OEM HITCH RECEIVER PLATFORM CRACKING
THE PETITION ALLEGES THAT CRACKS IN THE HITCH RECEIVER PLATFORM MAY LEAD TO SEPARATION OF THE TRAILER AND LOSS OF STEERING CONTROL

AS A RESULT OF THIS, AND THE PRECEDINGINVESTIGATION, RQ99-003, GM HAS EXPANDED THE POPULATION OF SAFETY RECALL 98V-150 (REPLACEMENT OF THE WINDSHIELD WIPER CONTROL MODULE) TO INCLUDE AN ADDITIONAL 1.77 MILLION VEHICLES.INCLUDED ARE VARIOUS 1994-1997 MY M/L VANS, S/T TRUCKS AND SUVS, & C/K TRUCKS AND SUVS.SEE ATTACHED TECHNICAL REPORT FOR DETAILS.
HEADLIGHT
BASED ON THE REPORTS RECEIVED BY ODI FROM CONSUMERS, FAILURE OF THE HEADLIGHT ELECTRICAL CIRCUIT MAY OCCUR IN THE SUBJECT VEHICLES, AND MAY RESULT FROM REPEATED ADJUSTMENT OFTHE TILT STEERING WHEEL IN THE SUBJECT VEHICLES.REPEATED ADJUSTMENT OF THE STEERING WHEEL POSITION MAY STRAIN THE WIRING IN THE HEADLIGHT CIRCUIT TO THE POINT OF FAILURE WHERE THEY ARE ATTACHED TO THE MULTI-FUNCTION SWITCH, RESULTING IN THE UNANTICIPATED LOSS OF HEADLIGHTS WHILE THE VEHICLE IS IN MOTION.ODI HAS CONCERNS REGARDING THE HIGH RATE OF WARRANTY REPAIRS FOR PROBLEMS POTENTIALLY ASSOCIATED WITH THE ALLEGED DEFECT IN THE SUBJECT VEHICLES, WITH THE RECOGNITION THE EACH INCIDENT OF HEADLIGHT FAILURE REPRESENTS A CONDITION OF VEHICLE DISABLEMENT.IN ADDITION, THE MANUFACTURER'S EVALUATION OF THE ALLEGED DEFECT HAS NOT BEEN RECEIVED, AND THIS OFFICE HAS INTEREST IN THE DETAILS OF THE METHODOLOGIES AND RESULTS OF THAT ASSESSMENT.

Tire: 265/70R16. New tire, bought less than 20 days earlier. Hit minor pothole on LA Hwy 1019, northbound on July 8, 2021. Immediately noticed a vibration originating from rt front area of Suburban. Speed limit on highway was 45 mph; I was driving at the required limit. Took vehicle to my tire man July 9, 2021. He explained that damage to tire was slipped belt, not the result of a thrown wheel weight. I had to replace a new tire with less than 100 miles on the tire. $190.

265/70/R16. Cruising down Interstate around Des Moines, Iowa at 65 mph. Hot day July 2021. Heard muffled "pop" noise. Few seconds later, suburban began to shake and vibrate violently. Approaching uncontrollable. Pulled off road onto side street. Noted bulge on top of tire. Located a Goodyear tire store in Ames, Iowa. Tire man identified the problem, ordered a new tire (Hankook brand). We spent unintended night nearby in hotel, had tire changed out at Goodyear Tire store, all other tires were inspected by qualified technicians, given a clean bill of health, and we continued on. No warning of impending tire failure. This incident was a second belt failure on same brand of tire within 30 days. First belt failure occurred about 20 days earlier on a same brand, and type tire that had less than 1,000 miles of wear on same vehicle.

WHILE DRIVING CONSUMER NOTICED THAT FLUID WAS LEAKING FROM UNDERNEATH THE VEHICLE. SUDDENLY VEHICLE STALLED. CONSUMER WAS UNABLE TO RESTART THE VEHICLE, AND IT WAS TOWED TO THE DEALER FOR INSPECTION. *AK THE CONSUMER HAS REPLACE THREE TRANSMISSIONS IN THIS VEHICLE. THIS LAST FAILURE CAUSED TRANSMISSION FLUID TO LEAK ONTO THE HOT EXHAUST PIPE NEARLY CAUSING A FIRE. *NM

This Preliminary Information communication provides information to the technician on how to unlock radios that are Delco TheftLock. Technician will use the radio to get the scrambled code. Technician will need to contact the radio unlock center to retrieve the unlock code from the scrambled code. The PI update to version D is to advise the unlock procedure is temporarily unavailable. E version update is to add administrative details.
