Unintended and Uncontrolled Acceleration
On February 16, 2010, NHTSA's Office of Defects Investigation (ODI) opened RQ10-003 to determine whether the scope of prior Toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, Toyota recalls 07E-082, 09V-388, 10V-017, and 10V-023.NHTSA also requested information regarding potential electronic causes of unintended acceleration in Toyota and Lexus vehicles.I. Pedal Interference from Floor mats, Carpet Covers and Plastic Pads in Carpets.During the RQ10-003 investigation, ODI reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agency's analysis, NHTSA requested that Toyota recall additional vehicles.Toyota complied with the agency's request.The details of these recalls are set forth more fully in Toyota's reports to NHTSA pursuant to 49 CFR Part 573.These recalls are as follows:Recall 11V-112: (1) model year (MY) 2004-2006 Toyota Highlander and Highlander Hybrid; and, (2) MY 2004-2007 Lexus RX330,RX350, and RX400h (hybrid model). The total estimated population under this recall is 769,379 vehicles.This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles.This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles.This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration.After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles.That study has concluded.Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.

The contact owns a 2007 Lexus RX350. The contact stated that over time she noticed that the dashboard was cracking and separating. The contact was concerned due to the proximity of the passenger’s side air bag housing and the crack in the dashboard. The contact was also concerned about an inadvertent deployment without the vehicle being involved in a crash. The local dealer was contacted and informed of the failure but offered no assistance. The vehicle was not repaired. The manufacturer was not contacted or informed of the failure. The approximate failure mileage was 113,000.

TL* THE CONTACT OWNS A 2007 LEXUS RX350. THE CONTACT'S WIFE STATED THAT UPON ACCELERATING THE VEHICLE AFTER EXITING A ROUNDABOUT, DUST BEGAN TO EMIT FROM THE EXHAUST PIPE. THE CONTACT'S WIFE DROVE TO A GAS STATION AND DISCOVERED THAT THE ENGINE OIL WAS EMPTY. THE CONTACT THEN REFILLED THE OIL BUT BY THE NEXT DAY, THE OIL HAD COMPLETELY LEAKED OUT OF THE VEHICLE. DUE TO THE FAILURE, THE CONTACT HAD THE VEHICLE TOWED TO LEXUS OF MAPLEWOOD (3000 US-61, MAPLEWOOD, MN 55109) WHERE THEY INITIALLY DISCOVERED METAL SHAVINGS IN THE OIL LINE. THE OIL LINE WAS REPLACED HOWEVER, THEY ALSO DISCOVERED THAT THE CAM GEARS IN UPPER ENGINE WERE DEFECTIVE AND NEEDED TO BE REPLACED. THE MANUFACTURER HAD BEEN NOTIFIED OF THE FAILURE AND STATED THAT THEY WOULD PROVIDE A CALLBACK. THE CONTACT HAD NOT RECEIVED A CALLBACK. THE VEHICLE HAD YET TO BE REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 83,000.

TL* THE CONTACT OWNS A 2007 LEXUS RX350. THE CONTACT STATED THAT THE DRIVER'S SIDE AIR BAG WARNING LIGHT FLASHED ON AND OFF CONSTANTLY. THE VEHICLE WAS TAKEN TO THE LEXUS OF GLENDALE (1221 S BRAND BLVD, GLENDALE, CA 91204, (818) 507-1300) WHERE IT WAS DIAGNOSED THAT THE WIRING HARNESS UNDER THE SEAT WAS DEFECTIVE AND NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 85,000.

DROVE 2007 RX350 APPROXIMATELY 15 MINUTES AND RETURNED TO RESIDENCE. 3 MINUTES LATER, WALKED OUTSIDE AND CAR WAS SMOKING FROM ENGINE AREA. WITHIN A MATTER OF 60-90 SECONDS THE CAR WAS COMPLETELY ENGULFED IN FLAMES AND FIRE DEPARTMENT IMMEDIATELY CALLED. FD ARRIVED 21 MINUTES LATER AND CAR WAS STILL FULLY IN FLAMES. ONCE FD EXTINGUISHED FLAMES, CAR WAS A TOTAL LOSS.

I WAS DRIVING ON A COUNTRY ROAD AND NOTICED A/C WAS ACTING ODD, THEN TEMP GAUGE WENT TO BOTTOM OF DIAL. I COULDN'T STOP ON THIS ROAD. I WAS TRYING TO GET TO GRANDDAUGHTER'S HOUSE. I STOPPED AT LIGHT AND COULDN'T START IT. I CALLED AAA AND CAR WAS TRANSPORTED TO LEXUS DEALER WHO TESTED CAR AND SAID ENGINE FAILED. THERE WERE 122000 MILES WHEN TESTED AT DEALER. THE CAR HAS BEEN DEALER SERVICED THE ENTIRE TIME. IT WAS SERVICED AT 120,000 AND ALL SYSTEMS OK. I'VE HAD AN 80 DATSUN THAT LASTED 250,000 MILES. THE DEALER SAID THAT USED ENGINE $8500. I EXPECTED MUCH BETTER FROM LEXUS. READING SOME NHTSA REPORTS THERE SEEM TO BE A RASH OF OIL LINES FAILING. THERE WAS NO WATER HOSE PROBLEM ACCORDING TO DEALER CHECKING IT

THE DASH STARTED CRACKING AND NOT FROM ABUSE. IT IS PARKED IN A COVERED GARAGE WHEN NOT IN USE. I'VE READ THE SAME COMPLAINTS. IT IS A MANUFACTURE DEFECT AND I'M AFRAID IT WILL AFFECT THE AIR BAG DEPLOYMENTS. THE MANUFACTURE SHOULD TAKE CARE OF THIS AND REPLACE THE DASH BOARD.

OIL STARTED SPEWING FROM UNDERNEATH THE CAR WHILE IT WAS IN MOTION ON A HIGHWAY . THE CAR WAS TOWED TO A LOCAL SHOP AND THE OIL COOLANT LINES (BLACK RUBBER HOSES) WERE REPLACED WITH METAL HOSES.

TSB: Some 2005 ? 2022 model year Lexus vehicles that have undergone water intrusion may exhibit a condition in which a musty smell is present. Follow the procedures in this bulletin to remediate the odor and address this condition. The purpose of this service bulletin is to provide general guidelines and procedures for odor remediation. This service bulletin provides a guide on how to prepare the interior of the vehicle prior to an odor remediation being performed, as well as contact information for an approved vendor who will arrange the remediation, and instructions on how to prepare the interior of the vehicle for reassembly once the remediation has been completed. TSB: Vehicles may display evidence of a fluid leak or fluid seep from powertrain components. This bulletin explains how to determine the differences between a fluid leak versus a fluid seep. Cases that are determined to be a ?leak? should be repaired accordingly and will be covered under warranty if applicable. Cases that are determined to be a ?seep? should be documented on the repair order and inspected at the next service interval. Follow the Repair Information in this bulletin to determine if the concern in question is a fluid leak or fluid seep and review the Repair Procedure section to address this condition.
