Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
Door Latch Failures
In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property.The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. Door Latch Failure
In January, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation based on consumers' allegations of doors failing to latch and/or inadvertently opening on model year (MY) 2012-2013 Ford Focus vehicles (subject vehicles).ODI has received 464 unique reports related to the alleged problem, with over 200 claiming that the door(s) opened inadvertently while the vehicle was in motion.One consumer alleged an injury that occurred while driving when a door inadvertently opened and then closed on a child's arm.Another alleged that a door that inadvertently opened struck a parked vehicle.In response to ODI's information request, Ford Motor Company (Ford) provided 500 unique reports related to the alleged problem on the subject vehicles.Ford reported no injury or accident claims.Ford also submitted 1,513 warranty claims related to door latch failures.Many customers indicated that the affected door could not be latched at all and some of them decided to restrain the door by other means in order to operate the vehicle.Some customers also reported door latch failures occurring on multiple doors and at different times.Additionally, Ford stated that MY 2014-2015 Ford Focus, 2013-2015 Ford Escape, 2013-2015 Ford C-Max, 2014-2016 Ford Transit Connect, 2015 Ford Mustang, and 2015 Lincoln MKC, all use the same door latch as the 2012-2013 Ford Focus.On September 6, 2016, Ford notified ODI that it would conduct a safety recall (16V-643) to remedy the door latches in the MY 2012-2015 Ford Focus and other Ford models mentioned above (total of approximately 2 million vehicles).According to Ford, a component within the door latch may break preventing the door from latching.Ford dealers will replace all four door latches in the recalled vehicles with a new more robust latch design that is currently in production.The recall action initiated by the manufacturer appears to address the safety risks.Accordingly, this investigation is closed.The ODI reports cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the identification (ODI) numbers found in the attached list.

Clutch assembly failed, the metal bracket weld cracked/failed resulting in the clutch pedal being inoperable. It's not available for inspection since I had it replaced and the defective assembly disposed of. My safety was at risk since I had extreme difficulty shifting gears on the road. The issue was confirmed at an independent service center - AAMCO of Huntsville, AL. No other entity inspected the part. The vehicle gave me no warning or indication of a problem other than the difficulty with starting and operating it.

I am filing a formal safety complaint regarding a catastrophic Electronic Power Assisted Steering (EPAS) failure and severe electrical short circuit on my 2015 Ford Mustang. This vehicle is exhibiting hard fault code B137F:09 (Steering Wheel Left Switch Pack: Component Failure / Short to Ground or Open). This electronic failure creates an immediate, severe safety hazard that goes entirely outside the scope of normal vehicle wear and tear. While driving, the vehicle's steering wheel pulls and jerks violently on its own, actively fighting my manual steering inputs. Furthermore, this internal electronic short creates a massive parasitic electrical draw that prevents the vehicle's computer modules from entering sleep mode when turned off. This has resulted in the primary vehicle battery draining completely flat and causing the vehicle to completely shut down, stall, or lose total electrical power altogether. A vehicle that forcefully steers itself away from the intended path of travel and suffers total electrical shutdown is a clear, life-threatening safety risk to myself and others on the road. Ford Motor Company is fully aware of this dangerous defect. They previously issued Safety Recall 15S18 for a very specific, limited batch of early-production 2015 Mustangs experiencing identical EPAS failures, replacing their steering gears for free. For the remaining 2015 Mustang owners experiencing the exact same hazardous failure, Ford quietly shifted the issue over to a Technical Service Bulletin (TSB / SSM 51312). By limiting the scope of Recall 15S18 purely due to the financial cost of a universal recall, Ford has left an entire population of 2015 Mustang owners in direct harm's way. Some owners received a free lifetime safety fix, while the rest of us are being forced to pay thousands of dollars out-of-pocket to correct an identical factory electronic defect. I request that the NHTSA immediately open an investigation into Ford's handling of the 2015 Mustang EPAS and more.

The driver-side seat belt retractor on my 2015 Ford Mustang GT Premium has failed. The seat belt retracts back into the retractor housing when released; however, it does not lock when pulled sharply and continues to extend freely. I confirmed this by repeatedly performing a rapid pull test while seated in the vehicle. The failure affects the seat belt restraint system because the belt does not appear to engage its locking mechanism. This could increase the risk of injury in a crash because the seat belt may not properly restrain the occupant during sudden deceleration or impact. The issue was inspected and confirmed by a Ford dealership service department. The dealer diagnosed the driver-side seat belt retractor as failed and recommended replacement of the retractor assembly. The dealer provided an estimated repair cost of approximately $550 plus tax. The vehicle was inspected by Smith Ford in Conway, Arkansas. The vehicle was also inspected for a separate seat belt recall, and the recall inspection passed. The dealer advised that the failed retractor was not related to the recall and would require customer-paid repair. There were no warning lights, messages, or airbag system warnings present when the failure was discovered. The primary symptom was that the seat belt would retract normally but would not lock during a rapid pull test. The exact date the failure first occurred is unknown, but the condition was identified during normal vehicle operation and later confirmed by the dealership.

The contact owns a 2015 Ford Mustang. The contact stated that while reversing, the rear-view camera displayed a black screen. The dealer was made aware of the failure, and the contact was informed that parts needed to be ordered to repair the vehicle. The contact stated the dealer had been contacted several times, and the contact was informed that the parts were unavailable. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact later received notification of NHTSA Campaign Number: 25V572000 (Back Over Prevention). The manufacturer was made aware of the failure, and a case was filed. The contact was informed that the parts were available. The failure mileage was approximately 37,000.

My passenger door latch pops open nearly every time I drive on the highway. The latches were previously the subject of Safety Recall 16S30. I had my vehicle inspected by a Ford Dealership. No repairs or work was deemed necessary at that time. In the years since, my door now comes unlatched. Infrequently at first and now with very alarming frequency. Safety Recall 20S30 was issued specifically to address these inspections and recalls being completed ineffectually. Somehow my VIN has been left off of this recall even though it was on the 2016 one. The dealership is trying to charge me nearly $2000 to fix a known safety issue. This seems unreasonable, unfair, and extremely dangerous.

I was driving down the highway with heater on and defrost. All of a sudden I hear a pop and my car started bogging down. I exited the highway to get out of danger, it then had a wrench light ( transmission) pop up and when I parked the car on the access road it started smoking a lot. I turned the vehicle off immediately and when I seen smoke coming from under the hood and into the cab of the car. I lifted the hood after seeing fire in the passenger wheel well. And then in the engine block towards the windshield in the middle of the engine. I am a fire fighter and it appeared to me that the fire was being fed by some kind of substance. Fort Worth Fire department showed up and put the fire out and took a report.

While driving, I attempted to depress my clutch pedal in order to bring my vehicle to a stop. My clutch pedal only went halfway to the floor. I was able to force it down to the floor using a higher than usual amount of force. After bringing my vehicle to a stop I inspected my clutch pedal and found that it had side to side play resulting from a cracked weld where the clutch pedal was welded to a shaft on the clutch/brake pedal assembly. Upon further research online I discovered that other owners of similarly aged mustangs have had identical issues to what happened to me.

The weld that holds the clutch pedal broke causing the clutch pedal to not engage properly. Driving in traffic during a down shift stop almost caused an accident. Which led to not being able to shift gears which left the car unable to drive and left the stranded on the side of the road. The weld that hold the clutch pedal in place is insufficient and weak. While researching this issue I found many instances where this has happened to other drivers.

Ford and Lincoln vehicles equipped with wired keyless entry keypad systems and accessory wireless keyless entry keypad systems may or may not come with a wallet card containing the master code. Unlike the integrated wired keypad, the accessory wireless keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool or from the label printed on the body control module (BCM). The Factory Keyless Entry Code application within the diagnostic scan tool will not provide an applicable master code for the accessory wireless keypad. If the wallet card for an accessory keypad is not available, the "Wireless RF Keypad Diagnosis Guide" can be referenced and provides direction on how to retrieve the master code. Pinpoint test HK in the Powertrain Control and Emissions Diagnosis (PC/ED) or in Section 303-14 of the Workshop Manual (WSM) has been updated to address this concern.
