Rearview Camera FMVSS 111 Noncompliance
On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs. Ford presented rate and scope data for many of its MY 2020 products during the August 2020 meeting indicating high failure rates for certain models.This RQ is being opened to investigate both the timeliness and scope of Ford's rearview camera recall (20V-575), and Ford's compliance with reporting requirements.The subject reports referenced in the opening resume are identified by the following ODI reference numbers which can be reviewed at NHTSA.gov: 11349813, 11344126, 11342696, 11341486, 11340254, 11337274, 11331444, 11330875, 11329235, 11316960, 11343521, 11341884, 11337226, 11344018, and 11339573.
Seat Belt Retractor Pretensioner Inadvertent Deployment
On November 22, 2023, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE) PE23021 to investigate complaints of front seat belt retractor pretensioner inadvertent deployment in model year (MY) 2019 Ford Expedition vehicles manufactured by Ford Motor Company. Some complaints alleged hearing a loud, explosion-like sound immediately followed by the seat belt tightening around the occupant’s torso area. The complaints did not state that a crash occurred that may have resulted in the deployment of the pretensioner system, suggesting that this deployment was inadvertent or otherwise unwarranted. On February 9, 2024, Ford issued safety recall 24V-099, recalling certain MY 2018-2020 Ford Expedition and Lincoln Navigator vehicles equipped with driver and front passenger seat belt retractor pretensioners built between October 1, 2018, and June 30, 2019. Ford described the defect as a seat belt retractor pretensioner assembly that may develop corroded squib pins, resulting in high resistance or an open circuit, which could result in an inadvertent deployment of a pretensioner and an increased risk of injury due to the loss of the pretensioner’s protection and a seat belt that remains in the locked position. This recall includes 77,574 vehicles. On November 15, 2024, ODI opened this Recall Query (RQ24015) after receiving 3 complaints involving vehicles not included in recall 23V-099 but that alleged a similar failure. As a result of this investigation, Ford issued safety recall 25V-197 on March 28, 2025, to expand the scope of 24V-099 to include MY 2018-2020 Ford Expedition and Lincoln Navigator vehicles equipped with driver and front passenger seat belt retractor pretensioners built between October 1, 2018, and July 31, 2020. This recall includes 105,322 vehicles, some of which are also included in 24V-099. Additionally, on May 23, 2025, Ford issued 25V-197 Amendment #1, which further expands the recall population’s scope for vehicles with build dates of September 2018. This amendment added 7,627 additional vehicles to the 25V-197 population. In response to ODI’s inquiries in RQ24015, Ford indicated that it cannot conclusively determine the root cause of the relevant failure. Ford was able to determine that certain pretensioners may develop corroded squib pins potentially resulting in inadvertent deployment, but it was unable to specify the cause of corrosion. Without a specific root cause, Ford assessed field performance of pretensioners to determine the scope of the recalls. ODI is aware of three vehicles that experienced pretensioner inadvertent deployment that are not included in either 25V-197 or 24V-099. Two of these vehicles are MY 2018 Lincoln Navigator vehicles built prior to October 1, 2018, and both experienced failures after more than six years of use/exposure while the recalled vehicles experienced higher rate of failures with less exposure. The remaining incident involves a MY 2021 Ford Expedition that was built after July 31, 2020. ODI is not aware of any other relevant failures in vehicles built after July 31, 2020. As such, ODI has not currently identified a safety defect trend for this issue in vehicles outside of the recalled populations. ODI will continue to monitor allegations of pretensioner inadvertent deployment in subject vehicles that are not included in 25V-197. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24015). The agency reserves the right to take additional action if warranted by future circumstances. The contact owns a 2020 Ford Expedition. The contact stated that while driving over 10 MPH with her husband inside the vehicle, while depressing the accelerator pedal, the vehicle hesitated, and the brakes independently engaged. No warning light was illuminated. A dealer was contacted. The vehicle was taken to the dealer to be diagnosed, and it was determined that the transmission needed to be replaced. The contact stated that the battery was replaced, and the vehicle was repaired. The contact stated that a software update for the rearview camera was performed. The manufacturer was not notified of the issue. The contact stated that the dealer refused to provide the telephone number for the manufacturer. The contact associated the failure with NHTSA Campaign Number: 26V402000 (POWER TRAIN); and the VIN was included in the recall. The contact was referred to the NHTSA Hotline for assistance. The approximate failure mileage was 117,000.

Last week I took the cover off our car seat to wash and the foam on the headrest was cracked all the way through. I have no idea when this happened or even how it could have. This car seat is barely 2 years old. I’ve never been in an accident or dropped this seat, and it is not used improperly.

I was driving home from vacation on I-77 in Virginia, and felt large jolt and heard several clicking type noises. Two separate service engine lights immediately came on the dashboard, and I was able to safely pull over. I had my vehicle towed to a nearby dealership to be looked at by a mechanic. The mechanic indicated the existence of safety recall notice 26S48/(NHTSA 26V402). I spoke with the mechanic who removed the diagnostics lights on the dashboard as prescribed by Ford, and attempted to see if they would be triggered again by driving the vehicle around. I described what had happened and the mechanic immediately said the description of this safety recall seems to be the culprit as to what was experienced. The parking pawl engaged while I was on I-77 traveling 70mph, causing the transmission to inadvertently shift into park causing the jolt and sounds I described. I contacted the dealership where I purchased the vehicle and Ford manufacturer directly regarding this recall, and was told as there is no remedy available. I have several concerns as the remedy is not expected until 5/31/2027 which is way too long of a period to have to wait when my vehicle has clearly been diagnosed with this recall issue, as shifting into park while driving presents a MAJOR safety risk that puts lives in danger. Furthermore, the parking pawl can cause significant damage to the vehicle's transmission and may make me liable for significant repair costs that 100% could be avoided by Ford providing a remedy in a reasonable amount of time. I need my vehicle to either be repaired, repurchased, or provided a loaner car by the dealership while I await this remedy to avoid this situation from happening and resulting in me having to pay for a new transmission or worse potentially causing an accident. This vehicle was inspected by the state of Pennsylvania in 2026.

The contact owns a 2020 Ford Expedition. The contact stated that while reversing at an undisclosed speed, the seat belt retractor exploded, and the seat belts became inoperative. The air bag warning light was illuminated. The contact received notification of NHTSA Campaign Number: 25V197000 (Seat Belts); however, the part to do the recall repair was not yet available. The local dealer was contacted and confirmed that the part was not yet available for the recall repair. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure. The failure mileage was approximately 110,361.

While driving on a public road, the recalled driver's side seatbelt pretensioner (Recall 26S34) spontaneously exploded without a crash. The seatbelt violently locked on my body, causing friction burns and bruising to my skin while the explosion caused hearing loss in my left ear. The vehicle is now un-driveable as the seatbelt no longer works and has been towed to a Ford dealership. The dealership has obtained possession of the vehicle stating it is not safe or legal to drive. We have contacted both the dealership and corporate ford for assistance with alternate transportation and have not been provided with any thus far. We have called 10 different times and visited the dealership in person twice & still no help. Since the incident, I experience high anxiety and stress whenever I am inside a motor vehicle. To add to this, the loss of my vehicle has caused severe logistical distress, disrupting my daily routine, which compounds my daily anxiety. It has been 10 days without any answers, only to be told if we don’t hear anything by mid August to call back and then we can escalate. This is unacceptable.

The contact owns a 2020 Ford Expedition. The contact stated that while driving 45 MPH and depressing the accelerator pedal, the vehicle started coasting, and thirty seconds later the vehicle operated as intended. The contact then heard a loud sound coming from the front of the vehicle. A dealer was contacted. The vehicle was taken the dealer where it was diagnosed and determined that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. Car is equipped with power running boards. Set to come down when the doors are opened on that side, and return up and tucked under when doors on that side are closed. Lately they have been glitching and stuck down, stuck up, or, the most dangerous - go back up under car when door is opened and stepping on to them - children's feet have almost been crushed in them tucking under the car. I have tried overriding the system in the car to make them either stay out or stay in regardless of when the door is opened or closed but it resets when powered off and you can only reselect it when in park the next time. If the power running boards fail to pull up and tuck under the car when the door is closed, you can hear the system trying to lift it over and over while driving. There is no warning when it malfunctions, you have to open the door and just wait a moment to let it decide what to do. Unable to replicate it at a repair shop. System had been reset for a safety recall and then another visit resulted in a drained battery. Both of those temporarily fixed the issue for a few days but then it goes back to the above.

ELECTRICAL SYSTEM; LANE DEPARTURE: BLIND SPOT DETECTION
Various 2020-2026 Ford and Lincoln vehicles equipped with blind spot information system (BLIS) may experience an inoperative blind spot monitoring with a "Blind Spot System Fault" message displayed in the instrument panel cluster (IPC). This may also be accompanied by diagnostic trouble codes (DTCs) B118C:11 and/or B118C:15 stored in the driver door module (DDM), and/or B118D:11 and/or B118D:15 stored in the passenger door module (PDM). This may be due to the wiring connector from the mirror to the BLIS module. If diagnostics lead to an internal mirror wiring harness fault, a new lower level BLIS light pigtail is now released for service. It is not necessary to replace the exterior mirror assembly. Use part number SC3Z-14A411-B to resolve the concern.

Some 2019-2024 Ford and Lincoln vehicles equipped with SYNC3 may experience various connectivity concerns with The Ford App Lincoln App including but not limited to: missing activation prompt pop-upon the vehicle's center display screen when attempting to activate vehicle through The Ford App Lincoln App, incorrect location global positioning system GPS reported through The Ford App Lincoln App, problems with remote commands being performed through The Ford App Lincoln App and or incorrect data information reported on The Ford App Lincoln App. These symptoms will be accompanied by diagnostic trouble code DTC B156D 89 in the TCU. This may be due to the accessory protocol interface module APIM software. To correct the condition, update the APIM software.

Some 2015-2021 Ford and Lincoln vehicles may exhibit an illuminated MIL with diagnostic trouble codes (DTCs) P0011, P0012, P0014, P0015, P0016, P0017, P0018, P0019, P0021, P0022, P0024 and/or P0025. Pinpoint test HK in the Powertrain Control and Emissions Diagnosis (PC/ED) or in Section 303-14 of the Workshop Manual (WSM) has been updated to address this concern.

Reason for update: update the Part List Description and Labor Times Description Some of the vehicles listed in the Model statement above may exhibit at least one of the following conditions: * Harsh engagement * Delayed engagement * Harsh shift * Delayed shift * Illuminated malfunction indicator lamp (MIL) with diagnostic trouble codes (DTC) P0751, P0752, P0756, P0757, P0761, P0762, P0766, P0767, P0771, P0772, P2700, P2701, P2702, P2703, P2704, P2705, P2707, P2708, P0729, P0731, P0732, P0733, P0734, P0735, P0736, P076F, P07D9, P07F6 and/or P07F7 stored in the powertrain control module (PCM) or transmission control module (TCM) This may be due to axial movement of the CDF clutch cylinder (7H351) sleeve causing hydraulic circuit leaks. NOTE: If internal transmission service is required to address a concern detected with the CDF clutch cylinder following this article, technicians should carefully inspect and replace other transmission components and flush transmission fluid cooler only as necessary to confirm proper function.
