The defect: MobilityWorks (WMK Inc.) is recalling certain model year 2006-2014 Ford E-150, E-250, and E-350 vans modified by MobilityWorks to be equipped with certain model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation. The affected lifts are equipped with folding link arms that may have pivot holes that are too big, allowing the bearings to move out of position.
The risk: If the bearings move out of position, the stowed platform may have excessive movement resulting in failure of the platform mounting. This failure could cause the wheel chair lift platform to fall out of the vehicle when the doors are opened, increasing the risk of injury for the lift operator.
The remedy: MobilityWorks will notify owners, and Ricon dealers will inspect the link arms for damage or for displaced bearings. The platform will be inspected for cracks. Any damaged parts will be replaced, free of charge. The recall began on December 15, 2015. Owners may contact MobilityWorks customer service at 1-800-769-8267, or Ricon Customer Service at 1-800-322-2884. Note: These vehicles and wheelchair lifts were subject to a similar recall in 2014. All vehicles that received that recall remedy are still affected by this recall as well.

The defect: Cummings Mobility Conversion & Supply (Cummings) is recalling certain model year 2006-2014 Ford E-150, E-250, E-350, Chevrolet Express, 2006-2008 Dodge Sprinter, and 2009-2014 Mercedes-Benz Sprinter, and 2013 Ford Transit Connect vehicles manufactured January 1, 2006, to October 8, 2014, equipped with certain model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation after January 1, 2006. The platform side plate of the affected wheelchair lifts may crack.
The risk: If the platform side plates crack, the lift platform can separate from the lift and come to rest against the vehicle's lift door. When the doors are opened, the platform may fall out, increasing the risk of injury to the lift operator.
The remedy: Cummings will notify owners to take buses to a Ricon dealer or service center who will install supplemental platform support bumpers. Any platform that has already started cracking will be replaced, free of charge. The recall began January 13, 2015. TL* THE CONTACT OWNS A 2009 FORD E-150. WHILE DRIVING 25 MPH, THE NUMBER SEVEN DRIVER SIDE SPARK PLUG FAILED. THE VEHICLE WAS TAKEN TO THE DEALER WHERE IT WAS DIAGNOSED THAT THE TREAD ON THE CYLINDER HEAD WAS STRIPPED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. TL* THE CONTACT OWNS A 2009 FORD E-150. THE CONTACT STATED THAT WHILE DRIVING AT VARIOUS SPEEDS, THE FRONT OF THE VEHICLE SHOOK VIOLENTLY OUT OF CONTROL. THE VEHICLE WAS TAKEN TO A DEALER, WHO WAS UNABLE TO REMEDY THE FAILURE. THE FAILURE RECURRED SEVERAL TIMES. THE CONTACT STATED THAT THE VEHICLE WAS TAKEN TO THE DEALER ON SEVERAL OCCASIONS WHERE THE TIRES AND THE STEERING DAMPER WERE REPLACED. IN ADDITION, AN ALIGNMENT WAS PERFORMED BUT THE FAILURE RECURRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 83,000.

ADULT MALE DRIVER, I'VE RAN A TRUCK SALES SINCE 1971, DRIVING ALL BRANDS AN THIS IS A FIRST AND THE MOST DANGEROUS 2 OR 3 MINUTES OF MY LIFE..... DRIVING A 2009 FORD F150 61,62 MPH ON CRUISE, COMING INTO LEARY GEORGIA AN APPROACHING 2 VEHICLES AHEAD OF ME, I TOUCHED THE BRAKE PETAL TO RELEASE THE CRUISE CONTROL. THIS IS THE START OF A NITE MARE. THE THROTTLE WENT WIDE OPEN, FROM 60 I GO TO 75 -80 I HAD TO PASS THE 2 CARS AHEAD IN TOWN OR REAR END THEM. I HAD BOTH FEET ON THE BRAKE AN THE TRANSMISSION KEPT DOWNSHIFTING GIVING IT MORE POWER. I TURNED THE IGNITION OFF AND THEN NO BRAKES, NO STEERING, NO LIGHTS. I WAS PREPARED TO HIT WOODS OR BUILDING TO DODGE THE CARS ON STREET, HAD A FULL 90 DEGREE RT TURN TO MAKE AN CROSS A RR TRACK, NEEDING BRAKES AND LIGHTS I TURNED IGNITION ON , FULL THROTTLE AGAIN, TURNED IGNITION BACK OFF AND STEERED UNDER A SERVICE STATIONS CANOPY AND BACK OUT ON THE STREET AND ACROSS JUMPED THE CURB AND INTO A GOD SENT VACANT LOT, GRASSY, WET AND SOFT. IT STOPPED. AFTER A FEW MINUTES AND TAKING MY NITROGLYCERIN ,I STOPPED SHAKING ENOUGH TO START THE ENGINE, IT IDLED. I LOCKED IT AND LEFT A NOTE ON THE WINDSHIELD TO NOT ATTEMPT TO DRIVE THIS VEHICLE WILL KILL YOU, WILL RUN AWAY FULL SPEED. ON APPROXIMATELY 8/25/2010, WHILE DRIVING ON THE HIGHWAY THE VEHICLE SUDDENLY SHUT DOWN ELECTRICALLY. IT COULDN'T BE STARTED BY AAA. IT WAS TOWED TO A LOCAL REPAIR SHOP AND THEY FOUND A BURNED WIRE THAT WENT FROM THE FUSE BOX TO A JUNCTION BOX. HE TOLD ME THAT THIS IS AN ENGINEERING DEFECT IN THE VEHICLE AS THE WIRE IS TOO CLOSE TO THE BRAKES AND WHEN IT GETS HOT THE WIRE BURNS OUT. HE ALSO SAID THAT THE VEHICLE CAN EVENTUALLY GO ON FIRE. IT WAS REPAIRED AND THEN ON 9/25/2011 THE SAME THING OCCURRED. I HAD THE VEHICLE TOWED TO AMERICAN FORD ON RT 17 SOUTH IN PARAMUS NEW JERSEY. THEY FOUND AGAIN A BURNED WIRE FROM THE FUSE BOX. I ASKED THEM NOT TO REPAIR THE VEHICLE UNTIL SOMEONE FROM THE NATIONAL SAFETY BOARD INVESTIGATES THIS VEHICLE. *TR

(DEFECTIVE LOCK/HANDLE) THIS INCIDENT OCCURRED WHEN THE DOOR WAS IN A LOCK POSITION AND A PASSENGER TRIED TO OPEN THE DOOR. SOMETHING SNAPPED. THEN THE DOOR WAS UNABLE TO OPEN EVEN AFTER THE DOOR IS UNLOCKED. AFTER MY FRIEND DID ME A FAVOR TO FIX IT, THE SAME THING HAPPENED ON THE PASSENGER DOOR. THE DOOR WAS LOCKED THEN MY COWORKER TRIED TO OPEN THE DOOR BEFORE I UNLOCKED IT FROM THE CAR ALARM, THEN A COMPONENT ON THE HANDLE SNAPPED. ELECTRICAL SYSTEM; STRUCTURE:BODY

Ford and Lincoln vehicles equipped with wired keyless entry keypad systems and accessory wireless keyless entry keypad systems may or may not come with a wallet card containing the master code. Unlike the integrated wired keypad, the accessory wireless keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool or from the label printed on the body control module (BCM). The Factory Keyless Entry Code application within the diagnostic scan tool will not provide an applicable master code for the accessory wireless keypad. If the wallet card for an accessory keypad is not available, the "Wireless RF Keypad Diagnosis Guide" can be referenced and provides direction on how to retrieve the master code. This guide is located under the Workshop Manual tab > Accessories > Installation > Wireless RF Keypad Diagnosis Guide > PPT C > Step C2.<br /><br /> Note: The only available keypad for some vehicles from the assembly plant is the accessory wireless keyless entry keypad. All accessory stick-on keyless entry keypads come with a wallet card containing the master code. If the accessory keypad was factory installed, this card will be placed in the glove box and should be handed to the vehicles owner during initial delivery. Unlike the integrated (wired) factory keypad, the accessory keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool. If the wallet card for an accessory keypad is lost, follow these steps to recover the code. Dealer Key Code access is granted through the Dealer Program Enrollment System (DPES); dealership management can authorize users. The authorized user must go to FMCDealer.com > Parts and Service > Parts Product Line Information > Keys/Entry Devices > Aftermarket Keyless Entry Keypad Code Access. Follow the instructions on the screen to obtain the master code for the accessory keypad.

Replacing the original equipment SYNC/Navigation system with either an aftermarket SYNC/Navigation kit or modifying original SYNC/Navigation parts to provide additional features is not recommended nor supported on any Ford, Mercury or Lincoln vehicle. Only approved Genuine Ford accessories or originally installed systems are supported and certified to function correctly with other vehicle systems. If you suspect a system has been modified, the following resources can be used to verify original equipment: Monroney Label (Window Sticker), Professional Technician Society (PTS) OASIS tab  select SYNC or HVBOM. For Genuine Ford accessories use: PTS Workshop Tab  select Dealer Installed Accessory or Accessory Installation.

The Accessory (Wireless) Keyless Entry Keypad can only be used with vehicles that are factory built with Remote Keyless Entry (RKE). The master code for the Accessory (Wireless) Keyless Entry Keypad can be retrieved from FMCDealer - Parts and Service - Parts Product Line Information - Keys/Entry Devices - Aftermarket Keyless Entry Keypad Code Access. Access to this site is granted through Dealer Program Enrollment System (DPES) and Dealer Principal, General Manager or Parts and Service Director can authorize users. Once authorized the retrieval process for master code can be obtained. The master code for the Accessory Keyless Entry Keypad cannot be retrieved from the body control module(BCM) with a diagnostic tool.

ELECTRICAL SYSTEM; POWER TRAIN:AUTOMATIC TRANSMISSION:CONTROL MODULE (TCM/PCM/TECM)

Some vehicles may exhibit a no start condition with an illuminated malfunction indicator lamp (MIL) and a DTC P161A after PCM replacement. This may be due to the parameter reset procedure not being properly completed. To clear this DTC, use the Ford Integrated Diagnostic System (IDS) service tool or Ford J2534 Diagnostic Software, select - Toolbox - Body - Security - PATS Functions - Parameter Reset. Follow the on-screen instructions to complete the procedure. This procedure will transfer the existing Key ID information between the modules without the need of having 2 programmed PATS keys present. There is no need to program keys when the PCM is going to be replaced. Additional information can be found in the Workshop Manual (WSM), Section 419-01. Missing or lost keys are not warrantable.
