The risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants, resulting in serious injury or death.
The remedy: GM will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. Interim letters notifying owners of the safety risk were mailed on February 22, 2021. Second notification letters will be mailed once the remedy is available. Owner notification letters were mailed on March 1, 2021. The defect: GM IS RECALLING CERTAIN MODEL YEAR 2006-2009 BUICK, LUCERNE; CADILLAC DTS; HUMMER H2; MODEL YEAR 2008-2009 BUICK ENCLAVE; CADILLAC CTS; MODEL YEAR 2007-2009 CADILLAC ESCALADE, ESCALADE ESV, ESCALADE EXT; CHEVROLET AVALANCHE, SILVERADO, SUBURBAN, TAHOE; GMC ACADIA, SIERRA, YUKON, YUKON XL; SATURN OUTLOOK; AND MODEL YEAR 2009 CHEVROLET TRAVERSE VEHICLES EQUIPPED WITH A HEATED WASHER FLUID SYSTEM (HWFS). A RECALL WAS IMPLEMENTED IN 2008 TO ADD A FUSE TO THE CONTROL CIRCUIT HARNESS TO ADDRESS THE POTENTIAL CONSEQUENCES OF A PRINTED CIRCUIT BOARD (PCB) ELECTRICAL SHORT. HOWEVER, THERE HAVE BEEN NEW REPORTS OF THERMAL INCIDENTS ON HWFS MODULES AFTER THIS IMPROVEMENT WAS INSTALLED. THESE INCIDENTS RESULTED FROM A NEW FAILURE MODE ATTRIBUTED TO THE DEVICE'S THERMAL PROTECTION FEATURE. THE SIGNIFICANCE VARIES FROM MINOR DISTORTION TO CONSIDERABLE MELTING OF THE PLASTIC AROUND THE HWFS FLUID CHAMBER.
The risk: IT IS POSSIBLE FOR THE HEATED WASHER MODULE TO IGNITE AND A FIRE MAY OCCUR.
The remedy: DEALERS WILL PERMANENTLY DISABLE AND REMOVE THE HEATED WASHER FLUID MODULE. AN UPDATED PAGE FOR THE OWNER MANUAL WILL BE PROVIDED AND INSERTED IN THE OWNER MANUAL TO DOCUMENT THAT THE FEATURE HAS BEEN PERMANENTLY DISABLED AND REMOVED FROM THE VEHICLE. Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Interior Door Handle Laceration Risk

The investigated concern involved chrome plating on interior door handles that could peel or separate from the plastic base material.This de-lamination could result in a sharp edge and pose a risk of laceration.In some cases, consumers reported minor hand/skin lacerations.Of these reported injuries, the vast majority of injuries were to adults.Ninety percent of claims associated with interior door handle de-lamination were related to the front doors.In addition to the above Tahoe/Yukon vehicles, other GMT 900 series C/K vehicles (Avalanche, Escalade, Sierra, Silverado, and Suburban) have a trim level that utilizes the chrome plated interior door handles and were included in the analysis.The resultant population group had a total of 746 complaint reports with 309 injury incidents and 43,619 unique VIN regular warranty claims over a combined 451,745 vehicle population.The door handle supplier was changed in September 2006 with new tooling and process in place by October 2006.The base population prior to the September 2006 vendor change accounted for the bulk (or 89%) of all complaints and comparable injury reports and warranty claims.Due to the nature of the failure (thin chrome plating peeling off the door handle), only minor hand/skin laceration injuries similar to a paper cut were recorded.All but a few did not require professional medical treatment such as surgical or liquid sutures (i.e. stitches).Less than 10% of injury reports had specifically involved a 3-10 year old child.This was due to the fact that 90% of the claims were associated with door handles located in the front seating area (where the window glass panels usually have less solar tinting than rear window glass panels) where children are not normally seated. In July 2010, GM issued a Special Coverage Policy (SCP 09239) on the base vehicles to extend the warranty coverage to 10 years/100k miles.Recent data indicate that the base population complaint and injury reporting have subsided since the implementation of the SCP campaign.In June 2011, GM added the remaining interim population (Sep-Oct 2006) to the SCP.A safety-related defect has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will monitor this issue and reserves the right to take further action if warranted by the circumstances.

The statistical data regarding problem experience in the table above are net counts for unique vehicles.Review of information in this PE discloses that while accounting for approximately two-thirds of the total population, MY 2007 subject vehicles are cited in more than 90 percent of the consumer complaints, injury reports, and warranty claims, respectively.In addition, improvements in the door handle manufacturing process during the MY 2007 production resulted in a decline in the occurrence of peeling/flaking of the thin chrome plating.Other environmental factors were also identified that may have influenced the occurrence of the condition which poses the alleged safety risk.Additional analysis is required to identify the specific vehicles most susceptible to peeling or separation of the chrome plating from the handle, and to assess the significance of its occurrence.This PE has been upgraded to an Engineering Analysis to facilitate further study.

IN A LETTER DATED AUGUST 27, 2008, GENERAL MOTORS CORP. (GM) NOTIFIED NHTSA THAT IT WILL CONDUCT A SAFETY RECALL (NHTSA RECALL NUMBER 08V-441) TO ADDRESS A SAFETY RELATEDDEFECT THAT CAN RESULT IN ENGINE COMPARTMENT FIRE IN APPROXIMATELY 850,000 VEHICLES EQUIPPED WITH A HEATED WINDSHIELD WASHER FLUID OPTION.THE SUBJECT VEHICLES OF THIS INVESTIGATION COMPRISE OVER THREE-QUARTERS OF THE RECALL POPULATION. ACCORDING TO GM, A SHORT CIRCUIT ON THE PRINTED CIRCUIT BOARD FOR THE WINDSHIELD WASHER FLUID HEATER MODULE MAY OVERHEAT THE CONTROL-CIRCUIT GROUND WIRE. THIS MAY CAUSE OTHER ELECTRICAL FEATURES TO MALFUNCTION, CREATE AN ODOR OR CAUSE SMOKE AND IN RARE CASES IT MAY CAUSE A FIRE.AS A REMEDY, GM DEALERS WILL INSTALL A WIRE HARNESS WITH AN IN-LINE FUSE. The heated seats in my 2007 GMC Yukon Denali caught fire to my car .The fur department came and the fire marshal came out and wrote a report stating it was from the heated seats what caused the fire when I need to I will abtain the report for the fire marshal I'm so glad my son and I were not in the car

The contact owned a 2007 GMC Yukon. The contact stated that after his son parked the vehicle outside near the sidewalk, and turning the off the vehicle, the contact's son exited the vehicle. Approximately 5-10 minutes later, his son’s mother informed him that there was smoke coming from the front of the vehicle. The contact’s son was then informed by his mother that the vehicle was on fire. The vehicle then became engulfed in flames. The Fire Department was called to the scene and extinguished the fire. During the fire, the vehicle was destroyed and later towed away from the scene. A Fire report was filed, and the contact was informed that the fire was probably an electrical fire. The contact stated that the fire had started on the driver’s side front of the vehicle however, the cause of the failure was not determined. The local dealer and manufacturer were not notified of the failure. The failure mileage was 130,000.

Travelling on an FM road, going 55 when Yukon began accelerating on its own, even when I took foot off the accelerator. tapped brakes, thinking cruise control had engaged. Vehicle swerved violently. Speed kept increasing. Saw I was traveling 75mph! Braked hard! Felt as if acceleration was fighting against the brakes. Then spun out in the middle of the highway, spinning around 1 1/2 times while fully braking. Finally I slowed enough to throw it into park, regardless of what it might do to my transmission, I had to stop the vehicle. I came to rest straddling the lane I had been in and the oncoming lane, facing the direction from which I came. Traffic behind me had seen my struggles in time to stop back far enough that they weren’t endangered and there was no oncoming traffic at that moment. The fuel propulsion system (throttle?) seems to be at fault. There was no warning on the dash about any issue prior to this. The issue was real, a danger to myself and others. A mechanic examined the vehicle and discovered accumulation of debris on top of connector under the hood. He believed just enough water caused the conductivity needed to trigger the acceleration. State that these vehicles allow more intrusion from wheel well area than most. He thoroughly cleaned all connectors and no further issues have happened (yet!)
