Recalls, owner-reported complaints, investigations and safety data

Data refreshed

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.

Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
1
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

The defect: Icon Vehicle Dynamics, LLC (Icon) is recalling all 05-UP FSD 2.5 Centerline Stabilizer Kits with part number 65050. The kits were sold as an accessory for certain 2005-2021 Ford F-250 4X4 and F-350 4X4 pickup trucks. The foam ring inside the stabilizer "through shaft" may block the piston ports, possibly causing the steering to lock up.
The risk: If the steering locks, it can increase the risk of a crash.
The remedy: Icon will notify owners, and dealers will install a new shock assembly with a revised foam ring and steel support disc, free of charge. The recall began December 21, 2020. Owners may contact Icon customer service at 1-951-689-4266.

The defect: Ford Motor Company (Ford) is recalling certain model year 2011-2015 Ford F-350, F-450, and F-550 trucks manufactured February 22, 2010, to January 30, 2015, and equipped with ambulance or fire engine preparation packages. The affected vehicles may detect an incorrect Exhaust Gas Temperature Sensor (EGT) fault indicating that the vehicle is too hot, causing the engine management system to shut down the engine and prevent its immediate restart.
The risk: Engine shut down and a subsequent cool down period may delay medical treatment or assistance to those needing it, increasing their risk of injury.
The remedy: Ford will notify owners, and dealers will update the Powertrain Control Module (PCM) software, free of charge. The recall began on April 22, 2015. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 15S09.

The defect: Iowa Mold Tooling Co., Inc. will notify owners and dealers to inspect the Muncie PTO pressure switch to determine if it is from the range of defective switches and if required, remove the pressure switch assembly and put a plug in the port where the PTO switch had been installed. This switch was never being utilized. This will be done free of charge. The recall is expected to begin in mid-July 2014. Owners may contact Iowa Mold Tooling Co., Inc. at 1-800-247-5958.
The risk: The leaking oil may soak into the thermal blanket covering the exhaust, which when combined with heat from exhaust, may result in an increased risk of fire.
The remedy: Iowa Mold Tooling Co., Inc. will notify owners, and dealers will install a new pressure switch kit that moves the switch away from the exhaust system, free of charge. The recall began on July 28, 2014. Owners may contact Iowa Mold Tooling Co., Inc. at 1-800-247-5958.

RQ14005ClosedRecall Query

Vehicle Commanded Engine Shutdown

Opened Jan 13, 2015
Closed Mar 15, 2016

The Office of Defects Investigation (ODI) opened this RQ to investigate allegations that the scope and remedy of safety recall 13V-535 were inadequate. Recall 13V-535 was limited to a relatively small population of Ambulance Package vehicles and ODI received complaints of pickup trucks and Fire Engine Prep Package vehicles being affected by the same defect. Ford manufactured approximately 555,985 subject vehicles between February 22, 2010 and January 30, 2015 that may be affected by defective EGT sensors and Powertrain Control Module (PCM) software calibration. The subject vehicles are equipped with four (4) EGT sensors to monitor engine operating conditions. An EGT sensor fault may produce false exhaust over-temperature signals. If these signals are produced, the Powertrain Control Module (PCM) implements a Failure Management Effects Mode (FMEM) and the driver receives a Stop Safely Now (SSN) system warning on the instrument cluster message center. The FMEM triggers a 70% reduction in available engine power over a period of no more than 45 seconds, during which it may not be possible to accelerate or maintain vehicle speed. The PCM then commands the engine to shut down when vehicle speed reaches approximately 1 mph. There is no immediate restart capability. Depending on the PCM software calibration, ignition key position and ambient temperature conditions, the engine may not restart until the end of a 10-60 minute cool down period.Ford and its component supplier(s) made numerous changes to the PCM software calibration and the design, material and manufacture of the EGT sensors over the period of subject vehicle production. This continuous product improvement affected vehicle performance regarding the alleged defect. Ford acknowledges the defect condition in the subject vehicles but argues that an unreasonable risk to motor vehicle safety is limited to Ambulance Package and Fire Engine Prep Package vehicles where commanded shutdown could impact emergency response and patient care. Ford also argues that the defect trend is in decline. Ford also initiated a new safety recall (15V-175) to address the 13V-535 remedy concerns in the Ambulance Package vehicles and Fire Engine Prep Package vehicles. The 15V-175 remedy reprograms the PCM with a new software calibration that stops a single EGT sensor fault to cause the vehicle to enter the FMEM shutdown mode.Ford also initiated an EPA emissions recall (14E03) for all subject vehicles that reprograms the PCM with the software calibration employed in safety recall 15V-175. Finally, Ford initiated a Customer Satisfaction Program (15M01) providing extended warranty coverage of 8 years or 80,000 miles for the the EGT sensors on certain MY2011-2012 subject vehicles.Due to the EPA emissions recall, that had a 66.6% completion rate in Ford's December 2015 progress report to the EPA, the defect trend is in decline. ODI is not aware of any crashes, deaths or injuries related to the alleged defect.ODI believes the alleged defect poses a safety risk but notes that the EPA emissions recall is addressing the safety problem.Compelling issuance of a duplicate recall would not represent a wise use of agency resources.Accordingly, this Recall Query is closed. This action is not a finding that no defect exists and NHTSA will take further action if such action is warranted.

I OWN A 2014 F350 DIESEL TRUCK, MY BUDDY OWNS A 2015 F350 DIESEL TRUCK, A MONTH OR SO AGO HE WAS DRIVING DOWN THE ROAD AND HIS TRUCK STALLED/SHUT DOWN ON HIM DRIVING DOWN THE ROAD CAUSING HIS STEERING WHEEL TO LOCK UP, HE WAS BARLEY ABLE TO GET IT ONTO THE SHOULDER AND IT LEFT HIM STRANDED SINCE HIS TRUCK WOULDN'T START BACK UP. HE BROUGHT IT TO A SHOP AND COME TO FIND OUT IT WAS HIS HIGH PRESSURE FUEL PUMP. FAST FORWARD A LITTLE FURTHER, HE DID SOME RESEARCH AND FOUND OUT THAT FORD PUT FAULTY FUEL PUMPS ON ALL THEIR DIESEL TRUCKS, THE PUMPS CAN'T HANDLE US DIESEL, IT'S NOT LUBRICATED ENOUGH FOR THE PUMPS, SO THE PUMP DESTROYS ITSELF AND SENDS METAL SHARDS THROUGH THE WHOLE ENTIRE FUEL SYSTEM. SO HE WAS STUCK PAYING A $6,XXX BILL TO GET IT FIXED. FAST FORWARD I FOUND OUT THEY MAKE A 'DISASTER PREVENTION KIT' SO WHEN THE PUMP FAILS THE METAL DOESN'T GET THROUGH THE WHOLE SYSTEM YOU ONLY HAVE TO REPLACE THE PUMP WHICH IS $1,XXX. SO I BOUGHT THE KIT AND HAD IT INSTALLED ON MY TRUCK BUT IT WAS TOO LATE MY SYSTEM GOT WIPED OUT. SO NOW I'M OUT $6,XXX TO GET IT FIXED WHICH I CAN'T AFFORD. FORD WONT DO ANYTHING ABOUT IT BECAUSE THERE'S NO RECALL. I'VE DONE MORE RESEARCH AND FOUND OUT THE NEW DIESEL TRUCKS FROM FORD STILL HAVE THE SAME FUEL PUMP. THIS SHOULD BE RECALLED, IT'S ONLY A MATTER OF TIME TILL OTHER PEOPLE START HAVING FAILURES. I WOULD BE PISSED IF I SPENT $70,XXX ON A TRUCK ONLY TO HAVE TO SPEND MORE TO FIX THE FUEL SYSTEM. NOBODY IS AWARE OF THIS ISSUES YET BUT SOMETHING MUST BE DONE ABOUT IT.

MY NEWER P/U JUST OUTSIDE OF WARRANTY IS LEAKING FROM THE RADIATOR. SEEMS TO BE ON ONGOING ISSUE WITH THE 6.7 L ENGINE. FORD WILL NOT STAND BEHIND THEIR PRODUCT AND ASSIST WITH THE LARGE COST OF FIXING. FORD DEALERSHIP TOOK A LOOK AT IT AND SAYS IT NEEDS A NEW RADIATOR. THIS SHOULD NOT BE HAPPENING AROUND 40K MILES. JUST A FEW DROPS ON THE GROUND EACH DAY ON THE GARAGE FLOOR.

THIS VEHICLE IS EQUIPPED WITH AUTOMATIC HEADLIGHTS. REGARDLESS OF SWITCH POSITION, THE INSTRUMENT CLUSTER LIGHTS STAY OFF WHEN THE SENSOR DETECTS ANY DAYLIGHT AT ALL. THE DESIGN OF THE DASH MAKES SEEING THE SPEEDOMETER EITHER EXTREMELY DIFFICULT OR IMPOSSIBLE, ON OVERCAST DAYS AND IN LOW LIGHT SITUATIONS. THIS IS A KNOWN ISSUE, BUT THE MANUFACTURER DOES NOT ACKNOWLEDGE IT AS A PROBLEM.

THE PICKUP TRUCK WAS PURCHASED BRAND NEW FROM THE DEALERSHIP, AND IT IS INHERENT TO WHAT IS KNOWN AS THE "DEATH WOBBLE." THIS MAINLY HAPPENS WHEN THE VEHICLE IS ON THE HIGHWAY GENERALLY WHILE IN THE SPEED BETWEEN 55 AND 60 MPH, DURING A COURSE OF SLIGHT TURN EITHER TO THE RIGHT OR TO THE LEFT WHEN IMPACTED BY A SLIGHT BUMP, THE TRUCK GOES INTO A VIOLENT SHAKING. THE ATTEMPT OF APPLYING THE BRAKES WOULD MAKE THE SYMPTOMS WORSE. THE ATTEMPT OF INCREASING SPEED WOULD AGAIN MAKE THE SYMPTOMS WORSE. STEERING IS ALMOST NONE EXISTENT AS THE STEERING WHEEL IS SHAKING PROFUSELY. FORD MANUFACTURER IS AWARE OF THIS ISSUE, YET NO RESOLUTION BY THEM. THE DEALERSHIP HAS BEEN GIVEN AMPLE OPPORTUNITY TO FIX THE TRUCK, BUT THEY HAVE NOT BEEN ABLE TO. THE PROBLEM CONTINUES AND IS ALSO PLAGUING OTHER SIMILAR VEHICLES. THE LINK ATTACHED HERE WILL SHOW YOU TWO SEPARATE EPISODES OF WHAT HAPPENS AND HOW DANGEROUS THIS VEHICLE IS TO THE PUBLIC. I ENCOURAGE YOU TO VIEW THEM, AS IT IS JAW DROPPING. SINCE I BOUGHT THE TRUCK IN MAY OF 2014 I HAVE EXPERIENCED UNEVEN BRAKING, MEANING MOST OF THE TIME WHEN I BRAKE, THE VEHICLE PULLS TO THE RIGHT HARD. LESS SO ON THE LEFT. ON AUGUST 6TH, 2014 I TOOK MY 2014 FORD F350XL SUPER DUTY TO VARNEY FORD IN NEWPORT MAINE TO HAVE THE VEHICLE LOOKED AT. THEY REPLACED BOTH BRAKE CALIPERS AND PADS. I DROVE THE MY TRUCK TO TENNESSEE ON AUGUST 15TH, 2014 WITH OUR 16,000LB 5TH WHEEL CAMPER ON THE BACK, DURING THE TRIP I HAD TO DO 2 EMERGENCY BRAKINGS IN 3 LANE TRAFFIC. THE TRUCK LURCHED TO THE RIGHT BOTH TIMES AND I HAD TO FIGHT THE WHEEL TO KEEP THE TRUCK STRAIGHT! IT KEPT DOING IT ON OUR WAY BACK HOME TO MAINE AS WELL. IT HAS DONE THIS FOR QUITE SOME TIME AND ON JULY 30TH OF 2015 I TOOK MY TRUCK TO HIGHT FORD IN SKOWHEGAN, MAINE FOR AN UNUSUAL NOISE IN THE DRIVE LINE. THEY PUT THE VEHICLE ON THE LIFT AND CHECKED EVERYTHING OUT. THE MECHANIC BROUGHT ME TO THE TRUCK THAT WAS ON THE LIFT AND SHOWED ME THE EXCESS BRAKE DUST ON THE RIMS. HE ASKED ME TO TRY TO TURN THE RIGHT SIDE WHEEL, IT WAS HARD TO TURN. HE TOLD ME THAT THIS IS WHERE THE PROBLEM COULD BE. THE LEFT WHEEL TURNED WITHOUT EFFORT BUT THE RIGHT SIDE WAS STUCK. I TOLD THE MECHANIC OF THE PROBLEMS THAT I HAVE WITH BRAKING AND THAT IT PULLS HARD TO THE RIGHT. THEY CHECKED ON BULLETINS AND THERE IS ONE BULLETIN ON UNUSUAL BRAKING ON THESE VEHICLES. THIS IS THE 3RD SET OF CALIPERS PUT ON THIS TRUCK AND STILL THE BRAKES ARE PULLING TO THE RIGHT. I AM HEADING OUT TO TENNESSEE ON AUGUST 14TH 2015 AND I AM QUITE CERTAIN THAT THIS PROBLEM WILL OCCUR AGAIN. THE DAY AFTER HAVING THE BRAKES "FIXED" FOR THE 3RD TIME THE RIMS ARE COVERED IN EXCESS BRAKE DUST. NOT ONLY IS THIS SLOWING MY VEHICLE DOWN TO GIVE ME LOWER MILEAGE IT IS ALSO A SAFETY HAZARD IF I HAVE TO BRAKE HARD AGAIN ON A CROWDED HIGHWAY AND SLAM INTO A FEW CARS AS WELL. WHAT HAPPENS THEN?

I ORDERED A NEW F-350 POWER STROKE DIESEL IN NOVEMBER. I WAITED UNTIL JAN 22 FOR IT TO BE DELIVERED TO ME. $65,000. WHEN I WENT TO PICK IT UP AT SUNRISE FORD IN FT PIERCE FLORIDA, IT LOOKED LIKE THE WHEEL HUBS WERE RUSTED SO I LOOKED UNDERNEATH. I COULD NOT BELIEVE WHAT I SAW. THE ENTIRE CARRIAGE TO THE DRIVE TRAIN WAS NOT ONLY CORRODED AND RUSTED, BUT HOLES WERE DEVELOPING IN THE DRIVE SHAFT, THE TIE RODS, THE CARRIAGE BOLTS, THE WHEEL SUPPORTS, THE U-JOINTS...NOT SURFACE RUST. IT WAS AS THOUGH THE TRUCK PARTS WERE TAKEN FROM A JUNK YARD. THE DEALER TOLD ME THEY ARE ALL LIKE THAT. SURE ENOUGH, WHEN I LOOKED UNDER ALL THE TRUCKS THEY WERE ALL ROTTED OUT...BEAUTIFUL BOX PUT ON ROTTED CHASSES. THE THING IS THAT WHEN YOU TOW TENS OF THOUSANDS OF POUNDS WITH THESE "WORK HORSES", IT IS THE CHASSIS THAT NEEDS TO BE STRONG AND WHEN IT IS ALREADY OXIDIZING, IT IS GOOD FOR NOTHING. I TOOK BACK MY DEPOSIT AND BEGAN TRAVELING TO NORTH PALM AND OTHER DEALERS....SAME THING...RUSTED AND ROTTED OUT CHASIS, CHASIS PARTS, DRIVE TRAIN AND PARTS AND BOLTS AND OTHER COMPONENTS. WE MADE THEM LIFT THE TRUCK SO I COULD SEE IF IT WAS AN OLD VEHICLE THEY WERE PASSING OFF AS NEW....9 MILES ON THE ODOMETER, TRAVELED BY FREIGHT...NOT DRIVEN...AND THE YEAR ON THE STICKER IN THE DOOR WAS 2014/JAN. WHEN I ASKED WHAT OTHER PEOPLE DO...HE SAID, "NO ONE EVER THINKS TO LOOK UNDERNEATH....BY THE WAY...THE CHASIS IS ONLY WARRANTIED FOR THREE YEARS...THEN FORD WASHES THEIR HANDS OF IT! WHAT HAPPENED TO AMERICA?THIS NEEDS TO BE PUT ON THE SOCIAL MEDIA. I FILMED THE ENTIRE UNDER CARRIAGE AND WOULD LIKE TO SEND THEM TO YOU. *TR

Some 2013-2016 Super Duty vehicles equipped with a 6.7L diesel engine and 2015-2019 Transit vehicles equipped with a 3.2L diesel engine may not pass a state smog test due to the exhaust gas system (EGS) monitor that will not run after completing the Exhaust Gas Sensor drive cycle located within Section 2 of the Powertrain Control/Emissions Diagnostics (PC/ED) Manual > Drive Cycles. This may be due to the vehicles diesel oxidation catalyst (DOC) age counter being under the 120-minute requirement. Engineering is investigating this issue and working on a solution.

STEERING

2024 F-Super Duty vehicles are equipped with XL-22 (WSS-M2C184-A1) high temperature power steering fluid that looks like clear hydraulic fluid. This fluid is an improvement in high temperature and thermal oxidative stability compared to Mercon LV. This power steering fluid is also compatible and highly recommended to use when filling or topping off power steering systems of all 2012-2023 F-Super Duty vehicles originally equipped with Mercon LV.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.
