Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
Loss of motive power due to broken crankshaft with no ability to restart.
On November 15, 2023, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE23020 to investigate allegations of loss of motive power in model year (MY) 2016 through 2019 Nissan Titan XD trucks equipped with 5.0L Cummins diesel engines. The complaints allege the condition was due to a failure of the engine crankshaft. The purpose of the investigation was to understand the scope, frequency, and safety related consequence of the alleged defect. During the investigation, it was found that the subject vehicles can experience a loss of motive power due to a failure of the crankshaft main bearings which can lead to a fractured crankshaft. This resulted in a loss of motive power and an inability to restart. An analysis of all complaint sources shows many vehicles exhibit knocking, vibration, malfunction indicator lamp (MIL) warnings, and/or poor engine performance prior to a total failure. Some consumers report driving their truck in for repairs only to be diagnosed with a failed crankshaft. ODI has not identified any injuries or fires attributable to this failure. ODI is aware of one complaint alleging a non-injury crash involving a curb strike. ODI is also aware of complaints of overheating starter motors and melted starter motor wiring occurring after prolonged attempts to start engines that have suffered a broken crankshaft. In its response to ODI's January 26, 2024 information request letter, Nissan provided information detailing several material and process changes involved in the production of the subject vehicle’s engine.These changes appear to coincide with incremental improvements in failure rates but there is no indication that a single factor was the primary cause of the failures. Analysis of ODI consumer complaints and manufacturer provided data found that failure rates decreased progressively for each model year. Review of failure mileage found that 50% of the failures occurred by 60,000 miles with a significant decrease in failures after 80,000 miles. Assuming average yearly mileage driven, the newest model year trucks (2019) would be near or past peak failure mileage. Given the low failure rate, the observable warnings available to the driver, and much of the fleet being past peak failure mileage, ODI is closing this Preliminary Evaluation with no further action. The closing of the investigation does not constitute a finding by NHTSA that a defect does not exist, and NHTSA will take further action if warranted by future circumstances. Cylinder 4-5 which caused catastrophic engine failure. CP4 fuel pump. Crank shaft broken, Metal flakes in oil. Differential service. Was on the road from [XXX] to [XXX] and broke down in [XXX]. Just bought the truck from Lakeland Automall on May 8th and broke down on May 31st. Was in a bad spot to pull over and from what I have gathered we were lucky the Truck didn't catch fire. The problem has been confirmed by the Nissan dealership in [XXX] and also by an independent. As of yet it has not been inspected by the manufacturer, police or insurance representatives. My next call will be to my insurance company for inspection. I was hoping I would be able to fix it on my own by to no avail. No warning lamps or messages before. My crankshaft broke on the highway. I was travelling at around 70mph. No warnings were given. I lost all power and had to limp it off to the side of the road. The engine was replaced under warranty. This is an ongoing issue for 2016-2019 Nissan Titans. Folks are left with worthless vehicles because they are outside of their warranty period when it happens.

The contact owns a 2018 Nissan Titan. The contact stated that while driving approximately 40 MPH over a ramp in the road, there was a loud banging sound coming from the front passenger’s side of the vehicle. Later, while depressing the brake pedal, there was a scraping sound coming from the front passenger’s side wheel, and the vehicle started jerking and vibrating and failed to properly stop the vehicle. The vehicle was eventually taken to the local Pep Boys, who determined that the front passenger’s side brake caliper bolt was missing from the brake caliper and informed the contact that the brake caliper needed to be replaced. The brake caliper was replaced, and the vehicle was repaired. The contact stated that the front passenger’s side wheel was damaged due to the failure and needed to be replaced. The dealer was notified of the failure, but no assistance was provided. The manufacturer was not contacted about the failure. The failure mileage was 127,000.

The contact owns a 2018 Nissan Titan. The contact stated that while driving at 40 MPH and accelerating, the vehicle shut off. The contact stated that there was an abnormal pop before the vehicle lost motive power and was pulled over to the shoulder of the roadway. The vehicle was towed to a local dealer, where it was diagnosed with a fractured crankshaft, rod bearings, and engine failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 127,000.

The contact owns a 2018 Nissan Titan. The contact stated that while refueling, there was an engine run-on after the vehicle was turned off. The contact determined there was a vapor lock issue and that air was entering the fuel system. The vehicle was difficult to start. There was an abnormal clicking sound coming from the vehicle. There were no warning lights illuminated. Upon research, the contact determined that the failure was like TSB: NTB19-049. The contact used a scanner and determined the failure was related to the fuel pump module. The local dealer was contacted and confirmed that parts were on back order. The contact called another local dealer, Victory Nissan Parts Center (307 Thompson Ln, Nashville, TN 37211); who confirmed that parts were backordered for approximately 6 to 8 months. The failure mileage was approximately 62,000.

The vehicle has recurring starting issues. The battery frequently becomes weak or completely unable to start, sometimes without warning. The issue occurs intermittently and appears electrical in nature. The vehicle may require jump starts or fails to start after sitting or after driving. This raises concern for an electrical or charging system defect. Battery has been replaced 4 times in the past 5 years. Jumped and charged numerous times in between.

Imminent catastrophic engine failure on 2018 Nissan Titan. The engine has developed severe cylinder wall scuffing and loss of compression (150psi vs 264psi spec) on multiple cylinders. This defect can cause loss of power and potential stalling at highway speeds, presenting a safety risk. The failure matches Nissan TSB NTB19-057 (knock/scuffing). Despite a perfect maintenance history, Nissan is denying warranty coverage by claiming there was no oil change service record because the maintenance was not done at a Nissan dealership. The local Nissan dealership confirmed the issue matches the TSB and an independent inspection confirmed the defect is mechanical (piston/cylinder clearance).

Don't really know the truck was running fine doing great and then all of a sudden started acting up they run a diagnostic on it and said it was the EMC was bad and I don't understand why because everything was fine on it

TELEMATICS SERVICE INFORMATION HINT: This bulletin applies to vehicles equipped with a 4G network compatible TCU. TCUs compatible with 3G network only are no longer supported. SERVICE INFORMATION The APPLIED VEHICLES that come equipped with a factory installed SOS switch, located near the overhead map lamp, also come equipped with a wireless communication device called a Telematics Communication Unit (TCU). With an active NissanConnect® Services subscription, the TCU communicates with the NissanConnect® Services Data Center to provide various security and convenience services. This bulletin contains important service procedures that must be performed properly in order to set-up and maintain the telematics system for the APPLIED VEHICLES. · A table has been provided on the next page that lists the steps required to successfully configure a TCU after it has been replaced. · A CHECK-OFF SHEET has been provided on page 20 to print and check off steps as they are performed. Staple the completed CHECK-OFF SHEET to the repair order.

FLUID LEAK FROM THE FRONT DIFFERENTIAL CASE ACTUATOR MOTOR PLASTIC THREADS SERVICE INFORMATION Figure 1 Figure 2 If the front differential has a fluid leak coming from the actuator motor plastic threads (Figure 1): a. Place an alignment mark on the splined actuator body and the gear carrier (Figure 2). b. Remove the actuator and thoroughly clean the threads, making sure not to allow RTV to fall into the differential case. c. Once clean, apply Three Bond sealant to the threads, and then reinstall the actuator motor, making sure to line up the alignment marks. d. Check the front differential gear oil level and top to specification. o Refer to the ESM: TRANSMISSION & DRIVELINE > DRIVELINE > FRONT FINAL DRIVE: MA235/MA210 > PERIODIC MAINTENANCE > FRONT DIFFERENTIAL GEAR OIL DO NOT replace the front differential due to a fluid leak from the actuator motor plastic threads unless there has been consequential damage due to fluid loss (bearing failure or gear noise) to the front differential.

Nissan dealers will inspect for the Exemption label and/or VECI label, and remove Exemption label if present and install VECI label if not present. ***** What Dealers Should Do**** 1. Verify if vehicles are affected by this Voluntary Emission Recall Campaign using Service Comm or DBS National Service History Campaign I.D. PD127. 2.
