DRIVER'S SHOE GOT CAUGHT BETWEEN 3 INCH OF SPACE BETWEEN ACCELERATOR PEDAL AND BRAKE PEDAL, CAUSING VEHICLE TO KEEP ACCELERATING AND HITTING 2 VEHICLES . THERE WAS NOT SUFFICIENT SPACE BETWEEN BOTH PEDALS. *AK
THROTTLE STUCK OPEN WHILE DRIVING DOWN A STEEP HILL. DRIVER PUT THE VEHICLE IN NEUTRAL, BRAKED AND SHUT THE ENGINE OFF. THE INCIDENT HAPPENED AGAIN ABOUT TWO MILES FURTHER ON THE HIGHWAY.
