warning light was illuminated. The contact stated that the vehicle was repaired, and the front wheel speed sensor was replaced; however, the failure persisted. The contact stated the failure had gotten worse over time. The contact also stated that while searching online, he became aware that the failure was a common failure with similar vehicles. The vehicle was not diagnosed or repaired by an independent mechanic or local dealer. The manufacturer was not made aware of the failure. The failure mileage was 181,000.

The contact owns a 2001 Toyota Sequoia. The contact stated that the vehicle was at the dealer for a diagnostic test; however, the dealer declined to perform the diagnostic test, stating that the sub frame was unstable, and an unknown recall campaign had expired. The manufacturer was made aware of the failure. The contact stated the dealer informed him there was a recall for the sub frame, however, the recall had expired. The failure mileage was unknown.

Rear axle is rusted to the point where differential fluid is leaking out of the body. There is extreme rust in general which has led to the failure of multiple components such as the oil pressure sending unit. Check engine light is on. This hasn't been verified by a shop or agency, only by me. These issues were discovered shortly after purchase this year.

While driving my 2001 Sequoia SR5 with only 74,750 miles on freeways at speeds between 55 and 65 mph, the SUV's braking system activates unexpectedly and without cause. The VSC check engine lighting and warning sound illuminate. My SUV veered to the right, the steering wheel vibrated, and it went into a skid when the brakes were abruptly engaged, causing it to skid. My SUV abruptly decelerated as if it had lost power. This situation was hazardous for other vehicles who could not tell if they were applying the brakes due to the absence of visible brake lights to prevent a collision. Fortunately, I responded defensively and avoided collisions with other vehicles and roadside obstacles. This issue is a safety concern; if it occurs at the wrong moment, a tragedy can be avoided. I've seen more than a hundred complaints from other proprietors and a completed NHTSA investigation. Sequoia models from 2001 and subsequent years were recalled for the same issue. Before a life is lost, Toyota must address this issue in all model years.

To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.

The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. It is the dealer’s responsibility to protect and maintain the quality of the vehicle’s paint finish after receipt at the dealership prior to the first sale. In areas known for high frequency and/or concentration of acid rain, frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.

Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: •Minor damage: requires only buffing to repair. •Moderate damage: usually requires neutralizing, color sanding, and buffing. •Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. It is the dealer’s responsibility to protect and maintain the quality of the vehicle’s paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.

This material protects from acid rain, environmental fallout, and rail contamination. Follow the Removal Procedure in this bulletin to remove the RapgardTM protective film within 90 days from initial application.

Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid. Follow the Repair Procedure in this bulletin to clean vehicles that may have been subjected to contamination by airborne iron particles such as rail dust during rail transportation or extended storage near industrial areas.
