Loss of motive power due to engine failure post recall remedy
On January 16, 2026, the Office of Defects Investigation (ODI) opened Recall Query RQ26001. This investigation was opened to assess the remedy effectiveness of Recall 25V-274. ODI has received complaints alleging engine failure in vehicles after the recall remedy had been completed, whether that remedy involved an oil viscosity change or a complete engine replacement. General Motors (GM) issued Recall 25V-274 to address concerns of engine failure in the L87 engines produced between March 1, 2021 and May 31, 2024. GM cited the root cause as a supplier manufacturing and quality issue. GM’s recall remedy for Recall 25V-274 consisted of two different repairs, either an oil viscosity change or an engine replacement, based on the results of the remedy inspection procedure. ODI continues to receive reports of engine failures after the completion of the recall remedy. To date, ODI has received 499 complaints alleging post-remedy engine failure in vehicles subject to Recall 25V-274, 473 of which involved an oil viscosity change and 26 a complete engine replacement. Additionally, ODI has received 191 reports of L87 engine failures in engines produced after the production period cited in Recall 25V-274. GM reported receiving 6,953 complaints of post-remedy engine failure. ODI is opening this Engineering Analysisto continue investigating complaints of post-remedy engine failure. ODI is expanding the scope of this Investigation to include engines produced after the production period cited in the recall. Loss of motive power due to engine failure
The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001). GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274. This presents a potential safety risk that warrants further investigation. ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues of vehicles built outside the recall scope. On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024. GM cited the root cause as a supplier manufacturing and quality issues. GM’s recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures which fall outside of the scope of recall 25V-274. Based on GM’s recall, ODI will close this PE. NHTSA will continue to investigate complaints of engine failure outside the scope of recall 25V-274 as part ofEngineering Analysis (EA25007). Vehicle has been maintained at GM authorized dealership according to manufacturer recommendations. 6.2L recall was performed as recommended by manufacturer at approximately 25k miles. Engine failed at 36000 miles. The failure created a safety situation where vehicle suddenly lost propulsion power on the interstate. GM replaced the engine with a remanufactured engine. Vehicle was out of service for approximately 90 days. My family has lost confidence in the vehicle and unfortunately because of the now known issues with this engine the vehicle has lost significant value, making it nearly impossible to trade the vehicle in for something reliable.

The hood molding flew off my vehicle as I was traveling down the interstate. I did not hit anything, it just randomly flew off of my vehicle. The piece flew off and just barely missed the vehcile behind me. If that would of hit the other vehicle, it would of caused damage to their vehicle, and potentially could of caused an accident. The piece that fell off was the trim piece that is diretly below the hood latch and above the front grille. There was no incidcators that this was going to happen. I see there was a recall on the same model and year vehicle as mine but for chrome grilles. The recall # is 25V-060.

At around 1,500 miles the truck reduced power. I was able to drive it to the dealership. The dealership reported the lifters had failed and it would be 5-6 months to repair because of the parts shortage.

While driving down the street pulling a trailer the engine stopped. I was able to get it to the side of the road. I called the GMC dealer and they dispatched a wrecker. No prior warnings, just a check engine light right before it failed and lost momentum. This is the second of three new trucks from GMC with engine failure, 1st was lifters, and the other two were bearing locked up. All maintenance was performed by the Dealership on all three trucks

The contact owns a 2022 GMC Sierra 1500. The contact stated that the vehicle intermittently failed to start and was jumpstarted on each occurrence. The contact purchased and installed a new battery; however, the failure persisted. The vehicle was taken to an authorized dealer, where it was diagnosed that a corroded ground strap had caused an electrical short to the battery. The dealer replaced the ground strap, and the vehicle was repaired. The contact associated the failure with Customer Satisfaction Program: N252515590; however, the VIN was not included. The failure mileage was unknown.

I request that GM and NHTSA investigate whether recall N242490120 / NHTSA recall 25V060 was properly performed and effective, determine why the deflector detached, and assess whether improper installation, defective parts, inadequate fastening, impact, road debris, or another cause was responsible. Evidence and Records Requested Please review or provide: • Complete recall-repair records, including repair orders, technician notes, procedures, parts and fastening specifications, photographs, repair date and mileage, personnel identities, and attachment-point inspections. • An explanation for the discrepancy between the April 18, 2025 myGMC date and April 28, 2025 GM VIN-lookup date. • Dealership inspection findings, photographs, measurements, and conclusions. • GM repair instructions, technical guidance, recall-validation records, and engineering analyses. • A written determination of the probable cause and whether this was a remedy failure, improper installation, defective part, unrelated damage, or unresolved cause. • Criteria for determining whether the grille must be replaced or repaired. • Warranty, goodwill, claim, technical-assistance, and customer-service records. • Similar complaints, warranty claims, field reports, damage reports, and recurrence data. Corrective Actions Requested I request that GM identify the cause, determine whether the grille must be replaced, and assess whether the attachment can be repaired. If the incident resulted from the recall condition, repair, improper installation, defective parts, or failure of the remedy, I request no-cost repair or reimbursement, including grille replacement. If GM attributes the incident to unrelated damage or an external event, I request the supporting evidence and technical basis. I request that NHTSA determine whether additional oversight or corrective action is warranted to prevent recurrence and road-debris hazards.

The contact owns a 2022 GMC Sierra 1500. The contact stated that while driving 25 MPH, the contact entered a parking lot and shifted the vehicle into park and the message “Shift to Park” was displayed. Additionally, the check engine warning light illuminated. The contact stated that the failure had occurred on several occasions. Due to the failure, the contact was unable to start the vehicle. The failure mileage was approximately 74,000.

General Motors has decided to conduct a Voluntary Emission Recall. Certain vehicles may have a condition which can cause the fuel pump power control module to fail. If this were to occur, fuel flow to the engine could be interrupted. Dealers will replace the fuel pump power control module.

This PI is created to inform GM dealers that some vehicles may intermittently fail to connect wireless Apple CarPlay or Android Auto after Bluetooth pairing during the first key cycle. The condition is caused by a software anomaly.

This preliminary information advises a dealer on how to email GDS2 session logs into Technical Assistance.
