Hyundai and Kia ABS Module Fires
The Office of Defects Investigation (ODI) has received eight Hyundai Motor America (Hyundai) and eight Kia America, Inc. (Kia) Part 573 Recall Reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (ABS) modules or Hydraulic Electronic Control Units (HECU) manufactured by the equipment supplier Mando. The Part 573 Recall Reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by Mando. Safety recalls included in this Audit Query (AQ) review include: Hyundai recalls 20V-520, 20V-543, 21V-161, 21V-160, 21V-303, 22V-056, 22V-810, and 23V-651. Kia recalls 16V-815, 20V-518, 20V-519, 21V-137, 21V-331, 22V-051, 22V-800, and 23V-652 The purpose of this AQ is to evaluate the timeliness and scoping of Hyundai and Kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.
Throttle control system vulnerabilities
The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) received a petition letter dated October 10, 2021, requesting an investigation of possible defects in the electronic throttle control (ETC) system on certain Kia vehicles resulting in the opening of DP21-003. Accordingly, Model Year (MY) 2005-2016 Kia Optima/K5, MY 2006-2015 Kia Sorento, MY 2007-2016 Hyundai Elantra, MY 2007-2016 Hyundai Santa Fe, MY 2006-2015 Hyundai Sonatas are all part of the subject population. The petition was originally filed as a Vehicle Owner Questionnaire that can be reviewed at NHTSA.gov under reference (ODI) number 11439675.On November 12, 2021, ODI opened Defect Petition DP21-003 to evaluate the petitioner?s request. ODI performed trend analyses of the VOQs related to vehicle speed control issues for all the subject vehicles and found that none of the 52 Model/Model Year Hyundai/Kia vehicles demonstrated any actionable trend that warranted an investigation.Petitioners submitted 1119 VOQs and non-VOQ cases in which sudden uncommanded acceleration (SUA) or loss of motive power (LOMP) incidents were alleged.For the 1119 reports, 220 were incidents involving crashes, from which 69 subject Hyundai-Kia vehicles were inspected by Hyundai/Kia dealer technicians, Hyundai/Kia engineers, and/or third-party representatives.ODI found the inspections identified no vehicle defects related to the ETC system.Of the 220 crash-involved reports, 13 Event Data Recorder (EDR) records were downloaded successfully and 11 of those indicated driver error: (i.e.) pedal misapplication.A condition where the accelerator pedal is mistakenly applied instead of the brake pedal when the driver wants to stop the vehicle.In addition, ODI reviewed a scientific journal paper published in Forensic Science International in 2016 by Mr. Park et al ., as well as related test videos. ODI does not believe there is any evidence of uncontrollable SUA occurrences in any of the tests reviewed. NHTSA's Office of Vehicle Safety Compliance (OVSC) reviewed the allegations regarding the potential noncompliance with Federal Motor Vehicle Safety Standard No. 124.Based on the information provided, the Agency concludes there is insufficient information for us to determine if a possible non-compliance exists.Finally, both Hyundai and Kia informed ODI that all their vehicles manufactured after 2012 were equipped with an ETC management system, in which the engine speed will go to idle when the system sees signals from both the brake pedal and accelerator pedal at the same time; this is commonly known as the brake-throttle override system.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Hyundai and Kia in response to an Information Request letter, and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that a formal investigation is warranted. After full consideration of appropriate factors, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA?s reasons for denial of the petition will be published.
Headliner Bracket Detachment
In March 2013, Hyundai issued a safety recall to reinforce the attachment of the metal support brackets to the headliner assembly in approximately 186,254 model year 2011-2013 Hyundai Elantra vehicles built in Korea and sold in the U.S. (see NHTSA recall 13V-115 for more information).The recall covers vehicles built up to March 5, 2013.After that date, Hyundai began using headliners with reinforced bracket attachments in the Korea-built Elantra vehicles.ODI has opened a Recall Query (RQ13-004) to investigate model year 2009-2012 Hyundai Elantra Touring vehicles.During EA12-008, ODI found these vehicles are equipped with a support bracket design and layout similar to that in the above-mentioned Elantra vehicles.Also, the headliner manufacturing processes used were similar for both models.Like the Elantra vehicles built before the recall, the Elantra Touring model headliner does not contain reinforced bracket attachments.This Engineering Analysis is closed.See attached report for additional information.The ODI report cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification number: 10457696
Side Curtain Air Bag Improper Deployment
The Office of Defects Investigation (ODI) is aware of one incident of an improper deployment of the driver-side curtain air bag that deployed as a result of a crash involving a model year (MY) 2012 Hyundai Elantra.An inspection of this vehicle showed that a metal bracket (stiffener) in the headliner assembly had detached and was forced downward by the deploying air bag.The metal bracket caused a laceration to the driver's ear and other possible injuries.Approximately 75,000 MY 2012 Hyundai Elantra vehicles, built in Korea and sold in the U.S., have stiffeners in the headliners while the MY 2012 Elantra vehicles built in the U.S. utilize a different headliner that does not use a stiffener. Thus, the latter vehicles are not included in this investigation.Hyundai has inspected the headliner assemblies from eight Elantra vehicles (which were sold in the Korean market) with production dates in the same range as the field incident vehicle.Hyundai reported to ODI that the metal brackets from these vehicles were found to be securely attached to the headliners.Hyundai also has conducted several static deployment tests in attempts to replicate the field incident. In one test, the metal bracket detached from the headliner but the test condition did not replicate or explain the condition observed in the incident vehicle.This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA12-008) to further assess whether incidents like the single field incident could potentially occur with other subject vehicles.In addition to the subject vehicles, ODI also will collect and review information on MY 2011 and 2013 Elantra vehicles that have headliners with stiffeners similar to those in the subject vehicles. The contact owns a 2012 Hyundai Elantra. The contact stated that while operating the vehicle, the brake pedal felt spongy while depressed. After parking the vehicle, the contact later restarted the vehicle, and the brake pedal extended to the floor while depressed. While inspecting under the hood, the contact discovered that there was no brake fluid inside the brake fluid reservoir. The cause of the failure was not yet determined. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 23V651000 (SERVICE BRAKES, HYDRAULIC), and a fuse was replaced. The manufacturer and the local dealer were notified of the failure. The failure mileage was 59,000.

I was going down the bridge on settlers landing road the yellow school bus was in the right turning lane with right turning signal on .as I approached the light in the middle straight lane to keep toward the landing (hotel) on settlers landing Hampton va the bus some what merged back towards the middle lane and that when the bus crashed into the car it went under the bus it’s totaled the windshield and my sunroof collided the airbags did not deploy something needs to be done about that that was dangerous.I got glass in my face my right hand had glass my head hit the steering wheel my body jerked. I hit my legs my whole body is hurting and very sore all over I can’t hardly walk my hand is swollen. Liability suit of airbag default needs to happen

*Wife litely drives this car avg 4k miles a year* She left the house for the grocery store the other day and after the first stop light, noticed the brakes were engaging on their own. Luckily she called me before going any further and damaging the pads and rotors, or worse - causing a fire or having an accident. I slowly drove it home and inspected fuses, brake fluid and checked online resources for anyone with similar issue. Turns out this is common? Very dangerous. Have never experienced this issue with any other car make. I had the car towed to HQ Hyundai (local dealer in Sanford FL) to address 2 open recall bulletins on this car. They immediately forced me to accept a $200 inspection fee (key ransom) just to do the recalls - siting safety of staff!?! They were informed that the brakes were remaining engaged and that they needed to disconnect the battery, but they left it in the tow yard over the weekend. I am sure they will try to sell us a battery now. Very upset with the response to this extremely unsafe issue, that we didn't cause from excessive, hard driving.

The contact owned a 2012 Hyundai Elantra. The contact stated that around 4:00 o'clock in the morning, she was awoken by the sound of her dogs barking; she then discovered that her vehicle had caught fire and had burned to the ground. The fire spread, which caught fire to a nearby mobile home and a 4-wheeler. The authorities were called to the scene, and the fire department managed to extinguish all three fires. No injuries were reported, and a police report was not filed. A neighbor's Ring doorbell camera captured the footage of the fire, which showed that the fire had started from underneath the hood of the vehicle. The manufacturer was notified of the failure, and a case was filed. The vehicle was a total loss and remained in the contact's possession. The failure mileage was unknown.

The contact’s son owns a 2012 Hyundai Elantra. The contact stated that while the contact’s son was driving at an undisclosed speed, the vehicle unexpectedly stalled, and the rear of the vehicle caught fire. The contact’s son pulled the vehicle over to the side of the road, and the fire extinguished on its own. No fire or police report was filed. The contact was informed by local police that a trail of oil had leaked from the contact’s son’s vehicle while driving. The vehicle was towed to the dealer. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and refused to replace the engine under an unknown related engine recall. The approximate failure mileage was 80,000.

To Whom It May Concern, I am submitting this complaint regarding a 2012 Hyundai Elantra equipped with the 1.8L Nu engine, which is experiencing premature internal engine damage that presents a potential safety risk. Vehicle details: Year/Make/Model: 2012 Hyundai Elantra Mileage: Approximately 134,500 miles Issue description: The engine produces a persistent ticking/knocking noise, most pronounced at cold start but now continuing even after the engine has warmed. A Hyundai dealership diagnosed the vehicle with piston slap and confirmed significant scarring on the cylinder walls, stating that the only repair is full engine replacement. Despite the presence of internal engine damage, no warning lights are illuminated. I am concerned this condition could lead to sudden engine seizure or loss of power while driving, creating a safety hazard. Additional observations include: Excessive crankcase blow-by observed when removing the oil fill cap while the engine is running Strong exhaust fumes at cold start Noise most noticeable at idle and low RPM Regular oil changes using manufacturer-recommended oil viscosity The dealership advised the condition was not repairable short of engine replacement and did not submit the concern for Hyundai corporate technical review (Techline), despite the known history of engine defects in Hyundai Nu engines. I am filing this complaint to document a pattern of premature internal engine wear that may not be adequately addressed through recalls or warranty programs, and which could result in sudden engine failure. The National Highway Traffic Safety Administration (NHTSA) has issued an urgent safety advisory regarding dangerous, counterfeit driver‑side airbag inflators manufactured by Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN), that have been identified in U.S. vehicles following prior crashes, theft, or non‑certified airbag replacements. These inflators are not approved for use, may rupture during deployment, and have caused serious injuries and fatalities. Please see attached announcement, provided by NHTSA, for more details.
AIR BAGS
The National Highway Traffic Safety Administration (NHTSA) has identified the presence of substandard and dangerous driver’s-side airbag inflators manufactured by Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN). These inflators were not supplied, approved, or authorized by the vehicle manufacturers and have primarily been found in vehicles that previously experienced a crash and underwent airbag replacement using non-genuine parts. These dangerous inflators have ruptured during deployment, posing a serious risk of injury or death due to metal fragments entering the vehicle cabin. To date, Hyundai is aware of three people who have died while driving a Hyundai vehicle with one of these substandard replacement airbag inflators that deployed in what likely would have been a survivable crash. NHTSA has issued a public advisory urging vehicle owners and service professionals to have vehicles inspected if there is reason to believe that a non-genuine replacement airbag may have been installed. Vehicles with salvage or rebuilt titles, prior airbag deployments, or unknown repair histories may present a higher risk. NHTSA strongly cautions against self-inspection by vehicle owners and recommends inspection by trained technicians.
ELECTRICAL SYSTEM
In response to this trend, we engineered a software upgrade that addresses the social media theft method and made it available to owners of eligible models free of charge at authorized Hyundai dealers since early 2023. In addition to the anti-theft software, we’ve developed a new zinc-reinforced ignition cylinder protector hardware upgrade that can be installed at the ignition cylinder location to provide additional protection against the social media theft method. Because your Hyundai vehicle with the VIN listed above (“Subject Vehicle”) was not factory-equipped with an engine immobilizer and was previously eligible for the software upgrade, you are now eligible until March 31, 2027 to have the ignition cylinder protector installed — if you request it — at no cost to you through an authorized Hyundai dealership.
ELECTRICAL SYSTEM
In addition to the Anti-Theft software provided under Service Campaign 993 (TSB 24-01-009H-1), a zinc reinforced ignition cylinder protector hardware upgrade is being made available to customers upon request. This supplemental security measure is to be installed at the ignition switch location and provides additional protection against the social media theft method. Vehicles not equipped with an immobilizer during production at the factory are eligible. The repair procedure shall be performed and claimed upon customer request only. Any future repairs to these vehicles in which an installed ignition cylinder protector impacts required parts and/or labor, shall be covered by HMA. Please see TSB 25-01-089H for details.
