Unintended and Uncontrolled Acceleration
On February 16, 2010, NHTSA's Office of Defects Investigation (ODI) opened RQ10-003 to determine whether the scope of prior Toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, Toyota recalls 07E-082, 09V-388, 10V-017, and 10V-023.NHTSA also requested information regarding potential electronic causes of unintended acceleration in Toyota and Lexus vehicles.I. Pedal Interference from Floor mats, Carpet Covers and Plastic Pads in Carpets.During the RQ10-003 investigation, ODI reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agency's analysis, NHTSA requested that Toyota recall additional vehicles.Toyota complied with the agency's request.The details of these recalls are set forth more fully in Toyota's reports to NHTSA pursuant to 49 CFR Part 573.These recalls are as follows:Recall 11V-112: (1) model year (MY) 2004-2006 Toyota Highlander and Highlander Hybrid; and, (2) MY 2004-2007 Lexus RX330,RX350, and RX400h (hybrid model). The total estimated population under this recall is 769,379 vehicles.This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles.This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles.This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration.After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles.That study has concluded.Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.
SUDDEN LOSS OF POWER STEERING ASSIST
THE OFFICE OF DEFECTS INVESTIGATION (ODI) OPENED PE06-055 BASED ON TWO COMPLAINTS OF LOSS OF EPS ASSIST IN 43,931 MY 2006 TOYOTA HIGHLANDER HYBRID VEHICLES.ONE OF THE COMPLAINTS ALLEGED THAT A LOSS IN POWER STEERING ASSIST RESULTED IN A CRASH.IN RESPONSE TO AN INFORMATION REQUEST LETTER FROM ODI, TOYOTA SUBMITTED INFORMATION ABOUT 6 ADDITIONAL CONSUMER COMPLAINTS, INCLUDING 2 ALLEGING CRASHES, AND 109 WARRANTY CLAIMS RELATED TO THE EPS ASSEMBLY IN THE SUBJECT VEHICLES.ODI?S ANALYSIS OF THE CRASH INCIDENTS REVEALED THAT EACH INCIDENT OF ALLEGED LOSS IN POWER STEERING ASSIST OCCURRED AT VERY LOW SPEEDS AND PRESENTED MINOR SAFETY CONSEQUENCES.TOYOTA'S RESPONSE ALSO IDENTIFIED THREE POTENTIAL ISSUES THAT COULD RESULT IN A LOSS OF EPS ASSIST IN THE SUBJECT VEHICLES.TWO OF THE CONDITIONS ARE RELATED TO ELECTRICAL CURRENT SURGES WITHIN THE EPS ECU.THE OTHER IS RELATED TO EPS MOTOR MAGNETS BECOMING DISLODGED BECAUSE OF POOR ADHESION.THE TWO ISSUES RELATED TO CURRENT SURGES CAN ONLY OCCUR WHEN THE VEHICLE IS BEING PARKED OR BEING OPERATED AT VERY LOW SPEEDS.THE THIRD ISSUE COULD THEORETICALLY OCCUR AT ANY SPEED, BUT OCCURS AS A RESULT OF A SPECIFIC TYPE OF IMPACT TO ONE OF THE FRONT WHEELS.TOYOTA STATES THAT IN ALL THREE CASES, AND DURING ANY LOSS OF POWER STEERING ASSIST TO THE VEHICLE, THE FRONT WHEELS CAN STILL BE DIRECTED WITHOUT ASSIST, MANUALLY, VIA THE STEERING WHEEL. ODI'S ANALYSIS OF THE COMPLAINT DATA AND OTHER INFORMATION SUBMITTED BY TOYOTA INDICATES THAT THERE IS A LOW OVERALL FAILURE RATE FOR ALL OF THE CONDITIONS COMBINED THAT CAN RESULT IN LOSS OF EPS IN THE SUBJECT VEHICLES (18 COMPLAINTS PER 100,000 VEHICLES AND A 0.2% WARRANTY CLAIM RATE) AND THERE IS NO EVIDENCE OF ANY FAILURE THAT COULD CAUSE A LOSS OF STEERING CONTROL.THE CHANGE IN STEERING EFFORT IS GREATEST AT VERY LOW SPEEDS WHEN THE STEERING IS AT OR NEAR END OF TRAVEL.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL CONTINUE TO MONITOR COMPLAINTS AND OTHER INFORMATION RELATING TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES AND TAKE FURTHER ACTION IN THE FUTURE IF WARRANTED.

ABS actuator/accumulator issue on 2006 Toyota Highlander hybrid limited. Internal component failure there are recalls on others but not this particular year. Was driving along hit the brakes and got hard pedal. Yes there were other indicator lights on including the VSC system and traction control I came to an intersection and I couldn't stop I couldn't even kind of stop it was all hard pedal were before it was normal as if there wasn't an issue. I've done a lot of research and I've seen a lot of forums and people are complaining that Toyota isn't backing this problem my 2006 Toyota is in perfect condition not a single problem except for this break issue and if you take it to Toyota the park cost $2,200 in about roughly over $5,000 to fix it well who wants to put that kind of money into a car of that year the problem is it's just a shame and a safety deal that it put my family's life at risk we could have crashed it if I wasn't an actual driver by trade I wouldn't have known how to react in other families could be harmed in this. It's worth looking into I did not need to make her police report because I veered off into a field that saved my family's life. As of now I'm hoping to open a recall campaign and just have that part replaced I could have a car that could drive another 100,000 miles and give and keepToyota good name

The contact owns a 2006 Toyota Highlander. The contact stated that while driving approximately 60 MPH, the vehicle failed to decelerate while the brake pedal was depressed. The contact stated that excessive pressure was needed on the brake pedal to stop the vehicle. The ABS and ESC warning lights were illuminated. In addition, there was an abnormal sound coming from underneath the vehicle. The vehicle was towed to a local independent mechanic, where it was diagnosed and determined that the ABS unit needed to be replaced. The vehicle was towed to another independent mechanic for a second opinion, and it was confirmed that the ABS unit needed to be replaced. The vehicle was taken to a local dealer, where it was diagnosed and determined that the brake actuator needed to be upgraded. The vehicle was diagnosed, and parts were ordered for the repair. The manufacturer was not made aware of the failure. The failure mileage was approximately 195,000.

My 2006 Toyota Highlander Hybrid Limited has an ABS fault that has caused my vehicle to undergo several maintenance issues and poses a serious safety hazard. My vehicle has less than 150,000, and over the course of the last year, my ABS light, my emergency brake light, my TSC light, and my tire pressure sensor light have all come on and refuse to go away. Additionally, the maintenance light, along with a "CHECK VSC SYSTEM" message has been on my dash for months and will not reset. My ABS has been making an intermittent buzzing sound that cycles every ten seconds when my car is on. That has been going one since shortly before the lights. When the lights came on, I noticed a serious change in my braking. The pedal would become spongy one moment and rock solid the next, or my car would brake intensely with the lightest pressure or require my foot basically to the floor to stop it. This has caused a serious issue for the safety of myself and my family. After using and OBDII scanner with ABS capabilities, the codes I received point to the ABS assembly failing to retain pressure, hence the persistent cycling to compensate for it loosing pressure. This was confirmed after the battery voltage/connection, relays, fuses, and speed sensors had all been tested. After doing further research, several other owners of similar configurations to mine share the same issue. My dashboard even makes an incredibly distracting ringing noise that can only be stopped by removing all the fuses and relays to the ABS entirely. This has not been caused as a lack of proper maintenance as this vehicle has only been in my possession for the past 3 years, of which before it came from the Toyota dealership. The dealership wants over $3,000 to fix the issue, and the part alone is $1,850 since the part has been discontinued. This fix costs more than the car is worth given its age and mileage. During any inclement weather, or in traffic, the car is unpredictable in how hard it might brake.

Toyota’s part number 44050-48190 brake actuator assembly failed, as it has in other models and is very commonly doing in the Highlander model. It is triggering other warning lights issues in the vehicle. Vehicle safety is at risk. Many others have reported the same issue.

I was on the interstate driving about 72 mph when the car just shut off all the dash lights came on and the dash it said check hybrid system I had no brakes or steering I got it off the road turned it off and back on again it started up and I drove it home but the dash still displayed check hybrid system , a week later I get in it to go to work and there’s an alarm that won’t cut off and the abs , traction control and brake light and warning light and tire pressure light are all on and the brakes are hard and there’s a weird noise coming from somewhere when u push on the brake and it says check VSC system on the dash now

The vehicle suffers from an ABS module failure. I've been researching that this is common problem with the Toyota Highlander Hybrid and presents a danger of failing brakes while driving. My brakes partially failed with my [XXX] son in the car. When attempting to stop, I had no hydraulic pressure and luckily was able to get the vehicle stopped after the car had regained some pressure. The vehicle had been running for several minutes, so it wasn't related to a fresh start and the system not having time to pressurize, if that's a thing. The brakes on my 06 Highlander hybrid AWD have been finicky at best since I’ve owned the vehicle. The car lunges forward when braking, all the way until a complete stop. While driving at around 55 MPH a medium sized dog ran out into the road. I braked hard but the brakes just lunging and trying to stop. At one point feels like it’s not braking anymore and just hope to veer out of the way. This can be fatal not only to the driver but to others driving as well. The fact that people are still having this issue, and Toyota hasn’t come up with a proper long term solution speaks to how much Toyota cares about its customer bases’ safety. It is honestly disgusting that people have had accidents and may have fatal and Toyota continues to not care. Please urge Toyota to look into this further and attempt to fix this permanently!

The driver's side sun visor does not stay up against the roof of the car and falls down obstructing the driver's view. It will not stay up and falls down immediately when pushed up against the roof. We have resorted to tying the visor up but then are unable to use the sun visor while driving.

Follow the procedures in this bulletin to remediate the odor and address this condition. The purpose of this Service Bulletin is to provide general guidelines and procedures for odor remediation. This Service Bulletin provides a guide on how to prepare and treat the interior of the vehicle for odor remediation. Refer to the applicable model and model year Repair Manual and the EPA (Environmental Protection Agency) website for the most up-to-date safety and precautionary guidelines.

Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: •Minor damage: requires only buffing to repair. •Moderate damage: usually requires neutralizing, color sanding, and buffing. •Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. It is the dealer’s responsibility to protect and maintain the quality of the vehicle’s paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.

The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. It is the dealer’s responsibility to protect and maintain the quality of the vehicle’s paint finish after receipt at the dealership prior to the first sale. In areas known for high frequency and/or concentration of acid rain, frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.

To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.

Toyota vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination. Follow the Removal Procedure in this bulletin to remove the RapgardTM protective film within 90 days from initial application.

Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid. Follow the Repair Procedure in this bulletin to clean vehicles that may have been subjected to contamination by airborne iron particles such as rail dust during rail transportation or extended storage near industrial areas.
