BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

The defect: Honda (American Honda Motor Co.) is recalling certain 2018-2020 Odyssey vehicles. Water may enter into the rearview camera, which can cause the rearview camera image to fail to display when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
The risk: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy: Dealers will replace the rearview camera, free of charge. Owner notification letters were mailed August 24, 2026. Owners may contact Honda's customer service at 1-888-234-2138. Honda's number for this recall is HOX. This recall expands previous NHTSA recall number 20V438. Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov beginning July 9, 2026.

The defect: Honda (American Honda Motor Co.) is recalling certain 2018-2021, 2023 Acura TLX, 2019-2024 RDX, 2017-2020, 2022-2026 MDX, 2017-2021, 2023, 2025 Honda Ridgeline, 2017-2022 Pilot, 2019-2021 Passport, 2018-2026 Odyssey, 2019-2022 Insight, 2019-2021 HR-V, 2018-2020 Fit, 2020-2022 CR-V Hybrid, 2017-2022 CR-V, 2017-2018, 2021 Civic Type R, 2017-2021 Civic hatchback, 2016-2020 Civic coupe, 2016-2022 Civic, 2017-2022 Accord Hybrid, and 2016-2022 Accord vehicles. The front passenger seat weight sensor may crack and short circuit, which can cause the air bags to deploy unintentionally during a crash.
The risk: Air bags that deploy unintentionally during a crash increase the risk of injury.
The remedy: Dealers will replace the seat weight sensors, free of charge. Owner notification letters were mailed July 7, 2026. Owners may contact Honda's customer service at 1-888-234-2138. The defect: Honda (American Honda Motor Co.) is recalling certain 2018-2022 Odyssey vehicles. A software programming error can cause the side and side curtain air bags to deploy unexpectedly.
The risk: Air bags that deploy unexpectedly can increase the risk of injury.
The remedy: Dealers will reprogram the supplemental restraint system (SRS) electronic control unit (ECU) software or replace the ECU, as necessary, free of charge. Owner notification letters were mailed May 27, 2026. Owners may contact Honda's customer service at 1-888-234-2138. Honda's number for this recall is UNW. Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov beginning April 16, 2026. The information in your report suggests that Honda (American Honda Motor Co.) may have been aware of this issue more than five business days before filing a report with NHTSA. The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline.  The subject engine failures were analyzed as part of a Recall Query (RQ24013).  It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation.  ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues.  To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

No Restart After Auto Start/Stop Engages

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. Inadvertent Deployment of Side Air Bags

On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.

Front windshield mounted sensor failure on 2020 Honda Odyssey. Disabled Honda Sensing safety suite which included lane keep assist, adaptative cruise control, auto high beams which all triggered warning messages on the dashboard which all occurred at the exact same time. This problem has been reproduced an inspected by Honda of Frontenac.

The power liftgate is no longer working and locked shut a few hours after multiple warning messages. There was heavy rain that night. It did randomly open and shut all or part of the way before that, sometimes after closing. This often happened after heavy rain when the van was not even in use. It could have popped open at any time, and then warning messages popped up and blocked the notifications and other dashboard information and distracted the driver. It was confirmed by the dealer and they recommended it get replaced, however, the electrical panel was corroded by water damage from a suspected leak in the roof rail. The dealer said that it would probably happen again and not to get it fixed until the leak was determined. Many others have reported this online regarding the Odyssey.

The contact owns a 2020 Honda Odyssey. The contact stated that while driving 45 MPH, the contact heard a knocking sound coming from the engine, and the vehicle went into LIMP Mode with the low oil warning light illuminated. The contact became aware of a burning odor coming from under the hood, and the vehicle needed to be towed. The vehicle was towed to the dealer, where it was diagnosed and determined that the connecting rod bearings had failed and the engine needed to be replaced. The vehicle was not repaired. The failure mileage was approximately 50,000.

Both sliding door keep stopping when closing or opening. Have to manually do it sometime. Rear door don't close anymore either only opens has to always be manually close.

The monocular camera failed causing the car to lose all ADAS features. Honda said this not a recall issue and I have to pay for the repair which is significant. Not only are all the ADAS features lost, but the dashboard flashes warnings and beeps reflecting the lost features

Stopped at a light, auto idle stop engaged, engine shut off normally. The moment I pressed the gas, everything died — engine and all electronics. I pressed the start button two or three times with no response, then it finally started. It was still in Drive. The hazards wouldn't turn on right after, then everything gradually came back to normal.

The contact owns a 2020 Honda Odyssey. The contact stated that while attempting to start the vehicle, several warning lights illuminated. The warning lights flashed continuously. The vehicle was taken to a dealer, where it was diagnosed with a failed multipurpose camera unit. The dealer determined that the multipurpose camera unit needed to be replaced; however, the contact was informed that the failure was not associated with the recall. The contact received notification of NHTSA Campaign Number: 26V423000 (Back Over Prevention). The contact stated that the failure was a known issue with the vehicle. The failure mileage was approximately 31,600.

On September 14, 2026, my 2020 Honda Odyssey experienced a sudden, severe low-pressure fuel pump failure while idling/parked. The engine began running extremely rough, surging repeatedly between 300 RPM and 1,000 RPM, knocking, and stalling. I was parked at the time, preventing a severe accident. Upon trying to restart, the vehicle turned over multiple times and would only catch when depressing the accelerator pedal, accompanied by cylinder knocking and an engine thermal spike. The vehicle was towed to an independent repair facility (Tuffy Auto Service). The diagnostic scan confirmed DTC P0087 (Fuel Rail/System Pressure - Too Low). A mechanical gauge test showed the low-side fuel pressure dropped to 10 PSI (far below the standard operating pressure of 50–60 PSI). The shop verified that the in-tank low-pressure fuel pump assembly failed internally and requires complete replacement. This failure, symptom pattern, diagnostic code (P0087), and component match the exact defect profile in NHTSA Recall 23V-858 (Denso Low-Pressure Fuel Pump Impeller Failure). However, when checking my VIN, American Honda claims this specific vehicle is excluded from the recall range. American Honda Consumer Relations subsequently refused goodwill assistance due to non-dealer service history and vehicle resale status. This creates a severe safety hazard. A vehicle experiencing a sudden drop to 10 PSI fuel pressure while driving in traffic will lose motive power, stall unexpectedly, and disable power steering and braking assist. I am submitting this report because American Honda has failed to include all affected production batches in Recall 23V-858. I request NHTSA expand Recall 23V-858 to include out-of-range 2020 Honda Odyssey VINs exhibiting identical low-side fuel pump failures.
